RESUMEN

Objective To evaluate the application effect of nursing management in the follow-up of patients with recurrent acute pancreatitis (RAP).Methods During January 2016 and January 2017,the clinical data of RAP patients admitted in Changhai Hospital of Navy Medical University was collected.Selfcontrol method was used to establish a prospective cohort study.Meticulous nursing management mode was applied to follow up the patients for 12 months.Patients' compliance,symptom improvement,patients' satisfaction and other markers are evaluated using questionnaires to assess the effect of meticulous nursing management once every 3 months.The data over 12 months were collected.Results Sixty-four RAP patients were finally included.The average follow-up period was 19.7 ±3.4 months.Before admission,the average disease course was(4.21 ± 3.95) years.30 patients (46.9%)had a history of drinking;33 patients (51.6%)had a history of smoking.25 patients (39.1%) had abnormal blood lipid and 40 patients (62.5%) had the dietary habit of high lipid and high protein.The etiology included alcohol (n =10,15.6%),high lipid (n =7,10.9%),biliary diseases (n =1,1.6%),idiopathic causes (n =46,71.9%) and abnormal BMI (n =49,76.6%).After the meticulous nursing management,the frequency of pancreatitis,VAS score of pain and alcohol intake situation was obviously lower than before intervention [(0.50 ±0.85) times/year vs(2.77 ±2.52)times/year;(1.84 ±2.54)vs(6.47 ±2.15);2 cases vs 30 cases].The number of patients who took medicine on time and BMI were obviously decreased[(52 cases vs 13 cases;(23.26 ± 3.85) kg/m2 vs (21.92 ± 4.27)kg/m2)],and the differences were statistically significant (P <0.01).Patients' satisfaction with this nursing model was 4.90 ± 0.56.Conclusions The implementation of the new meticulous nursing management model can effectively alleviate patient's symptom condition and improve the compliance behavior and life quality of the patients.

Objective To investigate the influence of Joint Commission International (JCI) accreditation on nursing practice.Methods Totally 30 medical institutions which had passed the JCI accreditation were investigated to compare the scores of nursing practice before and after JCI accreditation.Results Before JCI accreditation,the average standardized score of governance,leadership and management dimension was 81.20,but the scores of other dimensions were below 80.After the JCI accreditation,the quality improvement and patient safety(QPS) dimension scored 81.10,and scores of other dimensions were all above 90.The differences of all dimensions were statistically significant (P<0.001) before and after the JCI accreditation,the scores were all much greater than those before the accreditation,and improvement of AOP(77.37%)and IPSG(69.86%)was most obvious with statistical significance (P<0.001).Conclusion JCI accreditation plays a certain role in promoting nursing practice,making nursing process more systematic and comprehensive and the safety of nursing guaranteed.

Objective To evaluate the outcomes after implementing gestational diabetes mellitus(GDM) specialist practice model. Methods GDM specialist practice model contained four modules(first visit,follow-up visit,hospital-ization,postpartum),eight aspects of practice contents which included specialist evaluation,diet & exercise survey, diet & exercise prescription,GDM health guidance,weight management during pregnancy. Pregnancy outcomes were compared before and after GDM specialist practice. Results GDM specialist practice model was able to signifi-cantly decrease insulin utilization rate,reduce number of hospitalized patients due to poor blood sugar control,short-en length of hospitalization. Conclusion GDM specialist practice model can effectively save medical costs,and GDM specialist nurses play an important role in management of women with GDM.

