RESUMEN

El Consejo Internacional de Enfermeras (CIE) define a la Enfermería como los cuidados, autónomos y en colaboración, que se prestan a las personas de todas las edades, familias, grupos y comunidades, enfermos o sanos, en todos los contextos. Incluye la promoción de la salud, la prevención de la enfermedad y los cuidados de los enfermos, discapacitados y personas moribundas. A su vez Berbiglia y Banfield citan a Orem, ésta plantea que la práctica de la Enfermería en relación con los individuos se institucionaliza alrededor del proceso de una persona, la/el enfermera/o, que proporciona ayuda directa a otra persona, cuando ésta persona es total o parcialmente incapaz de ayudarse a sí misma en el cumplimiento de los cuidados diarios relacionados con la salud, a causa de la situación de salud existente. La misma autora define al estado de salud que indica la necesidad de asistencia enfermera como "la incapacidad de las personas de proporcionarse por sí mismas y continuamente el autocuidado (AC) de calidad y cantidad necesarios según las situaciones de la salud personal". Expresa también que los factores básicos condicionantes son "los factores internos o externos a los individuos que afectan sus habilidades para involucrarse en el autocuidado o afectan el tipo y cantidad de autocuidado requeridos". El objetivo, Sistematizar y acordar las intervenciones y cuidados de enfermería en la admisión del paciente con diabetes, con un enfoque desde los factores básicos condicionantes del autocuidado y la capacidad de la agencia de Autocuidado, que ingresan al servicio de urgencias de adultos, de un Hospital público, 2020. En este sentido el profesional de Enfermería cumple un rol fundamental dentro del equipo de salud para identificar el déficit de autocuidado y promover, gestionar e impulsar diversas estrategias para la promoción de la salud, la prevención de la enfermedad y su tratamiento[AU]

O mesmo autor define o estado de saúde que indica a necessidade de assistência de enfermagem como "a incapacidade das pessoas de proverem a si mesmas e continuamente o autocuidado (CA) de qualidade e quantidade necessárias de acordo com as situações de saúde pessoal". Também expressa que os fatores condicionantes básicos são "fatores internos ou externos aos indivíduos que afetam suas habilidades para se envolver em autocuidado ou afetam o tipo e a quantidade de autocuidado necessária". O objetivo sistematizar e acordar intervenções e cuidados de enfermagem na admissão do paciente com diabetes, com foco nos fatores condicionantes básicos do autocuidado e na capacidade da agência de Autocuidado, que entra no pronto-socorro de adultos, de um hospital público, 2020. Nesse sentido, o profissional de enfermagem desempenha papel fundamental dentro da equipe de saúde para identificar o déficit de autocuidado e promover, gerenciar e promover diversas estratégias de promoção da saúde, prevenção e tratamento de doenças[AU]

The objective is to systematize and agree on interventions and nursing care in the admission of the patient with diabetes, with an approach from the basic conditioning factors of self-care and the capacity of the Self-care agency, who enter the emergency department of adults, of a public hospital, 2020 In this sense, the nursing professional plays a fundamental role within the health team to identify the deficit of self-care and promote, manage and promote various strategies for health promotion, disease prevention and treatment[AU]

El bajo peso al nacer constituye una preocupación mundial, los programas diseñados para prevenirlo dependen de la identificación de gestantes susceptibles. Existe escasa evidencia en la literatura acerca del cuidado por parte del profesional de enfermería dirigido a mujeres con factores de riesgo de tener niños con bajo peso al nacer desde la Atención Primaria de Salud. No obstante, hay teorías y modelos conceptuales que pueden ser aplicados para guiar las intervenciones. La teoría de los sistemas de enfermería es la que asumen los autores de esta investigación por considerarla de gran utilidad en la Salud Pública, debido a que, en el ámbito de la responsabilidad personal por la salud, centrar el autocuidado en la iniciativa propia de quienes lo requieren, entendiéndolo como "la práctica de actividades que los propios individuos, inician y desarrollan en su propio beneficio, en la mantención de su vida, salud y bienestar". El objetivo de este artículo fue determinar la importancia de la aplicación de la teoría de los sistemas de enfermería, para el abordaje de intervenciones de enfermería en la prevención del bajo peso al nacer desde la Atención Primaria de Salud.Las conclusionesdan luces sobrelas mujeres con diagnóstico de riesgo reproductivo preconcepcional y condiciones de riesgo para tener hijos con bajo peso al nacer, en particular, deben incorporar a su vida diaria autocuidados que les permitan prevenir complicaciones. Las teorías ayudan a adquirir conocimientos que contribuyen a perfeccionar prácticas cotidianas mediante la descripción, explicación, predicción y control de los fenómenos; además facilita a los profesionales autonomía de acción como guía en aspectos prácticos, educativos y de investigación.


