RESUMEN

We developed poly-ε-caprolactone (PCL)-based nanoparticles containing D-α-tocopherol polyethylene glycol-1000 succinate (TPGS) or Poloxamer 407 as stabilizers to efficiently encapsulate genistein (GN). Two formulations, referred to as PNTPGS and PNPol, were prepared using nanoprecipitation. They were characterized by size and PDI distribution, zeta potential, nanoparticle tracking analysis (NTA), GN association (AE%), infrared spectroscopy (FT-IR), and differential scanning calorimetry (DSC). PNTPGS-GN exhibited a particle size of 141.2 nm, a PDI of 0.189, a zeta potential of -32.9 mV, and an AE% of 77.95%. PNPol-GN had a size of 146.3 nm, a better PDI than PNTPGS-GN (0.150), a less negative zeta potential (-21.0 mV), and an AE% of 68.73%. Thermal and spectrometric analyses indicated that no new compounds were formed, and there was no incompatibility detected in the formulations. Cellular studies revealed that Poloxamer 407 conferred less toxicity to PCL nanoparticles. However, the percentage of uptake decreased compared to the use of TPGS, which exhibited almost 80% cellular uptake. This study contributes to the investigation of stabilizers capable of conferring stability to PCL nanoparticles efficiently encapsulating GN. Thus, the PCL nanoparticle proposed here is an innovative nanomedicine for melanoma therapy and represents a strong candidate for specific pre-clinical and in vivo studie

Background: Shoulder Impingement is one of the most common causes of shoulder pain in adults. Scapular muscle imbalances results in impaired scapular orientation with altered scapular kinematics and altered glenohumeral rhythm. Purpose of study: To study the effect of motor control retraining of scapular stabilizers versus mulligan mobilisation with movement in shoulder impingement patients. Methodology: 60 patients with positive Hawkins, kinetic medial rotation and/or Neer impingement test were taken for the study as subjects. They were then divided into two groups of 30 each- Group A and Group B. Both the groups were assessed and reassessed for(i) pain status using VAS (Visual Analogue Scale) both at rest and on internal rotation, (ii)Shoulder Flexion, Extension, Shoulder Abduction, Adduction, Internal and External Rotation Range Of Motion (ROM), (iii)Functional Scale SPADI( Shoulder Pain and Disability Index) pre and post the intervention. Group A received motor control retraining of scapular stabilizers whereas group B received Mulligan’s mobilization with movement for 6 weeks (3 times in a week). Both the groups received conventional physiotherapy. Data analysis was done using Wilcoxon sign ranked test for intragroup comparison and Manwhitney test for intergroup comparison. Results: The results showed statistically significant improvement in shoulder pain, ROM and shoulder function in motor control retraining as compared to mulligan mobilisation with movement. Conclusion: Motor control retraining of scapular stabilizers is more effective treatment technique as compared to mulligan mobilisation with movement in shoulder impingement.

El ligamento colateral lateral (LCL), que forma parte de los estabilizadores de la rodilla, constituye una estructura importante dentro del complejo posterolateral de la misma (CPL), que restringe al varo, en los ángulos de flexión de esta, especialmente a los 30°, además de controlar la rotación externa. Son infrecuentes las lesiones que afectan a este complejo, etiológicamente entre el 50- 90% se asocian a otras lesiones ligamentosas con predominio del 60% de daño del ligamento cruzado posterior. La técnica de Larson se basa en la reconstrucción del LCL y del complejo poplíteo, utilizando una inserción proximal única en el epicóndilo femoral. Presentamos el caso de un paciente masculino de 28 años de edad, sin antecedentes clínicos, que acude debido a un varo forzado de la extremidad mientras realizaba actividad deportiva y tras recibir impacto directo en la región lateral interna de pantorrilla derecha, posterior a lo cual presenta dolor a los movimientos pasivos e imposibilidad a la deambulación. Además, presenta equimosis, edema, escoriaciones superficiales en cara lateral externa y dolor a la digitopresión en trayecto de ligamento colateral externo. A maniobras de exploración de rodilla: signo de bostezo en varo positivo, cepillado rotuliano positivo, hiperpresión de rotula positivo, limitación funcional a la flexión y extensión de la articulación. Esta técnica, Larson modificada, resulta como una de las más prometedoras por sus varias características, como la menos exigente y al mismo tiempo brindando resultados alentadores. Este procedimiento se fundamenta en la reconstrucción del LCL y del tendón poplíteo (TP) en conjunto, resumiéndose así la reconstrucción del Ligamento Poplíteo Peroneo (LPP) como estabilizador estático del TP, con su inserción a nivel de la cabeza del peroné en conjunto con la del LCL y la restitución de ambas inserciones en el epicóndilo femoral, tomando en cuenta la morfología del peroné y los ángulos de movimientos de la rodilla.


