RESUMEN

En la relación de pareja, el empleo del celular puede llegar a ocasionar conflictos, lo que afectaría la valoración de la unión conyugal. Objetivo: Determinar la asociación entre la satisfacción con la relación de pareja y los conflictos por el uso del celular en adultos peruanos. Materiales y Métodos. Estudio cuantitativo, de tipo no experimental, transversal y correlacional. La muestra fue constituida por 325 adultos peruanos mediante un muestreo no probabilístico. La información se recolectó mediante la técnica de la encuesta y como instrumento dos cuestionarios, la Escala de Valoración de la Relación propuesto por Hendrick y la Escala de Conflictos Relacionados al uso del celular de Roberts y David. Resultados. La satisfacción con la relación de pareja fue mayormente moderada para el 43,5%. Entre tanto, indicaron algún nivel de conflicto por el uso del celular el 72,9%. Además, no se encontró diferencias significativas en la satisfacción con la relación de pareja según el sexo. Conclusiones. Existe relación negativa moderada y significativa entre la satisfacción con la relación de pareja y los conflictos por el uso del celular en adultos peruanos (rs= -0.456), donde se alcanzó una p=0,000 (<0.05). De esta manera se sostiene que a mayor satisfacción, menores serán los conflictos percibidos por el uso del celular.


In the couple relationship, the use of cell phones can lead to conflicts, which would affect the assessment of the marital union. Objective: To determine the association between satisfaction with the couple relationship and conflicts due to cell phone use in Peruvian adults. Materials and Methods. Quantitative, non-experimental, cross-sectional and correlational study. The sample consisted of 325 Peruvian adults through non-probabilistic sampling. The information was collected using the survey technique and two questionnaires as instruments, the Relationship Assessment Scale proposed by Hendrick and the Conflicts Related to Cell Phone Use Scale by Roberts and David. Results. Satisfaction with the couple relationship was mostly moderate at 43.5%. Meanwhile, 72.9% indicated some level of conflict due to cell phone use. Furthermore, no significant differences were found in satisfaction with the couple relationship according to sex. Conclusions. There is a moderate and significant negative relationship between satisfaction with the couple relationship and conflicts due to cell phone use in Peruvian adults (rs= -0.456), where p=0.000 (<0.05) was reached. In this way, it is argued that the greater the satisfaction, the fewer the perceived conflicts due to cell phone use.

Objetivo: Determinar a associação entre a satisfação com o relacionamento conjugal e os conflitos devido ao uso de celular em adultos peruanos. Materiais e Métodos. Estudo quantitativo, não experimental, transversal e correlacional. A amostra foi constituída por 325 adultos peruanos através de amostragem não probabilística. As informações foram coletadas utilizando a técnica de survey e dois questionários como instrumentos, a Escala de Avaliação de Relacionamento proposta por Hendrick e a Escala de Conflitos Relacionados ao Uso de Celular de Roberts e David. Resultados. A satisfação com o relacionamento conjugal foi predominantemente moderada em 43,5%. Enquanto isso, 72,9% indicaram algum nível de conflito devido ao uso de celular. Além disso, não foram encontradas diferenças significativas na satisfação com o relacionamento conjugal de acordo com o sexo. Conclusões. Existe uma relação negativa moderada e significativa entre a satisfação com o relacionamento conjugal e os conflitos devido ao uso de celular em adultos peruanos (rs= -0,456), onde foi atingido p=0,000 (<0,05). Dessa forma, argumenta-se que quanto maior a satisfação, menores serão os conflitos percebidos devido ao uso de celular.

ABSTRACT Introduction: A systemic instrument is presented to measure the socioemotional network in relation to the partner and the person's perception of the impact of this intimate network on his or her partner for his or her classification. It is based on the idea that a nurtured social network brings positive benefits to one's nuclear partner. In order to verify this assumption both in research and in clinical practice, it is necessary to construct a complex instrument that allows reaching different dimensions within and outside the couple. Objective: The study seeks the construction and validation of the Multidimensional Couple scale to measure seven dimensions in the couple: emotional, cognitive, physical interest, protection, trust, respect and power, as well as an additional dimension to classify the type of couple. Method: An Exploratory Factor Analysis (EFA) and Exploratory Factor Analysis (CFA) were carried out to test the psychometric properties and the adequacy to the theoretical model. A total of 1149 people (71.5% women and 28% men) living in Mexico participated. Result: The presence of a scale formed by 7 dimensions in the couple and a second order factor is confirmed, which can be applied both by adjusting the answers to the couple itself and to other people different from the couple. The goodness-of-fit and reliability indices are satisfactory. Conclusion: This scale provides a psychometric instrument that allows the study of the relationship between the couple.