Low birth weight is a global concern; programs designed to prevent it depend on the identification of susceptible pregnant women. There is little evidence in the literature about care by the Nursing professional directed to women with risk factors of having children with low birth weight from Primary Health Care. However, there are theories and conceptual models that can be applied to guide interventions. The Theory of Nursing Systems is the one assumed by the authors of this research because they consider it very useful in Public Health, because in the field of personal responsibility for health, it focuses self-care on the initiative of those who they require it, understanding it as "the practice of activities that individuals themselves initiate and develop for their own benefit, in maintaining their life, health and well-being." The goal of this study was to determine the importance of the application of the theory of nursing systems, for the approach of nursing interventions in the prevention of low birth weight from Primary Health Care.The conclusions shed light on the women with a diagnosis of preconception reproductive risk and risk conditions for having children with low birth weight, in particular, should incorporate self-care into their daily lives that allow them to prevent complications. Theories help to acquire knowledge that helps to improve daily practices through the description, explanation, prediction and control of phenomena; it also provides professionals with autonomy of action as a guide in practical, educational and research aspects.

Los Adultos Mayores (AM+65) corresponden al grupo etario más afectado ante la acción del SARS-CoV-2, por lo que resulta relevante analizar la percepción de los Adultos Mayores de 65 años o más que asisten a la Confraternidad Lupita Nolvios (CLN) a la adherencia de los mecanismos de autocuidado ante el Covid-19 con énfasis en el acompañamiento del personal de enfermería (PE) de acuerdo al modelo de Dorothea Orem. Estudio cuantitativo de campo de enfoque descriptivo de cohorte transversal abarcando el periodo marzo-2020 a septiembre-2020. Los resultados indican que en el 08 de julio 2020 se alcanzó un pico de 21,88% de casos confirmados Covid-19 en AM+65 y un porcentaje acumulado de fallecimientos de 59,51% para el periodo del estudio. Se evidencia que los AM+65 conocen la importancia de su autocuidado aplicando en un 75,76% de las mismas, haciendo énfasis para Covid-19 en el uso de las mascarillas (64%; n=32/50). Resaltando finalmente, el papel de PE con un 64% que inciden en la formación del AM+65, en su autocuidado de acuerdo a su patología (74%) y un 68% perciben como influye el PE en subir la autoestima del AM+65 ante el Covid-19(AU)


Older Adults (AM+65) correspond to the age group most affected by the action of SARS-CoV-2, so it is relevant to analyze the perception of Older Adults aged 65 or over who attend the Lupita Nolvios Confraternity (CLN ) to adherence to self-care mechanisms in the face of Covid-19 with an emphasis on the support of nursing staff (NP) according to the Dorothea Orem model.Quantitative cross-sectional cohort descriptive field study covering the period March-2020 to September-2020. The results indicate that on July 8, 2020, a peak of 21.88% of confirmed Covid-19 cases in AM+65 was reached and a cumulative death rate of 59.51% for the study period. It is evident that AM+65 know the importance of their self-care by applying 75.76% of them, emphasizing for Covid-19 in the use of masks (64%; n=32/50). Finally, the role of PE with 64% that affect the formation of AM+65, in its self-care according to its pathology (74%) and 68% perceive how the PE influences in raising the self-esteem of AM+65 before the Covid-19(AU)

RESUMEN

Objective:To evaluate the effectiveness of Orem self-care model for elderly patients with hip fracture.Methods:The databases including Medline, Embase, Web of Science, Cochrane Library Database, Wanfang, CNKI, VIP, and CMB were searched from inception to September 2020. All randomized controlled trials (RCTs) of the Orem self-care model applied in elderly patients with hip fracture were included. Two investigators independently screened research, extracted data and assessed the quality of literature based on standard methods. RevMan 5.3 software was performed to conduct the Meta-analysis.Results:A total of 11 RCTs were included, involving 1 088 patients. Compared with conventional nursing model, self-care model could effectively reduce the incidence of complications during hospitalization ( RR=0.26, 95% CI 0.18-0.39, P<0.001) and improve the Harris scores ( MD=13.33, 95% CI 11.09-15.57, P<0.001) of elderly patients with hip fracture. However, the impacts of self-care model on alleviating postoperative pain was still uncertain. Additionally, the self-care model could shorten hospital stay, but the combined effect size was unavailable due to the high heterogeneity between related studies. Conclusions:In comparison with conventional nursing model, the Orem self-care model was more beneficial to shorten the hospitalization stay, reduce the incidence of postoperative complications and promote hip joint function of elderly patients with hip fractures. To confirm the effectiveness of self-care models in pain management for elderly patients with hip fractures, multicenter RCTs with high quality and large samples are needed.

