@#Abstract: Objective To explore the prevalence and bacterial strains of Francisella tularensis (F. tularensis) in wild rodents in Changbai Mountain area of China, and to further understand the epidemiological characteristics of F. tularensis infections in this area. Methods Wild rodents were captured from forest and forest-edge farmland from Kuandian County and Jianshi Forest District in the Changbai Mountain area, 2012-2014. DNA was extracted from the spleen tissues of the rodents, and the fopA gene of F. tularensis in wild rodents was detected using nested PCR. The infection rates were calculated for different areas and rat species. The bacterial subspecies of positive samples were identified using type-specific primers (C1/C4), and sequencing and comparative analysis were performed. Results A total of 133 wild rodents belonging to 6 rat species were captured. Among them, eight samples from three rat species (Apodemus agrarius, Apodemus peninsulae, Tscherskia triton ) were detected positive, with the overall positive rate of 6.01%. The positive rates of F. tularensis of Ji'an and Kuandian were 7.46% and 4.54%, respectively, and there was no difference in positive rates for different regions (χ2=0.117, P=0.732) and different rat species (χ2=0.641, P=0.986). The subspecies analysis showed that the detected 8 trains of F.tularensisall belonged to F.tularensis type B (F.subspecies subsp. holarctica). Genetic evolution analysis was performed on the fopA gene sequences of three positive samples (JA56, JA33, and JA38), which clustered together with Russia strains(CP009694.1, CP044004.1) and China strains (HM371344.1, HM371343.1) F.tularensis type B, with sequence similarities ranging from 99.21% to 99.47%. Conclusions Infection of F.tularensis subsp. holarctica existed in wild rodents in Changbai Mountain area of China, which suggests the existence of F.tularensis infection risks in this area.

RESUMEN

Objective:To envaluate the effect of public cardiopulmonary resuscitation (CPR) training in Ningnan Mountain area.Methods:Using the method of convenient sampling, 775 people in Ningnan Mountain area were investigated about CPR and first aid knowledge by questionnaire from January to December 2019. The awareness rate of CPR, operation score, and operation qualification rate of the public before and after CPR training in the primary trauma treatment (PTC) mode were observed.Results:After PTC training, the public's CPR knowledge score, operation score and operation qualification rate, in terms of first aid telephone, consciousness judgment method, identification of respiratory arrest, artificial respiration implementation method, effective CPR indication and extrathoracic cardiac compression position were significantly improved [first aid telephone score: 82.68±8.54 vs. 60.25±10.38, consciousness judgment method score: 79.46±10.82 vs. 58.35±9.26, identification of respiratory arrest score: 80.85±9.64 vs. 59.26±11.45, artificial respiration implementation method score: 81.54±9.48 vs. 56.47±10.54, extrathoracic cardiac compression site score: 80.35±10.48 vs. 59.56±9.85, effective indication of CPR score: 81.02±9.45 vs. 58.21±8.69, operation assessment score: 60.25±10.45 vs. 50.38±9.68, operation assessment qualified rate: 60.39% (468/775) vs. 12.13% (94/775), all P < 0.05]. Conclusion:PTC mode is helpful to improve the effect of public CPR training, which is worthy of clinical promotion.

RESUMEN

Objective To evaluate the effect of comprehensive schistosomiasis control measures based on infection source control in plateau mountain areas of Yunnan Province. Methods From 2006 to 2004,four administrative villages were selected as test areas from plateau canyon and plateau basin endemic areas in Jindun Town,Heqing County,two villages each type,and the comprehensive control measures were implemented,including the examination and treatment of schistosomiasis,Oncomela?nia hupensis snail survey and control,health education,improving drinking water and lavatories,banning grazing,constructing sanitary pen of livestock,replacing cattle with machine,etc. The schistosome infection state and snail status in 2006 were treat?ed as the baseline information,and the effect of the comprehensive measures were evaluated. Results The infection rate of hu?man in plateau canyon areas decreased from 4.94%in 2006 to 0.06%in 2014,and that of livestock decreased from 1.11%to 0. In plateau basin areas,there was only 1 case of schistosomiasis found in Xiaolian Village in 2007,and no any other cases found in the other years,the infection rates of livestock dropped from 7.38%to 0. Compared with 2006,the snail areas in the two type areas decreased by 74.89%and 75.30%,respectively,meanwhile,the percentage of snail area,the occurrence rate of frames with snails,as well as the average density of living snails also decreased,and no infected snails were found since 2008. Xidian and Xinzhuang villages in plateau canyon area reached the criteria of schistosomiasis transmission controlled in 2009,and Xiao?lian and Kangfu villages in plateau basin reached the criteria of transmission interrupted in 2014. Conclusions The comprehen?sive schistosomiasis control measures based on infection source control can effectively control the endemic situation of schistoso? miasis in plateau areas of Yunnan Province. In the future,we should pay an equal attention to the infection sources control and snail control to consolidate and amplify the achievement of schistosomiasis control.