RESUMEN Introducción: Se presenta un instrumento de origen sistémico para medir la red socioemocional en relación con la pareja y la percepción de la persona sobre el impacto de esta red íntima en su pareja para su clasificación. Se parte de la idea de que una red social nutrida aporta beneficios positivos en la propia pareja nuclear. Para la comprobación de este supuesto tanto investigación como en la práctica clínica, se hace necesaria la construcción de un instrumento complejo que permita alcanzar diferentes dimensiones dentro y fuera de la pareja. Objetivo: El estudio busca la construcción y validación de la escala La Pareja Multidimensional para medir siete dimensiones en la pareja: emocional, cognitiva, interés físico, protección confianza, respeto y poder, así como una dimensión adicional que permita clasificar el tipo de pareja. Método: Se ha llevado a cabo un Análisis Factorial Exploratorio (EFA) y Análisis Factorial Exploratorio (CFA) para comprobar las propiedades psicométricas y la adecuación al modelo teórico. Participó un total de 1149 personas (71.5% mujeres y 28% hombres) residentes en México. Resultados: Se confirma la presencia de una escala formada por 7 dimensiones en la pareja y un factor de segundo orden, que se puede aplicar tanto adecuando las respuestas a la propia pareja como a otras personas diferentes a esta. Los índices de bondad de ajuste y de fiabilidad son satisfactorios. Conclusión: Con esta escala se aporta un instrumento psicométrico que permite estudiar las diferentes dimensiones de la pareja y cómo estas se alimentan en base al establecimiento de relaciones con personas ajenas a la misma.

OBJECTIVE To establish HPLC fingerprint of Shuangdong capsules, and to study the spectral effect relationship of its anti-inflammatory effect. METHODS The fingerprints of 15 batches of Shuangdong capsules were established by HPLC,and the similarity evaluation was carried out; the foot swelling model was established to investigate the anti-inflammatory activity of Shuangdong capsules. The gray correlation analysis method was used to construct the spectral effect relationship for the anti- inflammatory effect of Shuangdong capsules using the swelling rate of rat foot and the levels of malondialdehyde (MDA), superoxide dismutase (SOD), prostaglandin E2 (PGE2), interleukin-6 (IL-6), IL-8, IL-1β and tumor necrosis factor-α (TNF-α) in right hindfoot tissues as the pharmacodynamic indexes of anti-inflammatory effects. RESULTS Overall 15 batches of Shuangdong capsules identified 20 common peaks, the similarities were all greater than 0.97, and a total of 8 chromatographic peaks were identified. According to the gray correlation analysis, the correlation degrees between the peak area and the foot swelling rate and the levels of MDA, SOD, PGE2, IL-6, IL-8, IL-1β and TNF-α in 15 batches of Shuangdong capsules were 0.621 1- 0.783 5, 0.564 3-0.827 9, 0.581 0-0.845 3, 0.564 9-0.855 0, 0.583 1-0.856 4, 0.576 5-0.863 5, 0.564 1-0.838 0 and 0.572 5- 0.851 3, respectively. Among them, the chemical components represented by peak 4 (geniposidic acid), peak 10 (chlorogenic acid) and the chemical composition represented by peak 2 were strongly correlated with anti-inflammatory efficacy indicators. CONCLUSIONS In this study, HPLC fingerprints of 15 batches of Shuangdong capsules were successfully established. Among them, geniposidic acid, chlorogenic acid may be its anti-inflammatory ingredients.

Polysaccharides are the important material basis of traditional Chinese medicine(TCM), and have various pharmacological activities such as immunomodulation, antitumor and anti-aging. Due to the large molecular weight of TCM polysaccharides, their structural analysis and oral absorption mechanism are facing technical challenges, and the current research on their structure-activity relationships has made some breakthroughs, while the research on their oral absorption mechanisms is relatively slow. In-depth study of the oral absorption mechanism of TCM polysaccharides is not only crucial for the interpretation of their action pathways and efficacy in vivo, but also helpful for the interpretation of their pharmacological effects, rational clinical applications and the discovery of new targets. In recent years, the application of fluorescent labeling and isotopic labeling methods has provided new technical means for the oral absorption studies of polysaccharides, which has promoted the development of oral absorption studies of TCM polysaccharides. In this paper, we reviewed the oral absorption pathways and labeling techniques of TCM polysaccharides, and concluded that they can be absorbed orally through transmembrane, cellular bypass, and M-cell-mediated transport, of which transmembrane pathway is the main absorption pathway, and summarized the labeling reactions of four fluorescent labeling and isotopic labeling methods with TCM polysaccharides, which can provide references for evaluating the absorption pathways of TCM polysaccharides, screening active TCM polysaccharides, establishing pharmacodynamic models and comprehensively elucidating the mechanism of TCM polysaccharides.

