RESUMEN

Objective To explore the prognostic value of preoperative serum pre-albumin in the patients with bladder urothelial carcinoma. Methods A retrospective study was conducted on the clinicopathological data of 320 patients with bladder urothelial carcinoma, who underwent radical cystectomy in Changhai Hospital of Navy Medical University (Second Military Medical University) between Jan. 2006 and Dec. 2014. Receiver operating characteristic (ROC) curve was drawn to determine the optimal cut-off value of preoperative serum pre-albumin level. χ2 test was used to analyze the relationship between preoperative serum pre-albumin level and clinicopathological characteristics. Log-rank test and Kaplan-Meier method were used to analyze the relationship between preoperative serum pre-albumin level and overall survival and relapse-free survival of the patients. Cox proportional hazards model was used to explore the risk factors of prognosis of the patients. Results Based on optimal cut-off value of 249.5 mg/L, the patients were divided into high pre-albumin (≥249.5 mg/L) group (162 cases) and low pre-albumin (<249.5 mg/L) group (158 cases). Compared with the patients in the high pre-albumin group, those in the low pre-albumin group were older and more prone to preoperative hydronephrosis, high pathological T staging, high grade classification, distant metastasis and intraoperative transfusion, and the differences were significant (all P<0.05). The patients in the low pre-albumin group had significantly shorter overall survival and relapse-free survival compared with the patients in the high pre-albumin group (both P<0.05). The results of multivariate Cox regression analysis demonstrated that high pathological T staging, lymphatic metastasis, distant metastasis, intraoperative transfusion and low pre-albumin level were independent risk factors for the overall survival (all P<0.05), and high pathological T staging, lymphatic metastasis, distant metastasis and low pre-albumin level were independent risk factors for the relapse-free survival (all P<0.05). Conclusion Low preoperative serum pre-albumin level is an independent prognostic risk factor for the patients with bladder urothelial carcinoma, and preoperative serum pre-albumin can be used as a common indicator for prognosis of bladder urothelial carcinoma patients.

Objective To study the relationship between the level of plasma pre-albumin and the patients′ self efficacy,quality of life and prognosis in patients with traumatic epilepsy.Methods Totally 120 cases of patients with craniocerebral trauma treated in our hospital from August 2015 to April 2016 divided into control group and observation group,according to whether patients with epilepsy,compared plasma albumin,self efficacy,quality of life score of the two groups,correlation analysis were used to investigate the relationship between plasma albumin level and self efficacy in patients with quality of life and clinical prognosis,logisitic regression analysis was used to study the risk factors of traumatic epilepsy in patients with poor prognosis.Results The CRAMS score,plasma pre-albumin,self-efficacy and life ability of the observation group were lower than those of the control group,the difference was statistically significant(P<0.05).Pearson correlation analysis showed that plasma pre-albumin was positively correlated with CRAMS score,self-efficacy and life ability(r=0.78,0.65,0.89,P<0.05).Logistic regression analysis showed that CRAMS decreased 0.2 points(P=0.015),plasma albumin decreased by 5 g/L(P=0.032)was an independent risk factor for prognosis of patients with traumatic epilepsy.Conclusion Plasma level of pre-albumin and CRAMS score are independent risk factors for death in patients with traumatic epilepsy,and should be paid attention to patients with brain injury.

Objective To investigate the clinical application of procalcitonin (PCT),pre-albumin (PA)and white blood cell (WBC)detections in children infectious diseases.Methods 116 children inpatients of infectious diseases in our hospital were select-ed for conducting the study and divided into the non-bacterial infection group (56 cases)and the bacterial infection group (60 cases) according to the different pathogenic microorganisms.Other 64 children with healthy physical examination in the outpatient depart-ment were taken as the control group.The blood PCT,PA levels and WBC count were detected in 3 groups,and which before and after treatment in the bacterial infection group were detected.The differences in PCT,PA levels and WBC count were compared a-mong the groups.Results The blood PCT level and WBC count in the bacterial infection group and the non-bacterial infection group were all higher than that in the control group,while the PA level in the bacterial infection group was lower than that in the non-bacterial infection group and the control group,the differences had statistical significance (P <0.05);the PCT level and WBC count after treatment in the bacterial infection groups were lower than those before treatment,while the PA level after treatment was higher than that before treatment with statistical difference (P <0.05).Conclusion The detection of PCT,PA and WBC count can be used as the indexes for diagnosing infectious diseases in children,and provides reference for the judgment of prognosis.

Objective To explore the methods of the nutritional support of children with severe sepsis.Methods The biochemical index,nutrition index,blood gas,blood routine and 24 hours urinary urea nitrogen of 198 children with severe sepsis were detected after hospitalization.Enteral nutrition,parenteral nutrition,or enteral and parenteral nutrition at the same time were selected for every children according to patients status.Children with parenteral nutrition were divided into glutamine (Gln) group and non-Gln group.Nutrition and metabolism indexes of survival and dead children were detected,including blood glucose,C-reactive protein,hemoglobin (Hb),pH,blood sodium,blood potassium,alanine transaminase,blood urea nitrogen,creatinine,blood lacticacid,lactate clearance rate,blood triglycerides,blood total cholesterol,serum pre-albumin (PA),retinal-binding protein (RBP),serum albumin (ALB) and 24 hours urinary urea nitrogen.The intake of calories were compared between survival and dead children before nutrition support (0 d),3 d and 7 d after nutrition support.Immunoglobulins of Gln group and non-Gln group were compared.Results The nutrition indexes of the survival group were higher than those of dead group [PA (130.0 ± 30.0) mg/L vs (50.8 ±20.5) mg/L,RBP(22.3 ±10.3) mg/L vs (15.7 ±6.7) mg/L,ALB(35.3 ±8.1) g/L vs (28.7 ±6.2) g/L,Hb(113.2 ±27.7) g/L vs (95.3 ±10.6) g/L,IgA(0.40 ±0.03) g/L vs (0.40 ±0.03) g/L,IgM(0.52 ±0.18) g/L vs (0.49 ±0.03) g/L] (P <0.05).The intake calories of survival group in 3 d and 7 d after nutrition support were (50.32 ±2.76) kcal/(kg·d) and (65.70 ±3.25) kcal/(kg·d),which were significantly higher than those of dead group [(32.54 ± 1.72) kcal/(kg·d) and (46.12 ± 1.08) kcal/(kg·d)).Among the survival children with parenteral nutrition,the levels of immunoglobulin in Gln group were higher than non-Gln group [IgG(4.93 ± 2.1) g/L vs (4.01 ± 1.03) g/L,IgA (0.31 ± 0.07) g/L vs (0.19 ±0.03) g/L,IgM(0.52 ±0.08) g/L vs (0.32 ±0.10) g/L] (P <0.05).Negative nitrogen balance was better in Gln group as(-2.5 ± 1.4) g/d than non-Gln group as (-5.3 ± 1.3) g/d(P <0.05).Conclusion Children with severe sepsis manifeste significantly increased metabolic rate and energy consumption,the protein decomposition utilization is greater than the synthetic with negative nitrogen balance.Children with more severe sepsis have lower metabolic indexes,nutrition indexes,and immunoglobulin.It is necessary for children with severe sepsis to use glutamine while having parenteral nutrition.

Objective To study the relationship of microinflammation,nutrition and common carotid artery intima-media thickness(CCA-IMT) for the early prevention and interference of ischemic stroke.
