La incontinencia urinaria de esfuerzo es la pérdida involuntaria de orina durante una maniobra de esfuerzo físico, ejercicio, estornudo o tos. Afecta aproximadamente al 15% de las mujeres de 30-60 años y su prevalencia es del 30-41%. Aunque existen terapias conservadoras para su manejo, muchas pacientes terminarán necesitando cirugía para su resolución. Las mallas suburetrales son alternativas para el manejo quirúrgico, existiendo dos vías de instalación, la transobturadora (TOT o TVT-O) y la retropúbica (del inglés tension-free vaginal tape o TVT), siendo esta última la que presenta mejores resultados y menos complicaciones posoperatorias. Objetivo: evaluar la tasa de efectividad y las complicaciones de la TVT en la Unidad de Piso Pélvico Femenino del Hospital El Carmen de Maipú entre los años 2015 y 2020. Materiales y Métodos: Se obtuvieron 715 registros de pacientes que fueron sometidas a TVT y se logró contactar telefónicamente con el 60,69% de ellas. Resultados: Los resultados muestran una tasa de efectividad del 94,8% y una tasa de complicaciones del 2,3%. Conclusión: Este estudio aporta evidencia local de los resultados posoperatorios en la IOE en pacientes que requirieron la instalación de una malla suburetal retropúbica, demostrando ser una cirugía altamente efectiva y segura.


Stress urinary incontinence is the involuntary loss of urine during physical exertion, exercise, sneezing, or coughing. It affects approximately 15% of women aged 30-60, with a prevalence of 30-41%. Although there are conservative therapies for its management, many patients will eventually require surgery for resolution. Suburethral sling are considered for surgical management, and there are two installation alternatives, transobturator (TOT or TVT-O) and retropubic (tension-free vaginal tape or TVT), with the latter presenting better results and fewer postoperative complications. Objetive: to evaluate effectiveness rate and complications of the TVT in the Female Pelvic Floor Unit of Hospital El Carmen de Maipú between 2015 and 2020. Materials and Methods: A total of 715 patient records were obtained for those who underwent TVT, and 60.69% of them were successfully contacted by telephone. Results: The results show an effectiveness rate of 94.8% and a complication rate of 2.3%. Conclusion: This study provides local evidence for the results of stress urinary incontinence that required the placement of a retropubic suburethral sling, proving to be a highly effective and safe surgery.

Chronic intestinal pseudo-obstruction (CIPO) is caused by a plethora of primary and secondary causes, dominantly involving the neuromuscular tissue, interstitial cells of Cajal, or the connective tissue framework. The lack of the connective tissue framework, known as desmosis, is evaluated by Masson's trichrome (MT) or picrosirius red stains, both of which are recommended in the London classification. We evaluated the orcein stain in detecting desmosis in comparison to the MT stain. We performed both orcein and MT stains in six previously published cases of complete or partial desmosis along with six age-matched controls. Our results showed comparable results of the orcein stain as compared to the MT stain. Additional advantages of lower cost and a clearer background in orcein stain were noteworthy, whereas MT stain can be used for the detection of additional pathology. We believe that orcein stain can be used as a cheap alternative in resource-limited settings.

Objective@#This study evaluated the functional and cosmetic outcomes after bilateral frontalis sling repair using either expanded polytetrafluoroethylene (ePTFE) or silicone rod for unilateral or bilateral severe congenital ptosis with poor levator function.@*Methods@#This was a non-comparative, interventional case series involving 30 patients with unilateral or bilateral severe congenital ptosis who underwent bilateral frontalis sling repair from February 2012 to November 2018. Medical records including pre- and postoperative photographs were reviewed. The functional and cosmetic outcomes were assessed.@*Results@#There were 15 patients with severe congenital ptosis who underwent ePTFE surgery and 15 patients who had silicone rod surgery. Baseline levator function measured was less than 5 mm in all patients. The mean follow-up duration after surgery was 15.6+6.9 months. Satisfactory eyelid contour was observed in all patients (100%). Postoperatively, all had good to excellent primary eyelid position and effective frontalis action on eyelid elevation. Complications included exposure of the silicone sling material after trauma at the brow area (n=1) and recurrence of ptosis (n=2).@*Conclusion@#Bilateral frontalis suspension in patients with severe congenital ptosis, whether bilateral or unilateral, resulted in excellent functional and cosmetic outcomes using ePTFE or silicone rod.

