RESUMEN

Objective To investigate the therapeutic effect and analgesic mechanism of percutaneous vertebral angioplasty in the treatment of painful osteoporotic thoracolumbar vertebral compression fracture.Methods80 cases of patients with painful osteoporotic thoracolumbar vertebral compression fracture in Shaoxing second hospital from May 2014 to May 2016 were selected, they were randomly divided into percutaneous vertebroplasty group (40 cases) and conservative treatment group (40 cases) two groups, the clinical efficacy, VAS scores and ODI scores of the two groups were statistically analyzed.ResultsThe total treatment effective rate of the percutaneous vertebroplasty group 95% (38/40) was significantly higher than the conservative treatment group (30/40) (P<0.05);The VAS scores and ODI scores of the two groups after treatment were significantly lower than before treatment (P<0.05);The differences of VAS scores and ODI scores before treatment between the two groups were not significant.The VAS score and ODI score after treatment of the percutaneous vertebroplasty group were significantly lower than the conservative treatment group (P<0.05).ConclusionThe therapeutic effect of percutaneous vertebral angioplasty in the treatment of painful osteoporotic thoracolumbar vertebral compression fractures is more significant than conservative treatment, it has better analgesic effect, so is worthy of promotion in the clinical.

RESUMEN

Objective To investigate the effect of percutaneous vertebroplasty and kyphoplasty (PKP) on bone density and serum leptin in patients with osteoporotic thoracolumbar compression fractures. Methods One hundred and two patients with osteoporotic thoracolumbar vertebral compression fractures and 100 cases of healthy control were treated with PKP surgery. The serum leptin and adiponectin, and the bone mineral density of the lumbar spine were determined. Results patients Levels of the serum leptin and adiponectin in patients with osteoporotic thoracolumbar vertebral compression fracture were significantly higher than those with non osteoporotic fracture patients (P < 0.05). One month post-PVP, the levels of serum leptin and adiponectin in the two groups were significantly decreased (P < 0.05). postoperative group of The lumbar bone density at 1 month post-operation was is significantly higher than that before operation in each group (P < 0.05, respectively), with no significant difference between the two groups. The levels of serum leptin and adiponectin were negatively correlated with the bone mineral density (BMD) (P < 0.05). Conclusions tLevels of serum adiponectin and leptin were both increased in osteoporotic or non bone osteoporosis thoracolumbar vertebral compressor fractures. The percutaneous vertebroplasty forming operation could effectively reduce the serum level of leptin and adiponectin, and improve the bone mineral density of the lumbar vertebrae.

RESUMEN

Objective To evaluate the influencing factors of recurrence of fracture after percutaneous vertebral augmentation.Methods From January 2007 to December 2013,256 cases patients with percutaneous vertebral augmentation in treatment of osteoporotic vertebral compression fractures in the Central Hospital of Chaoyang were retrospectively reviewed.After operation,the patients were divided into recurrent fracture group (59 cases) and no recurrence group (197 cases) according to whether postoperative recurrence of vertebral fracture.The age,sex,body mass index (BMI),bone mineral density,operation mode,bone cement injection volume,the existence of bone cement leakage and vertebral body height recovery rate were compared between the two groups respectively.Multi factor Logistic regression analysis was performed to analyze the correlation between the factors and the recurrence of vertebral body fractures.Results The age((75.9±4.6) years old vs.(69.9±5.0) years old),BMI((23.5±0.6) kg/m2 vs.(25.4±0.7) kg/m2),bone mineral density((-3.67±0.68)vs.(-2.75 ±0.98)),the existence of bone cement leakage rate(22.0% vs.5.1%) and vertebral body height recovery rate ((24.1±2.9) % vs.(14.9±3.0)%) between recurrent fracture group and no recurrence group had statistically significant differences (P=0.046,0.047,0.046,0.026,0.023).Multi factor Logistic regression analysis showed that age,BMI were not related to postoperative recurrence of fracture(P=0.140,0.137),and bone density,bone cement leakage and vertebral body height recovery rate were related to recurrence of fracture (P=0.018,0.000,0.000).Conclusion Low bone mineral density,existence of bone cement leakage and high body height recovery rate are the risk factors of recurrence of fracture after percutaneous vertebral augmentation.
