RESUMEN
Background: Thyroid nodules, common clinical occurrences, often require diagnostic assessment. Ultrasonography (USG) is primary non-invasive method for detection, with advancing technology enhancing detection capabilities. Fine needle aspiration cytology (FNAC) remains standard but poses risks and expenses. American college of radiology-thyroid image reporting and data system (ACR-TIRADS), introduced in 2017, offers a systematic scoring system based on ultrasound features. This study aims to evaluate USG guided by ACR-TIRADS for accurate nodule classification. Methods: A 1.5-year study at department of radio-diagnosis of BSMCH evaluated thyroid nodules using high-resolution USG based on ACR-TIRADS criteria, followed by FNAC for selected cases, comparing results for concordance. Results: Out of 47 thyroid nodules evaluated, 19.1% were malignant and 80% benign. ACR-TIRADS demonstrated sensitivity, specificity, and accuracy of 66.7%, 87.8%, and 82.9%, respectively. Higher ACR-TIRADS categories correlated with an increased risk of malignancy. Suspicious USG features such as hypo-echogenicity, taller-than-wide shape, lobulated margin, and punctate echogenic foci exhibited significant predictive value for malignancy, with varying levels of sensitivity and specificity. Overall, USG parameters demonstrated notable accuracy in identifying malignant nodules. Conclusions: ACR-TIRADS 2017 reliably predicts thyroid nodule malignancy, reducing unnecessary FNAC procedures, minimizing patient discomfort, and optimizing healthcare resources.
RESUMEN
Metastatic neuroblastoma often presents with prolonged fever, cytopenias and significant constitutional symptoms, before the abdominal mass is evident clinically. Clues like subcutaneous nodules and racoon eyes can help a clinician to suspect and evaluate appropriately to arrive at a timely diagnosis. We report a 3-year-old girl who had prolonged fever and constitutional symptoms for more than 2 weeks. On evaluation in outpatient visits, child was found to have cytopenia, hence was referred to our center for further management. Clinical examination revealed raccoon eyes with scalp nodules, hepatomegaly and abdominal mass predominantly over left lumbar area crossing over midline. On further investigations including radiological imaging and bone marrow biopsy child was diagnosed with metastatic neuroblastoma.
RESUMEN
As one of the pathogenic mechanisms contained in The Inner Canon of Yellow Emperor (《黄帝内经》), “disease with latent pathogen induced by a new pathogen” means that the induced new pathogen resulted to a combination of the latent previous pathogen and the new pathogen, which caused the disease. Based on this, it is believed that the change of “nodule-cancer transformation” of pulmonary nodules is actually based on the deficiency of original qi, and the new pathogen induces the latent pathogens like phlegm coagulation, qi stagnation, blood stasis, toxicity, so healthy qi can not drive the pathogens out, and the long-time detention generated into cancerous turbidity, and deve-loped into cancerous tumour at the end. Therefore, based on the three-stage treatment of unformed cancer, dense cancerous toxin, and developed cancer, the clinical practice applied six methods of clearing, expelling, dissipating, tonifying, harmonizing, and transforming, taking into account both the manifestation and root cause, moving the treatment window of pulmonary nodules forward, attacking the pathogens when the toxin was not yet overbearing, supporting the healthy qi before declining, delaying the process of nodules-cancer transformation, and providing ideas for the prevention and treatment of pulmonary nodules “nodule-cancer transformation” in traditional Chinese medicine.
RESUMEN
Based on the theory of qi, blood and fluids, and taking into account of the pathogenesis evolution process from constraint to phlegm, stasis and then mass in pulmonary nodules, an attempt has been made to construct a three-dimensional differentiation system for pulmonary nodules from the dimensions of time and space. The temporal progression of the early, middle, and late stages of pulmonary nodules reflects the pathological changes from constraint to phlegm and then stasis in the metabolism disorders of qi, blood and fluid. The spatial structures such as size, density, and morphology of pulmonary nodules reflect the pathological states of the duration, severity, and primary and secondary conditions of qi, blood and fluid metabolism disorders. Based on the temporal progression, the therapeutic principles have been proposed, which are dispelling pathogenic factors and promoting the use of beneficial factors to interrupt the growth momentum in the early stage, removing turbidity and dispersing phlegm to reduce the degree of nodules in the middle stage, and dispersing nodulation and eliminating abnormalities in the late stage. Based on the spatial structures, the suggested therapeutic methods are using wind herbs, employing multiple approaches to treat phlegm, and promoting blood circulation to resolve stasis, so as to provide theoretical reference for the systematic diagnosis and treatment of pulmonary nodules in traditional Chinese medicine.

