As drogas utilizadas para prevenção de convulsões podem impactar na movimentação dentária durante o tratamento ortodôntico. O objetivo deste estudo foi avaliar a influência de drogas anticonvulsivantes no tratamento ortodôntico. O desenho deste estudo é uma revisão narrativa da literatura onde uma busca eletrônica foi realizada sem restrição de idioma e data em três bases de dados (PubMed via MEDLINE, SCOPUS e BVS). Foram utilizados os seguintes termos para o PubMed e BVS: anticonvulsants drugs AND orthodontic movement e para o SCOPUS: anticonvulsants AND drugs AND orthodontic AND movement. Uma pesquisa complementar foi realizada nas referências bibliográficas dos estudos incluídos. Os artigos indicaram que o uso de medicação anticonvulsivante favorece o desenvolvimento de hiperplasias gengivais, xerostomia e alterações no metabolismo ósseo que modulam a taxa de movimentação ortodôntica. Dentro das limitações desta revisão, concluiu-se que não há contraindicações para tratamento ortodôntico em pacientes utilizando anticonvulsivantes, no entanto os ortodontistas devem estar cientes dos potenciais efeitos adversos advindos do uso desses medicamentos para que possam adotar medidas para mitigar esses riscos.
Drugs used to prevent seizures can have an impact on tooth movement during orthodontic treatment. This study aimed to evaluate the influence of anticonvulsant medications on orthodontic treatment. The design of this study is a narrative literature review in which an electronic search was carried out without language or date restriction in three databases (PubMed/MEDLINE, SCOPUS, and BVS). The following terms were used for PubMed and BVS: anticonvulsants drugs AND orthodontic movement and SCOPUS: anticonvulsants AND drugs AND orthodontic AND movement. A complementary search was carried out on the bibliographical references of the included studies. The articles indicated that using anticonvulsant medication favors the development of gingival hyperplasia, xerostomia, and alterations in bone metabolism that modulate the rate of orthodontic movement. Within the limitations of this review, it was concluded that there is no contraindication for orthodontic treatment in patients using anticonvulsants. However, orthodontists should be aware of the potential adverse effects arising from the use of these drugs so that they can adopt measures to minimize these risks.

Abstract Cephalometry is a morphological and descriptive diagnostic method that provides relevant data on skeletal and dentoalveolar malocclusions of patients seeking orthodontics treatment. Several authors have proposed different cephalometric measurements to determine facial growth direction and facial biotype, the results of these different measurements from the same patient do not always agree on the diagnosis. The aim of this study was to determine the level of agreement between Ricketts and Björk-Jarabak cephalometric analyses for the determination of facial growth direction and facial biotype in patients from a population of Yucatan, Mexico. A total of 260 lateral cephalograms of patients between 18 and 59 years of age were digitally traced using the Ricketts and Björk-Jarabak cephalometric analyses to determine the direction of facial growth and facial biotype using Dolphin Imagine software. Cohen's kappa statistical test was performed to establish the strength of agreement between the diagnostic results obtained by the cephalometric analyses. A poor diagnostic concordance strength was found for growth direction (K=0.105), and acceptable for facial biotype (K=0.362). The concordance strengths for each diagnostic possibility (level) ranged from slight to acceptable, except for the brachyfacial and dolichofacial biotypes, with a moderate strength of agreement. In conclusion, the Ricketts and Björk- Jarabak cephalometric measurements used for the determination of facial biotype and facial growth direction could suggest non-concordant diagnostic assessments in some individuals.
Resumen La cefalometría es un método diagnóstico morfológico y descriptivo que proporciona datos relevantes sobre maloclusiones esqueléticas y dentoalveolares de pacientes que buscan tratamiento de ortodoncia. Varios autores han propuesto diferentes mediciones cefalométricas para determinar la dirección del crecimiento facial y el biotipo facial, los resultados de estas diferentes mediciones de un mismo paciente no siempre coinciden en el diagnóstico. El objetivo de este estudio fue determinar el nivel de concordancia entre los análisis cefalométricos de Ricketts y Björk-Jarabak para la determinación de la dirección del crecimiento facial y el biotipo facial en pacientes de una población de Yucatán, México. Se trazaron digitalmente un total de 260 cefalogramas laterales de pacientes entre 18 y 59 años de edad utilizando los análisis cefalométricos de Ricketts y Björk-Jarabak para determinar la dirección del crecimiento facial y el biotipo facial utilizando el software Dolphin Imagine. Se realizó la prueba estadística kappa de Cohen para establecer la fuerza de concordancia entre los resultados diagnósticos obtenidos por los análisis cefalométricos. Se encontró una fuerza de concordancia diagnóstica deficiente para la dirección del crecimiento (K=0.105) y aceptable para el biotipo facial (K=0.362). Los niveles de concordancia para cada posibilidad diagnóstica (nivel) variaron de leve a aceptable, excepto para los biotipos braquifacial y dolicofacial, con un nivel de concordancia moderado. En conclusión, las mediciones cefalométricas de Ricketts y Björk-Jarabak utilizadas para la determinación del biotipo facial y la dirección del crecimiento facial podrían sugerir evaluaciones diagnósticas no concordantes en algunos individuos.

