RESUMEN

Background: Silicon oil is an important adjunct for achieving internal tamponade in the treatment of retinal detachment. Silicone oil tamponade often leads to narrowing of the angle and development of adhesions between the iris and anterior chamber angle structures, with consequential elevation of the intraocular pressure. The video showcases the management of these challenging scenarios. Purpose: To highlight the management of early synechial closures due to silicon oil tamponade. Synopsis: The video highlights the management of early synechial closure following silicon oil tamponade. Surgical pupilloplasty has been demonstrated to break the peripheral anterior synechias on intraoperative gonioscopy as well as on anterior segment optical coherence tomography (AS?OCT). Performing pupilloplasty in the early phase of development of peripheral anterior synechias (PAS) helps to break the existing synechias and prevent angle closure and sequential deterioration of vision. Highlights: Surgical pupilloplasty helps to relieve the post silicon oil?induced secondary angle closure glaucoma by breaking the peripheral anterior synechias and significantly opening the anterior chamber angles.

RESUMEN

A granular cell tumour is a tumour of neural origin and is mostly benign and rarely malignant and commonly found in the skin, oral cavity or digestive tract. This report describes a case of a 23 - year - old female with a palpable painless mass on the back near the left axilla with progressive growth. While her laboratory tests and radiological were within normal limits histopathological examination revealed round to polygonal PAS positive atypical cells with large vesicular nuclei and coarsely granular cytoplasm along with foci of spindling an d necrosis which on IHC were S - 100 and CD68 positive and negative for cytokeratin, desmin, EMA. Thus, rendering a diagnosis of a malignant granular tumour of primary origin. This case highlights the importance of accurate histopathological examination to r ecognise and further categorization of Granular cell tumours along with clinical and radiological correlation .

(AU)

This study compared the performance in the Rorschach Test (R-PAS) of people informed about the symptoms of schizophrenia and asked to respond to the Rorschach trying to impersonate a patient with this diagnosis (n = 40) with that of patients diagnosed with schizophrenia (n = 35). Participants responded to the Rorschach Test (R-PAS), the Magical Ideation Scale (MIS) and the Inventory of Problems-29 (IOP-29) trying to simulate schizophrenia. The results of the comparisons between the groups showed statistically significant differences for the variables (Hd), An, FQo, CFQ, CF-, P, M, PEC, WSumCog, MAH, GHR and Complexity. A positive and strong association was also found between the scores obtained in the IOP-29 and the gross scores of the MIS (r=.73, p=.0001). The findings show that even faced with attempts to distort the responses to the test, simulators cannot distance themselves from their perceptual accuracy. (AU)

RESUMEN

Objective:To establish ultra-performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) for simultaneous determination of six hepatotoxic pyrrolizidine alkaloids in Verbenae Herba, and to carry out preliminary risk assessment according to the research results. Method:An ACQUITY UPLC HSS T3 column (2.1 mm×100 mm, 1.8 μm) was used for analysis with 0.05% formic acid and 2.5 mmol·L-1 ammonium formate in water (A)-0.05% formic acid and 2.5 mmol·L-1 ammonium formate in acetonitrile (B) as mobile phase for gradient elution (0-12 min, 3%-8%B; 12-25 min, 8%-15%B; 25-26 min, 15%-3%B; 26-30 min, 3%B), the flow rate was 0.3 mL·min-1, the column temperature was 40 ℃, and the injection volume was 1 μL. MS system was operated by electrospray ionization (ESI) in the positive ion mode with multiple reaction monitoring mode. MS parameters of triple quadrupole and six analytes were optimized for qualitative and quantitative analysis. According to the determination results, the risk assessment was carried out by using margin of exposure (MOE) combined with transfer rate of hot water extraction. Result:Based on the instrument precision, linear range, repeatability, stability, recovery and other methodological validations, the results were in conformity with relevant standards of quantitative analysis. The linear ranges of intermedine, lycopsamine, intermedine N-oxide, lycopsamine N-oxide, echimidine N-oxide and echimidine were good (r≥0.999 0) between peak area and mass concentration in the ranges of 0.984-49.20, 0.994-49.70, 1.012-50.60, 1.032-51.60, 1.004-50.20, 1.016-50.80 µg·L-1, respectively. The average recoveries of these six analytes were 87.2%-94.2% with relative standard deviation (RSD)<4.0%. Their MOE values were >10 000. Conclusion:The UPLC-MS/MS established in this study is stable and feasible, which can provide scientific basis for the quality control and safety evaluation of hepatotoxic pyrrolizidine alkaloids in Verbenae Herba.