The lateral collateral ligament (LCL), which is part of the knee stabilizers, constitutes an important structure within the posterolateral complex of the same (CPL), which restricts the varus, in the flexion angles of this, especially at 30°, in addition to controlling external rotation. Lesions that affect this complex are infrequent, etiologically between 50-90% are associated with other ligamentous injuries with a predominance of 60% damage to the posterior cruciate ligament. The Larson technique is based on the reconstruction of the LCL and the popliteal complex, using a single proximal insertion in the femoral epicondyle. We present the case of a 28-year-old male patient, with no medical history, who attended due to a forced varus of the limb while performing sports activity and after receiving a direct impact on the internal lateral region of the right calf, after which he presented pain to passive movements and inability to ambulation. In addition, he presents ecchymosis, edema, superficial abrasions on the external lateral aspect and pain on acupressure in the course of the external collateral ligament. A knee examination maneuvers: positive varus yawn sign, positive patellar brushing, positive patella hyperpressure, functional limitation to flexion and extension of the joint. This modified Larson technique turns out to be one of the most promising due to its various characteristics, as the least demanding and at the same time providing encouraging results. This procedure is based on the reconstruction of the LCL and the popliteal tendon (PT) as a whole, thus summarizing the reconstruction of the Peroneal Popliteal Ligament (LPP) as a static stabilizer of the PT, with its insertion at the level of the head of the fibula in conjunction with the of the LCL and the restitution of both attachments in the femoral epicondyle, taking into account the morphology of the fibula and the angles of movement of the knee.

RESUMEN La sedimentación de partículas es un fenómeno común en dispersiones alimentarias, que está asociado con una calidad deficiente y se puede controlar usando diferentes aditivos, dentro de los cuales, se destacan el uso de hidrocoloides. Se evaluó el efecto de la concentración de goma xantano (0,025-0,075%), carboximetilcelulosa sódica (0,025-0,075%) y gel de aloe vera (0,5-1,5%), en las propiedades fisicoquímicas, sensoriales y estabilidad a la sedimentación, en bebidas elaboradas a partir de maíz dulce. No se observó variación significativa en el contenido de sólidos solubles totales, pH, densidad, acidez titulable, potencial ζ, tamaño de partículas, velocidad de sedimentación y parámetros de color con las concentraciones empleadas de hidrocoloides y de aloe vera. El potencial ζ indicó estabilidad ligera en las bebidas (>-30mV) y la floculación de la emulsión, lo que se evidencia en la distribución del tamaño de partícula (1-600μm). La viscosidad y la estabilidad a la sedimentación aumentaron con concentraciones >0,05% de GX y CMC, reflejándose en una mayor valoración sensorial de la apariencia. El aumento en la concentración de aloe vera no mostró efecto en la estabilidad de las bebidas.


