RESUMEN

Objective To investigate the clinical value of low molecular weight heparin (LMWH) in the (treatment) of severe acute pancreatitis (SAP) . Methods A prospective and randomized study on the (therapeutic) effects of conventioned therapy in 30 patients with SAP(C group), compared with 30 patients (treated) by conventional therapy plus LMWH(LT group). Results (1)The APACHE II scores in LT group were significantly lower than that in C group at 2 weeks after therapy (P0.05). The levels of fibrinogen in LT group were significantly higher than that in C group at 2 weeks after therapy (P

RESUMEN

Objective To evaluate the therapeutic effect of combination of sandostatin and growth hormone (GH) in the treatment of severe acute pancreatitis (SAP ). Methods Sixty patients with SAP were divided randomly into 3 groups:(1)Sandostatin treatment(ST) group (n=15);(2)combination of sandostatin with GH treatment(CT) group (n=30) ;(3)control group (n=15). The changes in serum IL-1, IL-6,TNF-? and albumin levels after treatment, and the incidence of complications, the duration of hospital stay and cost were compared among the 3 groups. Results The complications, mortality, duration of hospital stay in the CT group were significantly shorter than those in ST group and control group (all P

RESUMEN

Objective To investigate the pathogentic factor of severe acute pancreatitis(SAP)-associated lung injury and the protective function of octreotide combined with alinastatin on SAP-associated lung injury in rats. Methods Eighty SD rats were randomly divided into 5 groups.Sham operative(S)group,SAP group,(octreotide)(O)group, ulinastatin(U)group and octreotide + ulinastatin (O+U)group, and each group was divided into 6h,12h sub-groups.After AP models were induced, the serum concentration of amylase(AMY),tumor necrosis factor?(TNF-?),malondiadehyde(MDA),and the concentration of(myeloperoxidase)(MPO) in lung tissue were determined;and the pancreas and lung pathology were graded, the changes of the above-mentioned indexes after using octreotide and ulinastatin were compared.Results(1)Compared to SAP group,AMY,TNF-?,MDA,MPO,and pancreas pathology score were decreased significantly in each of the 3 therapy groups during the same period;and at 12h,in O+U sub-group,lung pathology score also decreased compared to SAP group(P

RESUMEN

Objective To explore the mechanism of lower molecular weight heparin(LMWH) in the treatment of severe acute pancreatitis(SAP). Methods Wistar rats were randomly divided into 3 groups: Sham group(S group),severe acute pancreatitis group(SAP group) and LMWH treatment group(H group). The serum amylase,IL-6,expressions of p65 mRNA and the activity of NF-?B were tested. Results The value of amylase and IL-6 in SAP group was significantly higher than that in H group (P

RESUMEN

Objective To evaluate the effect of local arterial infusion (LAI) of medicine on the treatment of servere acute pancreatitis(SAP). Methods The clinical data of 85 cases of SAP were retrospectively analyzed, and divided into three groups according to the admitted time:Group A. 42 cases admitted from February 1982 to December 1993,treated mainly by operation. Group B. 23 cases, admitted January 1994 to Auguest 1996, treated mainly by non operation. Group C. 20 cases, from September 1996 to Auguest 2000. treated mainly by LAI. Results The secondary infection rate in group A, B and C were 47%(20/42),26%(6/23) and 10%(2/20) respectively. The mortality in group A ,B and C were 36%(15/42),22%(5/23) and 5%(1/20),respectively. The difference in the secondary infection rate and mortality between group A , B and group C showed obvious significance (P

RESUMEN

Objective To investigate the effects of low molecular weight heparin (LMWH) on severe acute pancreatitis(SAP). Methods Retrospective analysis was made on the clinical features and therapeutic effect on the routine treatment (RT) group in 48 cases and LMWH treatment(LT) group in 31 cases. Results 28 (90.3%) out of 31 cases were cured while 3 (9.7%) died in LT group. 35(72.9%) out of 48 cases were cured whereas 15 (27.1%) died in RT group. The complications occurring in LT group (32.3%,10 patients 18 times ) was significantly lower than that in the RT group (62.5%,30 patients 59 times)(P

RESUMEN

Objective To investigate the effects of urinastatin on the production of inflammatory mediators and cytokines of acute pancreatitis(AP), and the effect of treating AP with urinastatin. Methods Serum levels of tumor necrotic factor ?(TNF ?), nitrogen oxide(NO), oxygen free radicals and amylase were determined in AP rats and patients with AP respectively. Effects of urinastatin treatment on the alleration of symptoms, signs and of pancreas in patients with AP were also examined. Results Urinastatin could apparently decrease the serum levels of TNF-?, oxygen free radicals and amylase in AP rats and the patients with AP, also alleviate the symptoms and signs of the patients with AP, and the effective rate of treating AP with urinastatin reached 90 percent. Conclusions Urinastatin, which can inhibit the production of inflammatory mediators and cytokines in AP, is an effective and cheap drug for AP.

RESUMEN

Objective To study the mechanism of 5 fluorouracil(5 FU) in the treatment of severe acute pancreatitis(SAP). Methods SD rats were randomly divided into three groups:acute pancreatitis group (AP group,n=12), 5 FU treatment group( 5 FU group,n=10),and control group (n=10). In 5 FU group,5 FU(1mg/100g) were injected immediately after AP was induced.12h after the AP was induced, blood samples were taken to determine nitric oxide(NO) and interleukin 6(IL 6). Results Compared with AP group,in 5 FU group, the survival rate increased, mean survival time prolonged, ascites volume decreased significantly, and the inflammatory mediators subsided. Conclusions NO and IL 6 may play an important role in the pathogenises of acute pancreatitis. The treatment effect of 5 FU for AP may through inhibit the inflammatory mediators.