BACKGROUND:In implant restoration in the aesthetic area,zirconium dioxide is gradually becoming the most commonly used upper restorative material and has achieved better clinical results.Resin-ceramic composite,a new type of dental restorative material,begins to try to be used as an upper restorative material in implant restoration,but there is less research on the application of this material in implant restoration. OBJECTIVE:To compare the biomechanical differences between resin ceramic crowns and zirconia all-ceramic crown implant restorations in three occlusal relationships for restoring maxillary central incisors. METHODS:The cone-beam CT image data of a patient with single-crown implant restoration of maxillary central incisor were selected,and the maxillary bone model was extracted by using Mimics 21.0 software,and the model was imported into Solidworks 2020 software.The crown,adhesive,abutment,central screw,and implant were modeled,and the model of single-crown implant restoration of maxillary central incisor was assembled.After giving the model material property parameters(resin-ceramic composite and zirconia for the upper restoration materials)in ANSYS Workbench 2021 R1 software,three occlusal relationships(edge-to-edge occlusion,normal overjet and deep overbite)were simulated and loaded to analyze the stress distribution of the resin-ceramic crown and zirconia all-ceramic crown implant restoration models. RESULTS AND CONCLUSION:(1)The stress concentration areas in the implant restoration models of the resin-ceramic crown group and the zirconia all-ceramic crown group in different occlusal relationships were distributed in the upper restoration loading point,the abutment-implant connection,the implant neck and the surrounding bone tissue.As the occlusal relationship changed from the edge-to-edge to normal and deep overbite,the peak equivalent forces of the restorative abutment,central screw,implant,and bone tissue in both the resin-ceramic and zirconia all-ceramic crown groups gradually decreased.The highest peak equivalent forces were observed for the upper restorations in deep overbite.The zirconia all-ceramic crown group had the highest peak equivalent force in the adhesive layer in the edge-to-edge relationship,and the resin-ceramic crown group had the highest peak equivalent force in the adhesive layer in the deep overbite.(2)In the edge-to-edge occlusion,the peak equivalent force of the adhesive layer and central screw in the resin-ceramic crown group was slightly smaller than that in the zirconia all-ceramic crown group,and there was no significant difference between the two groups in the peak equivalent force at the upper restoration,restoration abutment,implant,and bone tissue.The peak stresses in the upper restoration,adhesive layer,and central screw of the resin-ceramic crown group were slightly less than those of the zirconia all-ceramic crown group at normal fit,and there were no significant differences between the two groups in the peak equivalent forces at the restoration abutment,implant,and bone tissue.In deep overbite,the peak adhesive,abutment,and central screw stresses were greater in the resin-ceramic crown group than in the zirconia all-ceramic crown group,with no significant differences in the upper restorations,implants,or bone tissue.(3)The results showed that the upper restorative material had no significant effect on the stress distribution of the implant and bone tissue,and had some effects on the stress distribution of the upper restoration,adhesive,restoration abutment,and central screw,but the difference was not significant.The occlusal relationship has a significant influence on the stress distribution in all structures and bone tissue of the implant restoration.The resin-ceramic crowns have a buffering effect on the stresses in the case of edge-to-edge and normal occlusion.

