RESUMEN

OBJECTIVE: The purpose of this study was to evaluate the reliability and quality of radiomic features in glioblastoma multiforme (GBM) derived from tumor volumes obtained with semi-automated tumor segmentation software. MATERIALS AND METHODS: MR images of 45 GBM patients (29 males, 16 females) were downloaded from The Cancer Imaging Archive, in which post-contrast T1-weighted imaging and fluid-attenuated inversion recovery MR sequences were used. Two raters independently segmented the tumors using two semi-automated segmentation tools (TumorPrism3D and 3D Slicer). Regions of interest corresponding to contrast-enhancing lesion, necrotic portions, and non-enhancing T2 high signal intensity component were segmented for each tumor. A total of 180 imaging features were extracted, and their quality was evaluated in terms of stability, normalized dynamic range (NDR), and redundancy, using intra-class correlation coefficients, cluster consensus, and Rand Statistic. RESULTS: Our study results showed that most of the radiomic features in GBM were highly stable. Over 90% of 180 features showed good stability (intra-class correlation coefficient [ICC] ≥ 0.8), whereas only 7 features were of poor stability (ICC NDR ≥1), while above 35% of the texture features showed poor NDR (< 1). Features were shown to cluster into only 5 groups, indicating that they were highly redundant. CONCLUSION: The use of semi-automated software tools provided sufficiently reliable tumor segmentation and feature stability; thus helping to overcome the inherent inter-rater and intra-rater variability of user intervention. However, certain aspects of feature quality, including NDR and redundancy, need to be assessed for determination of representative signature features before further development of radiomics.

Objective: The aim of this study was to compare the use of semi-automated (Medax Velox 2; Poggio Rusco, Italy) and automated (Bard Magnum Biopsy Instrument; Covington, GA, USA) core biopsy needles, for ultrasound guided breast biopsy. Materials and Methods: A 14G semi-automatic spring loaded core biopsy needle with a 22-mm-throw (Medax Velox 2; Poggio Rusco, Italy) and 14-gauge automated needle device with a 22-mm-throw biopsy gun (Bard-Magnum Biopsy Instrument, Covington, GA, USA) were used for breast biopsies under ultrasound guidance on alternate months during the study period between July 2009 and May 2011. One hundred and sixty lesions were biopsied and specimens were sent for histological evaluation. Results: The automated needle obtained a higher number of histology reports at 84% (67/80) as compared with the semiautomated needle at 60% (48/80) (Fisher exact test, p value=0.023). Inadequate samples with the automated needle were much less at 9% (7/60) than with the semiautomated needle at 23% (18/60) (Fisher exact test, p value=0.028). The semi-automated needle showed slightly less fragmented samples. However, the number of fragmented samples with definitive diagnosis was slightly higher with the automated compared with the semiautomated needle, at 16% (13/80) and 13% (10/80) respectively. Compared with histology of 29 lesions that were excised, the semi-automated needle had higher sensitivity (100%) but lower specificity (75%) and accuracy (90%) compared with the automated needle (88% sensitivity, 100% specificity, 95% accuracy). Conclusion: Definitive diagnosis from the study samples slightly favours the use of automated core biopsy needle as compared to semi-automated core biopsy needle.

PURPOSE: This study compared the performance between the fully-automated external defibrillator (F-AED) and the semi-automated external defibrillator (S-AED) when used by laypersons. METHODS: Thirty-three laypersons participated in a mannequin simulation study as part of Cardio-Pulmonary Resuscitation (CPR) training courses. After 30 minutes of didactic education for the Automated External Defibrillator (AED), they watched a video on how to use the fully-automated external defibrillator (F-AED) and a semi-automated external defibrillator (S-AED) instead of a hands-on education. Laypersons performed the S-AED first, then the F-AED. Performances and shock delivery time intervals were recorded and evaluated. RESULTS: The performances in shock delivery were better with the F-AED, although the overall performance was statistically insignificant. In terms of shock delivery interval, the F-AED was shorter than the S-AED (54.48+/-2.84 sec vs. 64.76+/-3.57 sec, respectively, p<0.01). In the post survey, F-AED had a higher preference (F-AED vs. S-AED 23(70%) vs. 5(15%), respectively, p<0.001). CONCLUSION: The F-AED had a better performance and shorter shock delivery time interval than the S-AED. The F-AED should thus be considered for use, outside of the hospital, on cardiac arrest patients for early defibrillation.

