RESUMEN

PURPOSE: The goal of this study was to evaluate the clinical outcome of single short implants, less than 8 mm in length, placed in the posterior area. MATERIALS AND METHODS: A total of 128 patients (75 male and 53 female, mean age: 52.6±11.2 years) with 154 implants participated. Implant marginal bone loss, and survival and success rates were measured. RESULTS: The mean follow-up period was 51.35±24.97 months. A total of 128 implants, 8 mm in length, were placed in patients who had mean marginal bone loss of 0.75 mm. These implants had a survival rate of 95.3%. Twenty-six implants, 7 mm in length, were placed in areas with a mean marginal bone loss of 0.78 mm and had a survival rate of 96.2%. Both marginal bone loss and survival rate were not statistically different among the groups. In the maxilla, 34 implants showed a mean marginal bone loss of 0.77 mm and a survival rate of 97.1%. In the mandible, 120 implants showed a mean marginal bone loss of 0.75 mm and a survival rate of 95.0%. The average marginal bone loss around all implants was 0.76±0.27 mm at the last follow-up review after functional loading. The survival rate was 95.6% and success rate was 93.5%. CONCLUSION: In our study, single short implants less than 8 mm in length in the posterior areas had favorable clinical outcomes.

A reabilitação de pacientes parcialmente desdentados é um tratamento viável e com excelente prognóstico. Todavia, a disponibilidade óssea em altura é um fator crucial para o sucesso desse tratamento. O levantamento de seio maxilar, apesar da baixa qualidade óssea da maxila posterior, é considerado um procedimento bastante previsível. Entretanto, técnicas reconstrutivas em mandíbula não possuem a mesma previsibilidade, além de apresentar maior morbidade pós-operatória. Apesar da alta previsibilidade dos implantes com altura reduzida, poucos são os estudos que avaliam a real necessidade da esplintagem do mesmo em mandíbula posterior atrófica. Dessa forma, o objetivo do presente estudo foi avaliar a necessidade de um implante curto ser esplintado a outro. Após análise criteriosa da literatura concluiu-se que os implantes com altura reduzida unitários apresentam igual previsibilidade em relação ao esplintado. Todavia, a heterogeneidade dos estudos e a falta de ensaios clínicos randomizados justifica a elaboração de novas pesquisas.(AU)


Rehabilitation of partially edentulous patients is a viable treatment with an excellent prognosis. However, bone height availability is a crucial factor in the treatment success. The maxillary sinus lift, despite poor bone quality of posterior maxilla, is considered a predictable procedure. However, reconstructive techniques in the mandible do not have the same predictability and present more postoperative morbidity. Despite the high predictability of short implants, there are few studies evaluating the real necessity for splinting implants in posterior atrophic mandible. Thus, the objective of the present study was to evaluate the need for a short implant to be splinted to another. After analysis of the literature, this review concluded that unitary short implants presented equal predictability compared to splinted implants. However, the heterogeneity of the studies and the lack of randomized clinical trials justify the development of new research.(AU)

Se realizó una investigación descriptiva, longitudinal y prospectiva en la Clínica Estomatológica Provincial Docente "Mártires del Moncada" de Santiago de Cuba en el período comprendido desde septiembre de 2010 hasta julio de 2012, a fin de evaluar la evolución de 17 pacientes tratados con implantes roscados marca MIS, con abutmen o atache de bola, cargados inmediatamente, para lo cual se convirtió la misma prótesis que portaban en una sobredentadura mandibular soportada en la mucosa y retenida por el implante, pues su aparato protésico convencional carecía de la funcionalidad deseada. La rehabilitación con ese tipo de sobredentadura fue efectiva en aquellos pacientes con problemas en los principios biomecánicos de retención y estabilidad, así como también en los que presentaban una buena disponibilidad de hueso y en los que se colocaron implantes de mayor longitud, todos los cuales respondieron mejor al tratamiento; por consiguiente, se elevó su calidad de vida al proporcionarles confort y seguridad con el uso de su aparato protésico.


A descriptive longitudinal and prospective study was conducted in "Martires del Moncada" Provincial School of Dentistry of Santiago de Cuba from September 2010 to July 2012 in order to evaluate the progress of 17 patients treated with threaded implants MIS, abutmen or ball attachment, immediately loaded, for which the same prosthesis became a mandibular overdenture supported on the mucosa and retained by the implant, because their conventional prosthetic device lacked the desired functionality. The rehabilitation with this type of overdenture was effective in those patients with compromised biomechanical principles of retention and stability, as well as in those having a good bone availability and in those with greater length implants, thus responding better to treatment. Therefore, their quality of life increased by providing comfort and safety with the use of this prosthetic device.