Objective:To observe the impact of core stability training in the sling exercise therapy pattern on the balance of stroke survivors.Methods:Sixty stroke survivors were randomly divided into a control group ( n=30) and an experimental group ( n=30). In addition to routine rehabilitation training, the control group received traditional core stability training, while the experimental group underwent core stability training applying the sling exercise therapy pattern. Before and after 4 weeks of training, the standing balance of both groups was evaluated using the Prokin balance trainer, with the length and area of motion recorded when performing the balance test with the eyes open and closed. Surface electromyography was used to record the average EMG (AEMG) values of the bilateral erector spinae and multifidus muscles during the balance testing. Results:After the 4 weeks of training the length and the area of progression of the center of the pressure were both significantly smaller for both groups than before the training with the eyes both open and closed. The average length and area in the experimental group were significantly less than among the controls. The AEMG values recorded during the balance tests were significantly higher than those before the intervention for both groups, with the multifidus muscle averages on the affected side significantly greater in the experimental group than among the controls when performing the balance test with the eyes closed.Conclusion:Core stability training in the sling exercise therapy pattern is superior to conventional core stability training because it can better improve the balance of stroke survivors.

Stress urinary incontinence (SUI) following prostatectomy is an important and common problem in male, which can significantly reduce the quality of life. Urethral sling is a promising treatment for male SUI. It is easy to operate and can avoid the insertion of prosthesis, making it highly acceptable to patients. In recent years, with the advancement of technology, several kinds of slings have been put into clinical application and have achieved great outcomes. This article reviews the application of urethral sling in the treatment of SUI after prostatectomy.

【Objective】 To analyze the causes of the failure of previous operations in female patients with stress urinary incontinence (SUI), and to evaluate the efficacy and safety of modified transobturator outside-in tape (TOT). 【Methods】 A total of 42 female SUI patients who had failed in previous anti-incontinence surgery during Aug.2016 and Jul.2022 were enrolled. The causes of failure were analyzed. All patients were treated with modified TOT, and the operation time, estimated blood loss, duration of catheterization, symptom improvement and complications were observed. The outcomes were evaluated with the International Consultation on Incontinence Questionnaire Short Form (ICI-Q-SF), pad test and urodynamic study. 【Results】 All operations were technically successful. The average operation time was (32.4±18.6) min, and estimated blood loss was (54.2±34.5) mL. There were no obvious complications. After operation, the ICI-Q-SF score significantly decreased (P0.05). The overall effective rate was 88.1%. 【Conclusion】 Modified TOT can be used for the treatment of patients with failed anti-SUI surgery, which is simple and easy to perform, with a low complication rate and satisfactory postoperative results.

la incontinencia urinaria (IU), se define por la Sociedad Internacional de Continencia, como la perdida involuntaria de orina, una condición potencialmente tratable con disminución de la calidad de vida de las mujeres en diferentes aspectos, entre ellos la salud sexual con su pareja y su bienestar social. Se han descrito que solo el 44,9 % de las mujeres consultan, teniendo mayores tasas de depresión y ansiedad. El objetivo de esta investigación fue determinar la aparición de complicaciones intraoperatorias y posoperatorias con la realización de cintas transobturadora y retropúbica en el Hospital Militar Central en Bogotá durante el periodo 2015 al 2020. Se realizó un estudio observacional, descriptivo, retrospectivo de corte longitudinal, en el Hospital Militar Central en Bogotá; se incluyeron todas las pacientes con diagnósticos de incontinencia urinaria, tanto de esfuerzo, como de urgencias y/o mixtas, intervenidas con cintas medios uretrales libres de tensión por vía retropúbica o transobturadora. Las complicaciones posteriores al procedimiento quirúrgico la más prevalente fue el dolor con un 96,8 %, seguida de infección urinaria o expulsión de la banda con un 8,6 %; entre otras complicaciones menos frecuentes como hematomas, dispareunia y abscesos. La mayoría de las pacientes obtuvieron una adecuada respuesta clínica en el postoperatorio, el 77 % no requirió ningún manejo adicional. Las cintas mediouretrales, proporcionan un buen resultado con una baja incidencia de complicaciones quirúrgicas y recidiva, con mejoría de la calidad de vida de la paciente frente a su patología de incontinencia urinaria.