In 1 263 frozen section diagnosis results, the correct diagnosis rate was 95.65%, and the misdiagnosis rate was 4.35%. There were 55 misdiagnoses, including 18 (3.44%) invasive adenocarcinoma, 17 (5.82%) adenocarcinoma in situ, 7 (35.00%) mucinous adenocarcinoma, 4 (2.09%) minimally invasive adenocarcinoma, 3 (100.00%) IgG4 related diseases, 2 (66.67%) mucinous adenocarcinoma in situ, 1 (16.67%) atypical adenomatous hyperplasia, 1 (14.29%) sclerosing pulmonary cell tumor, 1 (33.33%) bronchiolar adenoma, and 1 (100.00%) papillary adenoma. Conclusion Intraoperative frozen section diagnosis still has its limitations. Clinicians need to make a comprehensive judgment based on imaging examination and clinical experience.
RESUMEN
@#Objective To explore the correlation between the imaging features of peripheral ground-glass pulmonary nodules and the invasion degree of lung adenocarcinoma, and the high risk factors for infiltrating lung adenocarcinoma under thin-slice CT, which provides some reference for clinicians to plan the surgical methods of pulmonary nodules before operation and to better communicate with patients, and assists in building a clinical predictive model for invasive adenocarcinoma. Methods Clinical data of the patients with peripheral ground-glass pulmonary nodules (diameter≤3 cm) in thin-slice chest CT in the First Affiliated Hospital of Soochow University from January 2019 to January 2020 were continuously collected. All patients underwent thin-slice CT scan and thoracoscopic surgery in our center. According to the pathological examination results, they were divided into two groups: an adenocarcinoma lesions before infiltration group, and an invasive lung adenocarcinoma group. The thin-slice CT imaging parameters of pulmonary nodules were collected. The nodular diameter, mean CT value, consolidation tumor ratio (CTR), nodular shape, vacuolar sign, bronchial air sign, lobulation sign, burr sign, lesion boundary, pleural depression sign, vascular cluster sign and other clinical data were collected. Univariate and multivariate analyses were conducted to analyze the independent risk factors for the infiltrating lung adenocarcinoma, and to analyze the threshold value and efficacy of each factor for the identification of infiltrating lung adenocarcinoma. Results Finally 190 patients were enrolled. There were 110 patients in the adenocarcinoma lesions before infiltration group, including 21 males and 89 females with a mean age of 53.57±10.90 years, and 80 patients in the invasive lung adenocarcinoma group, including 31 males and 49 females with a mean age of 56.45±11.30 years. There was a statistical difference in the mean CT value, nodular diameter, CTR, gender, smoking, nodular type, nodular shape, vacuolar sign, lobulation sign, burr sign, lesion boundary, pleural depression sign, vascular cluster sign between the two groups (P<0.05). However, there was no statistical difference between the two groups in age (P=0.081), lesion site (P=0.675), and bronchial air sign (P=0.051). Multiple logistic regression analysis showed that nodular diameter, mean CT value, CTR and lobulation sign were independent risk factors for differentiating preinvasive adenocarcinoma from invasive adenocarcinoma. At the same time, the threshold value was calculated by Youden index, indicating that the CTR was 0.45, the nodal diameter was 10.5 mm and the mean CT value was –452 Hu. Conclusion In the peripheral ground-glass pulmonary nodules, according to the patient's CT imaging features, such as mixed ground-glass nodules, irregular shapes, vacuoles, short burrs, clear boundaries, pleural indentations, and vascular clusters, have a certain reference value in the discrimination of the invasion degree of ground-glass pulmonary nodules. At the same time, it is found in this research that peripheral ground-glass pulmonary nodules with diameter greater than 10.5 mm, CT value greater than –452 Hu, CTR greater than 0.45 and lobulation sign are more likely to be infiltrating lung adenocarcinoma.