Orthodontic intrusion is a common treatment approach in managing orthodontic esthetic and functional problems, including gummy smile and deep bite. Intrusion often constitutes an integral part of orthodontic treatment to improve sagittal and vertical incisor relationships, to correct interincisal angle and, consequently, the gingival line and restore the esthetics of smiling. The choice of appliances and techniques used by practitioners varies radically among individuals, but the fundamental forces and moments they produce are universal. Appliances will always act according to the laws of physics. Therefore, understanding the basic biomechanical principles of intrusion may help in effecting efficient tooth control and help in achieving successful orthodontic treatment outcomes more predictable and consistent.

The aim of this study was to evaluate the effectiveness of motivational interviewing on improving oral hygiene behavior in patients with fixed orthodontics appliances. A parallel-group randomized clinical trial was conducted with 45 patients of the Orthodontic Clinic of Unibe's Postgraduate Unit. A computer-generated list of numbers created with SPSS V21.0 was used to randomly allocate participants into the experimental or the control group. Monthly oral hygiene instructions and an oral hygiene kit from GUM® with special orthodontic hygiene tools were given to the participants. Furthermore, the experimental group underwent motivational interviewing sessions facilitated by a periodontist who received training from two expert psychologists. These psychologists also provided supervision to the interviewer, ensuring the accurate implementation of the intervention. Simplified Oral Hygiene Index (SOHI), Gingival Index (GI), Periodontal Probing Depth (PPD) and Bleeding on Probing (BoP) were recorded at baseline, three and six months after the beginning of the study. The participant and the evaluator (another periodontist who recorded the data) were masked. Repeated-measures mixed- model analysis of variance and chi-squared test were conducted. Mean SOHI, GI, PPD and BoP scores did not differ significantly across the three time points (baseline, three and six months). Nevertheless, a significant interaction on SOHI scores (F (2, 58) = 3.463, p = .038, h2 = .052) was found between the sessions and the treatment conditions (experimental vs control group) at the third and sixth month. Motivational interviewing plus oral hygiene instructions appears to maintain a better control of dental biofilm and calculus in comparison with conventional oral hygiene instructions alone.
El objetivo de este trabajo fue evaluar la efectividad de las entrevistas motivacionales para mejorar el comportamiento de higiene oral en pacientes con aparatos ortodóncicos fijos. Se llevó a cabo un ensayo clínico aleatorizado de grupos paralelos con 45 pacientes de la Clínica de Ortodoncia de la Unidad de Posgrado de Unibe. Se utilizó una lista de números generada por computadora con SPSS V21.0 para asignar aleatoriamente a los participantes al grupo experimental o de control. Se les proporcionó a los participantes instrucciones mensuales de higiene oral y un kit de higiene oral de GUM® con utensilios de higiene especiales para ortodoncia. Además, el grupo experimental recibió sesiones de entrevista motivacional facilitadas por un periodoncista capacitado por dos psicólogas expertas. Estas psicólogas también supervisaron al entrevistador, asegurando la implementación precisa de la intervención. Se registró el Índice Simplificado de Higiene Oral (IHO-S), el Índice Gingival (IG), la Profundidad de Sondaje Periodontal (PS) y el Sangrado al Sondaje (SS) al inicio, a los tres y seis meses después del inicio del estudio. Tanto el participante como el evaluador (otro periodoncista que registró los datos) estaban enmascarados. Se realizaron análisis de varianza de medidas repetidas con modelo mixto y pruebas de chi-cuadrado. Las puntuaciones medias del IHO-S, IG, PS y SS no difirieron significativamente en los tres momentos (inicio, tres y seis meses). Sin embargo, se encontró una interacción significativa en las puntuaciones del IHO-S (F (2, 58) = 3.463, p = .038, h2 = .052) entre las sesiones y las condiciones de tratamiento (grupo experimental vs grupo de control) en el tercer y sexto mes. Las entrevistas motivacionales junto con las instrucciones de higiene oral parecen mantener un mejor control de la biopelícula dental y el cálculo en comparación con las instrucciones convencionales de higiene oral por sí solas.

