RESUMEN

A 67-year-old male patient presented with a plaque on the top of the head for more than 1 year.Physical examination showed an annular pale red plaque sized about 1.5 cm × 1 cm on the top of the head.It had a depressed center and papule-like elevated faint yellow boundaries, and there was no hair on the surface of the lesion.Histopathologic examination revealed milder epidermal hyperplasia,a small area of epidermal depression, confluence and hyperplasia of multiple sebaceous glands in the dermis, many keratinous cysts and funicular basophilic epithelial cells surrounded by hyperplastic collagen fibers in the superficial dermis.Immunohistochemical study showed positive staining for CD34 (hair mesenchymal cells),epithelial membrane antigen (EMA),cytokeratin 20 (CK20),but negative staining for bcl-2,P53,CD56 and collagen Ⅳ,with the proliferation index Ki-67 of 2%-5%.Based on the pathological results, the patient was diagnosed with desmoplastic trichoepithelioma complicated by sebaceous hyperplasia.

RESUMEN

Objective To compare the clinical efficacy and safety of carbon dioxide (CO2) versus high-frequency electroacupuncture based on the assistance of drilling and blunt dissection in the treatment of steatocystoma multiplex. Methods A randomized,self-controlled study was performed in 15 patients. The median line of the body served as the dividing line, and the lesions on the two sides of the median line of the body were randomly divided into 2 groups to be treated with CO2 laser or high-frequency electroacupuncture, which left micropathways into the cyst cavity. After the above treatment, contents in the cysts were squeezed out or drawn out with a mosquito clamp, and the cysts were bluntly dissected. The decrustation time, erythema subsiding time, adverse reactions and recurrence were compared among lesions at different sites or with different sizes. Results For the lesions at the same sites or with similar sizes after treatment, the decrustation time and erythema subsiding time were both significantly shorter in the CO2 laser group than in the high-frequency electroacupuncture group (both P < 0.05).There were significant differences in the decrustation time and erythema subsiding time among lesions at different sites or with different sizes (all P < 0.05).Mild pain occurred in the 2 groups, and no hematoma or secondary infection occurred. There were significant differences in the incidence of scar formation or pigmentation between the 2 groups (both P < 0.05).During the follow-up of 6-18 months, relapse occurred in 1 patient in each group. Conclusion Compared with the high-frequency electroacupuncture,CO2 laser assisted by drilling and blunt dissection shows better efficacy in the treatment of steatocystoma multiplex, and it can remove lesions at various sites at one time.

RESUMEN

The study aimed to verify the therapeutic action of isotretinoin in the treatment of sebaceous hyperplasia. During two months, 20 patients with sebaceous hyperplasia took isotretinoin at a dosage of 1mg/kg per day. Their skin lesions were counted and photographed before and after treatment and re-evaluated two years later. The average number of sebaceous hyperplasia lesions before treatment was 24 per patient. At the end of two months of therapy, the number of lesions decreased to 2 per patient. The statistically analyzed data showed a reduction in the number of lesions following isotretinoin use (p < 0.05). Two years after the end of the treatment, the average number of sebaceous hyperplasia lesions was 4 per patient. There were no severe side effects. Thus, the data analysis suggests that isotretinoin is a safe and effective drug for treating the disease under study.

RESUMEN

Sebaceous carcinoma is a rare and aggressive skin tumor. It can be located in any area of the body, the most commonly involved area being the periorbital region. It does not entail a typical clinical presentation, which explains the often late diagnosis. The aim of this report is to outline the rarity of the disease and its atypical clinical description, since to this day, inguinal ulcers with clinical manifestation have not been reported. We present and discuss a case of sebaceous carcinoma with an unusual clinical presentation, in an elderly male patient. The precise approach to genital ulcers, as shown in this case, is a diagnostic challenge that requires a great deal of effort on the part of the clinician.

Objectives To assess the prevalence of and risk factors for some sebaceous glands diseases (seborrhoeic dermatitis, acne, seborrhoeic alopecia and rosacea) and their relation to gastrointestinal dysfunction in adolescents. Methods A questionnaire survey was carried out for the epidemiologic information of sebaceous glands diseases from September to October 2001. A total of 4 600 youngsters aged from 12 to 20 years were recruited from 7 middle schools in Macau by random cluster sampling. The statistical software of SPSS 10.0 was used to analyze the data. Results The prevalence of seborrhoeic dermatitis, acne, seborrhoeic alopecia and rosacea was 2.7%, 45.5%, 1.6% and 0.7% respectively. Based on multivariate logistic regression analysis, the risk factors of sebaceous glands diseases included age, gastrointestinal dysfunction, season (summer), spicy and fried food, a family history of acne and the presence of long hairs on areolae of breasts. Gastrointestinal dysfunction was correlated with the incidence rates of seborrhoeic dermatoses (P = 0.007, OR = 1.208, 95% C.I 1.053 ~ 1.385). Conclusion Gastrointestinal dysfunction is an important risk factor for diseases of the sebaceous glands and correlates with the occurrence and development of these diseases.