ABSTRACT Particle sedimentation is a common phenomenon in food dispersions, which is associated with poor quality and can be controlled using different additives within which the use of hydrocolloids stands out. The effect of the concentration of xanthan gum (0.025-0.075%), sodium carboxymethylcellulose (0.025-0.075%) and aloe vera gel (0.5-1.5%), on physicochemical, sensory properties and sedimentation in beverages made from sweet corn, were evaluated. No significant variation was observed in the content of total soluble solids, pH, density, titratable acidity, ζ potential, particle size, sedimentation index and color parameters with the concentrations of hydrocolloids and aloe vera. The potential ζ indicated slight stability in beverages (> -30mV) and emulsion flocculation, which is evidenced in the particle size distribution (1-600μm). The viscosity and sedimentation stability increased with concentrations >0.05% of GX and CMC, reflected in a higher sensory assessment of appearance. The increase in aloe vera concentration showed no effect on the stability of the beverages.

RESUMEN A finales de la década de los '90, la cirugía de revascularización miocárdica sin circulación extracorpórea tomó gran auge, por lo que -en muchos centros de cirugía cardíaca- pasó a ocupar la manera más frecuente de realizarla, en aras de reducir sus complicaciones. Para ejecutarla, las diferentes firmas de fabricantes de equipos médicos pusieron a disposición de la comunidad quirúrgica estabilizadores retráctiles para facilitar el posicionamiento cardíaco por succión y presión, para revascularizar, sobre todo, los sectores lateral y posterior; además, para la estabilización del segmento coronario. Estos dispositivos han mostrado seguridad y eficacia; sin embargo, el paciente que se presenta constituye un caso aislado, donde se produjo disección epicárdica secundaria al uso de estabilizadores. A pesar de su complejidad, esta complicación pudo ser solucionada con técnica de parche pericárdico circular endomiocárdico y reparación ventricular, y el paciente tuvo una evolución satisfactoria.


ABSTRACT At the end of the 1990s, off-pump myocardial revascularization surgery gained momentum. Hence, a number of cardiac surgery centers adopted it as the most frequent surgical procedure to reduce complications. To implement it, the many medical equipment companies provided the surgical community with retractable stabilizers to facilitate cardiac positioning by suction and pressure, especially for lateral and posterior wall revascularization and coronary segment stabilization. These devices have been shown to be safe and effective; however, the patient presented is a rare case, in which epicardial dissection occurred secondary to the use of stabilizers. Although complex, this complication was eventually resolved with an endomyocardial circular pericardial patch technique and ventricular repair. The patient had a satisfactory outcome.

Of the different phases of bipolar disorder, bipolar depression is more prevailing and is more difficult to treat. However, there is a deficit in systemic research on the pharmacological treatment of acute bipolar depression. Therefore, consensuses on the pharmacological treatment strategies of acute bipolar depression has yet to be made. Currently, there are only three drugs approved by the Food and Drug Administration for acute bipolar depression : quetiapine, olanzapine-fluoxetine complex, and lurasidone. In clinical practice, other drugs such as mood stabilizers (lamotrigine, lithium, valproate) and/or the other atypical antipsychotics (aripiprazole, risperidone, ziprasidone) are frequently prescribed. There remains controversy on the use of antidepressants in bipolar depression. Here, we summarized the evidence of current pharmacological treatment options and reviewed treatment guidelines of acute bipolar depression from recently published studies.

In the majority of cases of bipolar disorder, manic episodes are usually brief and typically responsive to currently available psychopharmacological agents. In contrast, depressive manifestations are more prevalent and persistent, and can present as major depressive/mixed episodes or residual interepisode symptoms. The depressive phase is often associated with other neuropsychiatric conditions, such as anxiety spectrum disorders, substance use disorders, stressor-related disorders, and eating disorders. It is viewed as a systemic disease with associated ailments such as metabolic syndrome, diabetes mellitus, and cardiovascular disease. There is an increased rate of mortality not only from suicide, but also from concomitant physical illness. This scenario is made worse by the fact that depressive symptoms, which represent the main disease burden, are often refractory to existing psychotropic drugs. As such, there is a pressing need for novel agents that are efficacious in acute depressive exacerbations, and also have applicable value in preventing recurrent episodes. The rationale of the present review is to delineate the pharmacotherapy of the depressive phase of bipolar disorder with medications for which there is evidence in the form of observational, open-label, or double-blind randomized controlled studies. In the treatment of acute bipolar depression in adults, a comprehensive appraisal of the extant literature reveals that among mood stabilizers, the most robust proof of efficacy exists for divalproex sodium; while atypical antipsychotics, which include olanzapine, quetiapine, lurasidone, and cariprazine, are also effective, as demonstrated in controlled trials.

