RESUMEN

Based on the theory of "spleen dispersing essence" in Huangdi's Inner Canon, combined with the discussion of medical experts in previous dynasties and modern researches, this paper argues that "spleen not dispersing essence" is an important pathogenesis of diabetes, which runs through the whole process of diabetes, including the stage of its complications. In the pathological state, the spleen disperses semen, affecting the physiological functions of the viscera, and the occurrence of diabetic retinal, heart, kidney, peripheral nerve and other complications. In clinical practice, the general principle of "promoting the spleen and dispersing essence" is used to provide the idea of TCM syndrome differentiation for the treatment of diabetes and its complications.

RESUMEN

The original meaning of pathogenesis refers to the cardinal mechanism of the occurrence, development and change of disease, which has three characteristics: cardinal, dominant and directional. The original pathogenesis of primary osteoporosis includes aging and renal failure, which was determined through ancient Chinese medicine books and modern researches. The basic contradiction and evolutionary law of diseases from the level of original pathogenesis were identified, that contribute to the prevention and treatment of primary osteoporosis with Traditional Chinese Medicine.

RESUMEN

This article analyzes and discusses the theoretical basis and clinical significance of the treatment of primary liver cancer (PLC) from the perspective of “regulating and protecting Zhongzhou” in the spleen and stomach. The development and progression of PLC is associated with the damage of “Zhongzhou” in the spleen and stomach caused by various factors, and “ignoring Zhongzhou” and dysfunction of spleen and stomach are the important pathological mechanism of PLC, while strengthening the spleen and stomach and “caring for Zhongzhou” are important measures to strengthen vital Qi and fight against cancer. With reference to the research findings of modern studies, this article elaborates on the antitumor pharmacological effect of compound spleen/stomach-regulating prescription and related traditional Chinese medicine (TCM) drugs in improving immunity and inhibiting tumor cell proliferation, and clinical research findings further confirm the objective therapeutic efficacy and advantages of TCM drugs and prescriptions for strengthening the spleen and stomach and “caring for Zhongzhou” in the treatment of PLC. It is believed that during the syndrome differentiation and treatment of PLC, the acquired foundation of the spleen and stomach should be taken seriously, and it is necessary to “take care of Zhongzhou” and apply TCM drugs and prescriptions for strengthening the spleen, benefiting Qi, and strengthening the spleen and stomach, so as to improve overall clinical outcome.

Objective Through the analysis of endocrinology outpatient data of Jiangsu provincial hospital,the law and the pathogenesis of TCM for type 2 diabetes was discussed.Methods The data of patients who were diagnosed of type 2 diabetes in Jiangsu provincial hospital outpatient in January 2014-October 2014 were collected,which including the oral medicine,establishment of a database,statistical analysis of single herb drugs appear frequency and its clinical efficacy.Results We collected a total of 583 prescriptions which contain 281 kinds of medcicines.Among these medicines,107 medicines were used more than 30 times,with the most frequently used medicines were poria,root of herbaceous peony and Astragalus.The clinical efficacy of these 107 medicines belonged to 10 kinds,medinces with the tonics and antipyretic functions occupied highest proportion(frequency of 30.4%,22.2%),followed by promoting blood circulation,diuretics for eliminating dampness,and promoting qi flow.Conclusions The pathogenesis of type 2 diabetes was mainly deficieny of both yin and fluid,and dryness heat inside,accompanied by blood stasis,phlegm,Qi stagnation.Therefore the treatment should be nourishing yin and clearing away heat,besides invigorating blood circulation and eliminating stasis,removing dampness and promoting diuresis,and disperse the depressed liver Qi.

【Objective】 To establish preliminary diagnostic criteria of spleen-stomach damp-heat syndrome (SDS) in chronic superficial gastritis (CSG) by disease-syndrome combined differentiation method and modern epidemiological method. 【Methods】 A clinical questionnaire of SDS in CSG was drafted out based on literatures study and experts' survey. Questionnaire investigation was performed in 146 cases of CSG, which consisted of 81 SDS cases and 65 nonSDS cases. Mter the investigation, discriminant analysis was adopted to set up the discriminant functional equations and to perform frequency analysis. According to the results, the diagnostic criteria and the simplified diagnostic criteria of SDS in CSG were established. 【Results】 The preliminary diagnostic criteria of SDS in CSG were as follows: (1) the major symptoms were characterized as yellow and greasy tongue fur, fullness or distension or pain in the stomach, loose stool and poor appetite; (2) the secondary symptoms were characterized as bitter taste and sticky mouth, chest depression, thirst but unwilling to drink, fatigue and nausea; (3) diagnosis could be confirmed by the existence of yellow and greasy tongue fur accompanied by either 2 of the other major symptoms, or by either 1 of the other major and 2 of the secondary symptoms, or by more than 3 of the secondary symptoms. The simplified diagnostic criteria were characterized the co-existence of yellow and greasy tongue fur and fullness or distension or pain in the stomach. 【Conclusion】 The preliminary diagnostic criteria of SDS in CSG are scientific, objective and practicable to some extent.

