RESUMEN

Objective@#To explore the basic conditions and postoperative quality of life in patients undergoing oral vestibular thyroid surgery.@*Methods@#A total of 128 patients undergoing oral vestibular thyroid surgery were enrolled in the Department of Head and Neck Surgery from March 2015 to April 2018. On the basis of routine thyroid tumor care, we used telephone, WeChat public platform, WeChat group, QQ, and Email. Questionnaires such as information technology and clinic review. Correlation analysis was performed using SPSS 22.0.@*Results@#According to the analysis of SPSS statistical software, the postoperative recovery of the patients is good, and more than 95% of the patients have the feeling of "no" or "a little" for symptoms, and 91.8% (102/111) and 93.6% (104/111) of the patients have the perception of the general health status and life quality of the patients in the past 1 week respectively. Analysis of variance of postoperative life quality of patients, postoperative regression post was significantly correlated with postoperative social cognition, insomnia and economic difficulties of patients. The education level, occupation, marital status and family income of the patients all had significant influences on the postoperative economic status of the patients, and the differences were statistically significant (t=2.66-4.74, P<0.05). In the regression analysis of patients′ life quality, emotional function and physical function were independent risk factors affecting patients′ life quality, with statistical significance (t=-2.072, -5.564, P < 0.05).@*Conclusion@#Endoscopic thyroidectomy via oral vestibule has the advantages of good effect, beautiful appearance and high quality of life, which is worth popularizing and applying.

RESUMEN

Realizamos um estudo num CAPSII da cidade de São Paulo. Tratou-se de uma pesquisa participativa, portanto uma pesquisa qualitativa que teve como referencial metodológico o Comunity-Based Participatory Research (CBPR). Tivemos a Reabilitação Psicossocial (RP) como referencial teórico, e tendo em vista a meta de emancipação e contratualização dos usuários do CAPS, realizamos uma pesquisa ação junto a um usuário e sua rede social. Mapeamos a rede social significativa deste usuário através do Mapa de Rede de Sluzki (2006), e iniciamos um trabalho de Acompanhamento Terapêutico (AT) junto ao usuário. Durante o processo compusemos as reuniões de Intervenção em Rede e ao fim construímos um segundo Mapa de Rede no intuito de avaliar se houve mudanças na composição da rede após as intervenções realizadas através desta pesquisa. A rede deste usuário era formada por pessoas do CAPS e de sua família que ao longo do processo apresentaram muitos recursos, era uma rede que inclusive já utilizara o AT e as reuniões de rede como forma de cuidado ao usuário e a própria rede. Vivenciamos nesse período, muitos momentos significativos e identificamos uma série de situações em que usuário e rede eram capazes de produção de sentido em seu cotidiano. Foi possível perceber a partilha de afeto entre o usuário e a rede, e inclusive entre os próprios membros da rede, e ao mesmo tempo havia troca de violência principalmente na relação entre o usuário e sua família. Era uma rede ciente dos preceitos da RP, mas ainda assim, com o agravamento da violência e se avaliava que o usuário estava pondo em risco a vida dele próprio e de seus familiares. A rede se viu impotente para lidar com essa questão e agiu afastando e internando o usuário. Durante o processo pudemos perceber que não somente o usuário, mas a família passara por um processo recente de perda de contratos sociais, especialmente quanto a trabalho e habitação. Ainda que apresente recursos é uma família que passou por um processo de exclusão do sistema de organização social, e o sentimento de impotência parece antes tratar desse processo do que da violência entre eles em si. Se apresenta tentador responsabilizar a família ou o serviço pelo processo vivido, mas parece se tratar de um processo mais complexo em que não se está claro qual é o papel de cada elemento. Através da convivência no serviço percebemos que a universidade e as pesquisas de pós graduação já compõem com o que se tem produzido nos CAPS, nesse sentido, mais do que eleger culpados, isso nos leva a supor ser necessário que nós, enquanto parte do problema, passemos a entender qual o papel que estamos desempenhando no processo, e talvez aí possamos fazer alguma diferença no ciclo que leva a violência, a impotência e a internação de uma pessoa


We conducted a study in a CAPSII at the city of São Paulo. This study is composed by a participatory research, using as a methodological referential the Community- Based Participatory Research (CBPR). The Psychosocial Rehabilitation (PR) was our theoretical referential, and based on the goal of emancipation of CAPS's users, we performed an action research with a user and his social network. We mapped the significant social network of this user with the Network Map of Sluzki (2006) and initiated a Therapeutical Accompaniment (TA) with him. During the process we did Network Interventions meetings and in the end we built a second Network Map, to evaluate if it changed after the interventions we performed during this research. This user's network was composed by people from CAPS e from his family, which during the process presented many resources, a network that already had used the TA and the network meetings to provide care for the user and for the network itself. We lived, during the entire period, several significant moments and identified that the user and the network were able to produce meaning in his daily life. We could observe the sharing of affection between user and network and also between the members of the network. At the same time an exchange of violence could also be observed, especially between the user and his family. Although the network was aware of the RP's precept, with the escalation of violence it was evaluated that the user was putting his and his family's life at risk. The network felt powerless to deal with this problem and acted moving away from the user and interning him. During the process we could notice that not the user alone, but the whole family have passed by a recent period of social contract loss, especially regarding work and habitation. Even though his family presents resources, it is a family that passed by a process of exclusion from the social organization system and the powerless feeling seems to be related to this process rather to the violence between them. It is tempting to charge family or the service by the lived process, but it seems a more complex process and the role of each element is not clear. We could notice that university and post-graduation research compose with what have been produced in the CAPS. Hence, more than electing the guilty ones, this lead us to suppose that is necessary that us, as part of the problem, start to understand what is our role in the process, then we perhaps might make any difference in the cycle that leads to violence, impotence and people's internment

