RESUMEN

Geriatric giants account for the five most prevalent morbidities that cause immense decline in functional capacity and decreased quality of life. Immobility, instability leading to falls and fractures, incontinence, impaired cognition (dementia) and iatrogenesis affect majority of the ageing elderly leading to dependency, caregiving and associated burden among caregivers. Concept of non-pharmacological interventions in dementia care: interventions that involve non-pharmacological (non-medicinal) measures, have proven benefits to the people living with dementia (PLWD). music therapy, nature therapy that involves nature walks and living amidst natural spaces, have shown to have immense biophilia effects on the various organ systems in the human body. Immense health benefits have been documented among PLWD: dawn phenomenon is a clinical phenomenon where the PLWD experiences agitation, aggression and loud speech in the evenings associated with dementia. Out of all the seven studies published, there were significant improvements in mood, reduction in aggressive behavior, improving anxiety symptoms, reducing caregiver burnout and improving overall quality of life of the people living with dementia and their caregivers. With promising results among various interventional studies across the world, NPI is an important intervention with minimal to nil side effects with long term benefits on frequent administration.

Introdução: As úlceras no pé diabético surgem da interação complexa entreneuropatia periférica e doença arterial periférica, comprometendo a cicatrização após traumas. Objetivo: Explorar a diversidade de intervenções terapêuticas não farmacológicas que têm sido estudadas e avaliadas quanto à sua eficácia e segurança no tratamento de úlceras no pé diabético. Metodologia: Pesquisa do tipo revisão integrativa da literatura. Para obtenção dos resultados foi realizado um levantamento nas plataformas PubMed e Biblioteca Virtual em Saúde. Para elaboração dos resultados foram selecionados 21 artigos. Resultados: As intervenções encontradas foram oxigenoterapia hiperbárica, terapia de feridas por pressão negativa, uso de matriz dérmica, plasma rico em plaquetas, plasma atmosférico frio, tratamentos com curativos especiais e uso de solas rígidas, entre outros. Mostraram uma variabilidade na taxa de cicatrização e no tempo de fechamento da ferida, bem como na melhoria da regeneração tecidual. Conclusão: As pesquisas mostram uma diversidade de intervenções terapêuticas não farmacológicas utilizadas no tratamento de úlceras no pé diabético, ressaltando a necessidade de abordagens individualizadas e mais estudos para determinar a eficácia e segurança de cada intervenção (AU).


Introduction:Diabetic foot ulcers arise from the complex interaction between peripheral neuropathy and peripheral arterial disease, compromising wound healing after traumas. Objective:To explore the diversity of non-pharmacological therapeutic interventions that have been studied and evaluated for their effectiveness and safety in the treatment of diabetic foot ulcers. Methodology: An integrative literature review was conducted. The search for results was performed on the PubMed and Virtual Health Library platforms. Twenty-one articles were selected for result elaboration.Results:The identified interventions included hyperbaric oxygen therapy, negative pressure wound therapy, use of dermal matrix, platelet-rich plasma, cold atmospheric plasma, treatments with special dressings, and the use of rigid soles, among others. They exhibited variability in the healing rate and wound closure time, as well as improvement in tissue regeneration.Conclusion:The research demonstrates a diversity of non-pharmacological therapeutic interventions used in the treatment of diabetic foot ulcers, emphasizing the need for individualized approaches and further studies to determine the effectiveness and safety of each intervention (AU).


Introducción: Las úlceras en el pie diabético surgen de la interacción compleja entre neuropatía periférica y enfermedad arterial periférica, comprometiendo la cicatrización después de traumas.Objetivo: Explorar la diversidad de intervenciones terapéuticas no farmacológicas que han sido estudiadas y evaluadas en cuanto a su eficacia y seguridad en el tratamiento de úlceras en el pie diabético.Metodología: Investigación del tipo revisión integrativa de la literatura. Para obtener los resultados se realizó un estudio en las plataformas PubMed y Biblioteca Virtual en Salud. Para la elaboración de los resultados se seleccionaron 21 artículos. Resultados: Las intervenciones encontradas fueron oxigenoterapia hiperbárica, terapia de heridas por presión negativa, uso de matriz dérmica, plasma rico en plaquetas, plasma atmosférico frío, tratamientos con curativos especiales y uso de suelas rígidas, entre otros. Mostraron una variabilidad en la tasa de cicatrización y en el tiempo de cierre de la herida, así como en la mejora de la regeneración tisular. Conclusión: Las investigaciones muestran una diversidad de intervenciones terapéuticas no farmacológicas utilizadas en el tratamiento de úlceras en el pie diabético, resaltando la necesidad de enfoques individualizados y más estudios para determinar la eficacia y seguridad de cada intervención (AU).

