RESUMEN

Objective To explore the correlation between ostiomeatal complex(OMC)variation, nasal septum deviation and nasosinusitis with CT imaging.Methods The clinical and imaging data of 256 patients undergoing nasal and paranasal CT examination from June 1, 2015 to May 30, 2016 were retrospectively analyzed. Patients with paranasal sinus fractures, tumors and artifacts were not included in this study.Taking the image of standard coronal MPR built with thin axial sections at the level of middle of semilunar fissure as observing image,and the direction,location and extent of OMC variations were observed and recorded. The patients were divided into two groups according to the presence or absence of nasal septum deviation.OMC mutation,the incidence of unilateral mutation,incidence of paranasal sinusitis and the incidence of unilateral bilateral nasal sinusitis between the two groups was analyzed by χ2test with two independent samples. Then according to the presence or absence of OMC mutation, the patients were divided into two groups.The incidence of paranasal sinusitis and bilateral paranasal sinusitis were compared between the two groups. Results Two hundred and fifty six cases of CT imging of parannasal sinus were included in the study,96 cases were with nasal septum deviation, 160 were with non-deviation of nasal septum. The incidence rate of OMC variation between the two groups showed significant difference(χ2=38.40,P<0.05).The incidence rate of unilateral and bilateral OMC variation showed significant difference(χ2=13.70,P<0.05),among which 54 were unilateral variations,50(92.6%)variations were located at the same side of nasal deviation(side of the nasal cavity widened).The incidence rate of sinusitis between the two groups showed non-significant difference(χ2=0.50,P>0.05).The incidence rate between unilateral and bilateral nasosinusitis showed non-significant difference(χ2=2.25,P>0.05).Of the 256 cases,128 were with OMC variations,and 128 were with non-OMC variation.The incidence rate of nasosinusitis between the two groups showed non-significant difference(χ2=0.56,P>0.05). The incidence rate between unilateral and bilateral nasosinusitis showed non-significant difference(χ2=3.57,P>0.05). Conclusion Ipsilateral OMC variation occurred in nasal septum deviation might be a compensation to nasal cavity widening while not a obstructive factor in causing nasosinusitis.

Introdução. O estudo da anatomia da região naso-sinusal e de suas variações é de grande importância diagnóstica e para a definição de condutas de tratamento nas áreas da Odontologia e da otorrinolaringologia, principalmente em relação aos seios maxilares, regiões anatômicas diretamente relacionadas com os dentes superiores. Objetivos. Avaliar por meio da tomografia computadorizada de feixe cônico (TCFC) a prevalência das seguintes variações anatômicas do complexo ostiomeatal: concha bolhosa, células de Haller, desvios de septo nasal e aerações do processo uncinado e, ainda, avaliar a prevalência das afecções inflamatórias dos seios maxilares. A presença das variações anatômicas supramencionadas e de alterações apicais de dentes posteriores superiores serão correlacionadas com a presença das afecções inflamatórias dos seios maxilares. Material e Métodos. Foram avaliados 100 exames de TCFC de pacientes odontológicos escolhidos de forma aleatória de acordo com protocolos de aquisição que abrangiam as regiões anatômicas de interesse ao estudo. Todas as tomografias do estudo foram adquiridas através do tomógrafo i-Cat, modelo Classic e as imagens em formato DICOM foram visualizadas e analisadas através do software i-Cat Vision. Resultados. Os exames tomográficos avaliados eram de 100 pacientes, 43 do gênero masculino (média de idade de 23,7anos) e 57 do gênero feminino (média de idade de 25,8 anos). Todas as variações anatômicas em estudo foram visualizadas nos exames analisados. Oitenta e cinco indivíduos apresentaram uma ou mais de uma variação concomitantemente, enquanto 15 indivíduos não apresentaram qualquer tipo de variação anatômica. A maior prevalência foi de indivíduos que apresentaram duas variações anatômicas (38%). Desvio de septo nasal ocorreu em 67% dos pacientes. Considerando-se a possibilidade de ocorrência bilateral em cada paciente, concha média bolhosa, células de Haller e aeração do processo uncinado apresentaram, respectivamente, as seguintes...