Objective To investigate the intervention value of intensive care team in neonatal respiratory distress syndrome.Methods 110 cases of neonatal respiratory distress syndrome were selected,by using a random number table method they were randomly divided into the observation group and the control group,55 cases in each group.The control group was treated with routine care model,the observation group dedicated care team for critically ill children.The incidence of complications,mechanical ventilation time,cost of hospitalization,duration of hospitalization were compared after the care of children.Results The incidence rates of infection,abdominal bloating and intraventricular hemorrhage in the observation group (3.64%,1.82%,0.00%) were significantly lower than those in the control group(20.00%,16.36%,5.45%),the incidence of infection,abdominal distension between the two groups had statistically significant differences (χ2=4.852,P=0.027;χ2=7.040,P=0.008).The mechanical ventilation time,hospitalization time in the observation group [(11.23±2.17)d,(23.45±5.45)d]were significantly shorter than those in the control group[(16.78±4.52)d,(26.78±6.47)d],there were statistically significant differences between the two groups(t=8.209,P=0.000;t=2.919,P=0.004).The hospitalization costs of the observation group[(20 462.78±214.45) yuan] was significantly lower than the control group [(24 975.45±312.45)yuan],there was significant difference between the two groups(t=88.311,P=0.000).The total effective rate of the observation group was 98.18%,which was higher than 87.27% of the control group,there was statistically significant difference between the two groups(χ2=4.852,P=0.027).Conclusion Full implementation of critical care nursing team intervention on neonatal respiratory distress syndrome,can effectively reduce the incidence of children with complications,mortality,shorter hospital stays,reduce hospitalization costs,the effect is significant and should be introduced.

Objective To explore the moves that creating magnetic work environment in hospital, in order to improve the nurses job satisfaction and reduce the turnover rate then stable the nursing team. Methods Through a series of measures such as the establishment of scientific rational allocation of human resources, implementing fair performance evaluation and allocation mechanism, carrying out the nurse grading management system to create magnetic nursing work environment, the nurse job satisfaction and turnover rate were compared before and after the measures. Results In January 2014 a total of 520 questionnaireswere issued, 519 valid questionnaires, and the effective recovery rate was 99.81%(519/520);And 559 were valid of 577 issued questionnaires, which was 96.88% (559/577). The overall score of nurses′job satisfaction increased from (3.20±0.46) points in 2013 to (3.41±0.52) points in 2014 (t=9.40, P= 0.000); The nurse turnover rate fell to 1.97%(13/661) from 5.13% (30/585) (χ2= 10.27, P= 0.003). Conclusions That is very important for remain stable and high quality nursing team to carry out scientific and fine management according to the demand of nurses, so as to create a safe, harmonious, positive, cooperative magnetic nursing work environment which have development direction and space.

RESUMEN

The humanistic solicitude in nursing means that the medical staffs take sincerely the patients' life, health, basic rights, needs, personalities and dignities into account with humanitarian spirit, which plays an important part in nursing practice. To implant humanistic solicitude in teaching and to innovate its model in multiple ways by using related theories of pedagogy, learning advanced experiences from hospitals domestic and abroad can cultivate students′ sustained and stable concept of humanistic solicitude and motivate their empathy and caring emotions, thereby expanding their knowledge and promoting the abilities of utilizing humanistic solicitude in nursing practice.

Objective To analyze satisfaction of the clinical practice content of nursing postgraduates with different characteristic, and to provide transformative suggestions for the construction of clinical practice scheme of nursing postgraduates. Methods Using judgment sampling and snowball sampling, the questionnaire survey was conducted among 101 nursing postgraduates, and comparing satisfaction of clinical practice content of postgraduates with different age, work experience and teaching mode. SPSS 17.0 software package was used for data description and t test. Results The total satisfaction of clinical practice content of nursing postgraduates was low, and different postgraduates′satisfaction was different. On practical operation opportunity,> 30 years old of postgraduates′satisfaction was (3.95±0.87), which was higher than those 20~30 years old, (3.35±0.90), and the difference was statistically significant, t=2.748, P<0.01. On clinical practice assessment methods and frequency, the satisfaction of postgraduates with a fixed teacher was higher than those without a fixed teacher (3.40±0.74) vs. (2.83±0.87), and the difference was statistically significant, t=3.310, P<0.01. Conclusions At present, the clinical practice content of nursing postgraduates is in shortage, and need to be further improved.