Introducción: A nivel mundial, cada año 3,2 millones de muertes son atribuidas a la diabetes, lo que equivale a una de cada 20 muertes, 8700 muertes cada día, seis cada minuto y por lo menos una de cada diez se producen en adultos de 35 a 64 años de edad. Objetivo: Proponer una estrategia de enfermería en el autocuidado de los adultos mayores con diabetes mellitus. Métodos: Estudio de intervención y desarrollo en el Policlínico Dr. Rudesindo Antonio García del Rijo; del municipio y provincia de Sancti Spíritus, desde el año 2013 al 2017. Variables de estudio: edad, sexo, nivel de información de los adultos mayores, tipología de la diabetes mellitus, autocuidado de los adultos mayores y estado de salud de los adultos mayores con diabetes mellitus. Se utilizó entrevista estructurada a los adultos mayores con diabetes mellitus en la comunidad para conocer la mejoría del estado de salud, después de aplicada la estrategia se compararon las proporciones poblacionales mediante la prueba de McNemar. Resultados: Se lograron cambios significativos en el nivel de información de los adultos mayores, al finalizar se obtuvo 60,41 por ciento aceptable. Después de aplicada la estrategia el autocuidado estaba presente en un 85,45 por ciento y el estado de salud de los adultos mayores fue alto con un 84, 37 por ciento. Conclusiones: Con las acciones y actividades que se incluyen en la estrategia de enfermería para el autocuidado de los adultos mayores con diabetes mellitus se lograron cambios significativos en el autocuidado(AU)


Introduction: Worldwide, 3.2 million deaths are attributed to diabetes each year, a figure equivalent to one in 20 deaths, 8,700 deaths every day, or six every minute; and at least one in ten occur in adults aged 35-64 years old. Objective: To propose a nursing strategy concerning the self-care of older adults with diabetes mellitus. Methods: Intervention development study carried out in Dr. Rudesindo Antonio García del Rijo Polyclinic in Sancti Spíritus Municipality, in the province of the same name, from 2013 to 2017. The variables of the study were age, sex, level of information of older adults, type of diabetes mellitus, self-care of older adults, and state of health of older adults with diabetes mellitus. A structured interview was conducted on older adults with diabetes mellitus living in the community in order to find out the improvement in their health status. After applying the strategy, the population proportions were compared using the McNemar test. Results: Significant changes were achieved in the level of information of older adults; at the end, an acceptable 60.41 percent was obtained. After applying the strategy, self-care was present in 85.45 percent and the health status of older adults was high, accounting for 84.37 percent. Conclusions: With the actions and activities included in the nursing strategy for the self-care of older adults with diabetes mellitus, significant changes in self-care were achieved(AU)

Objective To explore the effect of dressing techniques under Orem self-care theory on the ability of daily living of stroke patients with hemiplegia. Methods About 96 stroke patients with hemiplegia were randomly divided into an experimental group and a control group, 48 cases in each group. The patients in the experimental group received training on dressing techniques under Orem self-care theory, while those in the control group received routine training on dressing techniques.The daily life ability of both groups was comfared. Result After one month, the Barthel scores in the experimental group were higher than those of the control group (P<0.05). Conclusion Dressing techniques under Orem self-care theory can improve the ability of daily living of stroke patients with hemiplegia.

Objective To investigate the performance of Orem’ sef-care theory application in the patients with a tendency of ovarian hyperstimulation syndrome (OHSS). Methods A total of 307 patients with a tendency of progressing OHSS, who were treated in Reproductive Medicine Center of Changhai Hospital of Second Military Medical University from Oct 2015 to Oct 2016, randomly received routine nursing management in combination with Orem’ sel--care theory (Orem’ sel--care theory group, n=165) or routine nursing management (routine group, n=142). The incidence and severity of OHSS of the patients in the two groups were compared and analyzed. Results Sixty-eight of 307 OHSS tendency patients finally developed OHSS, with 38 in the routine group (16 mild OHSS, 18 moderate and four severe), and 30 in the Orem’ sef-care theory group (20 mild, nine moderate and one severe). There was no significant difference in OHSS incidence between the two groups (P>0. 05); while the reduction of OHSS severity in the Orem’ sef-care theory group was more obvious than that in the routine group, and the difference was statistically significant (P<0. 05). Conclusion Orem’ sef-care theory nursing management has an advantage in reducing the severity of OHSS over routine nursing and is worthy of clinical use.