Objective To compare the differences of prescription pattern, adverse events, economic burden and drug-use adherence between preferred mood stabilizers and preferred antipsychotics in patients with bipolar disorder. Methods We investigated 240 cases of patients with bipolar disorder from 39 mental health institutions in 11 cities in Hebei province. The self-made questionnaire was used to investigate the demographic information, disease characteris-tics, prescription pattern and medical expense. The clinical global impression scale-severity of illness (CGI-SI) was used to assess the disease severity. The treatment emergent symptom scale (TESS) was used to assess the adverse drug reac-tions. The medication adherence rating scale (MARS) was used to assess drug therapy compliance in patients. Results One hundred fifty-two patients (63.3%) used antipsychotics as the first choice (antipsychotics group), 88 patients (36.7%) used mood stabilizers as the first choice (mood stabilizers group). The number of patients in-patient (90.1%vs. 76.1%), patients with psychotic symptom (27.0%vs. 11.4%), incidences of adverse events (46.1%vs. 31.8%), drug daily cost (me-dians 12.00 yuan vs. 8.37 yuan) and drug total cost (medians 344.61 yuan vs. 144.64 yuan) were larger in antipsychotics group than in mood stabilizers group (P0.05). Conclusion The bipolar disorder patients more frequently use antipsychotics as the first choice in Hebei province. The use of antipsychotics does not alter the combina-tion medication pattern. In addition, antipsychotics cause a higher incidence of adverse events and heavier economic bur-den compared with mood stabilizers, suggesting that mood stabilizers should be the first choice to bipolar disorder.

El presente trabajo consiste en una revisión de la evidencia que existe hasta la fecha respecto a tratamientos psicosociales y farmacológicos para el trastorno de ánimo bipolar (TAB) en población pediátrica. En cuanto a tratamientos psicosociales destacan: Grupo Psicoeducativo Multifamiliar de Fristad, el Programa Rainbow de Pavuluri, Terapia Focalizada en la Familia para adolescentes, Terapia Interpersonal y de Ritmos Sociales para Adolescentes y la Terapia Dialéctica Comportamental para adolescentes (DBT). Se ha visto que los tratamientos comparten ciertas características, tales como, psicoeducación, establecimiento de hábitos y rutinas, y el trabajo con las familias en términos de desarrollo de habilidades sociales, habilidades de comunicación, afrontamiento al estrés y resolución de conflictos, que serían de relevancia para la mejoría de la sintomatología anímica, la recurrencia de los cuadros anímicos, la adherencia al tratamiento y la prevención de recaídas. Las investigaciones realizadas en intervenciones farmacoterapéuticas muestran que los antipsicóticos atípicos son eficaces en el tratamiento de episodios agudos (maníacos o mixtos), y que podrían ser superiores a los estabilizadores del ánimo en este grupo etáreo. La escasez de estudios en el tratamiento de depresión bipolar y de largo plazo limitan las conclusiones del tratamiento farmacológico en estas fases. Tanto para intervenciones psicosociales como farmacológicas se necesita mayor investigación, ya que existen pocos estudios que aborden el tratamiento del TAB pediátrico, y faltan ensayos controlados aleatorios.