Se realizó un estudio descriptivo, longitudinal y prospectivo para describir la evolución de 55 pacientes tratados con 65 implantes unitarios de carga inmediata (marca MIS o Nano), colocados sobre huesos cicatrizados en la Clínica Estomatológica Provincial Docente de Santiago de Cuba durante el período comprendido desde septiembre de 2010 hasta enero de 2012. Entre los criterios más desfavorables para considerar la estética inadecuada en algunas restauraciones, sobresalieron el color y la posición de estas; asimismo, la higiene bucal deficiente, la masticación de alimentos duros y la no asistencia a los controles clínicos fueron las indicaciones más incumplidas. La rehabilitación con implante unitario de carga inmediata resultó exitosa en 86,2 % de los integrantes de la casuística y 38 de ellos (74,5 %) se mostraron satisfechos con los resultados terapéuticos de la técnica, cuya aplicación contribuyó a la integridad biopsicosocial y calidad de vida de los pacientes.


A descriptive, longitudinal and prospective study was carried out to describe the progress of 55 patients treated with 65 immediately loaded single implants (MI or Nano types), placed on healed bones in the Teaching Provincial Stomatological Clinic in Santiago de Cuba during the period from September, 2010 to January, 2012. Among the most unfavorable approaches to consider the inadequate aesthetics in some restorations, there were the color and their positions; also, the scarce buccal hygiene, the chewing of hard foods and the non attendance to the clinical appointments were the most unfulfilled indications. The rehabilitation with immediately loaded single implant was successful in 86.2% of the members of the case material and 38 of them (74.5%) were satisfied with the therapeutic results of the technique which application contributed to the bio psychosocial integrity and life quality of the patients.

RESUMEN

The replacement of a single missing anterior tooth with an implant-supported crown is a demanding therapy. This report describes a treatment in which an anterior maxillary implant was immediately restored with a provisional restoration. During the provisional phase, an optimal emergence profile was created by adjusting the provisional restoration. An impression was made with an individually fabricated impression post for an accurate reproduction of the established emergence profile and, finally, a screw-retained all-ceramic crown was placed. By implementing this protocol, an optimal definitive result could be achieved, together with immediate patient satisfaction.

PURPOSE: After the introduction of concept of osteointegration, dental implantology have been successful procedure in the dental field. Recently, it has shown successful results when used to restore single tooth missing. Considering the difference in bone quality of the mandible and maxilla, and the increased occlusal force in the posterior region, the success rates in each region may be different. In this study, success rates of single implants placed in the mandibular first and second molar areas were analyzed. MATERIALS AND METHODS: The subjects were patients (284 patients, 308 implants) who had been operated with single implant installation from 2002 to 2009 in seven dental clinics in Daegu city. One hundred sixty eight implants were placed in the mandibular 1st molar and 140 implants were placed in the mandibular 2nd molar. They were analyzed according to implant site, age, sex, length and diameter. RESULTS: The survival rates of single implant of this study were 97.6% in the mandibular 1st molar and 92.9% in the mandibular 2nd molar. In the mandibular 1st molar, 4 implants were failed. In the mandibular, 2nd molar, 10 implants were failed. CONCLUSION: The restoration of the mandibular 1st molar using single implant was found to be clinically acceptable treatment and showed higher survival rate than mandibular 2nd molar single implant. Single implant in mandibular 2nd molar needs careful consideration of poor bone quality, risk of overloading and anatomical structure of the mandible.

El edentulismo parcial puede ser eficazmente rehabilitado con la utilización de implantes oseointegrados. Con los avances de la implantología se ha alcanzado el restablecimiento tanto funcional como estético de áreas edéntulas y principalmente reduciendo el tiempo de tratamiento. Para lograr estos objetivos utilizando implantes oseointegrados, se han desarrollado innumerables técnicas quirúrgicas. En este relato de caso clínico se realizaron dos técnicas quirúrgicas concomitantes asociadas, la instalación del implante con carga inmediata e injerto de tejido conjuntivo. El objetivo de esta asociación de técnicas fue preservar la mucosa queratinizada en el CONIC® - Conexão Sistema de Prótese Ltda) fue instalado en la región con un excelente trabamiento inicial de 50 N.cm determinado por el torquímetro. Esta condición permitió optar por realizar carga inmediata. Por lo tanto fue seleccionado e instalado el abutment del tipo ceraone (Conexão Sistema de Prótese Ltda) y confeccionada la prótesis provisional de resina acrílica.