Dental crowding is a frequent characteristic of dental malocclusion and is the reduction in the perimeter of the arch that can manifest as space closure, rotation and/or movement of the teeth. The temporary dentition is important as it guides the eruption of the permanent teeth, thus establishing the ideal occlusion; the lack of spaces can cause disproportionate sizes between the jaws and the permanent teeth. Treatment options include the orthopedic phase and orthodontic phase in which the extraction of teeth may or may not be necessary. Timely treatment with personalized planning helps us achieve correct dental occlusion, which is essential for good chewing and correct passage of the bolus, in addition to playing an important role for phonation and the aesthetic results for the patient.

Objective: To evaluate the progression of caries around orthodontic brackets after the enamel has been exposed to lacteal products containing probiotics. Methods: Orthodontic brackets were bonded to the enamel surfaces. The test specimens were randomly divided into six groups: G1-negative control; G2-positive control, exposed to culture environment only (without microorganisms); G3-exposed to the cariogenic environment and the fermented cow's milk without probiotic; G4-exposed to the cariogenic environment and fermented cow's milk with probiotic; G5-exposed to the cariogenic environment and yogurt without probiotic; and G6-exposed to the cariogenic environment and yogurt with probiotic. The groups were placed in brain heart infusion medium, supplemented with 2% sucrose and with 1x106 cells/ml of Streptococcus mutans and Streptococcus salivarius (ATCC). The Shapiro-Wilk, Levene, Student t, Kruskal-Wallis, and Mann-Whitney tests were used. Results: all groups exposed to the ATCC strains showed lower final microhardness, compared to the negative control (p0.05). Conclusions: Lacteal products are not able to prevent the progression of caries around orthodontic brackets.

Este Estudio tuvo por objetivo determinar la incidencia de reabsorción radicular externa en premolares superiores e inferiores permanentes al finalizar el tratamiento de ortodoncia. Esta investigación fue realizada en pacientes entre 18 y 35 años que recibieron tratamiento de ortodoncia, sin extracciones, con técnica de autoligado en la ciudad de Guadalajara. Se consideraron 120 premolares, realizando mediciones longitudinales en las tomografías computarizadas de haz cónico al inicio y término del tratamiento. Utilizando el software Implant Viewer 3. Los valores registrados en el primer y segundo premolar superior tanto del lado derecho como izquierdo al inicio y término del tratamiento fluctuaron entre -4.946 y -7.801; p ≤ 0.0005 encontrándose diferencias estadísticas significativas. Mientras que en el primer y segundo premolar inferior tanto del lado derecho como izquierdo, fluctuaron entre -4.864 y -5.28; p ≤ 0.0005 encontrándose diferencias estadísticas significativas. Se concluye en este estudio que los dientes sometidos a movimientos durante el tratamiento de ortodoncia sufren modificaciones en el contorno de la raíz, cambios con consecuencias mínimas que no comprometen la funcionalidad del diente. Los premolares superiores presentaron mayores rangos de reabsorción radicular que los premolares inferiores.
The aim of this study is to determine the incidence of root resorption in permanent upper and lower premolars at the end of an orthodontic treatment. This research was carried out in patients aged between 18 and 35 years old who received orthodontic treatment, without extractions, with a self-ligating technique in the city of Guadalajara, Mexico. In this study, 120 premolars were considered, making longitudinal measurements with cone beam computed tomography at the beginning and the end of the treatment, using the Implant Viewer 3 software. The values recorded in the upper first and second premolars on both the right and left sides at the beginning and the end of the treatment fluctuated between -4.946 and -7.801; p ≤ 0.0005, finding statistically significant differences. In the lower first and second premolars on both the right and left sides, they fluctuated between -4.864 and -5.28; p ≤ 0.0005, also finding statistically significant differences. It is concluded that teeth subjected to movements during orthodontic treatment suffer modifications in the contour of the root, changes with minimal consequences that do not compromise their functionality. The upper premolars showed higher ranges of root resorption than the lower premolars.

