RESUMEN

Introduction: Osteoarthritis (OA) is a progressive, degenera?ve disease that leads to joint pain, tenderness, s?ffness, locking, effusion, reduced mo?on, swelling, crepitus, and disability. The pain in OA is the most significant clinical feature and impacts func?on, mobility, quality of life, and the reason for medical advice. Methods: Fi?y individuals with primary knee OA in the age range of 45–90 years were chosen at random for the research (N=50). The American College of Rheumatology’s diagnos?c criteria were employed to diagnose osteoarthri?s, and a visual analogue scale was u?lized to score the severity of pain. Knee OA was graded using the Kellgren-Lawrence (K-L) radiographic assessment method. The an?oxidant levels of superoxide dismutase in the synovial fluid were measured by using a spectrophotometric assay. Results: Grades 1, 2, 3, and 4 have SOD ac?vity values of 1.43±0.55, 1.44±0.72, 0.92±0.52, and 0.87 ±0.52 U/ml, respec?vely, in synovial fluid. Synovial fluid SOD ac?vity was higher in grades 1 & 2 of KOA as compared to grades 3 &4 and the difference was sta?s?cally significant (p<0.05). Conclusions: There was a link between K-L grade and synovial an?oxidant ac?vity level. In the late stages of knee osteoarthri?s, the an?oxidant enzyme (SOD) ac?vity was reduced.According to the results of this study, regular an?oxidant supplementa?on to early osteoarthri?s pa?ents may delay disease progression by improving the an?oxidant status of the knee, which neutralises free radicals and thus prevents car?lage damage.

RESUMEN

Objective To investigate the effects of 5-Aza-CdR (methylation transferase inhibitor) on gene expression levels of interleukin-1β (IL-1β),transforming growth factor-β (TGF-β) and nitric oxide synthase (NOS) in chondrocytes. Methods Chondrocytes from patients with osteoarthritis (OA) (n=22),rheumatoid arthritis (RA) (n=3) or trauma without rheumatic diseases (n=10) were collected and cultured. The mRNA expression levels of IL-1β,TGF-β and NOS were detected by real-time polymerase chain reaction (RT-PCR).Chondrocytes in trauma group were treated with 5-Aza-CdR(10μmol/L) for 72 h,and the mRNA and protein expression levels of IL-1β,TGF-β and NOS were detected by RT-PCR and ELISA,respectively. Results The mRNA expression levels of IL-1β,TGF-β and NOS were increased in OA and RA group as compared with trauma group (P<0.05),while they had no differences between OA and RA groups. After treated with 5-Aza-CdR,the mRNA and protein expression levels of IL-1β,TGF-β and NOS in chondrocytes rised in trauma group as compared with pretreatment (all P<0.05).Conclusions The mRNA expression levels of IL-1β,TGF-β and NOS in chondroytes are higher in OA and RA patients.5-Aza-CdR could increase the mRNA and protein expression levels of IL-1β,TGF-β and NOS by inducing relative gene methylation, which suggests demethylation might play a role in OA pathogenesis by influncing the inflammatory signal pathway or cell apoptosis.

RESUMEN

Objective To evaluate the clinical results of the PCL-retaining total arthroplasty in treatment of knee osteoarthris with valgus. Methods From June 2002 to January 2005,8 cases of osteoarthris with valgus knee were treated with PCL-retaining total knee replacement. The X-ray before operation showed average 21.6 degrees(ranging from 10 to 35 degrees) valgus. The cases were follow-uped from 6 to 36 months, each of them was evaluated with the Special Surgery Knee Scoring System. Results The X-ray after operation showed average 5.7 degrees(ranging from 3 to 9 degrees) valgus. The average HSS scores increased from preoperative 54 to postoperative 91.The range of motion was significantly improved from preoperative 83 degrees to postoperative 118 degrees. All of the cases were no pains after operation, and the function of knees were apparently improved. Conclusions The preliminary clinical results are quite satisfactory for the PCL-retaining total arthroplasty in treatment of osteoarthris with valgus knee. The adequate relaxing and balancing of soft tissue is the key factor to the success of total knee arthroplasty. The follow-up is necessary for further long-term outcome.

RESUMEN

Seven children (12 to 15 years) having madelung's deformity were considered for the study. All the children complained of aesthetic deformity with functional difficulties with their daily life activities. Ilizarov's technique by lengthening and re-axation of distal extremity of the radius was used as therapeutic modality. The short term follow up provides satisfactory results. However. long term study is needed to say with certainty that the technique prevent subsequent secondary osteo anthritis.
