RESUMEN

@#ObjectiveFollow-up of the clinical outcome of prosthetic disc nucleus replacement on patients with lumbar intervertebral disc herniation. Methods16 patients with lumar intervertebral disc herniation were implanted prosthetic disc nucleus from June to August 2003, including 10 males and 6 females. The operation levels were at L4-5 in 10 patients and L5~S1 in 6 patients. All patients were followed up from 28 to 32 months (averaged 30 months). The contents include pain, physical examination, Short-form of McGill Pain Questionnaire investigation, Visual Analogue Scales(VAS) and Present Pain Intensity(PPI), X-ray and MRI examination, etc. ResultsAfter surgery, 16 patients experienced clinical symptom relief, the displacement of PDN in 1 case and dislocation in 1case, the height of intervertebral space becoming narrower in 5 cases and endplate subsiding in 1 case. ConclusionProsthetic disc nucleus replacement acquired satisfactory after operation, but some complication may occurre in long-term.

RESUMEN

OBJECTIVE: The purpose of the current study is to investigate the effect of the prosthetic disc nucleus replacement on the mobility and height of the intervertebral disc and adjacent segments. METHODS: Thirteen patients who underwent L4-L5 prosthetic disc nucleus replacement were included in this study. A retrospective review of clinical and radiological data was conducted. The L4-L5 disc height and sagittal rotation angle of L3-L4, L4-L5, L5-S1 were measured in the static and dynamic lateral radiographs pre- and postoperatively. RESULTS: There were seven men and six women whose mean age was 37 years(range, 24-49 years). The mean follow-up period was nine months(range, 6-14 months). In all cases the L4-L5 motion segment demonstrated angular motion between flexion and extension with a mean of 4degrees(+/-2.3degrees) of sagittal rotation angle. The disc height increased from preoperative levels by 117%. There was no difference in angular motion of adjacent segments between pre- and postoperative data. CONCLUSION: The prosthetic disc nucleus replacement after discectomy is shown to restore the disc height and maintain segmental mobility.

RESUMEN

This article introduces a new technology of prosthetic disc nucleus replacement to treat lumbar disc herniation. The preliminary clinic experience, short time following ups, indications, contraindications, size and method of the technique were discussed. It was noted that some patients had short but heavy lumbago after the operation but the pain was relieved after oral medication. It will be a trend in the future spine surgery to restore the functions of spine unit. The prosthetic disc nucleus is a good choice before a total disc prosthesis replacing or spine fusion.