Introduction. The study of anatomy nasal-sinus and its variations is of great importance for the diagnosis and definition of behavior treatment in the areas of dentistry and otolaryngology, particularly in relation to the maxillary sinuses, anatomical regions directly related to the upper teeth. Objectives. Evaluate by beam computed tomography (CBCT) the prevalence of the following complex anatomical variations ostiomeatal: concha bullosa, Haller cells, nasal septum deviation and airings of the uncinate process, and also to assess the prevalence of inflammatory diseases of the sinuses jaws. The presence of anatomical variations above and apical changes of maxillary posterior teeth will be correlated with the presence of inflammatory diseases of the maxillary sinuses. Materials and Methods. We evaluated 100 CBCT examinations of dental patients chosen at random according to acquisition protocols covering the anatomical regions of interest to the study. All scans were acquired through the study of the tomograph i-Cat, Classic model and the images in DICOM format were viewed and analyzed using the software i-Cat Vision. Results. CT scans were evaluated for 100 patients, 43 males (mean age 23.7 years) and 57 females (mean age 25.8 years). All anatomical variations in the study were displayed in the images analyzed. Eightyfive subjects had one or more concomitant variation, while 15 subjects did not show any anatomical variation. The highest prevalence was individuals who had two anatomical variations (38%). Septal deviation occurred in 67% of patients. Considering the possibility of bilateral occurrence in each patient, concha bullosa, Haller cells and aeration of the uncinate process showed, respectively, the following rates: 31.5%, 20% and 9%. The inflammatory diseases of the maxillary sinuses showed the following distributions and prevalence rates considering bilateral analysis: in 74 maxillary sinuses were not visualized inflammatory disorders (37%)...

RESUMEN

Cholesterol Granuloma [CG] is a rare pathology found in the paranasal sinuses and is usually associated with middle ear infections. The etiology of sinonasal CG is not yet known. The clinical manifestations are nonspecific. Most patient presents with nasal discharge, facial pain and nasal obstruction similar to current case. The additional feature observed in the present case was bilateral CG of the maxillary sinus which was extending into ostiomeatal complex and into the oral cavity, whose clinical, imaging and histological characteristics were unique.

OBJETIVO: O objetivo deste trabalho foi verificar a freqüência e os tipos de variações anatômicas do complexo ostiomeatal. MATERIAIS E MÉTODOS: Foram revisadas 200 tomografias computadorizadas de pacientes com suspeita clínica de sinusopatia. RESULTADOS: Observamos um ou mais tipos de variações anatômicas em 83,5 por cento dos pacientes, as mais freqüentes envolvendo o corneto médio (84 por cento) e o septo nasal (34 por cento). Outras variantes anatômicas comuns encontradas foram a presença de aeração do agger nasi e células etmoidais infra-orbitárias (células de Haller). CONCLUSÃO: As variações anatômicas do complexo ostiomeatal anterior são muito corriqueiras. As mais freqüentes envolvem o corneto médio e o septo nasal.


OBJECTIVE: The present study was aimed at evaluating the frequency and types of anatomical variants of the ostiomeatal complex. MATERIALS AND METHODS: Computed tomography studies of 200 patients evaluated for clinical suspicion of sinusopathy were reviewed. RESULTS: Two or more types of anatomical variants were observed in 83.5 percent of the patients. The most frequent anatomical variants involved the middle turbinate (84 percent) and the nasal septum (34 percent). Other common variants found were the presence of pneumatized agger nasi cells and infraorbital ethmoid cells (Haller cells). CONCLUSION: The prevalence of anatomical variants in the ostiomeatal complex is high, the most frequent ones involving the middle turbinate and the nasal septum.

Objective To explore the features of ostiomeatal complex under endoscopic observation,so as to provide reliable landmarks for a safe and perfect endoscopic sinus surgery.Methods Twelve heads from adult cadavers(8 males and 4 females) were split axially on midline with the nasal septum removed,and nasal endoscopic operation was performed for the observation of tristar of groove with professional surgical instruments on anterior ethmoid,frontal sinus and maxillory sinus.Results The tristar of groove was consisted of beak of the ethmoidal bulla and its prolonging process,apex of the uncinate process and anterior peak of the middle turbinate,at the region of frontal recess.The structures called meatal groove,uncinate groove and bullar groove were observed around the ethmoidal bulla and the tristar of groove located at their origination.Under endoscopic view these structures looked like a triangular clefts,so it was named.The cells of the meatal groove located anteromedially to the tristar,the cells of the uncinate groove were anterolateral and just inferoposterior to the meatal groove,those of the bullar groove superoposteriorly located to the cells of the uncinate groove.The ostia of these cells were constant and did not connect each other,their locations at the tristar of grooves were fixed relatively.Conclusion Tristar of grooves is a key area for endoscopic frontal sinusotomy.Due to the great structural variations,the constant landmarks,which can be seen under sinus endoscope,and the regular pattern of the nasal sinuses distribution are important and can guide the endoscopic sinus surgery.

RESUMEN

Objective To evaluate the value of multi-slice spiral CT in nasal endoscopic surgery.Methods The multi-slice spiral CT data of 232 patients who were scanned prior to nasal endosopic surgery were analyzed.Results The nasal and sinus disease could be diagnosed correctly with multi-slice spiral CT.The ostiomeatal complex and the channels of the frontal sinus' drainage could be showed clearly.All the manifestation of the multi-slice spiral CT was in correspondence with the endoscopy showed in the surgery.Conclusion The multi-slice CT has the advantages of short scan time,less radiation dosage and multiplanar reconstruction.So it can display the nasal and sinus structures in detail,and has a great value in guiding the nasal endoscopic surgery.