Objective To explore the pathogenic mechanism associated with insulin resistance(IR) and adiponetin(APN) abnormality in carotid atherosclerosis(CAS) patients complicated with nonalcoholic fatty liver disease(NAFLD),and to study their relationships with traditional Chinese medical(TCM) syndrome types.Methods Seventy-four CAS patients complicated with NAFLD(group A) and 25 CAS patients(group B) were enrolled into the study.Fasting blood glucose(FBG),fasting serum insulin(FI),and serum APN levels were measured.IR and APN were evaluated by the homeostasis assessment(HOMA) method.HOMA-IR and APN in group A were compared with those in group B,and bivariate correlation analysis was made between the two indexes in group A.The relationship of HOMA-IR and APN with TCM syndrome types was analyzed in group A.Results (1) HOMA-IR was obviously increased,and APN was obviously decreased in group A(P

Objective The differences of von Willebrand factor(vWF),hypersensitive C-reactive protein(hs-CRP) and soluble P-selectin CD62P were observed in coronary heart disease(CHD) patients with different syndrome patterns,and the correlation of traditional Chinese medical(TCM) syndrome patterns with endothelial damage,inflammation and platelet activation in CHD patients was investigated.Methods One hundred and eighty-six CHD patients were differentiated as the syndromes of blockage of heart-blood-stasis,phlegm blocking heart vessels,deficiency of heart and kidney yin,deficiency of both yin and Qi,and deficiency of yang and Qi.Thirty healthy volunteers served as normal control.The blood levels of vWF,hs-CRP and CD62P were detected in the CHD patients and the normal controls.Results Blood vWF level in CHD patients was higher than that in the normal controls(P0.05).Blood CD62P in the syndrome pattern of deficiency of yang and Qi was higher than the other patterns(P

To investigate the expression of androgen receptor (AR) in affected area of androgenetic alopecia (AA) and the relationship between its expression and various syndromes of traditional Chinese medicine (TCM). [Me- thods ]Eighty cases of AA were classified as damp-heat syndrome of spleen and stomach and deficiency syndrome of liver and kidney. Expression level of AR in the alopecia scalp and non-alopecia scalp of the cases was detected with immunohistochemical method and was compared with 32 healthy volunteers.In the cases of AA, AR expression was positive in the granulocyte layer, spinous layer and basal layer of epiderm as well as in the fibroblast, vascular endothelial cells, nerve tissue, arrectores pilorum, sweat gland, sebaceous gland, hair root sheath and hair papilla of dermis;but it was negative in horny layer of epiderm, subcutaneous fat, internal root sheath, hair shaft cells and hair matrocytes. The total AR-positive rate and the AR-positive rate in epiderm, sweat gland, sebaceous gland, hair root sheath and hair papilla were higher in the alopecia scalp of the cases than those in non-alopecia scalp of the cases and in healthy volunteers (P0.05).[Conclusion]AR is related to the pathogenesis of AA and the local abnormal AR content may be one of the pathogenic factors of AA. Pathological feature and AR expression differ with the syndrome patterns: AR expression rate is higher in AA with damp-heat of spleen and stomach than those with deficiency of liver and kidney.

To investigate the influence of six exogenous pathogenic factors on asthma in children and to observe the changes of microscopic indexes in cold- and heat-type asthma. Logistic regression analysis was applied to analyze the influence of six exogenous pathogenic factors and the changes of serum level of IgE, eosinophil (EOS) counting, contents of third component of complement (C3) and C-reactive protein (CRP) were observed. Objective To explore the characteristics of TCM syndromes of acute coronary syndrome(ACS). Methods Clinical data of 127 ACS patients were collected and then a database was established.After that,the distribution of syndrome patterns was analyzed.Results The incidences of the following excess syndrome patterns were in a decreasing sequence:stasis syndrome,phlegm syndrome,cold syndrome and heat syndromes.The incidence of stasis syndrome was 90.5%,which was much higher than other excess syndromes.The incidences of the following deficiency syndrome patterns were in a decreasing sequence:heart-Qi deficiency syndrome,heart-Qi exhaustion syndrome,heart-yin deficiency syndrome,heart-Qi depletion syndrome,heart-kidney deficiency syndrome and heart-blood deficiency syndrome.The incidence of heart-Qi deficiency syndrome was 75.6%,which was much higher than other deficiency syndromes.The incidences of the stasis syndrome and cold syndrome in acute myocardial infarction(AMI)were higher than those in unstable angina(UA),and the incidences of heart-Qi deficiency syndrome and heart-Qi exhaustion syndrome in AMI were higher than those in UA(P

