RESUMEN

Objective To analyze the therapeutic effect of arthroscopic meniscus -plasty in the treatment of meniscal lesions.Methods Patients admitted to our hospital′s department of orthopedics were reviewed,and they were divided into two groups according to the surgical methods.The observation group was treated with meniscus -plasty,while the control group was treated with meniscectomy.The Ikeuchievaluation standard was compared at the time of the last follow -up.The Lysholm and IKDC score were compared before and after treatment.Results The excellent -good rates were 86.5% in the observation group,which was significantly higher than that in the control group (62.8%,χ2 =5.768,P =0.016).There was no difference in Lysholm and IKDC score between the two groups before treatment[(63.4 ±10.8)points vs.(61.7 ±9.5 )points,P >0.05;(54.6 ±7.5 )points vs.(55.1 ± 8.3)points,P >0.05].After treatment,the Lysholm[(92.5 ±5.9)points and (90.4 ±6.6)points]and IKDC scores[(85.4 ±9.2)points and (83.6 ±9.8)points]were all significantly increased (t =14.383,16.269,15.784, 14.552,all P 0.05).At the time of the last follow-up,both Lysholm scores[(78.5 ±8.2)points]and IKDC scores[(75.5 ±7.9)points]were decreased in the control group (t =7.413,4.220,all P <0.05),and there were significant difference between the two groups (t =6.165,4.029,all P <0.05).Conclusion Meniscus -plasty and meniscectomy have similar short -term effects on meniscal lesions,but meniscus -plasty may show better effects in the long term.

RESUMEN

The value of double contrast arthrography in meniscal lesions was studied in 58 patients with clinical findings suggestive of meniscal lesion. In all cases, arthrotomy was performed and meniscal lesions were found in 52 patients. The arthrographic findings were compared to clinical and operative findings and the following results were obtained. 1. The diagnostic accuracy of arthrography in meniscal lesions was 84.5% 2. The diagnostic accuracy of the clinical findings was 69%, but the accuracy was increased to 91.4% with the aid of arthrographic findings. 3. Arthrography would be unnecessary if the clinical findings highly suggest the meniscal lesion. 4. Arthrography is thought to be a useful diagnostic method for meniscal lesions.

RESUMEN

We meniscectomized 27 knees with meniscal lesions for disabling symptoms after the diagnosis by clinical evaluation and arthrography from January, 1970 to May,1979. Statistical analyses were as follows: 1. Male was affected 2 times more than female. The most meniscal lesions occurred in the age group between 10 and 29 years old (67%). 2. The lateral menisci were affected 2 times than the medial. In the former, the middle segments were affected most frequently and in the latter, the posterior horns. 3. The general diagnosis of torn meniscus for the 27 knees was correct by clinical evaluation for 19(71%), by arthrography for 20 (74%) and by two methods for 24 (88%). 4. In the follow-up study, satisfactory results were obtained in. 79%.

RESUMEN

We assessed the accuracy of clinical evaluation, arthrography, and arthroscopy in the diagnosis of meniscal lesions in twenty-nine knees in which arthrotomy and menisectomy were performed after evaluation by these three methods, At surgery, thirty menisci of twenty-nine knees were removed, of which twenty-eight were abnormal and two were normal. In these thirty menisci, a correct diagnosis was made clinically eighteen times, arthrographically twenty times, and arthroscopically twenty-seven times. Posterior horn tears of medial meniscus and discoid lateral meniscus were well visualized but lateral meniscal tears were poorly visualized on arthrogram. The errors of arthroscopy occurred in two tears confined to outer one half of medial menscal posterior horn, and in a normal meniscus as false positive. Based on this study, it was concluded that both the arthrography and arthroscopy are valuable techniques for the diagnosis of meniscal lesions, and that we can expect correct diagnosis of all the meniscal lesions by the use of complementary roles of the two and by the experiences.