The International Continence Society defines urinary incontinence (UI) as the involuntary loss of urine, a potentially treatable condition with a decrease in the quality of life of women in different aspects, including sexual health with their partner and their social well-being. It has been described that only 44.9% of women consult, having higher rates of depression and anxiety. The objective of this research was to determine the occurrence of intraoperative and postoperative complications with the performance of transobturator and retropubic taping at the Central Military Hospital in Bogota during the period 2015 to 2020. An observational, descriptive, retrospective, retrospective. All patients with diagnoses of urinary incontinence, both stress, emergency, and/or mixed, operated with tension-free mid-urethral tapes by retropubic or transobturator route were included. The most prevalent complications after the surgical procedure were pain (96.8 %), followed by urinary tract infection or expulsion of the band (8.6 %), among other less frequent complications such as hematomas, dyspareunia, and abscesses. Most patients obtained an adequate clinical response in the postoperative period, 77 % did not require additional management. Mediourethral tapes provide a good result with a low incidence of surgical complications and recurrence, improving the patient's quality of life concerning her urinary incontinence pathology.

Foi relatado que apenas 44,9% das mulheres consultam, apresentando taxas mais altas de depressão e ansiedade. O objetivo desta pesquisa foi determinar a ocorrência de complicações intraoperatórias e pós-operatórias com fita transobturatória e retropúbica no Hospital Militar Central de Bogotá, Colômbia, durante o período de 2015 a 2020. Foi realizado um estudo observacional, descritivo, retrospectivo de corte longitudinal no Hospital Militar Central de Bogotá, incluindo todos os pacientes diagnosticados com incontinência urinária, tanto de esforço, de emergência e/ou mista, operados com slings mediouretrais sem tensão via retropúbica ou transobturatória. As complicações mais prevalentes após o procedimento cirúrgico foram dor (96,8%), seguida de infecção do trato urinário ou expulsão da faixa (8,6%), entre outras complicações menos frequentes, como hematomas, dispareunia e abscessos. A maioria dos pacientes teve uma resposta clínica adequada no pós-operatório e 77% não precisaram de nenhum tratamento adicional. Os slings mediouretrais proporcionam um bom resultado com baixa incidência de complicações cirúrgicas e recorrência, com melhora na qualidade de vida da paciente com relação à sua patologia de incontinência urinária.

RESUMEN

Systemic tuberculosis mostly affects the lungs and ocular involvement is rare. The ocular manifestations are varied, and rarely peripheral ulcerative keratitis (PUK) occurs either due to direct invasion of the microorganism or due to immune reaction. We present a case of advanced PUK in a 28?year?old male patient diagnosed with pulmonary TB in the right eye with hourglass cornea. The ulcer was progressing circumferentially with necrosis of corneal stroma and undermined margins similar to Moorens ulcer, and was not responding to anti?tuberculosis treatment and topical steroids. We describe a modified novel surgical technique using tenons sling annular patch graft to manage the progressive advanced PUK with hourglass cornea to optimize visual and structural outcome in our patient.

Objective:To evaluate the long-term efficacy of AdVance sling bulbar urethral suspension and artificial urethral sphincter (AUS) implantation in the treatment of moderate to severe male stress urinary incontinence.Methods:The clinical data of 12 male patients with urinary incontinence who underwent surgical treatment in Peking University People's Hospital from June 2011 to June 2017 were retrospectively analyzed. The median age was 75(64-80) years. There were 9 cases after radical prostatectomy and 3 cases after transurethral prostatectomy. Patients had a median history of urinary incontinence of 3(1-9) years, and needed an average of 8(5-10) pads per day. Among them, 5 patients had moderate urinary incontinence and 7 patients had severe urinary incontinence. All patients underwent urinary incontinence surgery for the first time. Among the 12 patients, 6 received AdVance sling ball urethral suspension (AdVance group), and 6 received AUS implantation (AUS group). The median age of the AdVance group was 72 (64-73) years. The median number of pads used daily was 6 (5-8) tablets. Urinary incontinence Quality of Life questionnaire (I-QOL) score was (15.0±5.4). Five patients had moderate urinary incontinence and one patient had severe urinary incontinence. In the AUS group, the median age was 78(76-80) years old, the median daily pad use was 8(6-10) tablets, and the I-QOL score was (16.7±5.1), all of which were severe urinary incontinence. The daily pad usage, I-QOL and postoperative complications were recorded at 1 and 5 years after operation.Results:All patients completed the operation successfully. The postoperative follow-up was 5-7 years (mean 5.5 years). In AdVance group, 1 patient with severe urinary incontinence had no significant improvement in postoperative symptoms at 1 year after operation. The other 5 patients showed significant improvement in urinary incontinence symptoms. In the AdVance group, the median number of pads used per day was 2.5 (1-10), and the I-QOL score was (75.0±28.1), which were all significantly improved compared with that before operation ( P<0.05). The median number of pads used per day in the AdVance group 5 years after operation was 2.5(1-10), and the I-QOL score was (78.3±29.3), which were significantly improved compared with those before operation (all P<0.01). In the AUS group, no pad was needed at 1 year after operation, which was significantly improved compared with that before operation ( P<0.01). Urethral erosion occurred in 2 cases 3 years after operation, and the AUS was removed. Urinary incontinence recurred and returned to the preoperative state without reoperation. The other 4 cases did not need to use the pad 5 years after operation. In AdVance group, 3 patients had perineal pain within 3 months after operation, which was related to activity and relieved spontaneously. No wound infection, urethral erosion and other complications occurred. Urethral erosion occurred in 3 cases in AUS group. Conclusions:AdVance sling ball urethral suspension is effective for patients with moderate stress urinary incontinence and has fewer complications. AUS implantation is effective for patients with severe male stress urinary incontinence. However, the long-term complications of this operation may affect the postoperative efficacy.

