Abstract Background: Patients with higher thrombus burden have higher procedural complications and more long-term adverse cardiac events. Detecting patients with high thrombus burden (HTB) before coronary intervention could help avoid procedural complications. Objective: The research aimed to analyze the R wave peak time (RWPT) on the electrocardiogram to predict thrombus burden before coronary angiography in patients with acute ST-segment elevation myocardial infarction (STEMI). Materials and Methods: A total of 159 patients with STEMI were included in the study conducted at a tertiary medical center. The thrombolysis in myocardial infarction (TIMI) thrombus scale was applied to assess the thrombus burden. TIMI thrombus grades 0, 1, 2, and 3 were accepted as low; 4 and 5 had HTB. RWPT was measured from the beginning of the QRS complex to the R-peak from the leads pointing to the infarct-related artery. Results: Patients were divided into two groups according to their angiographically defined thrombus burden as low and high. The low thrombus burden group (LTB) comprised fifty-four patients, whereas the HTB group comprised 105 patients. In the LTB group, RWPT was 47.96 ± 9.17 ms, and in the HTB group was 53.58 ± 8.92 ms; it was significantly longer (p < 0.01). Receiver operating characteristic analysis showed that a cut-off value of preprocedural RWPT of > 46.5 ms predicted the occurrence of HTB with a sensitivity and specificity of 87.62% and 51.85%, respectively (AUC 0.682, 95% CI 0.590-0.774, p < 0.001). Conclusion: The present study evaluated the relationship between the RWPT and thrombus burden in STEMI patients. Based on the results, RWPT is an independent predictor of HTB.

Learning Objectives: Learn how to identify and differentiate the shark fin EKG pattern from other EKG patterns and abnormalities. Explore the potential diagnostic and prognostic implications of the shark fin EKG pattern, including its association with increased risk of arrhythmias and sudden cardiac death. Background: The Lambda-wave ECG, also known as the Shark fin or Thombostone pattern, has been linked to ST-elevation myocardial infarction's (STEMI) hazardous implications. It is characterized by merged QRS, ST, and T waves, forming a 'triangular QRS-ST-T waveform' or 'giant R waves.' This treacherous signature poses an alarming risk, with chances of cardiogenic shock and ventricular fibrillation leading to fatality. To tackle this peril, implementing swift thrombolysis or percutaneous intervention for reperfusion is critical for successful treatment. For optimal outcomes, the preferred setting to utilize ventricular assist devices is the ICU. Misdiagnosis can happen and be seen as either wide complex tachycardia or ECG changes induced by hyperkalemia. Imperative to prevent severe complications, early detection and treatment are crucial. We are reporting three cases of this Ekg pattern.

The automatic detection of arrhythmia is of great significance for the early prevention and diagnosis of cardiovascular diseases. Traditional arrhythmia diagnosis is limited by expert knowledge and complex algorithms, and lacks multi-dimensional feature representation capabilities, which is not suitable for wearable electrocardiogram (ECG) monitoring equipment. This study proposed a feature extraction method based on autoregressive moving average (ARMA) model fitting. Different types of heartbeats were used as model inputs, and the characteristic of fast and smooth signal was used to select the appropriate order for the arrhythmia signal to perform coefficient fitting, and complete the ECG feature extraction. The feature vectors were input to the support vector machine (SVM) classifier and K-nearest neighbor classifier (KNN) for automatic ECG classification. MIT-BIH arrhythmia database and MIT-BIH atrial fibrillation database were used to verify in the experiment. The experimental results showed that the feature engineering composed of the fitting coefficients of the ARMA model combined with the SVM classifier obtained a recall rate of 98.2% and a precision rate of 98.4%, and the

Objective: Poor R wave progression in right precordial leads is a relatively common electrocardiogram (ECG) finding that indicates possible prior anterior myocardial infarction (MI); however, it is observed frequently in apparently normal individuals. In contrast, reversed R wave progression (RRWP) may be more specific to cardiac disorders; however, the significance of RRWP in daily clinical practice is unknown. The purpose of this study was to clarify the significance of RRWP in clinical practice.Materials and Methods: We analyzed consecutive ECGs obtained from 12,139 patients aged ≥20 years at Mito Kyodo General Hospital in Ibaraki between November 2009 and August 2012. Our setting is a secondary emergency hospital in the community, and the study participants were inpatients or patients who visited the general or emergency outpatient departments. RRWP was defined as RV2 < RV1, RV3 < RV2, or RV4 < RV3. Regarding ECGs considered to show RRWP, we confirmed the presence or absence of an abnormal Q wave and whether ultrasound cardiography, contrast-enhanced computed tomography, coronary angiography, and/or left ventriculography were performed to obtain detailed information.Results: RRWP was identified in 34 patients (0.3%). Among these patients, 29 (85%) had undergone cardiac evaluation. The final diagnosis was previous anterior MI in 12 patients (41%) and ischemic heart disease (IHD) without MI in 5 patients (17%). All 17 patients with IHD had left anterior descending (LAD) artery stenosis. The other patients were diagnosed with dilated (two patients, 7%) and hypertrophic (one patient, 3%) cardiomyopathy, left ventricular hypertrophy (one patient, 3%), or pulmonary embolism (one patient, 3%). Only seven patients (24%) were normal.Conclusions: RRWP is rare in daily clinical practice; however, it is a highly indicative marker for cardiac disease, particularly IHD with LAD artery stenosis.