Objective To investigate the importance of a nomogram model based on biomarkers and CT signs in the prediction of the invasive risk of ground glass nodules. Methods A total of 322 patients with ground glass nodule, including 240 and 82 patients in the model and verification groups, respectively, were retrospectively analyzed. Independent risk factors for the invasive risk of ground glass nodules were screened out after using single and multiple Logistic analysis. R software was used to construct the nomogram model, and clinical decision curve analysis (DCA), receiver operating curve (ROC), and calibration curve were used for internal and external verification of the model. Results In this study, the independent risk factors for the invasive risk of ground glass nodules included systemic immune-inflammation index (SII), CYFRA21-1, edge, vascular cluster sign, and nodular consolidation tumor ratio (CTR). The area under the ROC curve of the constructed nomogram model had a value of 0.946, and that of the external validation group reached 0.932, which suggests the good capability of the model in predicting the invasive risk of ground glass nodules. The model was internally verified through drawing of calibration curves of Bootstrap 1000 automatic sampling. The results showed that the consistency index between the model and actual curves reached 0.955, with a small absolute error and good fit. The DCA curve revealed a good clinical practicability. In addition, nodule margin, vascular cluster sign, and CTR were correlated with the grade of pathological subtype of invasive adenocarcinoma. Conclusion A nomogram model based on biomarkers and CT signs has good value and clinical practicability in the prediction of the invasive risk of ground glass nodules.

Objective:To investigate the diagnostic value of multi-slice spiral CT(MSCT)perfusion imaging parameters in the differential diagnosis of benign and malignant solitary pulmonary nodules(SPN).Methods:A total of 80 patients with SPN admitted to our hospital from Oct 2021 to Oct 2022 were selected. All patients underwent MSCT perfusion imaging and pathological examination after admission.According to the histopathological examination results, the patients were divided into benign nodule group and malignant nodule group.MSCT perfusion imaging parameters(blood volume, mean transit time, blood flow, surface permeability coefficient)of the two groups were compared.Receiver operating characteristic(ROC)curve was used to analyze the value of MSCT perfusion imaging parameters in the differential diagnosis of benign and malignant SPN.Results:Among the 80 patients with SPN,47 were diagnosed as malignant nodules and 33 as benign nodules by pathological examination. There was no significant difference in mean transit time between 2 groups(P>0.05).The blood volume, blood flow and surface permeability coefficient in malignant nodule group were higher than those of benign nodule group(P<0.05).The results of ROC curve showed that the area under the curve(AUC)of blood volume, blood flow and surface permeability coefficient separately and in combination were 0.823(95% CI:0.721-0.926),0.855(95% CI:0.761-0.949),0.850(95% CI:0.752-0.948)and 0.963(95% CI:0.924-1.000)for the diagnosis of benign and malignant SPN,all of which had certain diagnostic value. When blood volume, blood flow and surface permeability coefficient were 4.405 ml/100 g,51.325 ml/(min·100 g)and 21.115 ml/(min·100 g),respectively, the best diagnostic efficiency could be obtained, and the combined diagnosis value was higher. Conclusion:The combination of blood volume, blood flow and surface permeability coefficient of MSCT perfusion imaging parameters have high value in the differential diagnosis of benign and malignant SPN,which can provide effective basis for the early diagnosis and treatment of benign and malignant SPN.