Introduction: Ornamental Angel fish (Pterophyllum scalare) is one of the main commercialized species from South America, with an important number of national and international dealers. Objective: To make a histological and morphometric description of P. scalare digestive tract. Methods: Nineteen adult individuals were used. Intestinal coefficient values were obtained, and a macroscopic and microscopic description of esophagus, stomach, and intestine (anterior, middle and posterior) was carried out. Samples were processed for paraffin embedding, and 5 µm sections were stained with H&E and P.A.S. Positive P.A.S. cells were counted. Lumen area and tissue area were measured in representative digitalized sections from each region. Results: P. scalare exhibited a short esophagus, followed by a blind bag shaped, scarcely muscular stomach, and a short intestine (intestinal coefficient 1.66 +/- 0.52). Four constitutive tissue layers were identified: mucosa, submucosa, muscular and in some areas also a serosa layer. Esophagus exhibit abundant globet P.A.S. + cells and shows the larger tisular area was observed. Luminal area and perimeter are larger in anterior intestinal region. Conclusions: The intestinal coefficient value (1.66 +/- 0.52), and the histological and morphometric characteristics of the evaluated organs, suggest that P. scalare is omnivorous with soft prey preferences.

RESUMEN

BACKGROUND: Hypoxia inducible factor-1 (HIF-1) is considered as the most activated transcriptional factor in response to low oxygen level or hypoxia. HIF-1 binds the hypoxia response element (HRE) sequence in the promoter of different genes, mainly through the bHLH domain and activates the transcription of genes, especially those involved in angiogenesis and EMT. Considering the critical role of bHLH in binding HIF-1 to the HRE sequence, we hypothesized that bHLH could be a promising candidate to be targeted in hypoxia condition. METHODS: We inserted an inhibitory bHLH (ibHLH) domain in a pIRES2-EGFP vector and transfected HEK293T cells with either the control vector or the designed construct. The ibHLH domain consisted of bHLH domains of both HIF-1a and Arnt, capable of competing with HIF-1 in binding to HRE sequences. The transfected cells were then treated with 200 µM of cobalt chloride (CoCl2) for 48 h to induce hypoxia. Real-time PCR and western blot were performed to evaluate the effect of ibHLH on the genes and proteins involved in angiogenesis and EMT. RESULTS: Hypoxia was successfully induced in the HEK293T cell line as the gene expression of VEGF, vimentin, and ß-catenin were significantly increased after treatment of untransfected HEK293T cells with 200 µM CoCl2. The gene expression of VEGF, vimentin, and ß-catenin and protein level of ß-catenin were significantly decreased in the cells transfected with either control or ibHLH vectors in hypoxia. However, ibHLH failed to be effective on these genes and the protein level of ß-catenin, when compared to the control vector. We also observed that overexpression of ibHLH had more inhibitory effect on gene and protein expression of N-cadherin compared to the control vector. However, it was not statistically significant. CONCLUSION: bHLH has been reported to be an important domain involved in the DNA binding activity of HIF. However, we found that targeting this domain is not sufficient to inhibit the endogenous HIF-1 transcriptional activity. Further studies about the function of critical domains of HIF-1 are necessary for developing a specific HIF-1 inhibitor.

Abstract Introduction: Theory of Mind (ToM) is one of the most important dimensions of Social Cognition (SC); it is of great interest for research in Development Psychology, Cognitive Psychology, Neuropsychology, and Social Neuroscience as well as for clinical practice. It has stimulated the creation and validation of instruments to asses this dimension as a cognitive process, but there are few instruments like these in Spanish and less to evaluating children and adolescents. Objective: This paper reports the adaptation into Spanish of four instruments to asses ToM: The Theory of Mind Battery, The Theory of Mind Inventory-2, the Reading the Mind in the Eyes and the Faux Pas Test. Method: The adaptation process was made in two steps first, translation and, second, cultural adaptation. To verify the results a pilot testing was carried out in two groups, one with children between 4 and 13 years old and other with children and adolescents between 6 and 17 years old. Results: Instruments adapted into Spanish are presented as results highlighting the main adjustments made to each tool. Discussion: The importance of creating a protocol with statistical validation to asses ToM are discussed.

RESUMEN

Introduction:Asthma is a chronic inflammatory condition of the lung airways resulting in episodic airflow obstruction. This chronic inflammation heightens the airway hyper responsiveness (AHR). Asthma is defined by the history of respiratory symptoms like wheeze, shortness of breath, chest tightness and cough that vary over time and in intensity together with variable expiratory airflow limitation.1 Aims & Objectives: To compare the effectiveness of a combination of inhaled anticholinergics (ipratropium bromide) and beta 2 agonists (salbutamol) compared with beta 2 agonists (salbutamol) alone for the treatment of children with acute exacerbation of asthma. Methods: This is a randomized comparative study conducted in the Department of pediatrics ASRAM Medical College during the period May 2018 to June2019. Total 66 children between the age group of 1-15 years with eligible diagnostic criteria were included in the study. Results: The percentage increase in predicted PEFR is better in Group II with a mean of 24.74 compared to Group I with a mean of 13.35. The p value is highly significant 0.001. The mean PAS at the start of the study is 10.5 in Group I and 10.7 in Group II. The mean PAS at the end of the study is 7.19 in Group I and 5.77 in Group II. The p value is 0.001 and is highly significant. The outcome is better in Group II when compared to Group I with a better decrease in PAS and better increase in percentage predicted PEFR. Conclusion: In the present study it has been proven the repeated doses of Ipratropium bromide combined to Salbutamol is beneficial and it reduces the broncomotor cholinergic tone.

