RESUMEN

Objective:To investigate the application effect of production-oriented approach (POA) for consulting ability teaching in general practice standardized residency training.Methods:Eighteen third-year general practice residents in the Second Affiliated Hospital of Harbin Medical University received a special training course on the consulting ability based on POA with the situational simulation teaching from September 2021 to March 2022. The consulting ability of residents was evaluated with Objective Structured Clinical Examination (OSCE) and Leichester Assessment Package (LAP) before and after the training, and the results were compared. Meanwhile, a questionnaire survey and semi-structured interviews were conducted to assess residents′ satisfaction with the course.Results:All 18 resident trainees (11 females and 7 males) aged 25 to 34 (27.3±2.4) years successfully completed the training. Both OSCE scores and LAP scores after the training were significantly higher than those before training (66.25±5.84 vs. 44.44±12.80, t=8.46, P<0.001; 65.78±7.05 vs. 38.33±14.2, t=1.47, P<0.001, respectively). The ability of medical history collection, physical examination, patient management, problem solving, doctor-patient relationship, preventive care and medical record writing were all significantly improved after the training compared with those before training ( t=2.464, 4.278, 8.997, 2.385, 10.35, 5.212, 7.578, all P<0.05). The questionnaire survey showed that all the 18 residents were satisfied with the reasonableness of the teaching content, the class time arrangement, the teaching progress arrangement and the instructors. In the dimension of training effectiveness, 18 participants all believed that the course was helpful to improve their humanistic quality and confidence in reception. In terms of self-promotion, all respondents believed that this course was able to promote their reflection ability and stimulate their learning interest. By coding and analyzing the contents of the semi-structured interviews with the residents, nine main themes were constructed, namely course deficiencies, course characteristics, course advantages, course suggestions, trainees′ deficiencies, trainees′ gains, trainees′ goals, previous teaching deficiencies, and instructor′s role. The results showed that all residents had a high degree of satisfaction with the course, and they thought the course was vivid and interesting and it is able to mobilize the enthusiasm of learning, promote learner to reflect. The residents also suggested to increase the teaching hours and to enrich teaching cases. Conclusion:The consulting ability training of general practice based on POA can effectively improve consulting ability, stimulate learning interest and improve independent learning ability of residents, which gains recognition from both faculty and trainees.

RESUMEN

The phenomenon of "Chinese cultural aphasia" in college English education in China is typically represented by "traditional Chinese medicine (TCM) cultural aphasia" in English learning among students in TCM colleges and universities, which is mainly caused by insufficient driving force for input and production in both teaching and learning. Based on the production-oriented approach, this study designs a teaching model oriented by "TCM culture" production in the ESP course of Introducing TCM Culture in English and conducts a practice research according to the teaching process of driving, facilitating, and evaluating. The practice research shows that the application of this model has effectively integrated "national sentiment" and "national consciousness" into the teaching and learning of the TCM ESP course, improved the TCM culture production ability among students, realized the educational effect of foreign language teaching, and promoted the teaching and research practice of teachers.

Objective:To explore the application of flipped classroom teaching in the standardized residency training of students in department of hepatic surgery from the perspective of production-oriented approach (POA) theory.Methods:Based on the implementation time of flipped classroom teaching (January 2017) from the perspective of POA theory in West China Hospital, 51 nursing students who were routinely trained in department of hepatic surgery before the implementation (January 2015 to January 2017) were included in the control group. After the implementation (January 2017 to January 2019), 51 nursing students under the residency training were included in the observation group. The changes of test scores, independent learning ability and professional awareness of the two groups of nursing students before and 6 months after the training were compared, and the teaching satisfaction after the training was recorded. SPSS 19.0 was performed for t test and chi-square test. Results:The total scores of the theoretical examination scores of the two groups of nursing students, the total scores of the clinical practice assessment scores, and the self-learning ability assessment scale for nursing college students (learning motivation, self-management ability, learning cooperation ability, information literacy) scores and total scores in each dimension were significantly higher than those before training, and the observation group was significantly higher than the control group ( P<0.05); the two groups of professional maturity scales (career goals, professional self-confidence, professional autonomy, professional value, dependence on relatives and friends, occupation reference) scores of all dimensions were significantly improved compared with those before the training, and the scores of the observation group (career goals, professional self-confidence, professional autonomy, professional value) were significantly higher than those of the control group during the same period ( P<0.05). There was no statistical significance in the comparison between the two groups' dependence on relatives and friends and occupational reference scores ( P>0.05); the teaching satisfaction rate of nursing students in the observation group was 96.08%, which was significantly higher than that of the control group (82.35%). Conclusion:The application of flipped classroom teaching from the perspective of POA theory in the standardized residency training of department of hepatic surgery can significantly improve the autonomous learning ability of nursing students, which is conducive to the improvement of test scores and professional awareness, with high teaching satisfaction.

Objective:To explore the application of the flipped classroom based on the production-oriented approach (POA) theory in the teaching of standardized training in the Department of Joint and Sports Medicine.Methods:A total of 37 standardized training physicians who rotated in the Department of Joint and Sports Medicine in Zhongnan Hospital of Wuhan University from January 2019 to December 2019 were selected as the control group, and the traditional teaching was adopted. Another 39 standardized training physicians who rotated from April 2020 to March 2021 were selected as the study group, and the flipped classroom based on POA theory was adopted. At the end of the rotation, the scores of theoretical knowledge and practical operation skills, the scores of mini-clinical evaluation exercise (Mini-CEX) and the teaching satisfaction were compared. SPSS 22.0 was used for t-test and Chi-square test. Results:The various scores and total score of theoretical knowledge and practical operation skills in the study group at the end of rotation were significantly higher than those in the control group ( P < 0.05). There was no significant difference in the various scores and total score of Mini-CEX between the two groups at the time of admission ( P > 0.05). The above scores in the two groups at the end of rotation were significantly higher than those at the time of admission ( P < 0.05), and the above scores of the study group were significantly higher than those of the control group ( P < 0.05). The overall teaching satisfaction rate of the study group was higher than that of the control group ( P < 0.05). Conclusion:The application of the flipped classroom based on POA theory in the teaching of standardized training in the Department of Joint and Sports Medicine can significantly improve the teaching effect and quality, and improve the clinical comprehensive ability and teaching satisfaction of the standardized training physicians, which is worthy of popularization and application.

RESUMEN

Living with internationalization, it becomes increasingly important to develop the ability of acquiring knowledge from and publishing professional articles international medical journals.However, current English teaching in colleges is predominantly by cramming method and without clear aims.As a result, students make little learning progress.Production-oriented Approach, proposed by Professor Qiufang Wen, is an important theory to reform the modern English Teaching.Based on this theory, we designed and performed the course of Medical English Writing and Literature Reading for graduate students.For the aim of literature writing, we matched the teaching content with the pertinent language ability to achieve the goal.The process of teaching was initiated with output, followed by input, then tested by another output.As a result, students′ writing was improved effectively in correctness and in awareness of the rules.In addition, their ability to comprehend difficult and long sentences was also found to be improved.We believe Production-oriented Approach conforms to the requirements of English Teaching in college and can contribute to the teaching goal.