The partial edentulism might be rehabilitated using dental implants. The advances in implantology had reached the functional reestablishment and esthetics in edentulism areas and decreasing the time of the treatments. These advantages of dental implants may be used in association with different surgical techniques. In this case report two surgical techniques were associated, the placement of dental implant with immediate loading and the connective tissue graft. The aim of this technique was the preservation of the queratinizated and bone tissue, periimplant marginal sealing, and obtaining esthetics in a short term period.

PURPOSE: This retrospective study was performed to analyze the relationship between complications of the posterior mandibular single crowns and distance from the adjacent teeth to the implant. SUBJECTS AND METHODS: Of the patients who presented Ewha Womans University Mokdong Hospital and Yonsei University Dental Hospital with missing of the posterior mandibular molar and restored with implant-supported 18 Single crowns between 1996 thru 2007, 115 patients had been followed after crown delivery. The subjects were divided into complication-followed group and a control without any problems. The distance from the most distal tooth to the implant were measured. The prosthetic and biologic complications were reviewed by the cantilever distance and analyzed by abutment type, age and gender statistically using SAS version 9.1 (SAS Inc., USA). RESULTS AND CONCLUSION The results were as follows; 1) The posterior mandibular single crown with cantilever showed higher incidence of follow-up complications upon logistic analysis (p<0.05). 2) The prosthetic and biologic complications are related with the cantilever distance with 2.1 odds ratio and 3.39 cut-off value of specificity and sensitivity by SPSS 12.0. 3) The complications are neither significant in abutment types nor age and gender.

STATEMENT OF PROBLEM: Little information is available about the buccal gingival level of multiple implant restorations. PURPOSE: This study was aimed to evaluate the relationship between width and height of buccal soft tissue around single and 2 adjacent implant restorations. MATERIAL AND METHODS: Four implant restoration groups (first and second molars, single second molars, posterior single restorations between teeth, and anterior single restorations between teeth) were randomly chosen from one dental institute. Each group comprised of 6 patients. After 6 months of function, silicone impressions were taken and stone models were fabricated for each restoration group. The stone models were cut in bucco-lingual direction at the most apical point of buccal gingival margin. The height and width of buccal supra-implant soft tissue were measured. One way ANOVA and Tukey HSD post hoc tests were performed to analyze the data obtained (P < .05). RESULTS: The most unfavorable width-height ratio was noted for the group, which was comprised of the second molar in the multiple adjacent (first and second molar) implant-supported restorations. The group also resulted in the shorter height of buccal supra-implant mucosa rather than that of anterior single implant restorations between natural teeth. CONCLUSION: To achieve a favorable level of buccal gingival margin, greater thickness of buccal supra-implant mucosa is required for the implant restorations without a neighboring natural tooth compared to the implant restorations next to a natural tooth.

BACKGROUND: TiUnite(TM) is a highly crystalline and phosphate enriched titanium oxide surface which has a unique porous surface structure. This improved implant surface enhances bone response and reduces healing period. It also assures early stability of implant. These help to increase the success of implant. The aim o f this s tudy i s to e valuate the survival r ate of TiUnite TM surfaced single implant. MATERIALS AND METHODS: A retrospective analysis of 89 TiUnite(TM) surfaced implants replacing a single tooth was assessed according to their dental record. The age of the patients ranged from 17 to 82 years (mean age: 45.8 +/- 14.6). Data were recorded regarding the survival rate of these implants. RESULTS: Fifty-two implants (57%) were placed in the maxilla, and 37 (43%) in the mandible. Over 75% were placed in the posterior area. Of the placed implants, 67% were the wide type, while 25% were the regular type and only 8% were of the narrow type. The single implants produced an overall clinical survival rate of 96.6% over the observation period (mean 17.9 months). Among 89 implants, only 2 implants were removed and one implant was submerged. CONCLUSION: According to t he se data, TiUnite(TM) surfaced implant in a single tooth restoration showed favorable survival rate although this study was done in a short term period.