BACKGROUND:Reactive oxygen species may be closely related to the occurrence and development of tendinopathy,but its exact role and related signal transduction mechanism have not been fully summarized. OBJECTIVE:To review current clinical or preclinical original studies,summarize the role of reactive oxygen species in tendinopathy and related signal transduction pathways and to explore its characteristics and whether there is a unified downstream pathway. METHODS:Relevant original studies in PubMed,Embase,Web of Science,as well as CNKI,WanFang,and VIP databases were searched by computer and the search results were screened and excluded according to the inclusion criteria.Ninety articles were finally included for review and analysis. RESULTS AND CONCLUSION:Reactive oxygen species affects the direction of tendon healing by simultaneously acting on tendon cells and the extracellular matrix,and it exhibits a bifacial effect in the treatment of tendinopathy.Concentration of reactive oxygen species may be the key to determining its direction of action.The possibility that low-dose reactive oxygen species can participate in the normal physiological healing of tendons or that tendon tissues are adaptive to stimulations may be the underlying mechanism that produces this characteristic effect.Reactive oxygen species affect the composition and structure of the extracellular matrix and normal tendon repair as well as maintain viability in response to external stimulations through matrix metalloproteinases,mitogen-activated protein kinases,mitochondrial apoptosis,the forkhead transcription factor O family,autophagy,inflammation,and antioxidant signaling pathways.Different reactive oxygen species stimulation intensities,durations,and external environments may cause different alterations in downstream molecular pathways and thus have different effects on the tendon.Due to the large gap in the number of literature included in the evaluation of the positive and negative effects of reactive oxygen species,it may cause some analytical error in the search for factors behind the characteristics of the action of reactive oxygen species in tendon.In addition,most experimental intervention conditions and results of interest are relatively homogeneous;therefore,the temporal and quantitative mechanisms of reactive oxygen species and the synergistic effects with other intervention factors have not been clarified,and the overall system of molecular actions of reactive oxygen species in tendinopathy has not been constructed.To conclude,reactive oxygen species might be involved in the treatment and prevention of tendinopathies as a beneficial factor in the future,and facilitate the exploration of oxidative stress signaling pathways and overall molecular action systems in tendinopathies thereafter,as well as lay the foundation for research on the therapeutic strategies of different antioxidants in tendinopathies to better prevent and treat tendon injury and degeneration.

BACKGROUND:Frog active peptides have rich activities,such as antibacterial and anti-tumor,and are expected to solve the problem of antibiotic resistance. OBJECTIVE:The active peptide QUB2984 was discovered in the skin secretions of Agalychnis callidryas.Its structure and properties were simulated by bioinformatics.The peptide was synthesized,purified,and identified and its biological functions were investigated. METHODS:Agalychnis callidryas skin secretions were collected by electrostimulation.The sequence of QUB2984 was obtained through constructing a cDNA library with isolated mRNA.BLAST was used for peptide sequence alignment.Besides that,Iterative Threading ASSEmbly Refinement(I-TASSER)and HeliQuest tools were used for protein secondary structure simulation.It was synthesized by solid-phase peptide synthesis,purified by reverse-phase high-performance liquid chromatography,and structurally confirmed by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry.The purified peptide was used to evaluate its biological activity.Its antibacterial effect was evaluated by the minimum inhibitory concentration method.Its cytotoxic effect was detected by MTT assay.Its safety was investigated by a hemolysis test. RESULTS AND CONCLUSION:(1)Peptide QUB2984 had basically α-spiral structure,with a relatively intact hydrophobic surface,and a certain destructive ability to biofilm.The third amino acid position of QUB2984 was composed of W and had a G-X-G structure.(2)The minimum inhibitory concentration of QUB2984 against gram-positive Staphylococcus aureus was 2 μmol/L,the minimum inhibitory concentration against gram-negative Escherichia coli was 2 μmol/L,and the minimum inhibitory concentration against the fungus Candida albicans was 8 μmol/L.(3)The active peptide QUB2984 had obvious inhibitory effect on human non-small cell lung cancer cells NCI-H838 at 10-5 mol/L concentration,and the hemolytic effect on horse red cells at 64 μmol/L concentration was 50%.(4)The results showed that QUB2984 had anti-bacterial and anti-cancer activity,and it had a positive charge of +3,which was conducive to contact with bacteria or cells.