To date, no effective treatment has been able to modify the pathological progression of osteoarthritis (OA).Current therapy can be broadly categorized into pharmacological and non-pharmacological approaches before considering surgical interventions. Non-pharmacological methods address lifestyle modifications, weight reduction and physical therapy, all aiming at alleviating mechanical stress on the affected joint.In this article, we focused on pharmacological treatment options, that primarily target pain reduction by reducing joint inflammation or restoring the altered synovial environment to a normal state.In this literature review , the main focus is on approved conservative therapies and examined emerging conservative strategies. Evaluating their advantages and limitations.

Medicinal plants are used to cure diseases, and their replacement is frequent and affects public health. The genus Baccharis has representatives within the medicinal flora of Argentina, although the replacement of the species of this genus known under the vulgar name of "carqueja" by Baccharis spicata has been detected i n herbalists or markets of herbal products. The genotoxic safety of this species has been established in previous work of our group. The aim of this study was to evaluate the antiviral activity of an infusion made from B. spicata leaves against hepatitis B virus with the HepG2.2.15 cellular system and to determine cytotoxicity in HepG2.2,15, A549 and Vero cell lines. Infusion of B. spicata was active to inhibit HBV replication with an EC 50 of 22.54 µg/mL and a CC 50 of 190 µg/mL.


Las plantas medicinales son empleadas para la cura de enfermedades, y su sustituc ión es frecuente y afecta a la salud pública. El género Baccharis posee representantes dentro de la flora medicinal de Argentina, aunque se ha detectado la sustitución de las especies de dicho género conocidas bajo el nombre vulgar de "carqueja" por Baccha ris spicata en herboristerías o mercados de productos herb arios . Se ha establecido la seguridad genotóxica de esta especie en trabajos previos de nuestro grupo. Este estudio buscó evaluar la actividad antiviral de una infusión elaborada a partir de hojas de B. spicata frente al virus de la hepatitis B con el sistema celular HepG2.2.15 y determinar la citotoxicidad en las líneas celulares HepG2.2.15, A549 y Vero. La infusión de B. spicata fue activa para inhibir la replicación del virus con un EC 50 de 22.54 µg/mL y un CC 50 de 190 µg/mL.

RESUMEN

Eucommia ulmoides, a plant belonging to Eucommiaceae, has a history of medical use for over two thousand years in China. The dried bark and leaves of this plant are usually used as medicinal materials. Due to the high safety in clinical application, E. ulmoides leaves were officially recognized for both medicinal and edible use by the food safety evaluation in 2019, providing a valuable resource for the development of food and health products. According to the traditional Chinese medicine theory, E. ulmoides has the effects of nourishing the liver and kidneys, strengthening sinews and bones, and calming fetus. Modern research has shown that different parts such as the bark, leaves, flowers, and seeds of E. ulmoides contain similar chemical components, including phenylpropanoids, terpenoids, flavonoids, phenolic acids, steroids, and polysaccharides. E. ulmoides exhibits diverse pharmacological activities such as lowering blood pressure and blood lipid and glucose levels, preventing osteoporosis and possesses anti-tumor, anti-bacterial, antiviral, anti-inflammatory, antioxidant, and hepatoprotective effects. Therefore, it holds great potential for the development of products with both medicinal and edible values. This review systematically summarizes the chemical constituents, pharmacological activities, and representative medicinal and edible products of different parts of E. ulmoides. It is expected to provide theoretical references for the clinical application of E. ulmoides and its active components and the development and utilization of the products with both medicinal and edible values. This review contributes to a deeper understanding of the medicinal properties of E. ulmoides and provides guidance for further exploration of its applications in the healthcare field. As a plant with both medicinal and edible values, E. ulmoides is expected to attract more attention in future research and contribute to human health.

