RESUMEN

En el presente trabajo se describe un caso de muerte súbita en una mujer de 70 años que fallece 20 días después de ser dada de alta tras ingreso por infección urinaria secundaria a litiasis renal complicada con shock séptico e insuficiencia renal y respiratoria. En el corazón se identificó una cardiopatía isquémica crónica y extensas áreas de miocardio calcificado. Además, se exponen los tipos de calcificaciones del miocardio existentes y sus causas; y se analizan aquellos factores que en el caso presentado han podido causar la calcificación del miocardio(AU)


In the present report, we describe a case of sudden death in a 70-year-old woman who died 20 days after been discharged after admission for urinary infection secondary to renal lithiasis complicated with septic shock and renal and respiratory failure. It has been identified chronic ischemic heart disease and extensive areas of calcified myocardium. Also, we review myocardial calfications subtypes and its causes, and we analyze the specifics of the presented case(AU)

RESUMEN

We report the case of a 21 years old woman without history of cardiovascular disease, who after her first euthocic delivery, did complain of genital infection of white parts complicated by pneumonia and pyothorax, isolating Streptococcus pyogenes, requiring intensive care, inotropic and vasoactive medication. Transesophageal echocardiography showed constrictive pericarditis and myocardial heterotopic calcification in the context of sepsis. Subsequently, the pericardial constriction and clinical symptoms improved, with persistent calcifications. The echocardiogram prior to delivery was completely normal.


Presentamos el caso de una mujer de 21 años sin antecedentes cardiovasculares quien posterior a primer parto eutócico simple presenta infección genital de partes blancas complicada con neumonía y piotórax, aislándose Streptococcus pyogenes, ameritando cuidados intensivos, inotrópicos y vasoactivos. El ecocardiograma transesofágico evidenció pericarditis constrictiva y calcificación heterotópica miocárdica en contexto de sepsis. Posteriormente mejoró el cuadro de constricción pericárdica y la clínica, persistiendo las calcificaciones. El ecocardiograma previo al parto se encontraba completamente normal.

RESUMEN

A 1-year-old male meerkat was found dead by the owner. The animal was clinically healthy and was regularly vaccinated for distemper virus. Necropsy revealed multifocal to confluent dry white areas in the myocardium, pneumonia and congestive hepatopathy. All the other organs, including gross vessels, were macroscopically normal. The heart showed histologically large, multifocal to confluent areas of mineralization of the myocardium and the wall of small coronary artery. Vascular calcifications were also observed in the hepatic portal tracts and kidneys arteries of small/medium sizes. The arterial lumen appeared narrowed and the wall thickened due to the calcification of the tunica media. In veterinary medicine, arterial mineralization is regarded as a metastatic calcification, as the result of hypercalcemia and/or hyperphosphatemia. However, today, the pathogenesis of medial artery calcification in humans seems to be the results of an active process resembling embryonic osteogenesis, rather than a mere passive process.

RESUMEN

Cardiac calcification usually occurs in patients with end-stage renal disease. However, rapid progression of cardiac calcification is rarely associated with secondary hyperparathyroidism of end-stage renal disease. We report a patient with end-stage renal disease who showed moderate left ventricular hypertrophy at the first echocardiography, and showed severe myocardial calcification and severe mitral valve stenosis 4 years later. We suspected a rapid progression 'porcelain heart' cardiomyopathy secondary to hyperparathyroidism of end-stage renal disease. The patient underwent parathyroidectomy, and considered mitral valve replacement.

Background & objectives: As adult dengue fever increases it also affects women with pregnancy. Dengue fever is mainly treated conservatively. However, complications like pre-eclampsia, pre-term labour, increased risk of caesarean section and fetal transmission have been noted. During dengue epidemic in our region we noted many women with dengue fever and observed certain different problems. Methods: A retrospective analysis of all pregnant women admitted in obstetric ward with dengue fever over a period of 18 months was done. Results: The striking feature observed was the presence of severe thrombocytopenia in 78.57% of the study population. In addition, coexistence of other vector borne diseases was also noted. Conclusion: Most cases only require conservative treatment. Only women who went into labour required platelet transfusion. Outcome seemed to correlate with gestational age of occurrence of dengue fever.
