RESUMEN

Objective@#Non-technical skills (NTS) are necessary to evaluate the comprehensive quality of surgeons. In this study, we proposed the concept of C-NTS, a scoring criterion for NTS based on real scenarios (for example, history taking) and video recording, and verified its practical application effects.@*Methods@#Study objects were divided into the tutor group and the student group. The tutor group contained four senior attending physicians in gastrointestinal surgery department of one tertiary hospital (all male with doctor degree). The student group had four rotating surgeons who were randomly selected from the same department in 2018 (two males and two females). Before and after the training, the tutor rated the same anonymous video by C-NTS. One-factor analysis of variance was used to compare the differences between and within the groups, and Kendall concordant coefficient was used to test the consistency by SPSS 22.0.@*Results@#After the tutor receiving training, Kendall concordant coefficient was increased from 0.425 to 0.853 and the latter suggested the difference had statistical significance (P=0.017), which confirmed the effectiveness of the training.@*Conclusion@#C-NTS has preliminarily solved the difficulty of homogenization of NTS assessment. Relevant C-NTS discussions and trainings for clinical tutors may be beneficial to save time and manpower in clinical teaching and evaluation.

RESUMEN

To meet the requirement of pathological practice and development, we systematically analyzed the situation of pathological residents training and the importance of initiating the clinical scientific research training. Additionally, we proposed the principle and implementation strategy for clinical scientific research training. According to features of pathological practice, we employed the modularized teaching to divide the training courses into several modules: discussion module for clinical pathology, lecture module for advanced research, and training modules for basic scientific theory, technology and writing skill. With these approaches, the systematic and structured system of standardized residents training is implemented to improve the clinical research ability of pathological residents.

The training of residents majoring in ultrasound needs not only to pay attention to the teaching of speculative knowledge,but also to "look at pictures and talk",which is based on a large number of diagnostic ultrasound cases,training the residents to find,discover and discriminate the lesions from the ultrasonic images to finally form a sharp recognition and diagnosis ability.This study used the instant messaging tool WeChat to establish a virtual learning platform and implement a case based teaching process through this platform,which can avoid the limitations of time and space,and make the communication between teachers and students faster.According to the questionnaire survey,residents generally believed that the training mode can effectively improve learning efficiency and professional ability and hope to add this training mode to the basis of traditional teaching mode.The results of resident case analysis showed that the diagnostic accuracy of residents gradually increased during the training process.The model of WeChat platform combined with case teaching can provide new ideas and methods for standardized training of resident doctors in ultrasound specialty.

RESUMEN

The application of continuous quality improvement program in standardized residency training examination in the central Hospital of Shanghai Jiading was introduced to inquire into the way to improve the quality of standardized training of resident doctors. Through the steps of FOCUS-PDCA, we continuously improved the examination content and quality compliance training personnel, and developed the examination process including test, training, supervision, learning and using. After the implementation of the project, the percentage of the departments that meet the residency training examination requirements has been increased from 33.3%to 100%in our hospital. The percentage of the students that passed the licensed medical skills examination was 96.6%in that year. 100%of the students passed the graduation comprehen-sive examination. Year-end evaluation showed that the students' satisfaction degree to the teachers increased from 94.5%to 98.2%. Thus, through the implementation of CQI project, we achieved the goal of promoting teaching and promoting learning.

In the course of the training of clinical specialty degree graduate education and standard-ized training of residents(referred to as"temporary residence"),students are required to obtain"four certifi-cates" in 3 years". Here, our department of Neurology reviewed residents who were trained by this training mode. Their ability of clinical neurological profession, clinical thinking and clinical research was with effective promotion through"one to one"teaching method,the tutor-responsibility system,PBL teaching and literature-reading meeting. The objective is to get the best effect in 3 years, to explore a more suitable training mode for these residents and train high-level clinicians with applied and academic types.

RESUMEN

To increase the rescue competency of residents, also the critical thinking, team dynamics, and the capability to handle with emergencies, Shanghai East Hospital developed a rescue course integrated with 24 standardized simulation cases learning and medical simulation education for residents training pro-gram. The instructor candidates should complete the simulation training and pass the instructor certification. Every course was monitored by the course director to ensure the teaching quality. After two and a half years of practice, it is found that the teaching of case teaching and simulation teaching is better than traditional teaching, and it plays an important role in the security of medical security.

Este estudio se enmarcó en la investigación: Caracterización del modelo en competencias profesionales de educación médica desarrollado en doce especializaciones clínicas y nueve quirúrgicas de la Facultad de Medicina, Universidad de Antioquia. Su objetivo fue indagar el estado del arte de la formación médica en los posgrados. La pregunta orientadora fue la siguiente: ¿hacia dónde se dirige actualmente la investigación en educación médica de posgrado? Se hizo un estudio descriptivo, no experimental, para el que se revisaron 12 bases de datos y se seleccionaron 28 artículos relacionados con la formación médica de posgrado. Se compararon, analizaron e interpretaron los hallazgos. Se evidenció un aumento del número de publicaciones sobre educación en las especialidades médicas. La tendencia investigativa en educación médica de posgrado apunta a la necesidad de hacer propuestas multi-intertransdisciplinarias y humanísticas basadas en el constructivismo; situar la evaluación como proceso haciendo énfasis en el aprendizaje y la participación de los estudiantes y construir sistemas de formación pedagógica de tutores y currículos interactivos y flexibles; es notoria la ausencia de estudios que promuevan la formación por competencias en los posgrados médicos.


