RESUMEN

We report a rare case of primary scrotal lipoma in a 20-year-old patient which clinically mimics a left triorchidism. The patients presented with the left testicular swelling and the ultrasonography of the scrotum revealed an inhomogeneous hyperechoic mass with an inconclusive report. The left scrotal hemi exploration was planned, and intraoperatively, it was found to be a paratesticular mass with an equivalent size corresponding to the left testicle, separate from the left testicle. The histopathology confirms the lipomatous swelling. The post-operative period remains uneventful and the patient has been on follow-up for the past 1 year with no relapse

Abstract Introduction: polyorchidism is an unusual pathology, about 200 cases in the world literature. Case report: we reported a case of polyorchidism in a 16-year-old male patient diagnosed by ultrasound and confirmed by magnetic resonance imaging. Discussion: most of the cases presented, there is a supernumerary testis, but there are reports on more than three, up to five testicles with supranumerical gonads on both sides of the scrotum. The diagnosis is usually performed in late puberty, incidentally, with a painless scrotal mass or at the emergency room, presenting a testicular torsion of the whole hemiscrotum or supernumerary testisalone, and the differential diagnosis should be made with epididymal cyst and spermatocele, besides other extra-testicular masses (hydroceles, varicoceles, lipomas, tumors.) and para-testicular masses (hernias, scrotal calculi). After the initial clinical evaluation, ultrasound is the first line subsidiary exam. Magnetic Resonance Imaging is very helpful, just in case the ultrasound diagnosis is uncertain. The supernumerary testishave the same Magnetic Resonance Imaging characteristics as the normal testes (intermediate signal intensity on T1- weighted images and high signal intensity on T2-weighted images).


Resumo Introdução: o poliorquidismo é uma patologia incomum, contando cerca de 200 casos na literatura mundial. Relato de caso: relatamos um caso de poliorquidismo em um paciente do sexo masculino de 16 anos, diagnosticado por ultrassom e confirmado por ressonância magnética. Discussão: na maioria dos casos apresentados, há um testículo supranumérico, mas há relatos de mais de três, até cinco testículos, com gônadas supranuméricas em ambos os lados do escroto. O diagnóstico geralmente é feito no final da puberdade, aliás, com massa escrotal indolor ou no pronto-socorro, apresentando torção de todo o hemiscroto ou somente do testículo supranumerário, e o diagnóstico diferencial deve ser feito com cisto epididimário e espermatocele, além de outros massas extratesticulares (hidroceles, varicoceles, lipomas, tumores) e massas paratesticulares (hérnias, cálculos escrotais). Após a avaliação clínica inicial, o ultrassom é a primeira linha do exame subsidiário. A ressonância magnética é muito útil se o diagnóstico por ultrassom não for certo. Os testículos supranumerários têm as mesmas características de ressonância magnética que os testículos normais (intensidade do sinal intermediário nas imagens ponderadas em T1 e alta intensidade do sinal nas imagens ponderadas em T2).

Introducción: El testículo supernumerario es una anomalía congénita urológica poco frecuente, que es definido como la presencia de más de dos testículos, ya sea de localización intra o extraescrotal. Los testículos supernumerarios son una rara anomalía a considerar durante el estudio de masas escrotales. La ecografía doppler y la resonancia magnética proveen elementos útiles para diagnosticar la presencia de esta entidad. Objetivo: Conocer la incidencia de testículos supernumerarios, anomalía infrecuente del sistema genital. Se realizó de forma retrospectiva la revisión de los casos operados por el servicio de Urología, según el registro de documentación clínica de nuestro centro entre los años 1970 y 2010, con un promedio anual de 250 casos intervenidos por año, siendo un solo paciente portador de esta anomalía. Se realizó una revisión de la literatura, encontrando solamente tres casos en Cuba y menos de 100 pacientes en el mundo. Se reportó el tercer caso de testículo supernumerario en Cuba.


Introduction: The polyorchidism is a few frequent urologic congenital anomaly defined as the presence of more than two testicles, located inside or outside the scrotum. The polyorchidism is a rare anomaly to be considered when studying scrotal masses. The Doppler echography and the computerized magnetic resonance imaging give useful elements to diagnose the presence of this entity. Objective: Knowing the incidence of the polyorchidism, infrequent anomaly of the genital system. Patients and methods. We retrospectively reviewed the cases operated in the Urology service, according to the Clinic Documentation records of our center in the period from 1970 to 2010, with a year average of 250 cases operated, and only one patient carrying this anomaly. We made a literature review, finding only three cases in our country and less than 100 patients in the world. We report the 3rd case of polyorchidism in Cuba.

RESUMEN

Polyorchidism is a very rare congenital anomaly and about 100 cases have been documented in the literature until today. Polyorchidism or supernumenary testis is defined as the presence of more than 2 histologically proven testes. In most case, polyorchidism have characteristics of unilateral involvement and three testis and mainly finding in left-side testis. And although patient age was various, average age was about 18-year-old in previous review of article. We report a rare case of polyorchidism that an old age patient had an incidentally found right accessory testis in operating a testicular injury.

RESUMEN

Polyorchidism is a very rare anomaly of the genitourinary tract and about 100 cases have been reported in the literature. Although polyorchidism is associated with undescended testis, inguinal hernia, testicular torsion, hydrocele, epididymitis, varicocele and cancer, the majority of the patients are asymptomatic and present with painless groin or testicular masses. We describe a case of polyorchidism in a 19-year-old man with varicocele and review the literature and current management of polyorchidism.

RESUMEN

Polyorchidism is a rare anomaly. It may result from transverse division of the urogenital ridge, a hypothesis that best explains the anatomical features of the supernumerary testicle. Until recently, histology provided the only proof of polyorchidism but new imaging techniques, such as sonography and magnetic resonance imaging(MRI), allows a highly accurate diagnosis of supernumerary testes rendering biopsy unnecessary. We report a case of polyorchidism in a 24 month-old-boy in whom the scrotal sonogram and magnetic resonance imaging confirmed the presence of 2 testicles without any evidence of tumor within his right hemiscrotum and who was treated conservatively and followed up 2 years.

RESUMEN

We describe an 22-yeer-old man with polyorchidism whose both ipsilateral testes were located in the inguinal area and scrotum. Polyorchidism is a rare anomaly with approximately 70 cases reported in the literature. In this report the literature is reviewed and the etiology of this anomaly is discussed briefly.