Background: Confidence in the appearance of teeth affects a person's physical and psychological health. The appearance of teeth is influenced by factors such as age, gender, educational level and economic status. The aim of this study was to find out the relationship between dental satisfaction and sociodemographic characteristics. Methods: A cross-sectional study has been conducted on 384 people aged 17 to 45 years in Samarinda City, Indonesia. The research instrument is a questionnaire on age, gender, educational level, family income, and six satisfaction questions about tooth appearance. Data analyzed descriptively and tested chi-square. Results: A total of 222 respondents (57.8%) felt dissatisfied with the appearance of their teeth. Respondents with a younger age tend to be 1.262 times more dissatisfied compared to older ones. There was a significant relationship between age and satisfaction with the appearance of the teeth (p=0.017). No significant relationship was found between teeth appearance satisfaction with gender, educational level and economic status. Respondents who did not want to do orthodontic care tended to be 1.920 times more satisfied than those who wanted to do ortodontic treatment. The most desirable treatments respondents wanted to improve dental aesthetics were bleaching (57.6%) and orthodontist treatment (43.2%). Conclusions: There is a significant correlation between teeth appearance satisfaction and age factor in the Samarinda Indonesian population who are 17 to 45 years old; there is a meaningful relationship between desire for orthodontic care and teeth satisfaction.

El manejo del anclaje en los tratamientos de ortodoncia ha sido un reto constante, por lo que la utilización de miniplacas aparece como un rescate a esta interrogante. Determinar las diferencias morfológicas en la zona de inserción de miniplacas ortodóncicas en las regiones anterior y posterior mandibular, comparandolas entre edad, sexo, biotipo y clase esqueletal. Se estudiaron 40 registros de cone-beam de pacientes y se realizaron mediciones de grosor del hueso, donde se identificaron los sitios que permitan posicionar una miniplaca y fueron comparados entre sexo, grupos etarios, biotipo y clase esqueletal. Se observó diferencias significativas entre hombres y mujeres a nivel anterior mandibular, en los hombres se observaron mayores grosores óseos y corticales. No se observaron diferencias significativas entre pacientes adultos y jóvenes a nivel anterior mandibular, pero sí hubo diferencias a nivel mandibular posterior. La zona 3 mm debajo del 2do molar no es una zona confiable para la inserción de mini placas, ya que muchos de los pacientes no presentaban hueso en esa zona. Las miniplacas son un recurso seguro de anclaje las cuales deben ser adaptadas a cada paciente.
The anchorage management in orthodontic treatments has been a constant challenge, the use of miniplates may be a viable solution to this query. The objective of this study was to determine the morphological differences in the area of insertion of orthodontic miniplates in the anterior and posterior mandibular regions, comparing them between age, sex, biotype and skeletal class. In this analysis 40 cone-beam records of patients were studied, bone thickness measurements were carried out, the sites that allowed the positioning of a miniplate were identified and compared between sex, age groups, biotype and skeletal class. Significant differences were observed between men and women at the anterior mandibular level, in men greater bone and cortical thicknesses were observed. No significant differences were observed between adult and young patients at the anterior mandibular level, but there were differences at the posterior mandibular level. The area 3 mm below the 2nd molar was not a reliable area for the insertion of mini plates, since many of the patients had no bone in that area. Miniplates are a safe anchoring resource which must be adapted to each patient.