RESUMEN
Lung cancer is one of the malignant tumors with the highest mortality and the fastest growing incidence, which seriously threatens human life and health. With the popularization of low-dose spiral CT and the enhancement of public awareness of physical examination, more and more ground-glass nodules have been detected.Accumulating studies have shown that for patients with nodules diameter≤2 cm and ground-glass opacity≥50% ,under the condition of ensuring the cutting edge, thoracoscopic sublobectomy or subsegmentectomy can more effectively preserve the lung function of patients, and has gradually become the recommended surgical method. In recent years, with the continuous improvement of thoracoscopic surgery technology, thoracoscopic subsegmentectomy and combined subsegmentectomy have been gradually carried out.Compared with lobectomy and segmentectomy, subsegmental resection can retain more normal lung tissue and reduce the loss of lung function under the condition of ensuring the safe cutting edge. However, thoracoscopic subsegmental resection requires a higher level of surgical technique and anatomical knowledge for the operator, and is rarely reported in relevant literature. Therefore, this article reviews the progress of anatomical subsegmentectomy and combined subsegmentectomy in the treatment of early non-small cell lung cancer.
RESUMEN
With the popularity of low-dose computed tomography(LDCT)in physical examination and the widespread concern of people about lung diseases after COVID-19 outbreak, the detection rate of pulmonary nodules in the population has increased year by year. The clinical cases of early lung cancer and multiple primary lung cancer are increasing, and the necessity of personalized treatment tailored to the diverse detection of pulmonary nodules is highlighted in the diagnosis and treatment process of pulmonary nodules. However, chest CT and traditional bron-choscopy not only have insufficient accuracy in preoperative localization of pulmonary nodules and the diagnosis of benign and malignant pulmonary nodules, but also have significant limitations in the diagnosis and treatment of patients who cannot undergo surgery. The emergence of electromagnetic navigation bronchoscopy(ENB)has greatly solved this problem and further improves the diagnosis and treatment process of pulmonary nodules.ENB is a new technology that relies on electromagnetic positioning technology to locate, biopsy, and minimally invasive treatment of pulmonary nodules through bronchoscopy. In this review,we mainly summarize the application and latest progress of ENB in the diagnosis and treatment of pulmonary nodules.

RESUMEN
Pulmonary nodules are classified as pure ground glass nodules, part-solid nodules and solid nodules. Surgery is the main treatment for pulmonary nodules. Localization of pulmonary nodules is helpful for accurate resection. The proportion of solid component, size, pathological subtype and lymph node evaluation of pulmonary nodules are the main basis for the selection of surgical methods and the extent of lymph node dissection, but there is no unified standard at present. The management of multiple pulmonary nodules is relatively complex and often requires multidisciplinary discussion. The application of ablation technology makes the treatment of pulmonary nodules more minimally invasive. The treatment concept of pulmonary nodules is rapid recovery and overall minimal invasion.

Objective:To compare the imaging quality and metabolic quantitative parameters of pulmonary nodules between Q. Flex whole information five-dimensional (5D) and conventional three-dimensional (3D) PET/CT imaging for clinical evaluation.Methods:Fifty-four patients (30 males, 24 females, age: 60(42, 75) years; 78 solid pulmonary nodules (maximum diameter≤3 cm) with abnormal uptake of 18F-FDG) from Tianjin Cancer Hospital Airport Hospital between June 2022 and August 2022 were enrolled in this retrospective study. All patients underwent 5D scanning and 3D, 5D reconstruction. Image quality scores, signal-to-noise ratio (SNR), SUV max, SUV mean and metabolic tumor volume (MTV) of pulmonary nodules of 5D group and 3D group were evaluated and compared with χ2 test and Wilcoxon signed rank test. Correlation of quantitative parameters between 2 groups were analyzed by using Spearman rank correlation analysis. Results:Thirty-five of 78(45%) pulmonary nodules with image quality score≥4 were found in 5D group, which were more than those in 3D group (22/78(28%); χ2=4.67, P=0.031). Meanwhile, SNR, SUV max, SUV mean, and MTV were significantly positively correlated between the 2 groups ( rs values: 0.86, 0.86, 0.85, and 0.95, all P<0.001). SNR, SUV max and SUV mean of pulmonary nodules in 5D group were significantly higher than those in 3D group, which were 37.46(18.42, 62.00) vs 32.72(16.97, 54.76) ( z=-4.07, P<0.001), 9.71(5.48, 13.82) vs 8.96(4.82, 12.63) ( z=-3.05, P<0.001) and 6.30(3.39, 8.94) vs 5.61(2.99, 7.63)( z=-4.07, P<0.001) respectively. MTV of pulmonary nodules in 5D group was significantly lower than that in 3D group, which was 1.72(0.66, 2.74) cm 3vs 1.98(1.06, 4.63) cm 3 ( z=-7.13, P<0.001). Quantitative parameters of lower lung field and nodules with maximum diameters of >10 mm and ≤20 mm based on 5D scanning changed most significantly compared with those based on 3D scanning ( z values: from -5.23 to -2.48, all P<0.05). Conclusion:Q. Flex 5D PET significantly improves the quantitative accuracy of SUV and MTV of pulmonary nodules, and the improvement of image quality is substantial without increasing the radiation dose, which has clinical practical value.

