RESUMEN

Objective To study the efficacy of minimally invasive treatment for lower extremities venous ulcers. Methods A total of 48 patients with lower extremities venous ulcers(56 hmbs)wereexamined with venography to identify the primary lesion that resulted ulcer. According to the type and degree of hemedynamie abnormality, high ligation of the great saphenous vein combined with endovenous laser treatment, subfascial endoscopic perforator surgery, and external banding valvuloplasty of the superficial femoral vein were done in 8 eases(8 limbs),high ligafion of the great saphenous vein combined with endovenous laser treatment and subfascial endoscopic perforator surgery were carried out in 24 cases(28 limbs),and high figation of the great saphenous vein combined with endovenous laser treatment was performed on 16 cases(20 linbs). In all cases, the ulcers were eircumferentially ligated at the same time. Results After the operations, superficial varicosis, pigmentations, and eczematous relieved remarkably in the 56 limbs. In 51 limbs, heavy feeling, ache, and edema disappeared 14-18 days after operation. The ulcers of the 56 limbs healed l6-48 days after operation.During 4 to 46 months follow up,ulcers recurred in 4 limbs(7.1%).Conclusions It is effective to correct the incompetence of the superficial, perforating, and deep veins and manage the surface of ulcers for lower extremities venous ulcers.Endovenous Iaser treatment and subfascial endoscopic perforator surgery are easily performing, minimally invasive, and safe operations. The recurrence of leg ulcers can be reduced by external banding valvuloplasty of the superficial femoral vein.

RESUMEN

Objective To compare operative and non-operative treatments for intraarticular fractures of distal radius by meta-analysis. Methods The literature which had been published in English or Chinese from 1988 to 2008 on comparison between operative and non-operative treatments of intraartieular fractures of distal radius were searched for on line.Three Chinese papers and 2 English ones were eligibly included in this meta-analysis.The clinical data of the 5 papers were collected for comparing wrist function, reduction on X-rays,complications and patient's satisfaction. Results Operative treatment resulted in better reduction on X-rays,better wrist function and fewer complications but lower patient's satisfaetion than non-operative treatment. Conclusion The operative treatment should be recommended for intraarticular fractures of distal radius, but meanwhile the needs and expectations of the patient should be also addressed.

RESUMEN

Objective To evaluate the diagnosis and surgical management of appandiceal mucoceles.Method The clinical data of 50 patients with appendiceal mucoceles surgically treated from January 2004 to January 2008 were reviewed retrospectively. Result The correct preoperative diagnosis rate was 8%.Eight patients were complicated with pseudomyxoma peritonei. The canceration rate was 4%.All the 50 patients recovered uneventfully after the surgery. There was no complications,no recurrence during the follow-up period, including the 2 patients with canceration. Conclusion It was difficult to make correct diagnosis of appendiceal mucoceles preoperatively. The diagnosis should always be in mind if the B-type ultrasonography indicated a cystoadenoma.Abdominal CT coupled with necessary digestive image examinations may improve the correct diagnosis rate. Some patients may be complicated with pseudomyxoma peritonei.Occasionally this abdominal mass was also associated with malignancy.Surgery is the management of choice, and the prognosis is good.

RESUMEN

Adjuvant radiotherapy can reduce resection range, increase the possibility of organ reservation and maintain satisfactory living quality without affecting local control rate.Adjuvant radiotherapy in combination with conservative surgery has become the standard of care for extremity soft tissue sorcoma. The indications and technological aspects as well as the perspectives are overviewed in this article.