By consulting ancient herbal books and modern literature, this paper systematically sorted out and researched the processing history, relevant processing norms in recent years, modern processing technology, chemical composition changes of processed products and their pharmacological mechanism of Scutellariae Radix, in order to provide a basis for the further development of Scutellariae Radix decoction pieces. According to the textual research of ancient books, there were many kinds of processing auxiliary materials of Scutellariae Radix, such as wine, vinegar, salt, honey, pig bile and so on, among which the wine processing was the most diverse and detailed, and the processed products such as raw products, stir-fried products, wine-processed products, fried charcoal products were still in use. The modern processing techniques of Scutellariae Radix mainly focus on the processing aspects of softening and slicing, wine processing and charcoal frying, and the research methods are relatively unified. At present, it is found that the changed chemical constituents of Scutellariae Radix after processing are flavonoids, polysaccharides, volatile oils and trace elements, etc. Pharmacological effects of processed products are hemostasis, antibacterial, anti-inflammatory, antioxidant, analgesic and antipyretic, treatment of lung diseases, treatment of colitis, etc. However, in the studies of Scutellariae Radix processing, there is a lack of research on the structural changes of chemical components caused by processing and a comprehensive comparative study on the pharmacological effects of various processed products. Based on this, it is suggested to carry out systematic research on the processing technology to processing mechanism, further explore the relationship between the change rule of material basis and pharmacological action before and after processing of Scutellariae Radix, and deepen the exploration of molecular mechanism and clinical application of processed products of Scutellariae Radix, in order to clarify the scientific connotation of the processing mechanism of Scutellariae Radix, and lay a foundation for the subsequent expansion of the application of Scutellariae Radix decoction pieces and the formulation of processing standards.

Idiopathic oligoasthenospermia (IO) has been increasingly emphasized in the diagnosis and treatment of male infertility. Oxidative stress damage directly affects sperm quality and spermatogenesis, constituting a major causative factor of IO. Firstly, due to its high content of polyunsaturated fatty acids, the sperm plasma membrane is highly sensitive to reactive oxygen species (ROS), leading to lipid peroxidation accumulation and even inducing ferroptosis. Secondly, deficient downstream key proteins in the base excision repair pathway render sperm unable to repair extensive DNA oxidative damage under oxidative stress. Simultaneously, under oxidative stress, the apoptotic pathway of sperm is cascade-activated, causing rapid loss of motility. ROS further disrupts the hypothalamic-pituitary-gonadal axis, inhibiting testosterone production and ultimately affecting spermatogenesis. Wuzi Yanzongwan,in line with traditional Chinese medicine theory of treating IO through "nourishing kidney essence and harmonizing Yin and Yang", clinically demonstrates its ability to improve sperm morphology, count, and motility, thereby enhancing male fertility. The research on the pharmacological constituents of Wuzi Yanzongwan primarily involves establishing a characteristic spectrum of Chinese medicine to achieve quality control and exploring the pharmacology of effective components. Studies have found that its main active ingredients consist of flavonoids and phenylpropanoids. Specifically, compounds such as hyperin, acteoside, kaempferol, and schisandrin A are identified as the primary active substances and quality control components. These compounds exhibit strong antioxidant activity and have been partly applied in research related to reproductive endocrine disorders. Tripterygium glycoside is primarily used for modeling of oxidative stress-induced IO. It leads to the accumulation of various lipid peroxides in testicular tissues and concurrently compromises the body's antioxidant capacity. Mechanistic studies have found that Wuzi Yanzongwan can inhibit elevated ROS levels in IO models and enhance the body's antioxidant capacity, thereby ameliorating inflammation, suppressing cell apoptosis, promoting testosterone production, and ultimately alleviating the decline in sperm quality and spermatogenesis caused by oxidative stress.

Neurological diseases are common and frequent in clinical practices, which are the main reasons that affect the quality of life and physical and mental health of patients seriously. Tianma Goutengyin (TGY) is from the New Significance of Patterns and Treatment in Miscellaneous Diseases, which was compiled by a famous doctor, HU Guangci. TGY is widely used in clinical practice and has the effects of calming the liver and calming the wind, clearing heat and activating the blood, tranquilization, and nourishing the liver and kidneys. Clinical studies have confirmed that modified TGY can be used either alone or in combination with acupuncture or western medicine to treat dementia, headache, vertigo, hypertension, insomnia, Parkinson's disease, stroke, epilepsy, and other common neurological diseases, with significant curative effect, few side effects, and high safety. The main active constituents of single flavor drugs in the composition of TGY mainly include gastrodin, gastrodia elata blume polysaccharides, p-hydroxybenzyl alcohol, rhynchophylline, baicalein, leonurine, stachydrine, geniposide, 2,3,5,4,-tetrahydroxystilbene-2-Ο-β-D-glucoside, pinoresinol diglucoside, pachymic acid, β-ecdysteroid, avicularin, etc. It has been found that TGY and these constituents have the effects of ferroptosis inhibition, anti-apoptosis, anti-inflammation, and oxidation resistance, and they can regulate neurotransmitters and autophagy, reduce cerebral edema reduction, lower blood lipid and blood pressure, and improve blood circulation through multiple targets and pathways. This paper reviewed the clinical application of and the mechanism of the whole prescription and single flavor drugs of TGY in the treatment of neurological diseases, so as to provide a theoretical basis for the clinical application of TGY and offer ideas for the follow-up mechanism research of this prescription.