Objective:To investigate the safety and effectiveness of tension adjustment technique using anatomical landmarks during retropubic midurethral synthetic sling.Methods:The data of 36 consecutive female patients with urinary incontinence, who had underwent retropubic midurethral synthetic sling procedure from January to August 2019 were analyzed retrospectively. The mean age was (60.83±7.93) years old and the body mass index was (24.43±2.44) kg/m 2. Among the recruited subjects, 36 had positive stress test and Marshall-Marchetti test. 20 (55.6%) were pure stress urinary incontinence, and 16 (44.4%) were mixed urinary incontinence. The severity of incontinence was classified into mild (5 cases, 13.9%), moderate (14 cases, 38.9%), severe (13 cases, 36.1%) and very severe (4 cases, 11.1%) using one-hour pad tests. Urodynamics were performed in 17 cases, with 5 (29.4%) presented detrusor overactivity, 3 (17.7%) possessed intrinsic sphincter deficiency. For each case, the tension of the sling was adjusted based on the anatomical landmarks, i. e. using an angled clamp attached closely to the pubic symphysis ventrally and the tip parallel to the edge of hymen dorsally. All patients were catheter-free right after the procedure. The subjective and objective effectiveness, and safety (the rate of urinary retension after surgery and postvoid residual volume 3 months later) were evaluated.The subjective cure rate was was defined as complete leakage free or very mild leakage during excessive bladder filling and fierce cough. The subjective effectiveness was defined as over 50% improvement of the leakage symptom. The objective cure rate was defined as a negative stress test. Results:For all 36 patients, the median hospital stays was 8 (5-95)h. No bladder perforation or transfusion cases. All patients were catheter-free right after the procedure, with no incidence of urinary retention. 27 patients completed a 3-month follow-up, with 22 had post-void residual data, 23 had subjective effectiveness data and 23 had objective effectiveness data. The median post-void residual was 7.5 (5-64) ml, subjective cure rate was 91.3% (21/23), and objective cure rate was 95.7% (22/23). 8.7% (2/23) reported difficult urination alleviated without the necessity of clinical interference. No urethra erosion or vagina extrusion was found. At 2-year follow-up, 34 patients completed assessment by phone. The subjective cure rate was 91.2% (31/34), with only 2.9% (1/34) reported difficult urination. Besides, at 3-month follow-up, there was no difference regarding the subjective cure rate [100.0%(12/12) vs. 81.8%(9/11)]or objective cure rate [91.7%(11/12) vs. 100.0%(11/11)] between patients with stress and mixed incontinence. No difference was noted among patients with mild, moderate, severe and very severe leakage[75.0% (3/4) vs. 100.0%(6/6) vs. 90.0%(9/10) vs. 100.0%(3/3)]. Of the 12 cases with urodynamic records, the presence of detrusor overactivity [66.7%(2/3) vs. 88.9%(8/9)] or intrinsic sphincter deficiency [0(0/1) vs. 90.9%(10/11)] did not significantly affected the cure rate of the procedure. At 2-year follow-up, there was no difference regarding the subjective cure rate between patients with stress and mixed incontinence [94.7%(18/19) vs. 86.7%(13/15)]. No difference was also noted among patients with mild, moderate, severe and very severe leakage[80.0%(4/5) vs. 100.0%(13/13) vs. 83.3%(10/12) vs. 100.0%(4/4)]. Of the 16 cases with urodynamic records, the presence of detrusor overactivity [60.0%(3/5) vs. 90.9%(10/11)]or intrinsic sphincter deficiency [66.7%(2/3) vs. 84.6%(11/13)]did not significantly affected the cure rate of the procedure.Conclusions:Tension adjustment using anatomic landmarks during sling procedure is safe and feasible for urinary incontinence, with minimum complications and residual volume, and high subjective/objective cure rate.

