RESUMEN

Objective To investigate the clinical value of the real-time shear wave elastography(RT-SWE) in the assessment of salivary gland lesions by measuring the hardness of parotid gland and submaxillary gland in patients with Sj?gren′s syndrome (SS). Methods Forty patients with SS were chosen as SS group,and 35 age and gender matched healthy volunteers were chosen as control group.The patients were divided into mild SS group ( n =18) and severe SS group ( n =22) according to the decrease of uptake and secretion of salivary gland in Salivary gland scintigraphy.The Supersonic AixPlorer real-time quantitative diagnostic shear wave elastography was used to detect the parotid gland and submaxillary gland in 35 healthy volunteers and 40 patients.The RT-SWE elasticity indices of mean(Emean) was obtained and draw the SWE technology in the diagnosis of mild and severe SS patients with parotid and submandibular glands of the ROC curves. Results ① There was no significant difference in the Emean of left and right glands among the three groups (all P > 0.05). The Emean of parotid gland and submaxillary gland in patients with SS was higher than that in control group,especially in the severe SS group(all P <0.05). ②The cutoff value,sensitivity,specificity and the area under ROC for Emean with SWE diagnosing the parotid gland lesions of mild SS were 7.75 kPa,94.4%,97.1% and 0.998,and those in severe SS were 10.65 kPa, 90.9%,97.1% and 0.999,respectively.The cutoff value,sensitivity,specificity and the area under ROC for Emean with SWE diagnosing the the submaxillary glands lesions of mild SS were 8.15 kPa,100%,91.4%and 0.998,and those in severe SS were 10.40 kPa,95.5%,88.6% and 0.994,respectively.Conclusions RT-SWE can be successfully applied to assess parotid gland and submaxillary gland′s hardness in patients with SS,and provide a new method for the clinical evaluation of salivary gland diffuse lesions.

Se presenta el caso clínico de un paciente de 45 años de edad, atendido en el Departamento de Cirugía Maxilofacial del Hospital Provincial Docente Clinicoquirúrgico Saturnino Lora Torres por presentar molestias en el suelo de la boca con 3 años de evolución. Después de un estudio minucioso se comprobó que se trataba de una sialoadenitis por sialolitiasis de la glándula submaxilar derecha. Se realizó sialolectomía canalicular intrabucal. El cálculo extraído tenía forma cónica, más ancho en su porción posterior, de 3,8 cm de largo y 4,2 cm de circunferencia externa, color amarillo-verdoso y consistencia dura friable


The case report of a 45 years patient was presented. He was assisted in the Maxillofacial Surgery Department of Saturnino Lora Torres Clinical Surgical Provincial Hospital in Santiago de Cuba due to discomfort in the floor of the mouth with a course of 3 years. After a detailed study it was confirmed that it was a sialadenitis due to sialolithiasis of the right submaxillary gland. An intraoral canalicular sialolectomy was carried out. The stone removed had a conic form, wider in its posterior portion, 3.8 cm long and 4.2 cm in the external circumference, yellow-green color and friable hard consistency

RESUMEN

Objective To explore if the expression of follicle-stimulating hormone ( FSH ) and its receptor ( FSHR) exist in the rat submandibular gland tissue , which may provide a theoretic evidence for further studying on adjusting function of the FSH in submaxillary gland .Methods Twenty normal SD rats , with a mean body weight of (200 ±20)g, were adopted to collect out the submandibular gland after abdominal cavity anesthesia .The method of in situ hybridization was employed on cellular localization of FSH and FSHR .RT-PCR was used to detect if FSH and its receptor mRNA exist in the rat submandibular gland .RNA was extracted from the submandibular gland , then FSH and the core cDNA of its receptor gene were obtained with reverse transcription polymerase chain reaction and analyzed on its sequence . Results The epithelial cells of serous acinus and granular convoluted tubule in submaxillary gland of rats had FSH and FSHR mRNA hybridized signals , which were detected in cytoplasm with negative nuclei .Specific bands of FSH cDNA and FSHR cDNA were identical to be 193bp and 413bp respectively.Conclusion FSH and FSHR can be synthesized by the epithelial cells of serous acinus and granular convoluted tubule in submaxillary gland of rats , thus this study further proves that the submandibular gland is the target organ of FSH .

ObjectiveTo investigate the localization G protein couple receptor 30 (GPR30) and its mRNA in submaxillary gland, and to supply theoretic evidence for further studying functional significance of the GPR30 in submaxillary gland of rats. Methods Four male SD rats were sacrificed by cervical dislocation after the intraperitoneal anesthesia, and excised the submaxillary glands. The distribution of GPR30 and its mRNA were studied through immunohistochemistry and in situ hybridization in the experiment. After isolation of the total RNA from the submaxillary gland, RT-PCR was conducted to obtain GPR30 cDNA by using the specific primers. The products of PCR were analyzed by sequencing with Sanger's method. Results The serous acinus epithelial cells and granular convoluted epithelial cells in submaxillary gland of rats showed GPR30 immunoreactivity, which were located in cytoplasm with negative nuclei. GPR30 mRNA hybridized signals were also detected in cytoplasm in the above cells. The products of PCR is identical to that of the GPR30 sequence of rats. Conclusion The serous acinus and granular convoluted epithelial cells not only express GPR30 but also may be a target organ by rapid estrogen signaling pathway in submaxillary gland of rats. This may be involved in the functional regulation of submaxillary gland.

RESUMEN

A method has been developed to isolate cells from the submaxillary gland of mouse by treatment with pronase. Three fractions of cells have been isolated having almost equal iodide concentrating activity. The isolated cells show time dependent uphill transport of iodide. The transport is substrate-saturable, having a Km value of 0·3 μΜ for iodide. The transport is sensitive to antithyroid drugs, metabolic inhibitors and to some extent to ouabain. Pseudohalide such as thiocyanate competes with the transport of iodide. Thyroid hormones or thyroid stimulating hormone have no significant effect on the iodide transport in these cells.

RESUMEN

Cell-free extracts of submaxillary glands from rat and goat iodinate exogenously added 3,5-diiodothyronine to form 3,5,3'-triidothyronine and thyroxine. This capacity was elevated after either thyroidectomy or exposure of rats to cold (3 ±1 °C) for 5 h. However, there was no further increase of iodination of 3,5-diiodothyronine when the thyroidectomized rats were exposed to cold stress. The submaxillary extracts have the ability to incorporate radioactive iodide into 3,5-diiodothyronine, 3,5,3'-triiodothyronine and thyroxine. The presence of 3,5-diiodothyronine was also detected in the soluble supernatant of submaxillary extract.