El presente reporte tiene como objetivo presentar de manera secuencial y visual la protracción maxilar de un paciente clase III esqueletal, utilizando el protocolo BAMP. Una vez realizado el análisis clínico del caso se decidió realiza un tratamiento ortopédico a través del uso de miniplacas de titanio con elásticos intermaxilares de clase III siguiendo el protocolo BAMP, junto a un tratamiento ortodóncico dento alveolar con aparatología fija bimaxilar prescripción Roth 0.22" Mini sprint forestadent. Se observa el éxito del enfoque interceptivo utilizando el protocolo BAMP. Este enfoque logró reducir tanto el tiempo como la complejidad del tratamiento ortodóncico, y también disminuyó la necesidad de someterse a una cirugía ortognática en la edad adulta. El tratamiento BAMP, diseñado para ciertas edades y crecimiento, se demostró exitoso en un paciente de 12 años sin potencial de crecimiento. Las miniplacas como anclaje son efectivas para protracción maxilar en varios casos, aunque se necesita seguimiento y educación postquirúrgica. Se recomienda retirar las miniplacas cuando no sean necesarias, adaptando el protocolo a cada paciente. En resumen, el tratamiento con miniplacas es eficaz para corregir anomalías Clase III esqueléticas.
The present report aims to sequentially and visually present the maxillary protraction of a Class III skeletal patient using the BAMP protocol. After conducting a clinical analysis of the case, it was decided to perform orthopedic treatment using titanium miniplates with Class III intermaxillary elastics following the BAMP protocol, in conjunction with dentoalveolar orthodontic treatment using fixed bimaxillary appliances with Roth prescription 0.022" Mini Sprint Forestadent. The success of the interceptive approach using the BAMP protocol is observed. This approach managed to reduce both the time and complexity of orthodontic treatment and also decreased the need for orthognathic surgery in adulthood. The BAMP treatment, designed for specific ages and growth stages, proved successful in a 12-year-old patient without growth potential. Miniplates as anchorage are effective for maxillary protraction in various cases, although post-surgical follow-up and education are required. It is recommended to remove the miniplates when they are no longer necessary, adapting the protocol to each patient. In summary, miniplate treatment is effective in correcting Class III skeletal anomalies.

The introduction of glass ionomer cements in orthodontics aimed to address the drawbacks of the acid-etch technique using composite resins, including demineralization, enamel damage, and potential allergic reactions. These cements release fluoride over time, potentially protecting against demineralization. Glass ionomer cements, despite their benefits in reducing adhesive residue and protecting against demineralization, showed higher bond failure rates compared to composite resins. This is attributed to their sensitivity to application techniques and moisture, along with a delayed setting time. In contrast, light-cured composite resins, preferred over chemically-cured resins in recent years, offer advantages such as ease of use, consistent handling, and controlled setting. However, early trials indicated higher bond failure rates for light-cured resins, a finding not consistently replicated in later studies. While glass ionomer cements offer certain advantages, their higher bond failure rate poses a significant limitation. Light-cured composite resins, with their user-friendly characteristics, have become the preferred choice in orthodontic bonding despite initial concerns about higher bond failure rates.

Open bite malocclusion is a condition where the upper and lower incisors do not overlap vertically. It involves factors like structure, dental issues and functional challenges. This review offers an overview of diagnosing, understanding the causes and managing bite malocclusions. The condition is noticeable by the gap between incisors, difficulties in biting and speech and habits like tongue thrusting. In children thumb sucking is often associated with this issue underscoring the importance of intervention. Diagnosis relies on assessments, cephalometric analysis and categorization based on dental or skeletal elements. The causes can be attributed to factors, environmental influences or behavioral habits. Various management strategies are available with treatments playing a role using methods like fixed appliances clear aligners, and innovative temporary anchorage devices (TADs). Surgical interventions such as surgery address discrepancies when necessary. Considering that open bites are age conditions, behavioral modifications along with intervention using orthopedic appliances are crucial for effective treatment. Advanced diagnostic tools like cone beam computed tomography (CBCT) contribute to treatment planning. The retention phase ensures stability of results while patient cooperation remains essential throughout the process. Collaboration between orthodontist’s maxillofacial surgeons and other specialists is vital for care. The field of orthodontics is continuously advancing and with research we can expect improvements in our understanding and treatment of open bite malocclusions.