This article is an updated revision of the psychosocial and pharmacological treatments for Pediatric Bipolar Disorder (PBD). Psychosocial treatments include: Fristad Multi-family Psychoeducation Groups, Pavuluri's Rainbow Program, Family Focused Therapy for adolescents, Interpersonal and Social Rhythm Therapy for adolescents and the Dialectical Behavior Therapy for adolescents. These interventions have common characteristics: psychoeducational interventions, habit and routine establishment, social and communication skills training and problem solving skills training. The evidence suggests that these characteristics would help to recover from mood symptoms, delay recurrent episodes, contribute to treatment adherence, and prevent recurrence. Research in pharmacologic interventions shows that atypical antipsychotics are efficacious in treating acute episodes (manic or mixed states), and could be superior to mood stabilizers in this age group. Lack of studies regarding treatment of bipolar depression and long-term treatment limit the conclusions for these pha- ses. Further research is warranted for both psychosocial and pharmacological interventions, specially randomized controlled trials.

RESUMEN

Bipolar disorder (BD) is a chronic disorder which usually has its onset in early adulthood. At one end of the spectrum is depression and at other is mania. Like many psychiatric illnesses, it is not treatable but its symptoms are completely manageable with medications. Commonly used drugs are mood stabilizers and atypical antipsychotics along with adjunctive medications such as anxiolytics and antidepressants. In general, a combination of these drugs is used for treatment. These drugs have significant adverse effects which add to the burden of the disease. Presently, there are 11 US Food and Drug Administration - approved drugs for management of acute mania, 3 for bipolar depression and 7 for bipolar maintenance. This review article details the use of these drugs in BD.

RESUMEN

Psychiatric disorders are among the most debilitating of all medical illnesses. Whilst there are drugs that can be used to treat these disorders, they give sub-optimal recovery in many people and a significant number of individuals do not respond to any treatments and remain treatment resistant. Surprisingly, the mechanism by which psychotropic drugs cause their therapeutic benefits remain unknown but likely involves the underlying molecular pathways affected by the drugs. Hence, in this review, we have focused on recent findings on the molecular mechanism affected by antipsychotic, mood stabilizing and antidepressant drugs at the levels of epigenetics, intracellular signalling cascades and microRNAs. We posit that understanding these important interactions will result in a better understanding of how these drugs act which in turn may aid in considering how to develop drugs with better efficacy or increased therapeutic reach.

OBJECTIVES: Mechanisms of clinical synergistic effects, induced by co-treatments of lithium and valproate, are unclear. Extracellular signal-regulated kinase (ERK) has been suggested to play important roles in mechanisms of the action of mood stabilizers. In this study, effects of co-treatments of lithium and valproate on the ERK1/2 signal pathway and its down-stream transcription factors, ELK1 and C-FOS, were investigated in vitro. METHODS: PC12 cells, human pheochromocytoma cells, were treated with lithium chloride (30 mM), valproate (1 mM) or lithium chloride + valproate. The phosphorylation of ERK1/2 was analyzed with immunoblot analysis. Transcriptional activities of ELK1 and C-FOS were analyzed with reporter gene assay. RESULTS: Single treatment of lithium and valproate increased the phosphorylation of ERK and transcriptional activities of ELK1 and C-FOS, respectively. Combined treatments of lithium and valproate induced more robust increase in the phosphorylation of ERK1/2 and transcriptional activities of ELK1 and C-FOS, compared to those in response to single treatment of lithium or valproate. CONCLUSIONS: Co-treatments of lithium and valproate induced synergistic increase in the phosphorylation of ERK1/2 and transcriptional activities of its down-stream transcription factors, ELK1 and C-FOS, compared to effects of single treatment. The findings might suggest potentiating effects of lithium and valproate augmentation treatment strategy.

