RESUMEN

Worster-Drought syndrome (WDS) is a special and rare type of cerebral palsy, which is characte-rized by pseudobulbar palsy, selective weakness of the muscles of the lips, tongue, and soft palate, dysarthria, dysphagia, salivation, and jaw spasm.The onset of WDS is linked with a variety of prenatal, intrapartum and postnatal factors.The report of family cases of WDS suggested the genetic predisposition.Although there is no cure at present, early genetic counseling, early diagnosis and intervention can reduce the morbidity of offspring and improve the symptoms of children with WDS, which will greatly reduce the burden on the family and society in the future.In the present study, this paper aims to explore the etiology, pathogenesis, clinical manifestation, imaging examination, diagnosis, disease spectrum and current treatment of WDS, thus improving the diagnostic and therapeutic efficacy.

RESUMEN

This article showed the work pattern of rehabilitation assessment, and made assessment of a patient who suffered from bilat-eral hemiplegia and pseudobulbar paralysis caused by multiple cerebral apoplexy. Finally, the special characteristics of rehabilitation treat-ment for a patient with bilateral paralysis were summarized.

RESUMEN

Objective To investigate the clinical management and therapeutic strategy for patients with pseudobulbar paralysis post stroke. Methods To retrospectively analyze the clinical data of 157 patients poststroke whom were in convalescence stage. Then patients with pseudobulbar paralysis were selected and whose epidemiology feature and treatment effect were analyzed. Results In 157 patients,55 patients(35.0%) were complicated with pseudobulbar paralysis,among whom,26 patients(26/55,47.3%) with hemorrhagic stroke,including basal ganglia hemorrhage 6(6/26,23. 0%) , cerebellar hemorrhage 12 (12/26,46. 2%) and brain stem hemorrhage 8 (8/26,30. 8%).29 patients suffered ischemic stroke(29/55,52.7%) ,including 9 cases with corticalis(9/29,31.0%),13 with brain stem infarction(13/29,44.8%) and 7 with cerebellar infarction(7/29,24. 2%). Among all the patients,32(32/55,58.2%) cases lesion were multiple and 23 cases(23/55,41. 8%) were simple. In the field of treatment effect of all patients,41 cases(42/55,76.4%) were with excellence, 11 with utility(11/55,20%) and 2 with inefficiency (2/55,3.6%). Conclusion The comprehensive management and compatible medical treatment should be the compatible clinical management and therapeutic strategy for patients with pseudobulbar paralysis post stroke.

RESUMEN

Objective To observe the clinical therapeutic effect of hyperbaric oxygen (HBO) therapy and swallowing training combined with Fengchi acupoint acupuncture on dysphagia caused by pseudobulbar paralysis after stroke. Methods Seventy-five patients were randomly divided into control group, HBO therapy group, swallow training group, acupuncture group and combined treatment group (n = 15). Patients in all groups were given cerebro-vascular disease routine treatment. In addition, the patients in HBO therapy group, swallowing training group and ac-upuncture group were given HBO therapy, swallowing training and Fengehi (GB20) acupoint acupuncture simultane-ously, respectively. The patients in combined treatment group were given HBO therapy combined with swallowing training and Fengchi acupoint acupuncture. Modified Kubota-Chiari's swallowing functional classification was used to assess swallowing function before and on the 3rd d,6th d,9th d, 12th d, 15th d, 18th d and 21st d after treatment. Results It was shown that the patients in all groups but the control group got significant improvement in swallowing after treatment, as demonstrated by the swallowing function scores (P < 0.05). Moreover, the priority order in im-provement of swallowing function and recovering rate were: combined treatment group > acupuncture group > swal-low training group > HBO therapy group > control group(P < 0.05). Conclusion The combination of HBO ther-apy, swallowing training and Fengchi acupoint acupuncture could act synergically to ameliorate dysphagia caused by pseudobulbar paralysis after stroke.

@#Objective To observe the effect of hyperbaric oxygen,rehabilitation training or acupuncture at Fengchi acupoint on pseudobulbar paralysis after stroke.Methods 90 patients were divided into group Ⅰ(A,B,C) or Ⅱ(A,B,C) randomly.Patients in all groups were given conventional therapy,while those in groups ⅠB and ⅡB were given hyperbaric oxygen,ⅠC and ⅡC were given swallowing training and group Ⅱ was given acupuncture at Fengchi acupoint additionally.They were assessed with Modified Kubota-Chiari's swallowing functional classification before and after treatment.Results In all groups except ⅠA,swallowing function scores improved after treatment significantly(P<0.05).The scores were different significantly between group Ⅰ and group Ⅱ after treatment(P<0.05).Except 3 d after treatment,the scores were different significantly between sub-group of group Ⅰ or group Ⅱon other time points(P<0.05).Conclusion The hyperbaric oxygen is more effective than conventional therapy on pseudobulbar paralysis,but less than swallowing training.Acupuncture at Fengchi acupoint can improve their efficacy.

Objective: To investigate the efficacy of scalp acupuncture in combination with body acupuncture for treating pseudobulbar paralysis. Methods: Eighty patients were randomly divided into treatment and control groups, 40 cases each. The control group was treated by the routine method of Western medicine and the treatment group by combined scalp and body acupuncture on the basis of the former. The curative effect was evaluated after treatment in both groups. The influence of the treatment on mean blood velocity(MBV) in anterior, middle and posterior cerebral arteries was observed by transcranial Doppler(TCD) in both groups. Results: The total efficacy rate was 97.5% in the treatment group and 12.5% in the control group. The recovery rate was 75% in the treatment group and 0% in the control group. There was a significant difference between the two groups(P< 0. 01). TCD showed that blood velocity in cerebral arteries was significantly increased and the unbalanced stasis of the right and left cerebral blood flow changed in the treatment group. A comparison of MBV between pretreatment and posttreatment showed P < 0.01. Conclusion: Scalp acupuncture in combination with body acupuncture has a good effect on pseudobulbar paralysis.

RESUMEN

Objective: To investigate the curative effect of combined acupuncture and medicine on pseudobulbar paralysis. Method:The patients were allocated into an acupuncture group of 50 cases and a control group of 48 cases by completely random sampling. The acupuncture group received western medicine combined with acupuncture and the control group received simple Western medicine. Results:The effective rate was 92.0% in the acupuncture group and 62.5% in the control group. There was a very significant difference in curative effect between the two groups(P< 0.01). Conclusion: The curative effect of combined acupuncture and medicine on pseudobulbar paralysis is significantly better than that of simple Western medicine.

RESUMEN

152 patients with pseudobulbar paralysis resulting from ischemic cerebral infarction were treated with polysaccharide sulphate (PSS) or Troxerutin (TR) and their clinical curative effect and laboratory parameters were studied. 76 patients (M46,F30,mean ages 62+6 y),were treated with PSS:PSS 150 mg in 5% Glocose Solution 500 mL iv. drip. qd,for 14 days as a course. The other 76 patients (M46,F30,mean age 63 + 7 y) were treated with (TR);TR 600 mg in hydroxyethyi-starch 500 mL iv. drip ,qd .for 14 days as a course. The results showed that the total effective rates of the two groups were 94. 74% and 77. 63% .respectively.
