RESUMEN

【Objective】 To evaluate the quality of suspended red blood cells (SRBC) under low-pressure and hypoxic conditions, in order to lay a theoretical foundation for airline delivery, air drop and storage of SRBC on plateau. 【Methods】 The low-pressure and hypoxic conditions (0.026 MPa and 0.047 MPa) were simulated by a pressure control device. SRBC were divided into 3 groups (5 bags/group, 1.5 U/bag). Each group was stored in 0.026 MPa environment at 2℃-6℃ for 24 hours (named 0.026 MPa), in 0.047MPa environment at 2℃-6℃ for 7 days (named 0.047 MPa), and in a blood storage refrigerator at 4℃ (named the control), respectively. The storage cell characteristics were examined on day 2, 9, 14, 28 and 35. 【Results】 We found that HCT, MCV, K+, Na+, FHb, hemolytic ratio, 2, 3-DPG and rheological properties in group 0.026 MPa and 0.047 MPa were not significant different compared with that in control (P>0.05). The consumptions of Glu in group 0.026 MPa and 0.047 MPa were significantly higher (P<0.05), and a transient increase in LAC concentration of group 0.026 MPa and 0.047MPa were observed, compared to the control. 【Conclusion】 The conditions of low-pressure and hypoxia have no significant effect on the quality of suspended red blood cells.

【Objective】 To investigate the quality changes of suspended red blood cells (SRBCs) prepared from the blood of Tibetan high Hb population, and explore the availability and safety of blood collected from Tibetan high Hb population. 【Methods】 The voluntary blood donors were grouped according to the Hb concentration at the initial screening: female blood donors from Tibet Autonomous Region (>3 500 m) with Hb≥190 g/L and male blood donors with Hb≥210 g/L were classified as plateau high hemoglobin group. A total of 13 male blood donors from Tibet Autonomous Region were recruited. And the female blood donors (n=13) with Hb(115~165) g/L and male blood donors (n=12) with Hb(120~185) g/L from Chengdu were classified as control group. Whole blood of 200 mL specification was centrifuged to remove the plasma, and MAP additive solution was added to prepare SRBCs, then SRBCs were divided into four aliquots (50 mL/bag and stored at 4℃. Parameters as blood routine, free Hb and hemolysis rate were measured aseptically at day 1, 14, 21, 35 of storage. And 10 mL SRBCs was used to extract membrane proteins for tyrosine phosphorylation detection of band 3 protein. 【Results】 The RBCs counts(×1012/L), hematocrit(%) and hemoglobin(g/L) of Tibetan high Hb group and control group were 6.76±0.95 vs 4.65±0.52, 63.3±6.8 vs 43.1±4.4 and 214.4±19.8 vs 143.2±16.9 (P<0.01). The erythrocyte deformability test on the day 1, 14, 21, 35 of storage showed that the deformability of SRBCs prepared from Tibetan high Hb group was significantly lower than that of the control group under shear stress of 3, 5.33, 9.49, 16.87, and 30 Pa, while the hemolysis rate of SRBCs prepared from the Tibetan high Hb group and the control group on the day 1, 14, 21, 35 were 0.050 2±0.040 2 vs 0.022 2±0.011 1, 0.055 4±0.043 vs 0.032 1±0.028 7, 0.061 2±0.025 9 vs 0.034 3±0.031 7 and 0.069 6±0.032 0 vs 0.044 0±0.033 3 (P<0.05). Western blotting showed that the cytoplasmic N-terminal Y21 of band 3 protein of SRBCs prepared from Tibetan high Hb group was highly phosphorylated. 【Conclusion】 The deformability of SRBCs prepared from the Tibetan high Hb group was significantly lower while the hemolysis rate of SRBCs was higher than that of the control group. The hemolysis rate of the SRBCs at the end of storage prepared from the Tibetan high Hb group meets the requirements of the national standard GB18469-2012(<0.8%). The increase of hemolysis rate of SRBCs prepared from the Tibetan high Hb group was closely related to the phosphorylation of band 3 protein.

【Objective】 To investigate the effect of leukocyte-depleted suspended red blood cells (lds-RBCs) storaged for different time on blood transfusion effect of patients with hematologic diseases and malignant tumors, as well as to evaluate the storage quality of lds-RBCs in blood stations. 【Methods】 Seven hospitals (4 tertiary-A hospitals and 3 secondary-A hospitals), applying for blood from our blood center, were selected. Blood transfusion cases (medical record) and related data (indicators) of patients with blood diseases and malignant tumors in those hospitals from December 2018 to May 2019 were collected, including disease diagnosis (type) before transfusion, demographic characteristics, date of solo transfusion of lds-RBCs, units of lds-RBCs [(1~2)U/bag, 1 U=200 mL whole blood], different storage duration (1~5 weeks) (bar code), and hemoglobin (Hb) 48 h before and after transfusion. The efficacy of lds-RBCs (storaged for different time) transfusion in patients with hematologic diseases and malignant tumors was evaluated by statistical analysis. 【Results】 A total of 3 557 patients with hematologic diseases and malignant tumors were enrolled in this study. No significant changes were noticed in transfusion efficacy by blood transfusion unit, gender and previous transfusion history (P > 0.05). The effective rate of lds-RBCs in patients with blood diseases and malignant tumors, stratified by storage duration, i. e. storaged for >1~2 weeks, >2~3 weeks, >3~4 weeks and more than >4~5 weeks, was 78.77% vs 77.68% vs 75.06% vs 70.37%, and 79.32% vs 76.73% vs 72.79% vs 67.65%, respectively(P<0.05), with lds-RBCs of 4-5 storage weeks presenting the lowest transfusion efficacy in both groups of patients. 【Conclusion】 The storage time of most lds-RBCs supplied by our center is moren than 3 weeks, and the transfusion effect of lds-RBCs stored for 5 weeks needs further observation. In order to ensure and improve the efficacy of blood transfusion, evidence-based medicine and information management are needed to help the clinical gasp the advantageous time of blood products and shorten the storage-to-transfusion time of red blood cells.