RESUMEN

Pulmonary artery sling (PAS) with bronchial bridge malformation is a very rare developmental malformation of vascular and trachea. In the past 2 years, we treated 3 children with pulmonary artery sling complicated with bronchial bridge, all of whom were clinically characterized by recurrent cough, asthma and dyspnea, which were confirmed by cardiac color ultrasound and chest CT three-dimensional reconstruction. All of the 3 children underwent surgical treatment, and no recurrent wheezing or respiratory tract infection occurred after surgery.

@#Objective To observe the effect sling exercise therapy (SET) and Tuina on radicular cervical spondylosis. Methods From August, 2015 to December, 2016, 72 patients with radicular cervical spondylosis were randomly divided into control group (n = 36) and trial group (n = 36), who accepted traction and SET+Tuina, respectively, for four weeks. They were measured F-wave conduction velocity with electromyogram from median nerve and ulnar nerve, somatosensory evoked potential (SEP), and current perception threshold (CPT) before and after treatment. The clinical ratio of improvement was calculated. Results The ratio of improvement was 83.33% in the trial group more than 58.33% in the control group (Z = 2.093, P < 0.05). F-wave conduction velocity increased in both groups after treatment (t > 12.059, P < 0.001), and increased more in the trial group than in the control group (t > 3.266, P < 0.01); while the latency of SEP decreased in N9 and N13 in both groups (t > 7.061, P < 0.001), and decreased more in the trial group than in the control group (t > 8.033, P < 0.001); the grade of CPT decreased in both groups (t > 8.895, P < 0.001), and decreased more in the trial group than in the control group (t = 8.913, P < 0.001). Conclusion The combination of SET and Tuina can promote the repair of nerve conduction of cervical spondylotic radiculopathy.

ABSTRACT Objective: Describe the medium-term safety of the tension free vaginal tape obturator (TVT-O) procedure in terms of complications, cure and changes in quality of life (QoL) after the surgery. Materials and methods: Descriptive historical cohort that included women over 18 years of age who underwent TVT-O due to objectively proven stress urinary incontinence, urethral hypermobility or mixed urinary incontinence in which the stress component predominated, confirmed on urodynamic testing between July 2013-April 2017, in a reference hospital located in the city of Murcia Spain. Women with previous anti-incontinence surgery, concomitant vaginal surgery and planning pregnancy were excluded. Follow-up was determined for each patient based on the time elapsed between surgery and the time when the research protocol was applied. Complications were stratified according to the modified Clavien-Dindo classification; also we evaluated subjective cure rate, quality of life using the ICIQ-SF score, before and after surgery. Results: The mean age was 52.6 (SD± 10.5) years and 80.1% of patients were at least overweight. The incidence of complications at 12 months was: 8.3% (12/144). We did not detect complications after this period in the followed patients at 24, 36 and 48 months. The subjective cure determined at 12, 24, 36 and 48 months was 62.5% (90/144), 59.09% (55/88), 50.81% (31/61) and 50% (7/14), respectively. There was a significant improvement in quality of life, as determined by the ICQ-SF mean score before and after surgery (13.76[6,34] vs 3.84[5.76]; p<0.05). Conclusions: The TVT-O surgery is a safe therapy associated with a low complication incidence at 12 months, an acceptable subjective cure rate in stress urinary incontinence, and quality-of-life improvement. Classifications of complications related to the insertion of the prosthesis and of those inherent to surgery, such as urinary tract infection, are required.