Objective To explore the influence of network intervention on lifestyle and behaviors habit of the middle -aged patients with hypertension.Methods All 300 middle -aged patients with hypertension were randomly assigned into the study group and the control group.Each group had 150 cases.All patients in the both two groups were given hypotensive drugs according to treatment specification of hypertension.The patients in the study group received a network intervention for 2 years,while the patients in the control group received an outpatient service follow-up.The difference of lifestyle and behaviors habit of the patients in the two groups were observed and compared. Results There were no significant differences with all items of the lifestyle and behaviors habit of the patients between the two groups before intervention(P >0.05 ).In the 12th end of month after intervention,there were no significant differences between the two groups with the items of smoking control,working and resting regularly,hobby and psychological balance(P >0.05),but other items in the study group were significantly better than those in the control group(Z =-3.062,-2.509,-2.239,-2.056,-2.126,-2.045,-2.023,all P <0.05).In the 24th end of month after intervention,all items in the study group were significantly better than those in the control group (Z =-4.078,-3.792,-2.080,-2.098,-1.985,-2.478,-2.173,-2.478,-3.290,-2.514,-2.730,all P <0.05 ).Conclusion The network intervention can significantly improve lifestyle and behaviors habit of the middle -aged patients with hypertension.Its effect is much better than that of outpatient service follow -up.

O projeto Articulando redes, fortalecendo comunidades é financiado pela FAPEMIG e pela Pró-Reitoria de Extensão da PUC Minas, com apoio do CNPq, e objetiva desenvolver ações por meio de metodologias participativas que contribuam para o fortalecimento e articulação de redes comunitárias e equipamentos políticos em três comunidades de Belo Horizonte: Lajedo, São Gabriel e Vila Cemig. Pequenas iniciativas de organização comunitária já existiam nas comunidades e, a partir de diferentes demandas direcionadas à Universidade, inicia-se uma parceria que, após alguns ajustes e processos, se organiza neste projeto. São desenvolvidas atividades locais em cada comunidade, utilizando-se de intervenções psicossociais, além de estratégias audiovisuais e multimídias. Ações conjuntas também são desenvolvidas entre as três comunidades que, dentre outros ganhos, permitem a construção de saber coletivo entre comunidades e universidade, discussão de casos similares existentes nas várias comunidades, busca conjunta de soluções e encaminhamento de questões e melhor aproveitamento de instrumentos de capacitação coletiva.


In the project Articulando redes, fortalecendo comunidades, subsidized by FAPEMIG and the PUC Minas Extension Deanship, with the CNPq support, using participative methodologies, the aim is to develop actions that contribute to the strengthening and the organization of the community networks and governmental institutions in three communities in Belo Horizonte: Lajedo, São Gabriel and Vila Cemig. Some time ago, inthese communities, there were small initiatives of community organization, and, because of a request from the communities to the university, a partnership started and, after a few processes and adjustments, it was structured into this project. Local activities have been developed within each community, with audio-visual and multimedia strategies, in addition to psychosocial interventions. Joint actions between the three communities are also developed and, besides other benefits, they allow the building of collective knowledge between the communities and the university, the debating about similar cases existing in many of the communities, the combined search for solutions, the routing of many questions and a better use of the instruments of collective capacitation.


El proyecto Articulando redes, fortaleciendo comunidades, financiado por la FAPEMIG y por el Pro-Rectorado de Extensión de PUC Minas, con apoyo de CNPq, busca desarrollar acciones que contribuyan al fortalecimiento y articulación de redes comunitarias, y equipamientos políticos en tres comunidades de Belo Horizonte: Lajedo, São Gabriel e Vila Cemig, por medio de metodologías participativas. En estas comunidades ya existían pequeñas iniciativas de organización vinculadas a liderazgos comunitarios; a partir de diferentes demandas direccionadas a la Universidad por parte de estos, se inicia una cooperación que, luego de algunos ajustes y procesos, se organiza en este proyecto. Son desarrolladas actividades locales en cada comunidad, utilizando intervenciones psicosociales, además de estrategias audiovisuales y multimedia. Acciones conjuntas también son desarrolladas entre las tres comunidades que, entre otros logros, permiten la construcción de un saber colectivo entre las comunidades y la Universidad, la discusión de casos similares existentes en varias de las comunidades, la búsqueda conjunta de soluciones y encaminamiento de problemas, así como un mejor aprovechamiento de los instrumentos de capacitación colectiva.