Objetivou-se propor uma estratégia de atenção para usuário de crack, por meio da aplicação do Processo de Enfermagem. Trata-se de estudo de caso, descritivo e exploratório com abordagem qualitativa, fundamentado em Neuman e em NANDANIC-NOC. O cenário foi o Centro de Atenção Psicossocial - Álcool e Drogas de Vitória-ES. Aplicou-se o questionário do padrão de consumo de crack, Escala de Gravidade de Dependência, Mini Internacional Neuropsychiatric Interview e Parental Bonding Instrument. Constatou-se a complexidade das situações relativas aos estressores intra, inter e extrapessoais. Foram discutidos três diagnósticos: 'conhecimento deficiente sobre o controle do uso de substâncias', 'enfrentamento ineficaz' e 'isolamento social', e sugeridas intervenções por meio de articulação entre a NIC, a NOC e a prevenção secundária de Neuman. Assim, é função primordial do enfermeiro: mobilizar os recursos do sistema - internos e externos - para estabilização e conservação da energia, de forma a fortalecer as linhas de resistência, além de considerar todas as variáveis do sistema e não só o uso do crack.


The objective was to propose a care strategy for crack users, through implementation of the nursing process. This is a case study, descriptive and exploratory with qualitative approach based on Neuman and the NANDA-NIC-NOC. Scenario: Psychosocial Care Center - Alcohol and Drugs from Vitória-ES. We applied the standard questionnaire of crack consumption, Addiction Severity Index, Mini International Neuropsychiatric Interview and Parental Bonding Instrument. It was found the complexity that lining the intra, inter and extra personal stressful situations. We discussed three diagnoses: 'poor knowledge about the substance use control ', 'ineffective coping' and 'social isolation', and interventions were suggested by articulation between NIC, NOC and secondary prevention of Neuman. It is therefore an important function of the nurse to mobilize system resources - internal and external - for stabilization and conservation of energy, in order to strengthen the lines of resistance, in addition to considering all the variables of the system and not only the use of crack.


El objetivo fue proponer una estrategia de atención para los usuarios de crack, por medio del aplicación del proceso de enfermería. Este es un estudio de caso, descriptivo y exploratorio con enfoque cualitativo sobre la base de Neuman y NANDANIC-NOC. Escenario: Centro de Atención Psicosocial - Alcohol y Drogas Vitória-ES. Se aplicó el cuestionario Padrón del consumo de crack, Escala de severidad de la dependencia, Mini Internacional Neuropsychiatric Interview y Parental Bonding Instrument. Se encontró la complejidad que revisten situaciones relativas a los estresores intra, inter y extra personal. Hubo la discusión de tres diagnósticos: "conocimiento escaso sobre control del uso de sustancias", "enfrentamiento ineficaz" y "aislamiento social", y hechas sugerencias por medio de la articulación entre NIC, NOC y prevención secundaria de Neuman. Por lo tanto, la principal función de la enfermera es movilizar los recursos del sistema - internos y externos - de manera que se fortalezcan las líneas de resistencia, además de considerar todas las variables del sistema y no sólo el uso del crack.

RESUMEN

PURPOSE: The purpose of the study was to determine the impact of regular nursing rounds on patient satisfaction with nursing care. METHODS: This was a controlled clinical trial in which 100 hospitalized patients in a medical surgical ward were allocated to control and experimental groups through convenience sampling. The experimental group received regular nursing rounds every 1e2 hours. Routine care was performed for the control group. Patient satisfaction with the quality of nursing care was assessed on the second and fifth days of hospitalization in both groups using Patient Satisfaction with Nursing Care Quality Questionnaire. RESULTS: On the second day, patient satisfaction scores of the two groups had no significant difference (p = .499). However, the intervention was associated with statistically significant increased patient satisfaction in the experimental group compared to the control group (p < .001). CONCLUSION: Implementing regular nursing rounds had a positive impact on patient satisfaction. This method may hence improve patient-nurse interactions and promote the quality of nursing care and patient satisfaction.