BACKGROUND: White matter hyperintensities (WMHs) on magnetic resonance imaging (MRI) have been considered as a reliable biomarker of small vessel damages. To evaluate the severity of WMHs, it is vital to develop reliable methods to measure the volume of WMHs. We applied open source software to measure WMH volume in the semi-automated way, and tested the reliability and validity by comparing with the commonly used qualitative rating scale. METHODS: Twenty five subjects with variable WMHs were recruited. ANALYZE 10.0 was used for the image processing and volumetric measurement of WMHs. The inhomogeneity and artifacts of signal were corrected with Insight Segmentation and Registration Toolkit in ANALYZE. For the gold standard of the WMH volumetric measurement, threshold method was applied with consensus of manual editing on each slice of the MRI images by two raters. Histogram of the all slices of the Fluid Attenuated Inversion Recovery (FLAIR) MRI was generated to calculate the optimal voxel intensity of threshold, and the lowest voxel threshold was decided as the mean+1.4 SD. The volumes of WMHs were generated by multiplying the area and the thickness of each slice. Inter- and intrarater reliability of the semi-automated volumetric and Scheltens'methods, and the association between the individual methods were analyzed. RESULTS: The semi-automated WMH volume at the threshold of 1.4 SD as well as the gold standard volume was well correlated with the Scheltens' visual scale (r=0.75, p<0.001). The semi-automated volumetry showed the excellent intra-rater (ICC=0.9929; 95% CI, 0.9840-0.9968) and inter-rater reliability (ICC=0.9830; 95% CI, 0.9620-0.9925), superior to the Scheltens' visual rating scale. CONCLUSIONS: The semi-automated volume measurement of the WMHs with Analyze was a valid and a reliable method to quantify subcortical white matter damages of various etiologies.

Avaliaram-se a degradabilidade da matéria seca e a cinética de fermentação ruminal das silagens de três genótipos de milheto (BRS-1501, NPM-1 e CMS-3), por meio da técnica in vitro semi-automática de produção de gases. O delineamento experimental utilizado foi o de blocos ao acaso em arranjo de parcelas subdivididas. Para a comparação dos genótipos, foi utilizado o teste SNK (P<0,05), e os parâmetros da cinética de fermentação foram ajustados ao modelo unicompartimental de France. A partir de 24 horas, a silagem do genótipo BRS-1501 foi a que apresentou maior produção acumulativa de gases. Os valores de degradabilidade da matéria seca apresentados às 96 horas de fermentação foram de 53,9 por cento para a silagem do BRS-1501, 51,7 por cento para a silagem do CMS-3 e 49,1 por cento para a silagem do NPM-1, sendo este o material com menor degradabilidade (P<0,05). Os potenciais máximos de produção de gases variaram de 145,18 para o genótipo CMS-3 a 155,02ml/g de MS para o BRS-1501. A silagem do genótipo BRS-1501 destacou-se entre as demais, em função do seu maior potencial de produção de gases e degradabilidades efetivas.


The dry matter digestibility and the fermentation kinetics of three pearl millet genotypes silages (BRS-1501, NPM-1, and CMS-3) were estimated by the semi-automated in vitro gas production technique, using a completely randomized blocks design in a split plot arrangement. The genotypes means were compared by SNK test (P<0.05) and fermentation kinetics parameters estimated by regression of gas production on time using the France model. The greater cumulative gas production from 24 hours on was obtained by BRS-1501 genotype silage. The dry matter degradability (DMD) values at 96 hours of incubation were 53.9 percent for BRS-1501, 51.7 percent for NPM-1, and 49.1 percent for CMS-3 silage. The lowest DMD (P<0.05) among the studied genotypes silages was observed for CMS-3 silage. The maximum potential of gas production ranged from 145.18ml/g of dry matter for CMS-3 genotype silage to 155.02ml/g of dry matter for BRS-1501 silage. The BRS-1501 genotype silage showed the highest potential of gas production and effective degradabilities.