MethodsThis study was conducted among 113 hepatitis B cirrhosis patients with DN, 105 hepatitis B cirrhosis patients with regenerative nodules (RN), and 70 hepatitis B cirrhosis patients with small hepatocellular carcinoma (sHCC) who were hospitalized in Guangdong Provincial Hospital of Traditional Chinese Medicine from May 2015 to March 2023. Related data were collected, including age, sex, liver function Child-Pugh class, TCM constitution type, and laboratory markers. A one-way analysis of variance was used for comparison of normally distributed continuous data between multiple groups, and the least significant difference t-test was used for further comparison between two groups; the Kruskal-Wallis H test was used for comparison of non-normally distributed continuous data between multiple groups; the chi-square test was used for comparison of categorical data between groups, and the Bonferroni correction method was used for further comparison between two groups. ResultsThe main TCM constitution types of hepatitis B cirrhosis patients with DN were Qi-deficiency constitution (27 patients, 23.89%), blood-stasis constitution (26 patients, 23.01%), and phlegm-dampness constitution (23 patients, 20.35%). There were significant differences between the three groups in the proportion of patients with phlegm-dampness constitution or damp-heat constitution (χ2=6.822 and 6.383, both P<0.05); the hepatitis B cirrhosis patients with RN had the highest proportion of patients with phlegm-dampness constitution (30.48%), followed by those with DN (20.35%) and those with sHCC (14.29%), while the hepatitis B cirrhosis patients with sHCC had the highest proportion of patients with damp-heat constitution (27.14%), followed by those with DN (16.81%) and those with RN (12.38%). There were significant differences between the hepatitis B cirrhosis DN patients with different TCM constitution types in sex, age, Child-Pugh class, prealbumin, albumin (Alb), aspartate aminotransferase, total bilirubin (TBil), total bile acid, and alpha-fetoprotein (all P<0.05). Compared with the male hepatitis B cirrhosis DN patients, female patients showed a significantly higher proportion of patients with Qi-deficiency constitution (χ2=4.895, P=0.027). Among the patients with Qi-deficiency constitution, the patients with Child-Pugh class A liver function accounted for a significantly lower proportion than those with Child-Pugh class B liver function (χ2=6.380, P=0.012), while among the patients with phlegm-dampness constitution, the patients with Child-Pugh class A liver function accounted for a significantly higher proportion than those with Child-Pugh class B liver function (χ2=8.515, P=0.004). The patients with phlegm-dampness constitution had significantly higher levels of prealbumin and Alb than those with the other four constitutions (all P<0.05), as well as significantly lower levels of TBil and total bile acid than those with damp-heat constitution (P<0.05); the patients with Yin-deficiency constitution had a significantly lower level of Alb than those with qi-deficiency constitution, blood-stasis constitution, or phlegm-dampness constitution (all P<0.05); the patients with Yin-deficiency constitution had a significantly lower proportion of patients with abnormal alpha-fetoprotein than those with non-Yin-deficiency constitutions (χ2=4.448, P=0.035). ConclusionHepatitis B cirrhosis patients with DN mainly have the TCM constitution types of Qi deficiency, blood stasis, and phlegm dampness. The patients with phlegm-dampness constitution seem to have a low probability of carcinogenesis, while those with damp-heat constitution and Yin-deficiency constitution have a relatively high risk of carcinogenesis.