RESUMEN

Objective To investigate a self-designed catheter pump by using computational fluid dynamics (CFD) method, so as to predict its hydraulic performance and risk of thrombosis formation. Methods The thrombosis prediction models proposed by Grigioni and Danny Bluestein were used. The shear stress and exposure time during platelet motion were calculated by CFD method, and parameters of platelet activation state (PAS) were obtained for prediction of thrombogenic performance. Results At the flow rate of 4 L/min and rotating speed of 10 000 r/min, the differential pressure of the pump reached 14.763 kPa and the hydraulic performance was proved to fit the requirement of left ventricular assist device. The PAS values of Grigioni model and Danny Bluestein model were 6.35×10-6 and 7.68×10-4,respectively, both at a very low level, indicating a low possibility of thrombus formation. Conclusions This study investigated the feasibility of thrombosis prediction based on simulation method and the predicted hydraulic performance and thrombosis will provide references for further design optimization.

RESUMEN

Objective To investigate the diagnosis and treatment of pulmonary alveolar proteinosis (PAP). Methods Clinical data of 3 children with PAP admitted from 2010 to 2016 were retrospectively analyzed. Results Three children were female and aged 3 years and 1 month, 3 years and 9 months, and 6 years and 4 months, respectively. The main symptom of the three children was cough along with or without anhelation. Two of them had longer course of disease. The therapeutic effect of conventional treatment was poor, and their chest CT indicated significant lesions. Fiberoptic bronchoscopy showed positive reaction by PAS staining, and the treatment effect was improved after bronchoalveolar lavage. Conclusions The main clinical symptoms of PAP are non-specific cough and shortness of breath. PAS staining and bronchoalveolar lavage are of great value in the diagnosis and treatment of PAP.

The question “What is a woman?” and the logic of exception on which a hysterical woman bases herself in her relationship with femininity reveal one of her beliefs: that the bar placed by Lacan on Woman does not exist. The distinction between this logic and the feminine logic of the “not-whole” is the opportunity to once again place beliefs on femininity within psychoanalytic theory and to question the paradoxical uses of “feminine” and “masculine” positions.

ABSTRACT Objective The aim of this study is to validate the adult version of “Faux Pas Recognition Test” created by Stone and colleagues (1998) as a reliable instrument assess and discriminate social cognition among schizophrenia patients and healthy controls. Methods This is a cross-sectional study with a total of 196 participants (mean age = 26.45; CI (95%) [25.10; 27.83]) 51% male. From those, 44 (22.4%) patients with schizophrenia and 152 (77.6%) healthy controls. The participants answered a short version of the Faux Pas Recognition Test, composed by 10 stories. Results Significant differences were found between both groups regarding their scores on Faux Pas Recognition Test (p = 0.003). Patients with schizophrenia had lower score, compared to healthy controls. Story 14 was the best to distinguish both groups, and Story 16, the worst. Among the questions of Faux Pas stories, the one related to intuition presented the most significant difference between the groups (p = 0.001), followed by the one related to understanding (p = 0.003). Conclusion The Brazilian version of the Faux Pas Recognition Test is a valid test to assess social cognition in schizophrenia and can be an important instrument to be used on the clinical practice.

OBJECTIVE: To investigate the expression level of miR-142-5p and its potential target gene endothelial PAS domain protein 1(EPAS1) in Stage III colorectal cancer during Transcatheter arterial infusion chemotherapy (TAI). MATERIALS AND METHODS: Illumina high-throughput sequencing was used to obtain miRNA expression profiles of paired tumor and adjacent normal tissues from one patient received TAI 1 week before the operation and another patient directly underwent an operation. The expression levels of miR-142-5p was measured with both high-throughput sequencing and quantitative real time-polymerase chain reaction. RESULTS: The expression levels of miR-142-5p, were significantly reduced in tumor tissues of stage III CRC, then significantly increased in tumor tissues receiving TAI and higher than tumor tissues without TAI. The apoptosis rate of HT-29 colon cancer cells was mildly increased after transfection with pre-miR-142. miR-142-5p could bind directly to the 3´untranslated region of endothelial PAS domain protein 1 and reduce its expression. CONCLUSIONS: miR-142-5p is a potential tumor suppressor in CRC and is upregulated in tumor tissues after TAI, suggesting its potential clinical values for testing the functionality of TAI and predicting the progress of CRC.

The Rorschach method has proven an effective tool for personality assessment and identification of psychopathological patterns, in particular schizophrenia. The aim of this study was to seek validity evidences for the Rorschach Performance Assessment System (R-PAS) in a Brazilian context. Both R-PAS and the Magical Ideation Scale (MIS) were administered in 70 adults, 35 schizophrenic patients, and 35 non-patients. The results of the comparisons showed statistically significant differences for the variables (Hd), An, FQo, FQu, FQn, P, DV, DR, INC, CON, MAH, MAP, GHR, PHR, EII-3 and TP-Comp (p<0.05). There were positive and strong magnitude associations to the score obtained in MIS and the variables of the R-PAS, EII-3 and TP-Comp (r=0.84 and 0.85, respectively). Therefore the results can be interpreted as evidence of validity for the R-PAS in a Brazilian context...