BACKGROUND:Observational studies have suggested that statins may have a protective effect against osteoarthritis,including knee osteoarthritis and hip osteoarthritis.However,the association between statins and the risk of osteoarthritis remains unclear. OBJECTIVE:To investigate the association between statins and the risk of osteoarthritis through Mendelian randomization analysis using summary data from large-scale population-based genome-wide association studies(GWAS). METHODS:Firstly,single nucleotide polymorphism data related to statins were obtained from the latest 9th edition of the FinnGen database,while data of osteoarthritis,knee osteoarthritis and hip osteoarthritis were obtained from the IEU Open GWAS,UK Biobank,and ArcOGEN(Genetics of Osteoarthritis)databases,respectively.The inverse variance weighted method was used as the primary analysis approach to evaluate the causal effects.The weighted median method,simple median method,weighted mode-based method,and MR-Egger regression were used as supplementary analyses.The causal relationship between statins and the risk of osteoarthritis,knee osteoarthritis and hip osteoarthritis was assessed using odds ratios(OR)with 95%confidence intervals(CI).Sensitivity analyses were conducted to validate the reliability of the results,including the Cochran's Q test for heterogeneity and the MR-Egger-intercept test for horizontal pleiotropy,as well as leave-one-out analysis to identify potentially influential single nucleotide polymorphisms. RESULTS AND CONCLUSION:The inverse variance weighted analysis demonstrated a negative causal relationship between genetically predicted statins and the risk of osteoarthritis(OR=0.998,95%CI:0.996-0.999,P=0.01),knee osteoarthritis(OR=0.964,95%CI:0.940-0.989,P=0.005),and hip osteoarthritis(OR=0.928,95%CI:0.901-0.955,P=4.28×10-7).MR-Egger intercept analysis did not detect potential horizontal pleiotropy(osteoarthritis:P=0.658;knee osteoarthritis:P=0.600;hip osteoarthritis:P=0.141).The results of this study provide evidence that statins reduce the risks of osteoarthritis,knee osteoarthritis and hip osteoarthritis as described in observational studies.Further research is needed to explore the specific mechanisms of statin treatment for osteoarthritis.

BACKGROUND:Clinical evidences have suggested a correlation between metabolic factors and sarcopenia.Blood metabolites have been found as biological factors underlying the mechanisms of musculoskeletal disorders.However,the causal relationship between blood metabolites and sarcopenia is unclear. OBJECTIVE:To explore the causal relationship between blood metabolites and sarcopenia-related traits through a two-sample Mendelian randomization analysis and to analyze their metabolic pathways. METHODS:A dataset of 486 blood metabolites and sarcopenia-related traits was obtained from public databases.The inverse variance weighting,MR-Egger and weighted median methods were used to assess the causal relationship of blood metabolites with muscle mass and strength across genders.Sensitivity analyses,including heterogeneity and gene pleiotropy,were performed to explore the robustness of the results.Metabolic pathway analysis of potential causal relationships was performed using the Metaboanayst 5.0 tool. RESULTS AND CONCLUSION:A total of 124 metabolites and sarcopenia-related traits were observed to have potential causal relationships(P<0.05).Mannose and 1-arachidonoylglycerophosphocholine were significantly causally associated with an increased muscle mass in males(P<1.03×10-4).Pentadecanoate and glycine were significantly causally associated with decreased muscle mass and muscle strength in females,respectively(P<1.03×10-4).Metabolic pathway analysis identified eight metabolic pathways associated with altered levels of muscle mass and muscle strength in sarcopenia,including the"glyoxylate and dicarboxylate metabolism"and"Glycine,serine and threonine metabolism."The identified metabolites are considered as useful circulating metabolic biomarkers for screening and prevention of sarcopenia in clinical practice,serving as candidate molecules for future mechanistic exploration and drug target selection.

BACKGROUND:Numerous clinical studies have suggested a close relationship between obesity and osteoporosis,but whether there is a genetic causal effect between obesity and osteoporosis remains unclear. OBJECTIVE:To explore the association between obesity and osteoporosis using summary data from a large-scale genome-wide association study(GWAS)through Mendelian randomization analysis. METHODS:Obesity data were derived from summary statistics of the Genetic Investigation of Anthropometric Traits(GIANT)and the UK Biobank(UKBB).Osteoporosis data were obtained from the Genetic Factors for Osteoporosis(GeFOS)consortium,including two bone density phenotypes:total body bone mineral density(BMD)and heel BMD.The inverse variance-weighted method was the primary analysis,with the Mendelian randomization method based on Egger regression(MR-Egger)and weighted median method as supplementary approaches to calculate the causal association between genetic variations related to obesity and osteoporosis.Sensitivity analyses were conducted to validate the reliability of the results.Heterogeneity was assessed using Cochran's Q test.Horizontal pleiotropy was assessed through the MR-Egger intercept test.Leave-one-out analysis was performed to evaluate the potential influence of single nucleotide polymorphisms on the combined inverse variance-weighted estimates. RESULTS AND CONCLUSION:(1)Impact of obesity on osteoporosis:In addition to body mass index and forearm BMD,body mass index,waist-to-hip ratio,body mass index-adjusted waist-to-hip ratio,and whole-body body mass index,heel BMD,forearm BMD,lumbar spine BMD,and femoral neck BMD were causally related to each other.Further Meta-analysis revealed that obesity increased the risk of BMD(odds ratio=1.07,95%confidence interval:1.03-1.12,P<0.01).(2)Impact of osteoporosis on obesity:Apart from arm BMD and lumbar spine BMD as exposure factors showing causal relationships with obesity,other datasets indicated no causal effect between total body BMD,heel BMD,femoral neck BMD,and obesity.Additional meta-analysis demonstrated that BMD did not increase the risk of obesity(odds rate=0.99,95%confidence interval:0.98-1.01,P<0.01).There is a causal relationship between obesity and osteoporosis,suggesting that obesity may be a risk factor for osteoporosis.However,no causal association is found between osteoporosis and obesity.

