RESUMEN

Abstract Background and aim Brucellosis is an infectious, contagious and zoonotic disease that occurs worldwide. The family members of an index case of brucellosis may be especially susceptible, due to sharing the same source of infection and similar risk factors for brucellosis. In this study, we propose to screen pediatric and adult family members of brucellosis index cases for detecting additional unrecognized infected family members. Materials and methods 114 family members of 41 pediatric patients with brucellosis were evaluated. All family members completed a brief questionnaire and were tested by a standard tube agglutination test (STA). Results The majority of family members (n = 96, 84.2%) were children. Among the 114 family members, 42 (36.8%) were seropositive, and 15 (35.7%) were symptomatic. The majority of the symptomatic seropositive family members (n = 12, 80%) had STA titers (≥1:640) higher than asymptomatic seropositive family members (n = 9, 33%; p = 0.004). Conclusion The routine screening of both pediatric and adult family members of index cases is a priority in endemic areas. Using this screening approach, unrecognized family members who are seropositive for brucellosis will be identified earlier and be able to receive prompt treatment.

RESUMEN

Medical education is the subject matter of both Union and State Governments and Medical Council of India is the sole supervising body of medical education and maintaining its standard. Problem of recognition of medical degrees / diplomas is prevalent in almost all the States of India and in most of the medical specialties. Holders of unrecognized degree / diploma may face problem of employment / promotion, etc. Thus, leading to filing of litigations in the court, some of which are decided and others are still pending in the Indian Courts. This paper deals with review of this problem, brief discussion of relevant court decisions, Government Orders and Medical Council of India (MCI) notifications, etc. thus, help in solving the problem to great extent by making concerned persons aware about the issue and to take initiative to solve the problem of very much public interest.

RESUMEN

Pulmonary rhodococcosis is an uncommon pulmonary infection in human beings, but the case reports have been increasing in HIV-infected patients. We report a female AIDS patient presenting with fever, productive cough and weight loss over 6 weeks. Chest x-ray showed a cavitary lesion with air fluid level in the right upper lobe infiltration. Sputa for AFB stain were negative for 3 consecutive days. Fiberoptic bronchoscopy was performed and bronchoalveolar lavage fluid was examined. It showed a gram-positive, weakly acid-fast coccobacilli. The culture grew only Rhodococcus equi. She was treated with erythromycin and rifampin and responded well.

OBJECT: Microaneuryms were unvisible in angiograms and incidentaly detected during operation. Increasing the frequency of endovascular surgery, they have dangerous risk during procedure. So we analyzed the unvisible reason and report operation method. MATERIAL AND METHOD: Twenty-six cases of angiographically unrecognized microaneurysm were operated from January 1993 to May 1998. They consisted of 3.8% of 684 operated cases of intracranial aneurysms at the same period. The most common site of angiographically unrecognized microaneurysm was the internal carotid artery(11 cases): one arose from the posterior communicating artery, 9 from the anterior choroidal artery, 1 from the internal carotid artery bifurcation. RESULT: According to the intraoperative findings, we analyzed the possible reasons why these microaneurysms were not visible in the pre-operative angiograms; 19 cases in low type, 4 cases in hidden type, 3 cases in sandwiched type. We performed direct aneurysmal neck clippings in 9 cases and wrap-clip reinforcements in 8 cases, wrappings in 9 cases. CONCLUSION: We have to consider that angiographically unrecognized microaneurysm may be found during the direct operation of intracranial aneurysm and also during the endovascular surgery. Also, the wrapping technique is found to be less secure and needs regular follow-up angiography.

RESUMEN

Renal cell carcinoma is a relatively rare tumor, accounting for approximately 3 percent of adult malignancies. Bilateral renal cell carcinoma, either synchronous or asynchronous, very rarely occurs and makes another urological dilemma. We experienced a case of unrecognized bilateral renal cell carcinoma ; which had been treated by radical nephrectomy after diagnosed as a left renal cell carcinoma, Robson stage IIIa, about 18 months later another mass was found in remaining kidney. Therefore, we performed wedge resection of the mass, and it was also diagnosed as a renal cell carcinoma. Reviewing of the initial abdomen-pelvis CF scan, mass in Rt. kidney had already been existed when left. renal cell carcinoma was diagnosed. This case was finally diagnosed with unrecognized bilateral synchronous renal cell carcinoma.