This study was framed in the research: Characterization of professional competency-based model in medical education developed in twelve clinical and nine surgical specializations at the Faculty of Medicine, University of Antioquia. Its aim was to inquire about the state of the art in medical postgraduate education. The guiding question was: Where is present-day research headed in medical postgraduate education. For this descriptive, nonexperimental work, 12 bibliographic databases were reviewed and 28 research articles related to graduate medical formation were selected. The findings were compared, analyzed and interpreted. The tendency in research on graduate medical education points to the need of having multi-inter-trans-disciplinary and humanistic proposals based on constructivism; to consider evaluation as a process emphasizing on learning and the participation of students, and to build systems of pedagogical formation of tutors and interactive and flexible curricula. The lack of studies that promote competencies-based training in postgraduate medical education is notorious.

RESUMEN

Objectives To explore the effect of the standardized residents training method on intern teaching in department of respiratory. Methods Sixty interns of Grade 2010 from the Second Military Medical School were randomly divided into 2 groups with 30 interns each. The traditional teaching method was adopted in control group, while the standardized residents training method was used in experiment group. When the rotating internship was finished in department of respiratory, the survey of satisfaction about teaching and the assessment of teaching effect were performed in two groups. The contents of examination included academic knowledge exam, clinical skills test, basic skills test and comprehensive quality assessment. Results There was no difference between two groups in academic knowledge exam (P>0.05). The total scores, clinical skills scores and basic skills scores of experiment group were higher than those of control group (P<0.05). And, higher satisfaction was ac-quired in experiment group(P<0.05). Conclusions Using the standardized residents training method can improve the effect of the intern teaching and teaching satisfaction and it can be widely applied in intern teaching.

@#Objective To observe the effect of case-based learning (CBL) applied in ward-round on residents training. Methods A total of 33 residents trained with routine ward-round and CBL ward-round in the neurological department were investigated with questionnaire. Results 30 effective answers were recalled. More of them reported that CBL ward-round was very significant in improving clinical thinking, mastering the specialized knowledge, developing autonomous learning and improving the communication with patients and their family members (P<0.05). Conclusion CBL ward-round is effective on improving the comprehensive professional abilities of residents.

OBJECTIVE: To determine the correlation of ultrasonographic estimation of fetal weight and actual birthweight and the impact of the level of resident s training on the results. METHODS: This was a prospective study of 90 women with term pregnancies. Ultrasound estimated fetal weight (EFW) was calculated by a pre-programmed Hadlock formula. Days from ultrasound to delivery were less than seven. The EFW was compared to the actual birthweight at delivery. The year of training of the resident that performed the ultrasound was recorded. Exclusion criteria included diabetes mellitus and known fetal anomalies. RESULTS: Mean age was 28 years, parity was 0 to 4 and mean gestational age was 38 weeks. There was an average over-estimation of 64.8 grams. The difference between mean EFW and mean birthweight was not significant (p = 0.067). The difference between mean EFW and mean birthweight when calculated according to year of residency was not significant, p = 0.075 and 0.402for junior and senior residents, respectively. CONCLUSION: There is good correlation between residents' ultrasonographic estimation of fetal weight and actual birthweight at the UHWI. There was no significant difference in correlation between senior and junior residents. Developments in computer technology might contribute to decrease in the learning curve.


OBJETIVO: Determinar la correlación de la estimación ultrasonográfica del peso fetal y el peso real al nacer, y el impacto del nivel de formación del residente en los resultados. MÉTODOS: Se trató de un estudio prospectivo de 90 mujeres con embarazos a término. El peso fetal estimado (PFE) por ultrasonido fue calculado mediante una fórmula de Hadlock preprogramada. Los días transcurridos desde el ultrasonido hasta el parto fueron menos de siete. Se comparó el PFE con el peso real en el parto. Se registró el año de formación del residente que realizó el ultrasonido. Los criterios de exclusión criterios incluyeron diabetes mellitus y anomalías fetales conocidas. RESULTADOS: La edad promedio fue 28 años; la paridad fue de 0 a 4; la edad gestacional fue de 38 semanas. Hubo una sobreestimación promedio de 64.8 gramos. La diferencia entre el PFE promedio y el peso promedio al nacer no fue significativa (p = 0.067). La diferencia entre el PFE promedio y el peso promedio al nacer calculada según el año de residencia no fue significativa, siendo p = 0.075 y 0.402 para médicos en la primera y última etapa de su residencia, respectivamente. CONCLUSIÓN: Existe una buena correlación entre la estimación ultrasonográfica del peso fetal, realizada por los residentes, y el peso real al nacer en el HUWI. No hubo ninguna diferencia significativa en la correlación entre los residentes en sus primeras y últimas etapas. Los desarrollos en la tecnología informática pueden contribuir a la disminución de la curva de aprendizaje.

RESUMEN

At present,the teaching resource for anesthetic residents training is more and more insufficient.The safe and standardized training course based on medical simulation teaching meets the training demand of comprehensive quality and professional skill for anesthetic residents training.Basic and advanced anesthetic training can raise the operational level while comprehensive anesthetic ability training can improve the ability of clinical thinking,organization,coordination and crisis management.The use of medical simulation teaching is insufficient in anesthetic residents training.It is highly recommended to guide anesthetic residents training by the government policy.

RESUMEN

During the course of standardized residents training,the performance appraisal management of the residents was used besides other regular management methods.The allowances of residents were decided monthly by the evaluation and assessment of many kinds of tests or examinations.Through this new measure,the learning enthusiasm of residents could be fully motivated.The authoritative effectiveness of management was enhanced enormously.Meanwhile,the quality of the educational and training has also been improved greatly.This method could be very useful for the training process.