Psicoestimulantes (PSE), estabilizadores del ánimo (EA) y benzodiacepinas (BDZ) son psicofármacos frecuentemente utilizados en psiquiatría infantojuvenil y cuya indicación ha aumentado considerablemente en los últimos años. Los PSE son empleados principalmente en el déficit atencional, siendo la terapia de primera línea en pacientes sin comorbilidad. Modafinilo y anfetaminas serían igualmente efectivos y se utilizan con similar frecuencia en muchos países sin embargo existe evidencia de mayores riesgos con anfetaminas. Otros fármacos como atomoxetina, a-agonistas, bupropión y antidepresivos tricíclicos (imipramina) se utilizan como tratamiento de segunda o tercera línea. Los EA son especialmente útiles en el tratamiento del trastorno bipolar (TB) y se consideran agentes de primera línea: litio, ácido valproico (sodio divalproex) y algunos antipsicóticos atípicos. Otros EA que han demostrado efectividad en TB son lamotrigina, carbamazepina, oxcarbamazepina y topiramato. Las DBZ han sido poco estudiadas en población infantojuvenil, pero su uso no es infrecuente. Si bien no son una indicación de primera línea en trastornos de ansiedad, están indicadas como tratamiento adyuvante especialmente diazepam. Fluracepam tiene un rol en el tratamiento de insomnio en mayores de 15 años. Aunque el tratamiento de elección de muchas patologías de psiquiatría infantojuvenil es claro en diversas guías clínicas, la complejidad terapéuticas de estos pacientes muchas veces hace necesario el uso de psicofármacos de segunda o tercera línea, muchos de ellos no aprobados por la Food and Drug Administration. Es estos casos cobra vital importancia el seguimiento estricto, con especial cuidado en los efectos colaterales, que pueden ser nocivos para el desarrollo del paciente.


Psycho stimulants (PSE), mood stabilizers (ME) and benzodiazepines (BDZ) are psychotropic’s frequently used in children and adolescents psychiatry and their indication has increased considerably in the last year. The PSAE are used mainly in attentional deficits being first-line treatment in patients without co-morbidity. Modafinil and amphetamines are supposed to be equally effective and used with similar frequency in many countries, however there is evidence of bigger risk with amphetamines. Other drugs such as Atomoxetine, a-agonist, bupropion, tricyclic antidepressants (imipramine) are used as second or third line treatment. The ME are especially useful as treatment of bipolar disorder (BD); lithium, divalproex sodium and some atypical antipsychotics are considered first line treatment. Other ME that have showed effectiveness in TB are lamotrigine, carbamazepine, oxcarbamazepina and topiramate. The BDZ haven´t been studied too much in young population, however its uses not uncommon. Although it is not a first line indication in anxiety disorders they are indicated as adjunctive treatment, especially diazepam. Fluracepam has a role in the treatment of insomnia in children over 15 years. While the treatment of choice for many pathologies of juvenile psychiatry is clear in several clinical guidelines, the therapeutic complexity of these patients often requires the use of psychopharmacology of 2nd or 3rd line, many of them not approved by the Food and Drug Administration in these cases is of vital importance a strict monitoring, with particular attention to side effects, which can be harmful to the patients development.

A obtenção de protoplastos de fungos, utilizando-se enzimas degradadoras de parede celular, tem sido o método mais utilizado em processos de transformação genética. Foram testados dois tipos de estruturas fúngicas (micélio e conídios), diferentes concentrações enzimáticas (5, 10, 20 mg), estabilizadores osmóticos (NaCl 0,7 mol.L-1 pH 5,7; (NH4)2SO4 1,2 mol.L-1 pH 5,8; KCl 0,7 mol.L-1 pH 5,8; MgSO4 0,7 mol.L-1 pH 5,5; Sacarose 0,5 mol.L-1 pH 5,7; SorbitoL 0,6 mol.L-1 pH 5,7) e seis tempos de exposição dos protoplastos ao sistema lítico, para estabelecer condições otimizadas de obtenção e regeneração de protoplastos de Colletotrichum gloeosporioides, agente relacionado a mancha manteigosa em cafeeiros. Protoplastos de C. gloeosporiodes foram obtidos em maior quantidade quando o micélio foi exposto durante 4 horas, com 10 mg.mL-1 de Lysing Enzime em KCl 0,7 mol.L-1 que se apresentou como melhor estabilizador osmótico, com frequência de regeneração de 11,64%.