BACKGROUND:Observational studies have suggested that statin drugs may have a protective effect on bone density,making them a potential treatment option for osteoporosis. OBJECTIVE:To evaluate the causal relationship between drug target-mediated lipid phenotypes and bone mineral density(BMD)using Mendelian randomization methods. METHODS:We obtained single nucleotide polymorphismsrelated to statin drugs and BMD data from the IEU Open GWAS database.The primary analysis method was the inverse variance weighted method,and we also used weighted median,simple median,weighted mode,and MR-Egger regression.We usedβ values and 95%confidence intervals(CI)to assess the causal relationship between statin drugs and BMD.Additionally,we conducted sensitivity analyses to validate the results,assessed heterogeneity using Cochran's Q test,examined for horizontal pleiotropy using the MR-Egger intercept test,and performed leave-one-out analyses to determine if individual or multiplesingle nucleotide polymorphism influenced the results. RESULTS AND CONCLUSION:There was a significant association between the statin target of action,3-hydroxy-3-methyl glutaryl coenzyme A reductase-mediated low-density lipoprotein cholesterol,and heel bone BMD(β=-0.086,95%CI:-0.117 to-0.055,P=5.42×10-8)and whole-body BMD(β=-0.193,95%CI:-0.288 to-0.098,P=7.35×10-5).The findings of this study support the protective effect of statin drugs on BMD.These findings not only deepen our understanding of the relationship between cholesterol-related genes and bone health but also reveal potential therapeutic targets for improving BMD.

RESUMEN

Acylcarnitines are metabolic intermediates of fatty acids and branched-chain amino acids having vital biofunctions and pathophysiological significances.Here,we developed a high-throughput method for quantifying hundreds of acylcarnitines in one run using ultrahigh performance liquid chromatography and tandem mass spectrometry(UPLC-MS/MS).This enabled simultaneous quantification of 1136 acyl-carnitines(C0-C26)within 10-min with good sensitivity(limit of detection<0.7 fmol),linearity(cor-relation coefficient>0.992),accuracy(relative error<20%),precision(coefficient of variation(CV),CV<15%),stability(CV<15%),and inter-technician consistency(CV<20%,n=6).We also established a quantitative structure-retention relationship(goodness of fit>0.998)for predicting retention time(tR)of acylcarnitines with no standards and built a database of their multiple reaction monitoring parameters(tR,ion-pairs,and collision energy).Furthermore,we quantified 514 acylcarnitines in human plasma and urine,mouse kidney,liver,heart,lung,and muscle.This provides a rapid method for quantifying acyl-carnitines in multiple biological matrices.

RESUMEN

Objective:To evaluate the pharmacodynamics of remimazolam tosilate inducing loss of consciousness (LOC) when combined with sufentanil in children.Methods:American Society of Anesthesiologists Physical Status classificationⅠ or Ⅱ pediatric patients of either sex, aged 3-6 yr, undergoing electronic bronchoscopy, were included in this study. ECG monitoring was carried out in all children after admission, sufentanil 0.1 μg/kg was intravenously injected slowly, and 3 min later remidazolam tosilate was intravenously injected. The dose of remimazolam tosilate was determined by the modified Dixon′s up-and-down sequential experiment, and the initial dose of remimazolam tosilate was 0.30 mg/kg. The dose of remimazolam tosilate in the next child was determined according to the the loss of consciousness, and the successive dose gradient was 0.05 mg/kg. Loss of eyelash reflex and Modified Observer′s Assessment of Alertness/Sedation Scale score reaching 0 and the occurrence of 8 crossover points where consciousness transitioned from non-disappearance to disappearance after 1 min of remimazolam tosilate injection were considered to be signs of LOC. The median effective dose (ED 50), 95% effective dose (ED 95), and their 95% confidence interval ( CI) of remimazolam tosilate inducing LOC were calculated using probit method. Results:When combined with sufentanil, the ED 50 and 95% CI of remimazolam tosilate inducing loss of consciousness were 0.461 (0.429-0.493) mg/kg, and the ED 95 and 95% CI were 0.515 (0.487-0.689) mg/kg. Conclusions:When combined with sufentanil, the ED 50 of remimazolam tosilate inducing LOC is 0.461 mg/kg and the ED 95 is 0.515 mg/kg in children.