Objective To explore the application of Orem self-care mode on hip function recovery of elderly patients with artificial hip replacement . Methods Fifty elderly patients with hip arthroplasty from 2014 January to December were set as control group and received routine nursing methods from 2015 January to December 50 elderly patients with hip arthroplasty , were set as observation group and received Orem self-care mode . The hip function of the two groups was evaluated by Harris scale. Result The hip joint function of observation group was higher than that of control group and the difference was statistically significant (P<0.05). Conclusion Orem self-care mode applied in nursing of elderly patients with hip arthroplasty can improve the recovery of elderly patients with hip function.

Objective To evaluate the effect of Orem intervention on the living quality and psychological state of elderly pa-tients with pulmonary tuberculosis.Methods Sixty-eight elderly patients with pulmonary tuberculosis were randomly divided into control group (n=31) and observation group (n=37).Orem intervention was applied in the observation group, while routine nurs-ing was used in the control group.The score of short-form-36 health survey (SF-36), self-rating depression scale (SDS), self-rating anxiety scale (SAS) and satisfaction were compared between the two groups.Results After the intervention, the SF-36 scores of observation group were significantly increased in 8 dimensions, compared with control group , with statistically significant differences (p<0.05).Compared with the control group, the SDS and SAS scores of the observation group were significantly de-creased statistically (p<0.05).The satisfaction rates of the control group and the observation group were respectively 77.4% (24/31) and 94.6% (35/37), with statistically significant differences (p<0.05).Conclusion Orem intervention can effectively im-prove the living quality and psychological state of elderly patients with pulmonary tuberculosis , worth clinical promotion.

Objective To investigate the application effect of Orem theory in the clinical care of patients with severe burns and its impact on the quality of life of patients. Methods All 60 patients with severe burns in accordance with the way randomly divided into control group(n=30) with the observation group(n=30), respectively, with the use of conventional care nursing model of Orem. Changes, satisfaction with care, quality of life after treatment were com-pared before and after the psychological status of nursing intervention. Results (1)Two nursing intervention psycho-logical status were significantly better than the pre-nursing intervention and nursing intervention group before and after the degree of psychological conditions improve significantly greater than in the control group, and the SAS and SDS scores were statistically significant differences (P<0.05); (2)the control group patient's care satisfaction 70.00%, was significantly lower than 93.33%of the observation group (P<0.05);(3)based on simple burns health Scale (BSHS-A),ob-served in patients after nursing intervention BSHS-A total score and dimension scores of obser vation group were sig-nificantly lower than the control group (P<0.05). Conclusion Compared with usual care model, the theory of Orem for the clinical care of patients with severe burns,the effect is significant,the quality of life of patients is significantly improved,should be widely used in clinical care.

Objective To explore the effect of Orem self-care pattern on quality of life of patients undergoing radiotherapy after breast cancer operation.Methods One hundred patients undergoing radiotherapy after breast cancer operation were randomly divided into control group and treatment group in equal number.The control group was treated by conventional nursing.Orem self-control pattern was used in the treatment group besides conventional nursing.QOL scale was employed to observe their quality of life after radiotherapy.Result The self-care ability and quality of life of the treated group were improved significantly comparing with the control group(P<0.05,for both).Conclusion Orem self-care pattern can effectively improve the quality of life of patients undergoing radiotherapy after breast cancer operation by way of helping them actively completing function exercises on the diseased limbs and propelling their early rehabilitation.

Objective To evaluate the effect of Orem self‐care mode on sense of well‐being in patients with schizophrenia by Me‐ta analysis .Methods The databases of CBM ,VIP ,CNKI and Wan Fang Data were searched .All randomized controlled trials(RCT ) about Orem self‐care mode for patients about sense of well‐being were included .Data collection and literature review were per‐formed by two reviewers independently .The RevMan 5 .2 software was taken for analysis .Results Three literatures were includ‐ed .All these articles were regarded as low quality ,and only one article′s score of methodological quality are more than three .The Meta analysis showed there were significant differences between the Orem self‐care mode group and the normal nursing mode group in sense of well‐being .Besides the scores of the Orem self‐care mode group are higher(MD = 16 .29 ,95% CI(14 .54 - 18 .05) ,P<0 .05) .Conclusion The Orem self‐care mode is better than normal nursing for sense of well‐being in patients .However ,trials are only three ,and most trials included in the review are of low quality .So large scale randomized controlled trials of higher quality are needed to confirm the result .