The isolation and regeneration of protoplasts from fungal cells, using several cell wall degrading enzymes, has been the most common method to prepare competent cells for genetic studies of filamentous fungi. In this work two types of fungal structures, different enzyme concentrations (5, 10, 20 mg), osmotic stabilizers (NaCl 0,7 mol.L-1 pH 5,7; (NH4)2SO4 1,2 mol.L-1 pH 5,8; KCl 0,7 mol.L-1 pH 5,8; MgSO4 0,7 mol.L-1 pH 5,5; Sacarose 0,5 mol.L-1 pH 5,7; Sorbitol 0,6 mol.L-1 pH 5,7), and six exposure times to establish optimum conditions of isolation and regeneration of protoplasts of Colletotrichum gloeosporioides, agent of blister spot on coffee, were tested. Protoplast of C. gloeosporioides were obtained in greater quantities when the mycelium was exposed for 5 hours with 15mg.mL-1 of Lysing Enzyme in 0.7 mol.L -1 of KCl as osmotic stabilizer, presenting a regeneration rate of 11.64%.

RESUMEN

Psychopharmacology has developed over approximately the past five decades. The remarkable proliferation of information in this area has made it difficult for clinicians to understand the characteristics of various psychotropic agents. Atypical antipsychotics including amisulpride, asenapine, aripiprazole, blonanserin, clozapine, iloperidone, lurasidone, olanzapine, paliperidone, quetiapine, risperidone, ziprasidone, and zotepine cause fewer extrapyramidal problems and have many clinical applications, but they can cause metabolic disturbances. Mood stabilizers and lamotrigine are widely used for bipolar disorder. Other novel anticonvulsants such as topiramate, oxcarbazepine, gabapentin, tiagabine, pregabalin, vigabatrin, levetiracetam, and riulzole have also been tested with diverging or inconclusive results. Antidepressants are commonly used in the clinical treatment of depression and anxiety disorder. However, the mechanism of action of medications used in the treatment of psychiatric disorders remains unclear. Understanding the mechanisms of action and clarifying the diagnosis may enhance the treatment outcome in psychiatry. In this review, we analyzed clinical pharmacology data for each drug within a class and discussed clinical strategies for administering currently available antipsychotics, mood stabilizer/anticonvulsants, and antidepressants widely used for various psychiatric indications.

In this study,thermo-adapted(Ta)PPR vaccines were assessed for their stability at 25,37,40,42 and 45℃ in lyophilized form using two extrinsic stabilizers(lactalbumin hydrolysate-sucrose(LS)and stabilizer E)and in reconstituted form with the diluents(1 mol/L MgS04 or 0.85% NaCl). The lyophilized vaccines showed an expiry period of 24-26 days at 25℃,7-8 days at 37℃ and 3-4 days at 40℃. LS stabilizer was superior at 42℃ with a shelf-life of 44 h,whereas in stabilizer E,a 40 h shelf-life with a comparable half-life was observed. At 45℃,the half-life in stabilizer E was better than LS and lasted for 1 day. Furthermore,the reconstituted vaccine maintained the titre for 48 h both at 4℃ and 25℃ and for 24-30 h at 37℃. As both the stabilizers performed equally well with regard to shelf-life and half-life,the present study suggests LS as stabilizer as a choice for lyophilization with 0.85% NaCI diluent,because it has better performance at higher temperature. These Ta vaccines can be used as alternatives to existing vaccines for the control of the disease in tropical countries as they are effective in avoiding vaccination failure due to the breakdown in cold-chain maintenance,as this vaccine is considerably more stable at ambient temperatures.

