RESUMEN

Objective:To explore the correlation between the ratio of C-reactive protein (CRP) to albumin (CAR) and the syndrome type of Wei-Qi-Ying-Xue in patients with severe coronavirus disease 2019 (COVID-19).Methods:A case-control study was conducted to select 63 severe patients with COVID-19 admitted to the Dongzhimen Hospital of Beijing University of Chinese Medicine from December 2022 to December 2023, including 50 severe cases and 13 critical cases. The clinical data of the patients were collected. According to the syndrome differentiation of Wei-Qi-Ying-Xue, there were 21 cases of Qi syndrome, 20 cases of Ying syndrome and 22 cases of Xue syndrome. The differences of CRP, ALB and CAR levels in patients with different Wei-Qi-Ying-Xue syndromes were compared. Spearman correlation test was used to test the correlation between CRP, ALB, CAR and the Wei-Qi-Ying-Xue syndrome type, and the receiver operating characteristic (ROC) curve was used to detect the diagnostic efficacy of CRP, ALB and CAR on the Wei-Qi-Ying-Xue syndrome type. Results:There was a statistically significant difference in the clinical classification of Western medicine among the three groups ( P<0.05). The CAR of the Ying group and the Xue group was higher than that of the Qi group ( P<0.05), while there was no statistically significant difference in age and comorbidities (all P>0.05). The CRP of the Xue group was higher than that of the Qi group ( P<0.05), and the ALB of the Ying group and the Xue group was lower than that of the Qi group (all P<0.05). Correlation analysis showed that there was a correlation between the Wei-Qi-Ying-Xue syndrome type and CRP, ALB and CAR ( P<0.05), among which CAR changed most significantly with the change of Wei-Qi-Ying-Xue syndrome type. ROC curve analysis showed that CRP, ALB and CAR had good diagnostic value for Qi syndrome and Xue syndrome ( P<0.05). The critical values of the diagnosis of Qi syndrome were 48.57 mg/L, 34.20 g/L and 2.97. The critical values of the diagnosis of Xue syndrome were 28.30 mg/L, 26.6 g/L and 5.96. Conclusions:CAR ratio is correlated with the Wei-Qi-Ying-Xue syndrome type of severe COVID-19 patients, and its level changes are in line with the evolution law of Wei-Qi-Ying-Xue syndrome. CAR≤2.97 is contributed to the diagnosis of Qi syndrome, and CAR>5.96 is contributed to the diagnosis of Xue syndrome. CAR may be an objective index related to the Wei-Qi-Ying-Xue syndrome type of severe COVID-19 patients.

RESUMEN

TCM proposes that the core pathological mechanism of depression is"deficient qi with stagnation and heat",with the following pathogenic characteristics and evolution patterns:"deficient qi"as the nature, and deficiency in nature is in spleen, and deficiency in superficiality is in brain;"stagnation"is the superficiality, and qi stagnation, phlegm stagnation, and blood stagnation are in the brain collaterals;"heat"fires the brain collaterals, depression raised the heat, and excessive heat accumulated to stagnation.Based on the understanding of the pathogenesis of depression caused by stress sensitization in modern medicine, this article explored the potential association between this mechanism and the core pathogenesis of"deficient qi with stagnation and heat".It proposed that tonifying deficiency, promoting circulation, and clearing heat are the basic treatment principles for depression.By inhibiting inflammatory reactions and improving the stress sensitization state of neurons and glial cells,TCM compound formulas can exert multi-target and multi-dimensional therapeutic characteristics.

RESUMEN

Chronic heart failure(CHF)is a common and frequent clinical condition with complex pathogenesis. The micro-inflammatory state exists in the development of CHF and is manifested by a continuous increase in inflammatory proteins and inflammatory cytokines in the circulation, affecting the body's metabolism and immune function.Micro-inflammatory state of CHF belongs to the syndrome of deficiency in nature and excess in superficiality in the TCM field. The basic pathogenesis of CHF is"deficiency qi retention";the nature is the deficiency of heart energy and the superficiality is retention of qi,blood stagnation, phlegm, water and dampness, with"deficiency, stagnation and water"running throughout the disease. Therefore, based on the theory of"deficiency qi retention",this article took"reinforcing deficiency and removing stagnation"as its basic treatment method, regulated the body's immunity and inflammatory indicators such as serum high-sensitivity C-reactive protein, tumor necrosis factor-α,in order to alleviate the micro-inflammatory state, which is of great significance for the prevention and treatment of CHF.