La elaboración del presente trabajo tiene la finalidad de aplicar el modelo teórico de Dorothea Orem como fundamento teórico para la aplicación del proceso enfermero a una paciente con diagnóstico clínico de osteogénesis imperfecta (OI), mismo que afecta sus requisitos de autocuidado universal. Para la elaboración de los diagnósticos de Enfermería se utilizó el formato PES (Problema, Etiología, Signos y síntomas), se realizó una jerarquización de los mismos con base en prioridades, y posteriormente, para la elaboración del plan de cuidados, recurrimos a la Enfermería Basada en Evidencia, para lo cual se consultaron diferentes bibliotecas virtuales. La evaluación de las intervenciones de Enfermería fue favorable, colaborando a la resolución de los requisitos alterados y a la adquisición de competencias necesarias en la familia para compensar las deficiencias.


The preparation of this paper aims to apply the theoretical model of Dorothea Orem as theoretical foundation for the application of the nursing process in a patient with clinical diagnosis of osteogenesis imperfect (OI), it affects their universal self-care requirements. For the development of nursing diagnoses used the PES format (Problem, Etiology, Signs and Symptoms), there was a hierarchy of them on a priority basis, and subsequently, to the development of the care plan based nursing resort to in evidence, to which were consulted different virtual libraries. Evaluating nursing interventions was favorable, contributing to the resolution of the changed requirements and the acquisition of skills needed in the family to compensate for deficiencies.

Determinar los factores condicionantes en el desarrollo de episodios depresivos en universitarios, a partir de los elementos básicos considerados en la Teoría General del Déficit de Autocuidado de Dorothea Oremmetodología: Estudio cuantitativo, prospectivo, descriptivo, correlacional-causal y de corte transversal. Muestra de 819 alumnos de los diversos semestres y carreras (Licenciatura en Enfermería, Licenciatura en Administración, Contador Público, Ingeniero Civil y Licenciatura en Mercadotecnia). Se utilizó un instrumento creado exprofeso para medir los diversos factores, con un alfa de Crombach de 0.92. El procesamiento estadístico se realizó a través del programa SPSS versión 17, utilizándose análisis descriptivo y regresión lineal logística

RESUMEN

Se aplicó el Proceso de Atención de Enfermería (PAE) en el hospital “Adolfo López Mateos” acorde al enfoque metodológico enfermero, empleando la teoría del déficit del autocuidado de Dorothea E. Orem, dirigido a un paciente masculino de 24 años con diagnóstico médico de politraumatismo, traumatismo craneoencefálico grado III y hemorragia subaracnoidea, con escala de valoración neurovascular Fisher II. El objetivo de este trabajo es que el profesional de enfermería adquiera opiniones, opciones, conocimientos y habilidades para diagnosticar, brindar tratamientos de enfermería e identificar los factores de riesgo que ayuden al paciente a su pronta recuperación o a mantener su estado de salud-enfermedad no sólo en el aspecto biológico, sino también en el aspecto psicosocial y espiritual, con el fin de prevenir patologías secundarias. Los aspectos teóricos están en proceso de desarrollo y perfeccionamiento. Por dichas razones, la investigación es el puente que une la teoría con la práctica y en donde la búsqueda del conocimiento es la base para formar el juicio y fundamento de las acciones de enfermería.


We applied the Nursing Care Process (SAP) at the Hospital “Lic. Adolfo López Mateos” nurse according to the methodological approach, using the self-care deficit theory of Dorothea E. Orem, aimed at a male patient of 24 years with physician diagnosed multiple trauma, head injury grade III subarachnoid hemorrhage, with rating scale neuro-vascular Fisher II. The aim of this work is that the nurse acquires opinions, options, knowledge and skills to diagnose, provide nursing treatments and identify risk factors that helps the patient to a speedy recovery or maintain their health and disease not only the biological aspect, but also the psychosocial aspect and spiritual in order to prevent secondary conditions. The theoretical aspects are under development and refinement. For these reasons, the research is the bridge between theory and practice where the pursuit of knowledge is the base to form the judgment and nursing actions.