@#The functional health and stability of the oral and maxillofacial system is one of the basic goals of orthodontic treatment. Currently, it is believed that, in general, the condyle is located in the center of the joint fossa when the mandible is in an intercuspal position (ICP) in healthy normal people. At this time, the function of the temporomandibular joint (TMJ) is stable. Due to orthodontic tooth movement and subsequent occlusal changes, patients with malocclusion may experience related remodeling of the temporomandibular joint, especially changes in the position of the condyle. The position of the mandibular condyle is traditionally evaluated using a condylar position indicator. However, this method lacks consistency in obtaining condylar position changes. In recent years, in the clinical application of orthodontic treatment, cone beam computed tomography (CBCT) has become the first choice for examination. CBCT can accurately measure the interarticular space and determine changes in condylar position. This article reviews the CBCT assessment of condylar position and related research on condylar position changes in patients with malocclusion before and after orthodontic treatment. The literature review results indicate that there are differences in the condylar position of patients with different malocclusions, and the condylar position may also change before and after orthodontic treatment. With a lower radiation dose, CBCT has higher accuracy in evaluating the condylar position in patients with malocclusion who undergo orthodontic treatment, thus promoting further study of the mechanism of condylar position changes in patients with malocclusion in the future and providing more accurate and personalized guidance for patient treatment.

Objective To explore the effect of cannabinoid receptor 2(CB2)on orthodontic tooth movement(OTM)rate and periodontal tissue reconstruction of pressure area in mice.Methods Thirty CB2-/-male mice and thirty littermate control WT male mice were individually accepted the orthodontic appliance at their age of 6 weeks.The mice were respectively scarified at 3 days,7 days,14 days and 21 days after the operation.Then the tooth movement distance was examined through the stereomicroscope.Hematoxylin-eosin staining was performed to explore the biological responses of periodontium at the distal mesial root pressure area.Anti-tartrate acid phospha-tase staining was performed to calculate the number and distribution of osteoclasts at the distal mesial root pressure area,and MMP-9 was evaluated by immunohistochemistry to examine the number of MMP-9(+)monocytes and multinucleated cells in the same district as the TRAP staining.Results Compared with those WT mice at 3,7,14 and 21 days,OTM distance showed a gradual increased tendency according with experimental time over 21 days.The widths of periodontal ligament on the pressure side were markedly greater in CB2-/-mice than WT mice at 7,14 and 21 days(P<0.000 1).The numbers of TRAP positive osteoclasts were significantly greater in CB2-/-mice than those in WT mice at 14 days of OTM(P<0.001).MMP-9 immunohistochemical staining showed that the number of MMP-9(+)monocytes and multinucleated cells was more in CB2-/-mice than that in WT mice at 14 days of OTM(P<0.05).Conclusion The absence of CB2 accelerates orthodontic tooth movement under or-thodontic force.The absence of CB2 reinforces bone resorption in orthodontic tooth movement compressive area dur-ing orthodontic tooth movement.

Objective To investigate the effect of dynamic monitoring of occlusal force on the final therapeutic effect and the change of periodontal supporting tissue during combined periodontal orthodontic treatment.Methods The periodontal clinical index of 20 patients with traditional periodontal orthodontic treatment and 20 patients with combined periodontal orthodontic treatment assisted by T-Scan Ⅲ and Anycheck digital occlusion analysis system were compared before,during and after treatment,as well as the changes of bite force,bite time and tooth mobility in the experimental group.Results The depth of periodontal pocket(PD),loss of attachment(AL),bleeding index(BI)and tooth looseness were significantly reduced after combined periodontal orthodontic treatment in both groups.In the control group,the percentage of anterior and posterior biting force changed obviously,and the occlusion force balance was improved.Conclusion The combined treatment of periodontitis and orthodontics can improve the periodontal tissue of patients with periodontitis,and T-Scan system can observe and guide the adjustment of occlusal and better achieve occlusion force balance.

Objective:To investigate the effects of 3 adhesives on the bond force and durability of polished and glazed zirconia ceram-ics to orthodontic metal brackets respectively.Methods:Universal adhesives,Single Bond Universal(SBU)and Prime&Bond Universal(PBU)were respectively used to bond polished and glazed zirconia to metal braces of maxillary central incisors using TransbondTM MIP(TM)as the control.The shear bond strength(SBS),the fracture morphology and adhesive residual index(ARI)were examed after wa-ter bath or water bath-thermal cycling storage.Results:The adhesive(P<0.001)and storage conditions(P<0.001)significantly af-fected the shear bond strength of zirconia to brackets.There was no significant difference between the polished or glazed groups(P=0.09).SBU showed the stronger SBS and lower ABI,there were significant differences in ARI scores among the 3 cements(P<0.001).Conclusion:SBU may have better bonding performance than PBU and TM in the orthodontic bonding of polished or glazed zir-conia surfaces to the zirconia ceramics.