This study was performed to investigate the survival rate of single implant used in maxillary anterior region during follow up periods. 231 patients whose single missing tooth in maxillary region had been replaced with 237 implant at the periodontal dept. of Yonsei University Hospital between February 1993 and December 2004. The following results are compiled from 231 patients who received single implant surgery. 1. The major cause of single tooth loss in maxillary anterior region is trauma, followed by periodontal disease and congenital missing. 2. The total survival rate of single implant placed in maxillary anterior region is 94.5%. 3. The survival rate of single implant placed in type II and type IV was 100% and in type lll was 92.7%. As for the bone quantity, the survival rate in type A(100%) was most, followed by type B(97.3%) and type C, D (93.5%). 4. The survival rate of implant placement combined with advanced surgery was 94.4%. The results showed that the placement of single implant is considered as a reliable treatment option for a single missing tooth in maxillary anterior region except in certain extreme conditions especially with poor bone quality and quantity.

RESUMEN

For a missing teeth, orthodontic treatment may be a better choice of treatment in comparison to a conventional prosthetic replacement such as FPD, resin bonded prosthesis in view of aesthetics, periodontal health and function. Occasionally after an orthodontic treatment, an insufficient space may occur. The mini-implant could be an alternative in situations of narrow ridge dimension, where conventional root form implant could be compromised. The aim of this clinical report is to describe how a space that could not be restored with a traditional root form endosteal implant was managed and to present a technique to achieve optimal anterior esthetics in single implant restoration.

Osseointegrated implnats have proven to be successful in both full and partial edentulous patients since the 1960s and recently have shown successful results when used to restore single tooth missing. However, in most studies reporting the success of single implants, single implants replacing anterior teeth are more frequently mentioned than posterior single implants. Moreover, in studies regarding posterior single implants, the replaced region seemed to be variable; the maxilla, mandible and areas from the first premolar to the second molar were mentioned. However, considering the difference in bone quality in the mandible and maxilla, and the increased occlusal force in the posterior region, the success rates in each region may be different. In this study, the cumulative success rates and amount of bone loss of single implants replacing the mandibular first and second molar, respectively, were compared and analyzed to come to the following conclusion. 1. The 20 (20 persons) single implants that were placed in the mandibular first molar region were all successful and showed a 100% 5 year cumulative success rate. Among the 27 (24 persons) single implants replacing the mandibular second molar, 8 failed (27.63%) showing a 5 year cumulative success rate of 70.37%. 2. Among the 8 failed implants, one showed symptoms of postoperative infection and one complained of parenthesia. 6 implants failed after functional loading; 5 showed mobility and one resulted in fixture fracture. 3. After the attachment of the prosthesis, there was no significant statistical difference regarding the marginal bone loss in group 1 and group 2 during the checkup period (P>0.05). In conclusion, restoration of the mandibular first molar using single implants was found to be an excellent treatment modality, and when replacing mandibular second molars with single implants, poor bone quality and risk of overloading must be considered.

The main disadvantage of cement-retained implant restorations is their difficulty in retriev-ability. Advocates of cemented implant restorations frequently state that retrievability of the restoration can be maintained if a provisional cement is used. The purpose of this study was to find the optimal properties of provisional luting cements and the surface treatment of abutments in single implant abutment system. 30 prefabricated implant abutments, height 8mm, diameter 6mm, 3-degree taper per side, with light chamfer margins were obtained. Three commercially available provisional luting agents which were all zinc oxide eugenol type ; Cavitec, TempBond and TempBond NE were eval-uated. No cement served as the control. TempBond along with vaseline, a kind of petrolatum(2:1 ratio) was also evaluated. Ten out of thirty abutments were randomly selected and abutment surfaces were sandblasted with 50 mu m aluminum oxide. Another ten abutments were sandblasted with 250 mu m aluminum oxide. A vertical groove, 1 mm deep and 5mm long was cut in each twenty abutments. Ten of them were sandblasted with 50 mu m aluminum oxide. The full coverage casting crowns were cemented to the abutments with the designat-ed provisional luting agent. Specimens were stored in distilled water at 37 degrees C for 24 hours. Each specimen was attached to a universal testing machine. A crosshead speed of 0.5mm/min was used to apply a tensile force to each specimen. Within the limitations of this in vitro study, the following conclusions were drawn: 1. Tensile bond strength of provisional luting cements in no surface treatment decreased with the sequence of TempBond NE, TempBond, Cavitec, TempBond with vaseline, no cement. 2. Tensile bond strength more increased by surface treatment. Sandblasting with 250 mumaluminum oxide exhibited the highest tensile bond strength in the abutment cemented with TempBond NE and sandblasting with 50 mu m aluminum oxide exhibited the highest tensile bond strength in cemented with TempBond. 3. In the aspect of a groove formation, tensile bond strength significantly increased in TempBond with vaseline only and the others had no significant effect on tensile bond strength.