With intensified aging, Alzheimer's disease has become a serious problem in China's health field. In the field of traditional Chinese medicine (TCM), Alzheimer's disease mainly describes cognitive deficits such as dementia and amnesia. After the inheritance and summary by medical experts of successive generations, the theory of "toxin damaging brain collaterals" has become a mature pathogenesis hypothesis of this disease. Blood stasis, as one of the main viral pathogens, is also closely related to the theory of Alzheimer's disease in modern pharmacology. Chuanxiong Rhizoma is used frequently in clinical prescriptions for Alzheimer's disease. As the main component of Chuanxiong Rhizoma, tetramethylpyrazine has a series of pharmacological effects on the cardiovascular system such as vasodilation, anti-platelet aggregation, anti-atherosclerosis, and anti-myocardial ischemia, which reflects the effects of Chuanxiong Rhizoma in activating blood circulation and removing blood stasis. However, few studies have focused on the effect of tetramethylpyrazine on the pathogenesis of Alzheimer's disease. From the perspective of TCM theory and modern pharmacology, this article discussed the effects of tetramethylpyrazine on the pathology and pathogenesis of Alzheimer's disease from the aspects of cardiovascular function, oxidative stress, inflammatory response, mitochondrial function, and cholinergic system and made prospects for the future application of tetramethylpyrazine to prevent and treat Alzheimer's disease.

Objective:To study the the mechanism of action of Huanglong Mixture in the treatment of cough variant asthma (CVA) in children based on the IL-4/signal transduction and activator of transcription 6 (STAT6) signaling pathway using network pharmacology methods, molecular docking techniques, and in vitro cell experiments.Methods:The components and targets of various TCM components in Huanglong Mixture were searched in TCMSP database, HERB database and literature, and the disease targets of CVA were found in Gene Cards database, OMIM database, DrugBank database and PharmGkb database. The STRING database was used to construct the protein-protein interaction network, and Cytoscape 3.9.1 was used for topology analysis to screen out the core targets. The disease-drug-component-target network was constructed to screen out the core components. The KEGG enrichment analysis and GO enrichment analysis of the intersection targets were performed using Metascape software. PDB protein database, PubChem, Autodock and R language were used for molecular docking verification of core targets and core drug components. Finally, rat primary airway smooth muscle cells were cultured, modeled with interleukin-4 (IL-4), and p-STAT6 expression in the cytoplasm and nucleus was detected by Western blot.Results:A total of 122 effective components were obtained, including quercetin, kaempferol, luteolin and so on. The core targets included JUN, ESR1, TP53, MYC, HIF1, etc. GO enrichment analysis involved biological processes such as response to external stimuli, response to oxygen levels, positive regulation of protein phosphorylation, and regulation of cellular stress response. KEGG enrichment analysis showed that the main pathways of Huanglong Mixture in treating CVA included advanced glycation end product-glycation end product receptor (AGE-RAGE) signaling pathway, phosphatidylinositol-3-kinase-protein kinase B (PI3K-Akt) signaling pathway, tumor necrosis factor (TNF) signaling pathway, Janus kinase/signal transduction activation factor (JAK-STAT) signaling pathway. Molecular docking found that the core targets and core drug components had good combination. Cell experiments also confirmed that Huanglong Mixture could inhibit p-STAT6 entering the nucleus.Conclusions:The effective components and targets of Huanglong Mixture in the treatment of CVA are successfully predicted. The mechanism of Huanglong Mixture in the treatment of children with CVA may be related to the inhibition of IL-4/STAT6 signaling pathway.

Objective:To explore the potential mechanism of Huayu Pills in the treatment of TNBC (triple-negative breast cancer) using network pharmacology.Methods:TCMSP (Traditional Chinese Medicine Systems Pharmacology), PubChem, STITCH and SwissTargetPrediction databases were used to analyze the potential targets of Huayu Pills. TNBC disease targets were screened based on the TCGA (The Cancer Genome Atlas) database. The drug-disease mapping target was constructed by PPI network, key target screening and module analysis, and the DAVID database was used for GO function annotation and KEGG signal pathway enrichment analysis. SD rats were orally administered 3.94 g/kg of Huayu Pills decoction for 4 days to prepare medicated serum. HUVEC cells were randomly divided into a control group and an experimental group using a random number table method. The experimental group received intervention with Huayu Pills containing serum for 24 hours, inoculated in matrigel's solidified 48 well plate and HUVEC angiogenesis was observed 3 hours later.Results:130 possible targets of Huayu Pills in the treatment of TNBC were obtained. VEGFA is the core target. The cascade of angiogenesis, hypoxia, and blood coagulation may be the main functions and key signals of Huayu Pills in the treatment of TNBC. In vitro studies have shown that serum containing Huayu Pills can promote the normalization of tumor blood vessels in HUVEC cells.Conclusion:Huayu Pills may promote tumor vascular normalization (angiogenesis, hypoxia, coagulation cascade reactions) through VEGF targets, and can assist immune checkpoint inhibitors in the treatment of TNBC.