[ Objective ] To investigate the decisive factor of syndrome differentiation of systemic lupus erythematosus (SLE) , so as to provide its experimental evidence. [Methods] Serum levels of soluble intercellular adhesion molecule-1 (sICAM-1) and soluble vascular cell adhesion molecule-1 (sVCAM-1) in SLE patients with yin deficiency of the liver and kidney (group B) and with excessive toxic heat (group C) were detected by enzyme-linked immunosorbent assay (ELISA) . Serum levels of sICAM-1 and sVCAM-1 in SLE patients were compared with those in healthy volunteers (group A). RESUMEN

[Objective] To investigate the relationship between serum leptin level in chronic renal failure (CRF) and Qi or yang deficiency syndrome of spleen and kidney. [Methods] Thirty-eight cases of CRF and 16 healthy subjects as control were enrolled in this study. The CRF patients were classified as Qi /yang deficiency syndrome of spleen and kidney (pattern 1) and non-Qi/yang-deficiency syndrome of spleen and kidney (pattern 2). Serum leptin level was detected by enzyme-linked immunosorbent assay and nutrient parameters such as serum albumin (ALB), C-reation protein (CRP) and pre;ilbumin (preAlb) levels were also observed. The relationship of leptin level and nutrient parameters with Qi or yang deficiency syndrome of spleen and kidney was observed. [Results] The serum leptin level of pattern 1 was higher than that of pattern 2 ( P 0.05) . [ Conclusion ] Serum leptin may be the partial material base of Qi or yang deficiency syndrome of spleen and kidney and this is worthy doing further research. The uncertain relationship between nutrient parameters and Qi or yang deficiency syndrome of spleen and kidney may be due to the increase of type Ⅱ error which results from the small sample size.

[Objective]To investigate the relationship of TCM syndrome patterns with gastrointestinal motility and hormones in functional dyspepsia (FD). [Methods] Among 30 cases of dyskinetic FD, 16 with liver depression and qi stagnation were allocated to Group B, 14 with spleen and stomach deficiency to Group C and 20 healthy volunteers served as control group ( Group A). Gastric evacuation, elctrogastrogram (EGG), serum motilin and migrating motor complex (MMC) wave of gastric antrum and duodenum during digestion were examined and analysed statistically. Objective To explore the relationship between syndrome differentiation of diabetic retinopathy (DR) and pattern of visual evoked potential (P-VEP).Methods One hundred and eighty-six eyes from 104 cases of DR were allocated to three groups according to TCM syndrome:qi-yin deficiency (Group A),qi stagnation and blood stasis (Group B) and yin-yang deficiency (Group C).P-VEP induced by various space frequency (0.388?Hz,1.55?Hz) and pictures of various colors (black/white,red/magenta,blue/green) was observed in the three groups and compared with that in the normal control group (Group D).Results One hundred and three eyes in Group A mainly manifested as simple DR,47 eyes in Group B and 36 eyes in Group C mainly manifested as proliferative DR.The latency of P 100 was prolonged and the amplitude was decreassed in all groups of DR,but the latency of P 100 was much prolonged in Group B,the amplitude was much decreased in Group C and the changes were less in Group A.Conclusion P-VEP varies in different TCM syndrome types of DR,and determination of P-VEP under various stimulating conditions may serve as an index of syndrome differentiation of DR.

RESUMEN

One hundred and twenty cases of restenosis after percutaneous transluminal coronary angioplasty (PTCA) were subjected to TCM syndrome analysis.PTCA and support implantation were performed by routine method. The standard of TCM syndrome differentiation is subordinated to Professor Deng Tietao. The results showed that 78 cases (65%) were differentiated as Qi deficiency with phlegm-blood stasis, 13(11%) as yin deficiency with phlegm-blood stasis and 29 (24%)as phlegm-blood stagnation. It is indicated that Qi deficiency with phlegm-blood stasis is the main syndrome and reinforcing Qi dissipating phlegm and activating blood-flow may be a reasonable way of preventing and treating restenosis.

Objective To explore the relationship of TCM syndromes in chronic heart failure (CHF) with left ventricular mass index (LVMI) and left ventricular ejection fraction (LVEF). Methods Two hundred and ninety-six CHF patients were classified into the syndromes of Qi deficiency (21 patients), Qi deficiency and blood stasis (98), fluid retention due to Qi deficiency and blood stasis (72), blood stasis due to the deficiency of Qi and yang (56), and blood stasis and fluid retention due to the deficiency of Qi and yang (49). LVMI and LVEF in different syndromes were campared.Results LVMI presented an increasing sequence and LVEF presented a decreasing sequence in the syndromes of Qi deficiency, Qi deficiency and blood stasis, fluid retention due to Qi deficiency and blood stasis, blood stasis due to the deficiency of Qi and yang, and blood stasis and fluid retention due to the deficiency of Qi and yang.