Objectives To evaluate the values of notched on R wave in inferior leads of electrocardiogram in infant with secundum atrial septal defect. Methods To observe and compare the prevalence of notch on R wave in inferior leads combined with incomplete right bundle branch block (IRBBB) in 162 cases with secundum atrial septal defect (group I) and 162 cases without heart disease (group II). Results The prevalence of notch on R wave in all the three inferior leads, in at least one inferior lead with IRBBB and in all the three inferior leads with IRBBB were 27.16%, 14.20%and 10.49%respec-tively in group I, and were 3.09%, 1.85%and 0.62%in group II respectively, and the differences between two groups were statistically significant (P<0.005). The specificities of notch on R wave in diagnosis of atrial septal defect were 96.91%, 98.15% and 99.38% respectively. Conclusions Notch on R wave in inferior leads is an independent electrocardiographic sign of secundum atrial septal defect in infants, and thus can be used as a diagnostic parameter.

RESUMEN

A realtime system used for acquisition and analysis of ECG based on LabVIEW is presented in this paper.This realtime system can accomplish the functions such as the acquisition and display of ECG and the calculation of Instantaneous Heart Rate(IHR)and the Average Heart Rate(AHR).The system can be implemented easily.In addition,the user can modify and add functions of the system according to his needs conveniently.

RESUMEN

This paper introduces a practical and real -time R wave detection algorithm as well as its realization and adjustment by DSP/BIOS of RTOS. With DSP/BIOS of RTOS applied, the adjustment and evaluation of real-time program for ECG are facilitated and the program stability is enhanced.

BACKGROUND: Exercise electrocardiogram is the most widely used non-invasive test in those patients suspected of having coronary artery disease. However, sensitivity and specificity of this test are not satisfactory, especially when the exercise induced ST-segment depression is used as a single criterion of myocardial ischemia. Although many investigators have tried to improve diagnostic accuracy with R-wave amplitude change in addition to ST-segment depression, controversy exists whether this new criteria improve the test accuracy for coronary artery disease or not. The purpose of this study is to determine the test accuracy of R-wave amplitude change induced by exercise combined with the conventional ST-segment criterion for myocardial ischemia. METHODS: We reviewed our records of patients who visited to Korea University Anam Hospital with chest pain from January, 1998 to July, 1999. We included 130 patients with chest pain who had a tredmill test followed by a coronary angiography within 2 months. Patients with change in ST-segment depression (delta STD)> or = 1.00 mm and delta STD> or = 1.00 mm with R wave amplitude decrease (-delta R)> or = 1.00 mm in the same lead in any of 12 leads and delta STD> or = 1.00 mm with R wave amplitude increase (+delta R)> or = 1.00 mm in the same lead in any of 12 leads were compared. According to the findings of coronary angiograms, patients were divided into 4 groups ; normal coronary artery group, mild coronary artery stenosis group (> or = 30% to or = 50% to or = 70%). RESULTS: Fifty three patients (40.7%) had normal coronary angiograms and 77 patients (59.3%) had coronary artery stenosis. There was no significant difference in gender and age. But, patients with coronary artery stenosis had more diabetes mellitus, hyperlipidemia, smoking, previous myocardial infarction history and angina during exercise testing than those without coronary artery stenosis. The sensitivity of exercise EKG is significantly decreased when combined with delta R (delta STD, 74.0%, delta STD with -delta R, 45.5%, delta STD with +delta R, 30.0%, p<0.01), but the The test accuracy is delta STD; 73.7%, delta STD with -delta R; 61.5%, delta STD with +delta R; 57.7%. CONCLUSION: When ST-segment depression is combined with R-wave amplitude change as a marker for myocardial ischemia, the specificity is increased, but the test accuracy of exercise EKG is not improved.

Diabetic complications such as cystopathy and retrograde ejaculation are slowly progressive disease without any specific signs and symptoms so it is difficult to diagnose its complication and prevent it. Diabetic autonomic neuropathy is known as a neurologic lesion with combined involvement in cardiovascular and genitourinary systems. The purpose of this study is to assess correlation between diabetic cystopathy and heart rate variation on consecutive EKG waves and to detect early cystopathy in patients with decreased heart rate variation. We have studied 12 cases of diabetic cystopathy and 9 cases of retrograde ejaculation who were managed in Seoul Eulji Hospital from Mar. 1992 to Aug. 1993 and concluded as follows. Diabetic patients with autonomic neuropathy shows decreased heart rate variability(HRV) (CV%: 2.16+/-0.97) than diabetic patients without autonomic neuropathy (CV%: 3.29+/-0.58, P <0.0001 ), it means that decreased HRV on consecutive EKG waves well correlates to diabetic autonomic neuropathy such as cystopathy and retrograde ejaculation so we think that the measurement of HRV provides a simple test to be concerned urologic complications such as cystopathy or retrograde ejaculation.