AIM To study the neoflavonoids from Dalbergia cochinchinensis Pierre ex Laness and their anti-hypoxia/reoxygenation injury activities on H9c2 myocardial cells.METHODS The 70%ethanol extract from D.cochinchinensis was isolated and purified by silica gel,Sephadex LH-20 and reverse-preparative HPLC,then the structures of obtained compounds were identified by physicochemical properties and spectral data.The CCK-8 method was used to detect their activities on H9c2 cells and protective effects on hypoxia-reoxygenation injury of H9c2 cells,and their structure-activity relationship was analyzed.RESULTS Twelve compounds were isolated and identified as latifolin(1),5-O-methyllatifolin(2),mimosifoliol(3),5-O-methydalbergiphenol(4),dalbergiphenol(5),cearoin(6),2,4-dihydroxy-5-methoxy-benzophenone(7),2-hydroxy-4,5-dimethoxybenzophenone(8),melannoin(9),2,2′,5-trihydroxy-4-methoxybenzophenone(10),dalbergin(11),4-methoxydalbergione(12).The dalbergiphenols and dalbergins had little toxicity to H9c2 cells,and dalbergiphenols had strong activity against hypoxia-reoxygenation injury of H9c2 cells.CONCLUSION Compound 8 is a new natural product.Compounds 4,9 are isolated from this plant for the first time.Dalbergiphenols may be the main neoflavonoids against hypoxia-reoxygenation injury of H9c2 cells.

[Objective]To explore the construction measures of a new pattern of tutor-postgraduate relationship under the background of striving for"double first-class"traditional Chinese medicine(TCM)university,in order to continuously improve the quality of postgraduate training in TCM universities.[Methods]Under the background of striving for"double first-class"TCM universities,this paper introduces the practical significance of building a good mentoring relationship,and analyzes the outstanding problems existing in the mentoring relationship in the current TCM universities from the perspective of tutors and postgraduates-the two main subjects.[Results]Through the establishment of a good mentoring relationship,the tutor-postgraduate relationship has generally been positively influenced,and the communication between teachers and students has become closer and more efficient.Through a series of positive incentive mechanisms,tutors are more active in providing support and help to postgraduates in academic guidance,academic guidance and life care,and postgraduates are more active in cooperating with tutors in various scientific research work.The quantity and quality of academic papers published by postgraduates have been significantly improved,and the scientific research and teaching work of the tutor team have been carried out more efficiently with remarkable progress.[Conclusion]Starting from the three aspects of strengthening the positive core value of"respecting teachers and medicine",perfecting the relevant system and correcting the imbalance of the tutor-postgraduate relationship,the School of Basic Medicine of Zhejiang Chinese Medical University focuses on exploring the new mode of building a good tutor-postgraduate relationship,and puts forward feasible suggestions through a series of measures.This mode has significantly improved the tutor-postgraduate relationship.It not only plays a positive role in improving postgraduates'professional level and communication-scientific research-innovation ability,but also provides a guarantee for tutors to complete teaching and scientific research work successfully,which is worthy of further improvement and promotion.

Objective To screen the pharmacodynamic material basic components of Artemisiae Annuae Herba and study its antioxidant activity in vitro by investigating the spectrum-effect relationship between the HPLC fingerprints of 11 batches of Artemisiae Annuae Herba (dried aerial part of Artemisia annua L.). Methods The determination was performed on Aglient C18 column (250 mm×4.6 mm, 5 μm) with mobile phase consisted of 0.2% phosphoric acid solution-Methanol (gradient elution) at the flow rate of 1.0 ml/min. The column temperature was indoor temperature, and detection wavelength was 220 nm, with sample size of 10 μl. Using isochlorogenic acid A as reference, HPLC fingerprints of 11 batches of samples were determined. The common peaks of 11 batches of samples were identified and recorded through TCM chromatographic fingerprint similarity evaluation system (2012 edition). Using scavenging rate of DPPH and ABTS free radical as pharmacodynamic indicators of antioxidant effects, SIMCA 14.1 analysis software was used for PLSR to establish the spectra-effect relationship. Results There were 48 common peaks on 11 batches of sample, 11 components were identified as scopoletin, scoparone, isochlorogenic acid B, A, C, luteolin, apigenin, chrysosplenetin, artemisinin, artemisetin and artemisinic acid. The scavenging activity of 11 batches of samples to DPPH and ABTS free radicals was detected. The spectrum-effect relationship showed that isochlorogenic acid A, B, C and scoparone were positively associated with its antioxidant capacity, and variable projection value was greater than 1. It was suggested that these components were the material basis of antioxidant effect in Artemisiae Annuae Herba. Conclusion This study investigates the antioxidant capacity of different substances in Artemisiae Annuae Herba in vitro, and proves that isochlorogenic acid A,B,C and scoparone play a major role for the antioxidant capacity.