RESUMEN

The two injuries of fluid and qi are the pathological basis of atrophy syndrome, and also the result of the imbalance of"yang transforming into qi,yin forming shape".Deficiency of yang transforming into qi,imbalance of yin forming shape would result in dysfunction of visceral functions, internal generation of water, phlegm, and blood stasis, and loss of nourishment in limbs and nine orifices, and then later result in atrophy syndrome.Based on the theory of"yang transforming into qi,yin forming shape",this article discussed Professor Gao Ying's treatment of myasthenia gravis with insufficient yang transforming into qi by using the method of promoting yang transforming into qi;the method of tonifying the kidney and regulating the shape could be used to treat multiple sclerosis with imbalance in yin forming shape;yin-yang tonifying method could be used to treat motor neuron disease with both yin and yang damaged. In the treatment of atrophy syndrome,it is necessary to examine the syndrome and seek the cause, distinguish between yin and yang, adjust biases, and provide TCM treatment in the early stages of the disease to delay the progression of the disease and improve prognosis.

RESUMEN

[Objective]To explore the etiology, pathogenesis and clinical treatment of histiocytic necrotizing lymphadenitis(HNL)based on the insidious pathogen warm disease theory.[Methods]To analyze the etiology, pathogenesis, characteristics of symptoms and transmission of the disease in Chinese medicine, and summarize the treatment principles based on the descriptions about insidious pathogen warm disease in ancient literature and modern researches on HNL,and cite a clinical case for verification.[Results]The pathogenesis of the HNL is mainly characterized by deficient healthy Qi leading to latent evil. The specific manifestation is that exogenous evils are latent in the Moyuan, which causes Yang Qi to be blocked and depressed and turns into heat.Evil heat spreads from the Moyuan to the Shaoyang tri-Jiao,leading to the generation of phlegm, static blood, turbid evil and toxin in the body, which in turn leads to disease. In terms of treatment, supporting healthy Qi and eliminating the evil is regarded as the law of treatment, and the emphasis is eliminating the evil.Aiming at the three pathogenetic links of latent evil, depressed heat and internal production of pathological products, the following treatment principles are formulated:expelling evils from Moyuan, dredging tri-Jiao and promoting the flow of Qi to make the evil heat go out, clearing away endogenous pathological products, removing toxin and dispersing knots. The focus of supporting healthy Qi is the protection of Qi and Yin as well as recuperation after recovery, which prevents evil Qi from remaining and causing the disease to reoccur. The medical case cited was a patient with HNL treated by applying the theory of insidious pathogen warm disease.Damp-heat and toxin brewing, phlegm combined with static blood was the traditional Chinese medicine(TCM)pattern of this case. The the prescriptions were based on Shengjiang Powder combined with Sanren Decoction, and Ganlu Xiaodu Pill successively, added and subtracted according to the syndrome, and the case achieved a significant effect.[Conclusion]The effect of treating HNL based on the insidious pathogen warm disease theory is quite good, which can provide new ideas and methods for the diagnosis and treatment of HNL.

RESUMEN

This paper summarized Professor LI Haisong's clinical experience in treating spermatorrhea based on the view of "spirit controlling essence and qi". It is emphasized that the heart spirit has the function of controlling the essence and qi of the human body, believing the frenetic stirring of heart spirit and the insecurity of essence gate are the core pathogenesis of spermatorrhea, LI advocates to regulate the heart spirit first and take into account of the zang-fu organs, essence and qi simultaneously for the treatment. Treatment should be performed according the clinical syndromes differentiatied. For those with heart spirit failing to nourish syndrome, it is recommended to supplement heart qi, nourish spirit and consolidate essence with self-made Yangxin Mijing Formula (养心秘精方). In case of heart fire hyperactivity, the method of clearing heart heat and draining fire, calming spirit and consolidating essence should be used, and self-made Xiexin Gujing Formula (泻心固精方) is recommended. For heart-liver qi constraint, it is advised to soothe the liver and calm heart, calm the mind and regulate essence with self-made Jieyu Anshen Tiaojing Formula (解郁安神调精方) which is a modifcation to Chaihu Shugan Powder (柴胡疏肝散). In terms of deficiency of both heart and liver, the treatment principle is supplementing spleen and nourishing heart, calming the mind and controlling essence, for which self-made Xinpi Tongtiao Shejing Formula (心脾同调摄精方) modified from Guipi Decoction (归脾汤) can be used. For deficiency of both heart and kidney, it is better to nourish the kidney and calm heart, calm the mind and consolidate essence with self-made Xinshen Liangzi Tianjing Formula (心肾两滋填精方) that modified from Shuilu Erxian Elixir (水陆二仙丹) and Wuzi Yanzong Pill (五子衍宗丸). Prescriptions are used to treat the root by harmonizing the zang-fu organs, nourish the spirit by regulating qi and blood, and calm the mind by taking special medi-cinals, and they should be flexibly modified according to the disease.