BACKGROUND:There is an increasing demand for orthodontic treatment,and periodontally accelerated osteogenic orthodontics(PAOO)technique can make it possible to move orthodontic teeth that are limited by thin alveolar bone. OBJECTIVE:To investigate the biomechanics of orthodontic tooth movement before and after periodontally accelerated osteogenic orthodontics(PAOO)surgery to increase alveolar bone volume using the three-dimensional finite element method. METHODS:A patient undergoing PAOO surgery before orthodontic treatment to increase bone volume on the labial side of the mandibular anterior region was selected.The patient was under invisible orthodontics.Two three-dimensional finite element models were constructed based on the patient's preoperative and 6-month postoperative cone beam CT data.Both models simulated the movement of tooth 33:experiment Ⅰ:distal-central movement of 0.25 mm;experiment Ⅱ:lingual movement of 0.25 mm;and experiment Ⅲ:intrusion movement of 0.10 mm.The stress distribution and initial displacement trend of tooth 33,periodontal ligament and surrounding alveolar bone under the action of the invisible aligner were analyzed before and after the PAOO procedure. RESULTS AND CONCLUSION:Dental stress analysis:In the same orthodontic tooth movement,the maximum Von-Mises stress and overall stress values of tooth 33 were all higher before surgery than after surgery;there were similar distribution areas of maximum equivalent stress and overall distribution trends of Von-Mises stress before and after surgery.Periodontal ligament stress analysis:In the same orthodontic tooth movement,the maximum Von-Mises stress and overall stress values of the periodontal ligament were higher before surgery than after surgery,and there were similar distribution areas of the maximum equivalent stress and overall distribution trends of Von-Mises stress before and after surgery.Alveolar bone stress analysis:In the same orthodontic tooth movement,the maximum Von-Mises stress values of the alveolar bone around tooth 33 were higher before surgery than after surgery,while the equivalent stress distribution showed a gradual decrease from the top of the alveolar ridge to the root.Initial displacement analysis:In the same orthodontic tooth movement,the initial displacements in the main displacement direction for all six observation points of tooth 33 were smaller before surgery than after surgery,and showed a tendency to gradually decrease from the tooth tip to the apex.Therefore,there were differences in the biomechanical characteristics of orthodontic tooth movement before and after the PAOO surgery.With the clear aligner,the postoperative equivalent stress values on the dentition,periodontal ligament,and surrounding alveolar bone were lower than before the surgery,and the initial displacements of the orthodontic teeth after the surgery are larger than before.These findings suggest that PAOO can release the restriction of thin alveolar bone on the movement of orthodontic tooth by increasing alveolar bone thickness,effectively improving the force on the roots,periodontal ligament,and alveolar bone,avoiding the stress concentration on orthodontic tooth in the thin alveolar bone area that can cause complications when moving,and improving the efficiency of tooth movement.

BACKGROUND:Most of the studies on combined orthodontic-orthognathic treatment of skeletal class Ⅲ malocclusions have focused on the improvement of the patient's lateral appearance and recovery in the later stages of the treatment,while there are fewer studies observing the microcosmic nature of the alveolar bone remodeling of the lower anterior teeth. OBJECTIVE:To evaluate the therapeutic effect of lower anterior tooth decompensation and alveolar bone remodeling in patients with skeletal class Ⅲ malocclusion before and after orthodontic-orthognathic treatment based on oral X-ray lateral films and oral cone-beam CT. METHODS:From January 2015 to May 2023,15 patients with skeletal class Ⅲ malocclusion who underwent orthodontic-orthognathic surgery at Qingdao Hospital of Rehabilitation University were enrolled.All patients underwent lateral cephalography and cone beam computed tomography before and after treatment.Cephalometric measurement items related to the angle and line distance,lip/lingual bone cracking length(d-La/d-Li)and bone cracking/bone fenestration of the lower anterior teeth before and after treatment were measured. RESULTS AND CONCLUSION:Lateral X-ray films showed that the amount of alveolar bone remodeling after decompensation of the lower anterior teeth showed significant changes compared to before treatment.The root of the tooth moved significantly towards the center of the alveolar bone,and the specific data was closer to normal data,but there were still some differences compared with normal individuals.Based on the cone-beam CT measurement,the bone cracking/bone fenestration length and width of the alveolar bone were improved in almost all the teeth after orthodontic-orthognathic combined treatment,alveolar bone remodeling in some teeth even reached the level of healthy individuals.Before treatment,most patients often experienced bone fenestration/cracking on the lip/lingual side of the lower incisor due to compensatory tooth growth.However,during the preoperative orthodontic stage,decompensation triggered alveolar bone remodeling and significant changes in tooth angle.Preoperative orthodontic treatment caused the upper anterior teeth to retract and the lower anterior teeth to tilt and control the root,but the amount of decompensation before surgery was often insufficient.In the orthognathic surgery stage,the jaw was removed through the positioning guide plate,the maxilla moved forward,and the mandible retreated.During the postoperative orthodontic process,the effect of fine adjustment was better.Although there is a certain degree of recurrence trend in the position of teeth and jawbones,the postoperative orthodontic treatment is closer to the normal value.

