RESUMEN

Herein, we report a case of in-the-bag dislocation of a WIOL-CF® polyfocal full-optics intraocular lens (IOL), without a history of trauma. A 56-year-old man was referred to our hospital with the chief complaint of sudden-onset visual disturbance in his left eye. He had undergone uneventful phacoemulsification with WIOL-CF® IOL implantation in the left eye at the local clinic 7 years prior. In fundus examination, IOL-capsular bag complex dislocated into the posterior vitreous was observed. We believe this is the first report of in-the-bag dislocation of a WIOL-CF® IOL that has been subluxated or dislocated in a characteristic pattern, not an in-the-bag pattern.

RESUMEN

PURPOSE: To report a case of acute angle closure after cataract surgery using an accommodative intraocular lens (IOL), WIOL-CF® (GELMED, Praha, Czech). CASE SUMMARY: A 46-year-old male patient underwent phacoemulsification and implantation of WIOL-CF® into the capsular bag. Seven months after the surgery, a sudden increase in intraocular pressure (IOP) associated with angle closure was observed. Ultrabiomicroscopy revealed a dislocated WIOL-CF® that was pushing the peripheral iris anteriorly. Despite the use of IOP-lowering medication and peripheral laser iridotomy, IOP was not controlled. After the use of cycloplegics, the angle was widened and IOP decreased; however, after nine days, the WIOL-CF® was completely dislocated into the anterior chamber and so was removed. CONCLUSIONS: When performing cataract surgery using WIOL-CF®, a possibility of dislocation of IOL and subsequent angle closure should be considered.

PURPOSE: To report 2 cases of WIOL-CF(R) intraocular lens (IOL) (Gelmed International, Kamenne Zehrovice, Czech Republic) dislocation after neodymium-doped yttrium aluminum garnet (Nd:YAG) laser capsulotomy. CASE SUMMARY: A 78-year-old female was referred for IOL dislocation. She was implanted with WIOL-CF(R) IOL 18 months prior. Two months after WIOL-CF(R) implantation, she received Nd:YAG laser capsulotomy at a local clinic. Pars plana vitrectomy and transscleral fixation of IOL were performed. CONCLUSIONS: This is the first report of WIOL-CF(R) IOL dislocation after Nd:YAG laser capsulotomy in unvitrectomized eyes. When Nd:YAG laser capsulotomy is performed after WIOL-CF(R) IOL implantation, IOL dislocation should be considered even in unvitrectomized eyes.

PURPOSE: To evaluate the prognostic factors to determine the uncorrected near visual acuity of patients who undergone routine cataract surgery and WIOL-CF(R) (Gelmed international, Kamenne Zehrovice, Czech Republic) accommodative intraocular lens implantation. METHODS: This study evaluated 46 eyes of 27 patients who had undergone routine cataract surgery and WIOL-CF(R) accommodative IOL implantation. We checked visual acuities at near and distant before surgery. And we also checked age and gender of patients and manifest refraction, corneal astigmatism and axial length of eyes at that time. We analyzed association between these factors and uncorrected near visual acuity at postoperative 6 and 12 month. RESULTS: Univariate linear regression analyses between uncorrected near visual acuity and preoperative patients characteristics suggested that the age of patients was the only statistically significant independent variable on uncorrected near visual acuity at postoperative 6 (p < 0.001) and 12 (p < 0.001) month. Multiple regression analyses also revealed the same results (p = 0.021 at postoperative 6 month and p = 0.042 at postoperative 12 month). CONCLUSIONS: This study suggest that the age is the one of the most important prognostic factors of postoperative uncorrected near visual acuity after WIOL-CF(R) accommodative intraocular lens implantation.