RESUMEN

El proceso de atención en enfermería (PAE) tiene como finalidad brindar atención integral y especializada a cada agente de cuidado, impulsando y conservando la salud del usuario cuando lo requiera. Este PAE se realizó en el Hospital “Lic. Adolfo Ló- pez Mateos” y se utilizó la teoría del déficit de autocuidado de Dorothea Orem. Se trata de Jesús (nombre ficticio), masculino de 66 años con un padecimiento de insuficiencia renal crónica y peritonitis bacteriana; HAS (hipertensión arterial sistémica), DM T2 (diabetes mellitus tipo 2) descontroladas debido a una falta de adhesión al plan terapéutico.


The completion of the Nursing Care Process is designed to provide comprehensive care to each agent specialized care by developing and maintaining the health of the user when required. The Nursing Care Process (SAP) at the Hospital “Lic. Adolfo López Mateos”, was applied using Dorothea Orem’s theory in conjunction with his theory of self-care deficit. This is Jesus, 66 years old male with chronic renal disease, bacterial peritonitis, HAS (hypertension), T2 DM (diabetes mellitus type 2) uncontrolled due to a lack of adherence to therapeutic plan.

RESUMEN

Objective To study the influence of Orem self-care theory on postpartum quality of life and role change of parturients with natural labor. Methods 100 parturients with natural labor in our hospital from May 2010 to July 2011 were selected as research object,and they were randomly divided into the control group and the observation group with 50 cases in each group,the control group was taken care with routine nursing methods,the observation group was given with nursing according to Orem self-care theory.Then the SCL-90 score,situation of role change and satisfaction degree of two groups before nursing and on the second and fifth days after nursing were analyzed and compared. Results The SCL-90 score of the observation group after the nursing on second and fifth days was all lower than those of the control group,the better rate of role change was higher than that of the control group,patients'satisfaction degree was higher than that of the control group. Conclusions The Orem self-care theory plays an important role in improving the quality of life and role change of parturients with natural labor after delivery,and it can significantly improve satisfaction degree of parturients.

RESUMEN

Objective To discuss the application value of Orem's self-care mode in the nursing of patients with stroke. Methods 88 patients with acute stroke in our hospital were selected from January 2008 to January 2011.All patients were divided into the control group and the observation group with 44patients in each group. The control group was treated by conventional medical care, and the observation group was treated by Orem's self-care mode. The ADL of two groups was observed before and after treatment. Results After treatment, the Barthel index of the control group and the observation group were significantly increased. But compared with the control group, the Barthel index of the observation group increased significantly. Conclusions By Orem's self-care mode, patients with stroke can improve their ADL and their quality of life.

Este proyecto aborda analíticamente aspectos conceptuales del Modelo de Autocuidado de Orem, que han sido utilizados en la investigación que adelanta el Grupo Cuidado de la Salud de la Universidad del Tolima. El ejercicio que se presenta no es una fórmula mágica para dar respuesta a la implementación de modelos en enfermería. Esencialmente traza una ruta metodológica y conceptual como propuesta de aplicación del Modelo en la práctica clínica.


This project addresses analytically conceptual aspects of Orem's Self Care Model, which has been used in the investigation being conducted by the Health Care Group, University of Tolima. Charting a path as methodological and conceptual proposal for implementing the model reference in clinical practice. The exercise presented below is not a magic formula to answer this postulate. Essentially addresses theoretical elements of Orem model, which served as support for its implementation in our institution.

Objective To explore the nursing effects of self-care body posture in patients after modified radical mastectomy. Methods The clinical data of 102 patients with modified radical mastectomy were randomly divided into the study group and the control group, all patients met the following standard:clear consciousness, effective communication, no paralysis of limbs, no fracture. In the study group, according to Orem"self-care theory" concept, patients carried out self-care body posture with mainly focus on self-emancipated and the body center of gravity position, in the control group patients were given traditional posture care with mainly emancipated assistance. The first time getting out-of-bed, the occurrence of various complications, the comfort level of emancipation and posture, as well as nursing satisfaction on the first day after surgery were observed and compared between the two groups. Results The two groups showed significant difference in the first time out-of-bed, anxiety, the comfort level of emancipation and posture, as well as nursing satisfaction. There was no significant difference between the two groups in dizziness, limb swelling, flap effusion, flap necrosis. No pressure ulcer occurred. Conclusions Making use of self-care body posture in patients after modified radical mastectomy can improve the satisfaction and comfort of the patients, and improve the patients' recovery, without increasing the complication, which is worth promoting.