RESUMEN Objetivo: describir la seguridad a mediano plazo del procedimiento con el cabestrillo suburetral transobturador dentro-fuera (en inglés: tension free vaginal tape obturator TVT-O) en términos de: complicaciones, cura y cambios en la calidad de vida después de la cirugía. Materiales y métodos: cohorte histórica descriptiva que incluye mujeres mayores de 18 años intervenidas de TVT-O por incontinencia urinaria de esfuerzo (IUE) objetivamente comprobada, hipermovilidad uretral o incontinencia urinaria mixta en la que predominó el componente de esfuerzo, confirmada en prueba urodinámica entre julio del 2013 y abril del 2017 en un hospital de referencia ubicado en la ciudad de Murcia, España. Se excluyeron mujeres con cirugía previa de incontinencia, cirugía vaginal concomitante y planificación del embarazo. El seguimiento se determinó para cada paciente por el tiempo transcurrido desde la cirugía hasta el momento en que se aplicó el protocolo de investigación. Las complicaciones se estratificaron según la clasificación de Dindo-Clavien modificada, además, se evaluaron la tasa de curación subjetiva y la calidad de vida mediante el International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF) antes y después de la cirugía. Resultados: la edad media de las pacientes fue de 52,59 (DE±10,46) años, el 80,1 % tenía al menos sobrepeso. La incidencia de complicaciones en los primeros 12 meses fue del 8,3%. No detectamos complicaciones después de este periodo en las pacientes seguidas a 24, 36 y 48 meses. La curación subjetiva determinada a los 12, 24, 36 y 48 meses fue del 62,5% (90/144), 59,09% (55/88), 50,81% (31/61) y 50% (7/14), respectivamente. Hubo una mejoría significativa en la calidad de vida, en términos de puntuación media ICQ-SF antes y después de la cirugía (13,76 [6,34] vs 3,84 [5,76]; p < 0,05). Conclusiones: el TVT-O es una terapia segura, con baja tasa de complicaciones a los 12 meses, aceptable tasa de curación subjetiva de la IUE y una mejora en la calidad de vida. Se necesitan clasificaciones de complicaciones relacionadas con la inserción de la prótesis y que incluyan complicaciones inherentes a la cirugía, como la infección del tracto urinario.

Abstract Objective To evaluate whether performing preoperative urodynamic study influences postoperative urinary symptoms of women with stress urinary incontinence that underwent transobturator sling. Methods Retrospective analysis of patients treated for stress urinary incontinence by transobturator sling from August 2011 to October 2018. Predictor variables included preoperative urodynamic study, age, incontinence severity, body mass index, preoperative storage symptoms and previous anti-urinary incontinence procedure. Outcome variables were postoperative subjective continence status, storage symptoms and complications. Logistic regression after propensity score was employed to compare outcomes between patients who underwent or not pre-operative urodynamic study. Results The present study included 88 patients with an average follow-up of 269 days. Most patients (n = 52; 59.1%) described storage symptoms other than stress urinary incontinence, and 38 patients (43.2%) underwent preoperative urodynamic studies. Logistic regression after propensity score did not reveal an association between urinary continence outcomes and performance of preoperative urodynamic study (odds ratio 0.57; confidence interval [CI]: 0.11-2.49). Among women that did not undergo urodynamic study, there was a subjective improvement in urinary incontinence in 92% of the cases versus 87% in those that underwent urodynamic study (p = 0.461). Furthermore, postoperative storage symptoms were similar between women who did not undergo urodynamic study and those who underwent urodynamic study, 13.2% versus 18.4%, respectively (p = 0.753). Conclusion Preoperative urodynamic study had no impact on urinary incontinence cure outcomes as well as on urinary storage symptoms after the transobturator sling in women with stress urinary incontinence.


Resumo Objetivo Avaliar a influência do estudo urodinâmico pré-operatório nos resultados miccionais pós-operatórios em mulheres com incontinência urinária de esforço submetidas a sling transobturador. Métodos Análise retrospectiva de mulheres com incontinência urinária de esforço submetidas a sling transobturador entre agosto de 2011 e outubro de 2018. As variáveis preditoras pré-operatórias, entre outras, foram a realização do estudo urodinâmico, gravidade da incontinência e sintomas urinários de armazenamento. As variáveis de desfecho pós-operatórias foram o status subjetivo da continência, sintomas de armazenamento urinário e complicações cirúrgicas. A regressão logística após o escore de propensão foi empregada para comparar os resultados entre os pacientes que foram submetidos ou não ao estudo urodinâmico pré-operatório. Resultados Foram incluídas no presente estudo 88 pacientes com um seguimento médio de 269 dias. A maioria das pacientes apresentava sintomas miccionais de armazenamento (n = 52; 59,1%) concomitantes à incontinência urinária de esforço. Um pouco menos da metade das pacientes (n = 38; 43,2%) foram submetidas a estudo urodinâmico pré-operatório. A regressão logística após o escore de propensão não revelou associação entre os resultados de continência urinária e a realização de estudo urodinâmico pré-operatório (odds ratio 0,57; intervalo de confiança [IC]: 0,11-2,49). Além disso, os sintomas de armazenamento urinário pós-operatórios foram similares entre as pacientes que não realizaram e aquelas que realizaram o estudo urodinâmico, 13,2% e 18,4% respectivamente (p = 0,753). Conclusão O estudo urodinâmico pré-operatório não teve impacto nos resultados de continência urinária, bem como nos sintomas de armazenamento urinário após o sling transobturatório.