Objective To understand the status of water contact and labor practice of residents in plateau mountain schisto?somiasis endemic areas in Yunnan Province so as to provide the evidence for formulating the strategy of prevention and control of schistosomiasis in the next step of work. Methods A total of 412 residents aged 6-65 years old in 2 villages were randomly sampled and investigated with questionnaires for their water contacts and labor practices in plateau valley schistosomiasis endem?ic areas. Then the schistosome infection status of the residents was surveyed with the indirect hemagglutination assay IHA . Re?sults Among the 412 residents investigated the rate of water contact was 88.35% and the main causes of water contact were watering and swimming and playing. The percentages of residents who had schistosomiasis history were 1.16% 0.00%30.80% 3.85% and 0 in them who had swimming and playing water?contacts bathing watering washing hands and others respectively χ2=38.96 P<0.01 . The positives of IHA in the above?mentioned residents were 18.60% 0.00% 37.60%23.08% and 0 respectively χ2=12.61 P<0.05 . Conclusions The productive infested water contact is the main way of schistosome infection. Therefore the changes of labor practices and water contact of the residents are very important for the ef?fective prevention and control of schistosomiasis transmission.

Background: The fi eld isolates of P. falciparum may display variant forms and different frequencies. This study was designed to know the diversity of allelic type of MSP-1 block 2 among P. falciparum isolates collected in a mountain and a coastal area in West Sumatera, Indonesia, and compare mountain and coastal area. Methods: A total of 56 P. falciparum infected blood samples, collected from 27 patients attending local health facilities in South Solok district in a mountain region and 29 patients attending a local health facilities in South Coastal district region, West Sumatera, Indonesia were used in this study. The regions flanking the highly polymorphic characters, block 2 for MSP-1, were genotyped by allele-specific nested-PCR to analyse the population diversity of parasite. Sequence analysis of the polymorphic regions of MSP-1 was also conducted to identify allelic diversity in the parasite population. Results: Diverse allelic polymorphism of MSP-1 was identified in P. falciparum isolates from a mountain area and a coastal area in West Sumatera, Indonesia, and most of the infections were determined to be mixed infections. Sequence analysis of MSP-1 block 2 revelaled that 16 different alleles for MSP-1 (3 for K1 type, 2 for MAD20 type and 2 for RO33 type) were identified. Conclusion: Extensive genetic polymorphism with diverse allele type was identified in MSP-1 in P. falciparum field isolates from a mountain and a coastal area. A high level of mixed infections was also obcserved, as was a high degree of multiplicity of infection.

RESUMEN

Objective To anderstand the metagenesis of Oncomelania snails in the mountainous regions so as to control the spread of snails and the epidemics of schistosomiasis.Methods Observation spot was established at a typical snail habitat close to Puge county,Sichuan province from February 2008 to July 2009.Random sampling was applied to determine the place of each frame during the observation.All the snails in each frame were collected and numbers counted in the laboratory,with the number of mating pairs in each frame also observed.Snails being collected were measured for the body indices and the dissection was carried out to identify gender composition.survival status and the number of live snails in each frame counted.Line graphs of the body indices.mating pairs in each observed months,bar graphs of the snail density,proportions of gender together with the maturity of the snails in each month were drawn.Results The number of live snail existed the whole year and its density fluctuated.All the three kinds of body indices showed the same time trend and a dynamic circulation.The young snail existed all year around and arose constantly in proportion from May,becoming the dominant snailin October to replace the adult snails.The young and adult snails also showed a dynamic alternative.The gender composition showed no significant difference during each month.The number of the mating pairs was more on April.May and June annually,when were the snail's main multiplying stage.Conclusion In mountain area.the young snails existed through all the year while adult snails appeared to be dominantin each month except for October.Oncomelania snail showed a circular process of metagenesis which started in May and finished in October.The snail population presented a dynamic equilibrium.It was concluded that ecological studies on Oncomelania snail were extremely relevant,either to optimally apply the existing control measures or to develop alternative measures for snail control,ecologically or biologically.

OBJECTIVE To approach practicable management of biosafety in clinical laboratory department of mountain area grass roots hospital.METHODS The status and existing problem of biosaety management in clinical laboratory department of mountain grass areas root hospital were investigated and analyzed.RESULTS A series of problems such as facilities,construction arrangement,work environment,and personnel′s biosafety consciousness were existing.CONCLUSIONS The biosafety is significant in clinical laboratory department.Medical institutions should execute strictly relative policies and regulations,and think highly of and consummate biosafety protection.