BACKGROUND:Temporomandibular joint disorders are closely related to high stress in temporomandibular joint.With the change of molar position after tooth reduction extraction,the establishment of new occlusal relationship often leads to the change of internal stress environment of the temporomandibular joint. OBJECTIVE:To analyze the stress distribution of temporomandibular joint in patients undergoing orthodontic reduction tooth extraction with different degrees of molar forward movement using the three-dimensional finite element model of the maxillary complex and temporomandibular joint. METHODS:A case of individual normal occlusal patient was selected from the Orthodontics Department of Qingdao Municipal Hospital,Shandong Province,and the finite element models of 1/3 anterior molar space(extraction of four second premolar teeth)before and after reduction and 2/3 anterior molar space(extraction of 4 second premolar teeth)after reduction were established based on the cone-beam CT and MRI data.ABAQUS software was used to analyze the stress distribution of various parts of the temporomandibular joint during the interposition of tooth tips. RESULTS AND CONCLUSION:The stress distribution of the condyle,articular disc,and osteoarticular fossa in the model before and after the reduction was basically the same.The stress of the condyle was mainly distributed in the anterior and apical part of the condyle,the stress of the articular disc was mainly distributed in the middle band and lateral part of the articular disc,and the stress of the articular fossa was mainly concentrated in the anterior and apical part of the articular fossa.However,the equivalent stress value of the condyle,articular disc and articular fossa decreased after reduction.After orthodontic reduction extraction,the equivalent stress values of condyle and articular disc in the 1/3 anterior molar space model were smaller than those in the 2/3 anterior molar space model.From the perspective of biomechanics,orthodontic reduction extraction can reduce the stress of the temporomandibular joint and provide a good biomechanical environment.

BACKGROUND:The application of orthodontic force triggers autophagy in the periodontal tissue via diverse signaling pathways,augmenting or attenuating the activity of relevant cell types such as periodontal ligament cells,osteocytes,osteoclasts,and osteoblasts,thus facilitating the process of periodontal remodeling. OBJECTIVE:To review the research progress in orthodontic force mediated autophagy in periodontal tissue and its impact on orthodontic tooth movement. METHODS:The PubMed,Web of Science,China Biology Medicine disc and CNKI were searched for literature published from 2010 to 2023 to summarize the progress in orthodontics-related autophagy.And 76 papers were finally included in the analysis and discussion. RESULTS AND CONCLUSION:Orthodontic force can trigger a series of biochemical signal changes through periodontal mechanical receptors and aseptic inflammation they cause,leading to autophagy in periodontal tissue.Subsequently,autophagy generates corresponding feedback through cascaded amplified signaling pathways such as Phosphoinositide 3-kinase/protein kinase B,Hippo,and mitogen-activated protein kinase pathways,promoting periodontal tissue remodeling and ultimately achieving tooth movement and stability.Orthodontic force-induced autophagy can differentially regulate bone resorption on the tooth pressure side and bone formation on the tension side.Related targets have good prospects in the clinical application of orthodontic treatment.Orthodontics and autophagy have complex mechanisms.However,existing research has only focused on exploring the role of autophagy in orthodontic tooth movement.Further exploration is needed to investigate the mutual regulatory effects between autophagy and orthodontic tooth movement,as well as the interactions between upstream mechanical receptors and signaling pathways involved in related pathways.