Objective:To explore the effect of sling-massage exercise on neck type cervical spondylopathy (NTCS). Methods:From June, 2020 to February, 2021, 68 patients with NTCS from the Second Affiliated Hospital of Shandong University of Traditional Chinese Medicine were randomly divided into control group (n = 34) and observation group (n = 34). The control group accepted routine massage, while the observation group accepted sling exercise and massage based on weak link test, for four weeks. They were assessed with McGill Pain Qusetionnaire (MPQ) and Neck Disability Index (NDI) before and after treatment, and measured median frequency (MF) of the upper trapezius muscle on the affected side with electromyography biofeedback before treatment, after treatment, two weeks after treatment and 4 weeks after treatment. Results:Four cases in the control group and three cases in the observation group dropped down. The scores of MPQ and NDI decreased in both groups after treatment (|Z| > 4.879, P < 0.001), while MF increased (P < 0.001). There was no significant difference in scores of MPQ and NDI between the two groups after treatment (|Z| < 1.100, P > 0.05), while MF was less in the control group than in the observation group (t = 2.184, P < 0.05). MF decreased more in the control group than in the observation group, and was continually less in the control group than in the observation group after treatment (t > 3.770, P < 0.001). Conclusion:Sling-massage exercise is as effective as massage on NTCS immediately, and for more time.

Abstract The placement of a suburethral sling is standard treatment for stress urinary incontinence. The transobturator technique (TOT) emerged as an alternative to minimize the risks of the blind insertion of needles, leading to a lower rate of perforation complications compared with the retropubic approach. We present a case of injury to a branch of the left obturator artery following the placement of a urethral sling using TOT, followed by intense bleeding and hemodynamic instability, which was treated with embolization.


Resumo Sling de uretra média é o tratamento padrão para a incontinência urinária de esforço. A abordagem transobturatória (TOT) surgiu como alternativa para minimizar os riscos da inserção às cegas das agulhas com taxa de complicações perfurativas menores quando comparadas à abordagem retropúbica. Apresentamos um caso de lesão em ramo da artéria obturatória esquerda após sling TOT que evoluiu com sangramento intenso e instabilidade hemodinâmica, sendo tratado com embolização.

RESUMEN

Closed loop small bowel obstruction is a surgical emergency, which when left untreated leads to vascular compromise resulting in intestinal ischemia, necrosis and perforation. We report the case of a 61 years old female with past surgical history of hysterectomy and a mid-urethral sling, who presented to the emergency department for abdominal pain and obstipation. She was found on imaging to have a closed loop small bowel obstruction. An exploratory laparotomy revealed an adhesive band encompassing the distal terminal ileum, visceral peritoneum and the Mid‐urethral slings mesh. This is a rare complication that, to our knowledge, has not been reported in the surgical literature. This paper will discuss the clinical presentation, diagnostic studies, therapeutic intervention and outcome of this unique case.

ABSTRACT Introduction : The surgical management of stress urinary incontinence uses the transobturator tape (TOT) technique since 2006. We present a study applying the suburethral tape as a better alternative that does not require the TOT needle. Objective : To demonstrate the advantages of the suburethral tape placement technique over the transobturator tape technique in the surgical treatment of stress urinary incontinence. Methods : Observational comparative study using the non- invasive suburethral tape placement versus the transobturator tape technique. Results : Fifty patients were treated with TOT for stress urinary incontinence and other 50 patients were managed with suburethral tape placement (UTP) in the previous shift the same day or the preceding day. The TOT patients were 56 years old on average, and time of the procedure was 18 minutes on average. Complications included pain in the inguinal area (13 cases), mesh extrusion (2 cases), and suture dehiscence of anterior colporrhaphy, presence of left inguinoperineal granuloma due to mesh reaction and bruising in the left obturator area (one case of each). The 50 patients treated with UTP were 56 years old on average, operating time was 7 minutes on average, and there were no complications. The cost of the TOT kit was about US$ 1 000.00, and the suburethral tape cost was US$ 50.00. Conclusions : In this study of stress urinary incontinence treatment, the suburethral tape technique was safer than the transobturator tape technique and had a lower cost.