At present, there are many difficulties in the diagnosis and treatment of acute vestibular syndrome (AVS). For example, complex and difficult identification of the cause of disease, uneven diagnosis and treatment levels of clinical doctors, weak humanistic care awareness, lack of communication skills, intrinsic affinity and other reasons, which make it difficult for AVS patients in the process of diagnosis and treatment, and cannot receive timely and effective treatment, resulting in an exacerbation of doctor-patient conflicts. Therefore, it is recommended to explore new paths of AVS diagnosis and treatment work using the humanistic care concept, respect each other between doctors and patients, build a team of medical staff with the value orientation of “humanistic care”, and promote the organic unity of theory and practice of “humanistic care”, with a view to better promoting the implementation of AVS diagnosis and treatment work, helping more patients rebuild confidence, enhancing quality of life, and improving the doctor-patient relationship.

RESUMEN

The medical education perspective of the Johns Hopkins model mainly includes the integration of clinical, teaching, and scientific research, integrating medical humanities elements and conducting elite education with small class teaching, long learning, and high elimination rates. This model has made remarkable achievements in the practice of introducing it into the field of medical education several times, and also had many positive effects on the doctor-patient relationship in the first half of the 20th century. The contemporary doctor-patient relationship has generated new negative sides, mainly including reduced trust and compliance caused by the proliferation of medical information, increased aggression in patients due to information asymmetry between doctors and patients, and conflicts arising from difficulties in doctors-patient shared decision-making. These can be mitigated by taking action through drawing on the medical education perspective of the Johns Hopkins model and transforming it into a more in line with contemporary situations, as well as taking the cultivation of medical talents as the starting point, so as to promote the improvement of doctor-patient relationship.

This paper analyzed the legal issues existing on the medical side in 20 medical damage cases in obstetrics from January 2020 to June 2022 in China Judgment Documents Network, and hoped to explore the paths that the medical side can use for reference in building a harmonious doctor-patient relationship in the legal context. Firstly, medical institutions should establish rescue evaluation systems and early warning mechanisms for critical pregnancy and maternal, so as to ensure the health and safety of life of pregnant women and newborns, as well as prevent the occurrence of adverse obstetric events. Secondly, strengthening the cultivation of the humanistic quality among obstetric medical practitioners is the basis of the harmonious obstetrical doctor-patient relationship. Thirdly, the training and learning of relevant laws and regulations such as the Civil Code of the People’s Republic of China and the Physician Law of the People’s Republic of China should be strengthened. Fourthly, the patients’ informed consent should be respected, the process and procedures of fulfilling informed consent should be standardized by the systems, and the forms for fulfilling informed consent should be further explored. Fifthly, the medical risk-sharing mechanism should be explored and this paper suggested that postpartum women purchase obstetric accident insurance to reasonably share medical risks. Sixthly, medical social workers should be introduced, doctor-patient communication should be strengthened, and the obstetric risk awareness of the whole society should be enhanced.

De Xin Ji Medical Case, in a narrative and discussion manner, emerged the doctors’attention to the temporal aspect of life, the shaping of personality, the promotion of plot and the intersubjectivity and ethics of treatment, as well as displayed rich storytelling characteristics. Based on the perspective of doctors, the author Xie Xinghuan conducted multiple-dimensional narrative reflections from perspectives of the relationship between doctors and patients and society, patient compliance, as well as self-medical behavior, which presented the doctor’s introspection and deep understanding of human nature. By depicting the interactive details between the characters, the medical case showed the relationship between doctors and patients, doctors and other doctors, and doctors and disciples, which not only contained the core connotation of narrative medicine, but also highlighted the unique cultural attributes of traditional Chinese medicine diagnosis and treatment.