Chuanxiong Rhizoma,the dried rhizome of Ligusticum chuanxiong Hort.(Umbelliferae),has the effects of ac-tivating blood,promoting the circulation of Qi,dispelling pathogenic wind,and relieving pain.Chuanxiong Rhizoma contains phthalides,alkaloids,phenolic acids,and polysaccharides.Pharmacological research indicates that Chuanxiong Rhizoma has various pharmacological activities on the cardiovascular system,nervous system,and respiratory system,which are mainly manifested as anti-cerebral ischemia,anti-thrombosis,analgesia,anti-inflammation,antioxidation,and anti-asthma effects.In this paper,the chemical constituents and pharmacological effects of Chuanxiong Rhizoma are systematically summarized in order to provide references for its clinical application and resource development.

RESUMEN

Paeonia Radix Alba is the dried root of Paeonia lactiflora Pall.in the ranunculus family,which is mainly produced in Zhejiang(Hang Paeony),Anhui(Hao Paeony)and Sichuan(Chuan Paeony).It has the effects of calming liver,relieving pain,nourishing blood,regulating menstrual period,collecting Yin,and preventing perspiration.Similar to Paeonia Radix Alba,Paeonia Radix Rubra is the dried root of Paeonia lactiflora Pall.or Paeonia veitchii Lynch.in the ranunculus family.Although the chemical constituents are similar,the efficacy and clinical application of the two herbal medicines are obviously different owing to their various processing methods and techniques.Modern studies have shown that Paeonia Radix Alba is rich in multiple chemical constituents,mainly including monoterpenoids and their glycosides,triterpenoids and their glycosides,volatile oils,flavonoids,and polyphenols,and has a wide range of pharmacological effects such as analgesia,anti-inflammation,anti-depression,liver protection,anti-tumor,and anti-hyperglycemia.This paper reviews the progress of research on effective constituents of Paeonia Radix Alba and their pharmacological effects to provide references for the comprehensive development and utilization of Paeonia Radix Alba.

Radix Tetrastigmatis is a traditional medicinal herb in Zhejiang and Fujian provinces.It mainly contains flavonoids,organic acids,phenols,volatile oils,triterpenes and sterols,polysaccharides,etc.It has the effects of anti-tumor,anti-inflammatory,anti-viral,hepatoprotective,and can prevent and treat chronic obstructive pulmonary disease.This article reviewed the chemical composition and pharmacological effects of Radix Tetrastigmatis,and looked forward to its development prospects,so as to provide reference for its rational development and utilization.

RESUMEN

Objective To evaluate the effects of different non-pharmacological interventions on postpartum depression,and to provide the evidence for the formulation of relevant intervention programs.Methods Databases,including China National Knowledge Infrastructure(CNKI),China Biomedical Literature Database,Wanfang Database,VIP database,PubMed,EMBase,the Cochrane Library,were systematically searched for randomized controlled trials(RCT)on the effects of non-pharmacological interventions on postpartum depression.The search time limit was from the establishment of the database to July 2022.Network meta-analysis was performed using Stata17.0 and Review Manager 5.3 software.Results A total of 43 studies were included,including 14 non-pharmaceutical interventions,involving 4451 parturients.The results of network meta-analysis showed that compared with the usual care group,exercise therapy[SMD=-5.41,95%CI(-6.94,-3.89),P<0.001],cognitive behavioral therapy[SMD=-4.72,95%CI(-6.13,-3.31),P<0.001],acupuncture and moxibustion[SMD=-4.52,95%CI(-8.01,-1.03),P<0.001]and the comprehensive psychotherapy[SMD=-4.64,95%CI(-8.53,-0.75),P<0.001]had better improvement effects on postpartum depression.The results of the area under the cumulative rank probability map showed that exercise was the best non-pharmacological intervention to improve postpartum depression.Conclusion Exercise has the best effect on improving postpartum depression,and it is recommended that clinical nursing care should be prioritized.However,a large number of high-quality RCTs are still needed for verification.