RESUMEN Introducción . Entre los tratamientos quirúrgicos de la incontinencia urinaria de esfuerzo se emplea la técnica de la cinta transobturatriz (TOT, por sus siglas en inglés) desde el 2006 hasta la actualidad. Se presenta un estudio con la aplicación de la cinta uretral (UTP, por sus siglas en inglés) como mejor alternativa al no usar la aguja del TOT. Objetivo . Demostrar las ventajas de la aplicación de la cinta suburetral versus la cinta transobturatriz en el tratamiento quirúrgico de la incontinencia urinaria de esfuerzo. Métodos . Estudio observacional, comparativo con la técnica no invasiva empleando cinta suburetral versus la técnica de la cinta transobturatriz. Resultados . Se evaluó 50 pacientes intervenidas con TOT, quienes precedieron un turno el mismo día o el día precedente a las pacientes operadas con la cinta uretral. El promedio de edad fue 56 años y el tiempo promedio del procedimiento fue 18 minutos. Las complicaciones consistieron en dolor en la zona inguinal (13 casos), extrusión de malla (2 casos) y, una de cada caso, dehiscencia de sutura de la colpoporrafia anterior, presencia de granuloma inguinoperineal izquierdo por reacción a la malla, hematoma en zona obturatriz izquierda. En los 50 casos tratados con cinta suburetral, el promedio de edad fue 56 años, el tiempo operatorio 7 minutos y no existió complicación alguna. El costo del kit de la TOT fue alrededor de US$ 1 000.00; la cinta suburetral costó US$ 50.00. Conclusiones . En la presente serie de manejo de la incontinencia urinaria de esfuerzo, la técnica de la cinta suburetral resultó más segura que la técnica de la cinta transobturatriz y fue de menor costo.

Urge urinary incontinence (UUI) is one of the most troublesome complications of surgery of the prostate whether for malignancy or benign conditions. For many decades, there have been attempts to reduce the morbidity of this outcome with variable results. Since its development in the 1970s, the artificial urinary sphincter (AUS) has been the "gold standard" for treatment of the most severe cases of UUI. Other attempts including injectable bulking agents, previous sphincter designs, and slings have been developed, but largely abandoned because of poor long-term efficacy and significant complications. The AUS has had several sentinel redesigns since its first introduction to reduce erosion and infection and increase efficacy. None of these changes in the basic AUS design have occurred in the past three decades, and the AUS remains the same despite newer technology and materials that could improve its function and safety. Recently, newer compressive devices and slings to reposition the bladder neck for men with mild-to-moderate UUI have been developed with success in select patients. Similarly, the AUS has had applied antibiotic coating to all portions except the pressure-regulating balloon (PRB) to reduce infection risk. The basic AUS design, however, has not changed. With newer electronic technology, the concept of the electronic AUS or eAUS has been proposed and several possible iterations of this eAUS have been reported. While the eAUS is as yet not available, its development continues and a prototype device may be available soon. Possible design options are discussed in this review.

Objective:To observe the effect of pelvic floor neuromuscular electrical stimulation (NMES) and sling exercise training (SET) on diastasis recti abdominis after parturition. Methods:From September to December, 2019, 90 patients with rectus abdominis separation > 2 cm and pelvic floor muscle strength above grade III were randomly divided into three groups, accepting simple pelvic floor NMES (group A), pelvic floor NMES and SET (group B), and simple SET (group C), respectively, for four weeks. The separation distance of rectus abdominis was measured before treatment, two weeks and four weeks after treatment, and four weeks follow-up. Results:The separation distance of rectus abdominis decreased in each group after treatment (F > 8.327, P < 0.01); and it was the least in group B (F > 8.327, P < 0.01), while the multiple comparison results showed that there was no significant difference between group A and group C (P > 0.05). Conclusion:Both pelvic floor NMES and SET can similarly relieve the diastasis recti abdominis after parturition, and it is more effective in combination.
