RESUMEN

Objective To analyze and evaluate the hospital bed efficiency and development trend of 54 tertiary public hospitals in Shandong Province from 2013 to 2020 and to provide a reference for improving efficiency in the use of hospital beds and rationally allocating medical and health resources. Methods Based on the panel data of 54 tertiary public hospitals in Shandong Province from 2013 to 2020,three indicators of bed utilization rate, bed turnover times and the average length of stay of patients were selected to describe the differences of bed use efficiency in each category by using variance analysis and to evaluate the bed use efficiency in tertiary public hospitals in Shandong Province. Results The results of the study show that the bed utilization rate of tertiary public hospitals in Shandong Province decreased from 2013 to 2020,but it is still higher than the national average level. The results of variance analysis showed that in 2019,the overall bed utilization rate of 54 third-level public hospitals reached 95.9%,which was overloaded. The number of bed turnover and the average day of hospitalization were improved, but there were significant differences in bed use efficiency among hospitals with different bed size and institutional attributes(P<0.05).Conclusion Shandong Province should strengthen the rational allocation of medical resources, improve the level of hospital management, formulate relevant policies to promote the rational use of hospital bed resources, and further improve hospital bed efficiency.

RESUMEN

Objective:To explore the construction of a quantifiable comprehensive evaluation index system for the core professional capabilities of clinical engineers, a system matching the routine work of tertiary public hospital′s department of biomedical engineering, and supporting the future development of the discipline. Methods:Based on analysis of such factors as hospital development planning, clinical diagnosis and treatment characteristics, and routine work description of the clinical engineering department, as well as literature review and expert consultation, a preliminary evaluation index system was developed for the core professional capabilities of such engineers. Then the Delphi method was applied for two rounds of expert consultation, with experts invited to evaluate the importance of core indexes, while the analytic hierarchy process was used to calculate the weight values of these indexes. Based on the evaluation index system for core professional capabilities of clinical engineers, a survey questionnaire was developed and the score criterion was defined independently, and then a questionnaire survey was conducted from February to April 2023 on clinical engineers of the tertiary public hospitals′ department of biomedical engineering to evaluate their core professional capabilities. Results:The comprehensive evaluation index system for the core professional capabilities of clinical engineers was composed of 4 first-level indexes and 14 second-level indexes. The weights for the 4 first-level indexes, namely job responsibilities, service abilities, extension abilities, and communication abilities were 0.200, 0.350, 0.250, and 0.200, respectively. Among the 14 second-level indexes, the weights of equipment management ability, engineering technology transformation ability, scientific research ability, personnel quality, and medical communication ability ranked top five, weighting 0.097, 0.091, 0.089, 0.088, and 0.087, respectively. A total of 106 clinical engineers were selected for a questionnaire survey, and the self-scoring found 6 of them as " excellent", 36 as " good", 38 as " moderate", and 26 as " poor" in terms of core professional capabilities. Conclusions:The evaluation index system for the core professional capabilities of clinical engineers in this study proves rational and practical, and can provide reference for the core professional competence evaluation, talent cultivation, and clinical engineering discipline of these professionals.

RESUMEN

Objective:To analyze the impact of the national tertiary public hospital performance assessment(hereinafter referred to as the national examination) on the functional orientation of the provincial public hospitals in Zhejiang province, for reference to improve the allocation of provincial medical resources and high-quality development of public hospitals. Methods:The data came from the hospital financial data of 17 provincial tertiary public hospitals in Zhejiang province from 2012 to 2021 and the DRG quality performance analysis report of the tertiary hospitals in Zhejiang province from 2020 to 2021. The information of the number of employees and actual number of beds at the end of the period, etc., were extracted to analyze the overall operation of the hospital. The ratio of outpatient times to discharged times and the proportion of discharged patients undergoing surgery, as well as the proportion of discharged patients′ day surgery and level-4 surgery in hospital and in province were took as evaluation indicators to analyze the functional positioning of tertiary public hospitals. Results:Compared with 2012 to 2018, the average number of employees at the end of the period and actual number of beds in 17 hospitals from 2019 to 2021 increased by 761 and 303 respectively, and the average number of hospitalization days decreased by 2.26 days. The average ratio of outpatient times to discharged times in the hospital decreased from 175.76 in 2012 to 67.51 in 2021. The average proportion of discharged patients undergoing surgery in general hospitals increased from 0.39 in 2012 to 0.46 in 2021, and that in non-general hospitals decreased from 0.67 to 0.43. The average proportion of discharged patients undergoing day surgery in hospital and in province increased from 0.20 and 0.03 in 2020 to 0.23 and 0.04 in 2021 respectively, and the average proportion of discharged patients undergoing level-4 surgery in province increased from 0.04 to 0.05.Conclusions:The national examination could be conducive to strengthening the functional orientation of public hospitals. After the national examination, the ratio of outpatient times to discharged times in the provincial tertiary public hospitals in Zhejiang province had declined as a whole, the proportion of discharged patients in general hospitals had increased, but the proportion of level-4 surgery in hospitals needs to be further improved. The author suggested that we should continue to strengthen the operation mechanism of functional positioning of provincial public hospitals, improve the service capacity of hospitals for difficult and critical diseases, and improve the performance evaluation system of tertiary public hospitals.

RESUMEN

Objective@#To dynamically study the use of antiseptic drugs at tertiary public hospitals in the localities for general information.@*Methods@#In this paper, the panel data were analyzed using dynamic monitoring data from the " monitoring network for clinical application of antiseptic drugs" . Such means as descriptive statistical analysis, single-factor analysis and logistic regression analysis were used to learn the use and monitoring of such drugs.@*Results@#We found that the use of such drugs at tertiary hospitals has improved significantly nationwide in recent years, yet with some gaps up to the standard. Given such progress, the economic and social burden caused by the unreasonable use of antiseptic drugs calls for more attention.@*Conclusions@#It is imperative to strengthen the supervision of such drugs and promote their rational use so as to reduce the social and economic burden.

RESUMEN

Objective To evaluate operating efficiency of 68 Zhejiang Provincial tertiary public hospitals in 2015 and analyze the reason of the efficiency differences among different types of hospitals so as to put forward correspondent policy suggestions. Methods Adopting the three-stage DEA model analysis upon filtering the external influencing factors and random error to the hospital and quantitatively evaluating the comprehensive technical efficiency, pure technical efficiency and scale efficiency of the hospitals. Results The result shows that it is necessary to carry out three-stage DEA model analysis. The mean value of hospital efficiency increases by 0.1,which is higher than before adjustment.Provincial hospitals efficiencies are higher than those of the non-provincial hospitals.Efficiencies of general hospitals are higher than those of the traditional Chinese Medicine(TCM) hospitals and the specialized hospitals. Conclusion Specialized hospitals utilize more resources than the TCM hospitals. The efficiency of provincial hospitals are relatively high, but they are facing the risk of excessive expansion. There is room for efficiency improvement.Prospective policies need to be initiated to make the hospital administrators change their management concept, control the scale of hospital, promote the fine management so as to achieve sustainable development.Furthermore, policies are needed to be adopted to support the development of TCM hospitals.