Anticonvulsivantes y estabilizadores del ánimo principalmente el ácido valproico, lamotrigina y carbamazepina, poseen una alta incidencia de reacciones adversas a medicamentos (RAM) severas, como eritema multiforme, Síndrome Stevens- Johnson y necrolisis epidérmica tóxica, asociadas. Existen signos de alarma para su sospecha diagnóstica precoz, que permiten indicar la temprana suspensión del fármaco sospechoso e iniciar la terapia de soporte únicas medidas que han demostrado una clara disminución en la mortalidad. La inmunoglobulina G intravenosa se recomienda por su seguridad, sin embargo, su rol en disminuir la mortalidad es contradictorio. Los corticoides no han demostrado cambios en la mortalidad comparados con la terapia de soporte exclusiva. Se ha intentado mantener el tratamiento con lamotrigina, por sus cualidades terapéuticas, pese a la aparición de RAM cutáneas. De hecho, en estudios recientes en pacientes que han desarrollado RAM leves a este producto se ha demostrado un éxito de reexposición de 85 por ciento-87 por ciento mediante una lenta titulación de la dosis.


Anticonvulsants and mood stabilizers mainly valproic acid, lamotrigine and carbamazepine are medications that have a high incidence of severe adverse drug reactions (ADRs), such erythema multiforme, Stevens- Johnson syndrome and toxic epidermal necrolysis. Early diagnosis based in systemic and cutaneous alarm signs have been described, allowing premature discontinuation of suspected drugs and start supportive therapy; these are the only measures that have that have shown clear reduction in mortality. The use of intravenous immunoglobulin G is recommended for their safety, but studies regarding their role in reducing mortality are conflicting. Corticosteroids have not proved changes in mortality compared with exclusive supportive care. Due to therapeutic quality Lamotrigine is used despite the incidence of ADRs. In fact in recent studies patients with mild ADRs to this drug have shown between 85 percent-87 percent of success, when patients are re-exposed through a slow increasing in dosage.

The objective of this study was to assess the clinical evidence for or against mood stabilizers as a treatment for Alzheimer's disease (AD). We searched 5 databases from their inception to January 2010. Five randomized clinical trials of mood stabilizers to treat human patients suffering from AD were included. These trials assessed the effectiveness of mood stabilizers as an adjunct treatment to conventional anti-dementia drugs on behavioral and psychological symptoms, especially on agitation. Methodological quality was assessed using the Jadad score. The results suggested a significant effect in favor of placebo on the Mini-Mental Status Examination [n=270, weight mean difference (WMD), -0.89; 95% confidence intervals (Cis) -1.69 to -0.09, P=0.03] and on the Neuropsychiatric Inventory total (NPI total) (n=51, WMD, 3.71; 95% Cis 0.15 to 7.26, P=0.04). There were no significant differences in change scores on total Brief Psychiatric Rating Scale (BPRS total),NPI/BPRS agitation, Cohen-Mansfield Agitation Inventory total and Physical Self Maintenance Scale between mood stabilizers and placebo. Only one of these studies was free of methodological limitations (Jadad score=5). In conclusion, based on the existing evidence, mood stabilizers are ineffective or even harmful as a treatment for AD.

La conjuntivitis alérgica es una de las causas más comunes de consulta externa en oftalmología y tiene una muy alta relación con las reacciones alérgicas sistémicas. La fisiopatología de las reacciones de hipersensibilidad nos hace entender los cuadros clínicos característicos de esta patología y sus diferentes grados de severidad. El diagnostico diferencial es amplio, sin embargo hay claves diagnosticas que nos orientan rápidamente al diagnostico correcto. El tratamiento se enfoca en las medidas farmacológicas y no farmacológicas, utilizándolas en conjunto para disminuir la recurrencia del cuadro y prevenir secuelas visuales.


Ocular allergies are the one of the most common pathologies in ophthalmology and have high association with systemic allergic reactions. The physiopathology of the hypersensitivity reactions make us understand the different manifestations and severity of this pathology. Differential diagnosis is wide but many diagnostic clues guide us quickly to the right disease. Treatment basis are pharmacological and non pharmacological, using them together to prevent relapses and visual sequelae.