OBJECTIVE To evaluate the impact of the plateau environment on suspended red blood cells (SRBCs). METHODS  Plain SRBCs (from healthy people living in Beijing) were stored in Beijing, Lhasa and Linzhi at 4? for 35 and 42 d, respectively. Plateau SRBCs (from healthy people living in Lhasa) were stored at 4? in Lhasa for 35 and 42 d. Hemolysis rate, hemoglobin content, hematocrit and cell survival rate were examined. The extracellular acidification rate of erythrocytes was detected by the extracellular flow analysis technology before baseline glycolsis, glucose response and glycolysis capacity were calculated.RESULTSCompared with the plain SRBCs preserved in Beijing for 35 and 42 d, there was no statistically significant difference in the hemolysis rate, hemoglobin content, hematocrit, cell survival rate, baseline glycolysis, glucose response or glycolysis capacity of SRBCs stored in Lhasa. In SRBCs stored in Linzhi, except the hemolysis rate and glycolysis ability for 35 d(P<0.05), there was no statistically significant difference in other indicators. Compared with plateau SRBCs preserved in Lhasa, after preservation in Lhasa for 35 and 42 d, plain SRBCs had no statistically significant difference in all the indicators above.;CONCLUSION The plateau environment (Lhasa or Linzhi) does not affect the quality of stored SRBCs.

Objective To compare the influence of washing processed of banked suspended red blood cells (RBCs) by cell saver and the influence by zero-balanced ultrafiltration(Z-BUF) on the inflammatory response after cardiopulmonary bypass(CPB) in infants.Methods Sixty infants with ventricular septal defect (VSD) were randomly divided into a control group (group A,20 cases), group B(20 cases)and group C (20 cases).Banked suspended RBCs were washed by cell saver before priming in group B,in group C the banked suspended RBCs were treated with Z-BUF and in group C the banked suspended RBCs were primed directly without any pre-treatment.Samples of the arterial blood were obtained respectively before the start of the CPB (T1), when the CPB stopped(T2) ,2 h after CPB (T3), 12 h after CPB (T4) and 24 h after CPB (T5).The levels of tumor necrosis factor-α (TNF-α), interleukin(IL)-6, IL-8, IL-10 were detected and analyzed comparatively among 3 groups.Results The levels of TNF-α, IL-6, IL-8 ,IL-10 in 3 groups at T2,T3 ,T4 showed a rising trend markedly,and the above four indicators of A,B and C at T4respectivelywere:(110.3±14.0) ×10-9g/L,(90.6±10.3) ×10-gg/L,(103.3±9.7) ×10-9g/L;(54.1 ± 6.5) ×10-9 g/L,(39.3±4.2) ×10-9 g/L, (46.2±5.7) ×10-9 g/L;(96.8 ±9.2) ×10-9 g/L, (82.5 ±6.5) × 10-9 g/L,(88.4±5.1) ×10-9 g/L;(228.4 ±42.9) ×10 9 g/L,(171.5 ±26.4) ×10-9 g/L,(202.9 ±42.8) × 10-9 g/L.The levels of TNF-α and IL-8 in group B and group C were significantly lower than those in group A at T2, T3 ,T4 and T5(all P <0.05) ,but there was no significant differences in the levels of IL-6 and IL-10 among 3 groups at T5;the levels ofTNF-α,IL-6,IL-8 ,IL-10 in group B were significantly lower than those in group C at T2,T3 and T4(all P < 0.05).Conclusions Processing of banked suspended pre-RBCs with cell saver and Z-BUF can relieve systemic inflammatory response, and the effect of washing with cell saver is better compared with that of Z-BUF.

Objective To investigate the clinical application of suspended red blood cells in Liuzhou,in order to make the transfusion of blood components more reasonable and safer.Methods Retrospective data were collected from 226 patients,who had received the transfusion of 980 units of suspended red blood cells between 2000 and 2006.Results Suspended red blood cells could be used widely to different kinds of patients,and most patients benefit from the treatment.The transfusion of leuko-reduced RBC was even more effective.The age,gender,category of disease,disease severity,hemoglobin level and the amount of transfusion were the key points that affect the therapeutic effect of transfusion.Traumatic hemorrhage was another important factor.Unreasonable clinical transfusion still exists.Conclusion Transfusion of suspended red blood cells is an effective treatment.But when and how to transfuse depend on individual cases.
