RESUMEN

The first objective of this research was to evaluate the effectiveness of XP-Endo Finisher on dentinal tubule penetration of irrigation solution using confocal laser scanning microscopy. The main purpose of this research was to compare the effect of cold lateral condensation, continuous wave obturation and core-carrier based techniques on sealer penetration. Sixty mandibular premolars were prepared and allocated into two experimental groups (n=30) as the final irrigation technique and obturation technique experiment. In the final irrigation technique experiment, final irrigation was performed with XP-Endo Finisher, passive ultrasonic irrigation (PUI) and conventional needle irrigation (CNI) (n=10). The roots in the obturation technique experiment were also assigned into 3 groups and obturated with cold lateral condensation, continuous-wave obturation and core-carrier techniques (n=10). The most effective activation method, which emerged as a result of the first part of this study, was used as the final irrigation method in the obturation technique experiment. Then, all roots were sectioned in 1-mm-thick slices at 3mm from the apex for scanning. In terms of depth and percentage of material penetration, CNI exhibited significantly the lowest values and no significant difference was found between others. Also, there was no significant difference among obturation methods. In conclusion, XP-Endo Finisher and PUI are more effective than CNI on irrigant penetration. Sealer penetration into dentinal tubules is independent of obturation techniques.


El objetivo principal de esta investigación fue evaluar la eficacia de XP- Endo Finisher en la penetración de la solución de irrigación en los túbulos dentinarios mediante microscopía de láser confocal. Se prepararon sesenta premolares mandibulares y se distribuyeron en dos grupos experimentales (n=30) según el tipo de método de evaluación utilizado. En el experimento de la técnica de irrigación final, la irrigación final se realizó con XP-Endo Finisher, irrigación ultrasónica pasiva (PUI) e irrigación con aguja convencional (CNI) (n=10). Las raíces en el experimento de la técnica de obturación también se asignaron en 3 grupos y se obturaron con técnicas de condensación lateral fría, obturación de onda continua y portador de núcleo (n=10). El método de activación más eficaz, que surgió como resultado de la primera parte de este estudio, se utilizó como método de irrigación final en el experimento de la técnica de obturación. Luego, todas las raíces se seccionaron en muestras de 1mm de espesor. En términos de profundidad y porcentaje de penetración del material, CNI exhibió significativamente los valores más bajos y no se encontraron diferencias significativas entre los demás. Además, no hubo diferencias significativas entre los métodos de obturación. En conclusión, XP-Endo Finisher y PUI son más efectivos que CNI en la penetración del irrigante. La penetración del sellador en los túbulos dentinarios es independiente de las técnicas de obturación.

RESUMEN

Autoimmune hemolytic anemia (AIHA) has been rarely reported worldwide or from India as the underlying cause of anemia in malaria. We hereby present a case of complicated Plasmodium falciparum malaria with concomitant warm AIHA in a 31-year-old male. Direct Antiglobulin Test (DAT) was positive and elution studies showed pan-agglutination reaction. Clinico-hematological and serological follow-up of the patient was done post artesunate treatment until day 9. We suggest that it is important to establish the immune basis of anemia in malaria patients for guiding the treatment plan for the clinicians and providing packed red blood cell transfusion if required.

Objective:To investigate the effects of thermobaric charge explosion simulated gas on long-term neurobehavior and hippocampal neurogenesis in rats.Methods:A total of 48 male SPF grade SD rats aged 8-10 weeks were randomly divided into control group, 5 min exposure group, 10 min exposure group and 15 min exposure group, with 12 rats in each group. Twenty-eight days after inhalation of infection, the anxiety-like behavior of rats was evaluated by an elevated cross maze, and the learning and memory function of rats was evaluated by two-way active avoidance experiment. The number of positive cells of rat hippocampal dentate gyrus neural stem cells marker molecule neural epithelial cell protein (SOX2) and mature neuron marker molecular neuronal nuclei (NeuN) was detected by immunofluorescence staining. Western blot was used to detect SOX2 and NeuN protein expression in the hippocampal tissues of rats. GraphPad prism 8.0 software was used for data analysis.The comparison of repeated measurement design data was carried out by repeated measurement ANOVA.One-way ANOVA was used for inter group comparisons, and Tukey test was used for pairwise comparison. Hippocampal nerve cells were counted using the Image J software.Results:(1) The experimental results of the elevated cross maze showed that the percentage of arm opening and the percentage of open arm residence time in each group had significant group effects ( F=22.31, 5.43, all P<0.05). The percentage of open arm entry times of rats in the 5 min, 10 min and 15 min exposure group ((28.85±1.47)%, (15.04±4.69)%, (12.66±2.89)%) and the percentage of residence time in open arm ((12.12±2.64)%, (12.16±1.11)%, (8.73±3.52)%) were all lower than those of the control group ((65.40±1.86)%, (42.92±3.12)%) (all P<0.05). There were no statistically significant differences in pairwise comparison among the three exposure groups (all P>0.05). (2)During the memory acquisition period, the results of repeated-ANOVA showed that the time main effect ( F=56.46), the group main effect ( F=16.64) and the interaction effect had significant differences( F=4.21)(all P<0. 05). The difference values of active avoidance number between the 4th day and 1st day among the four groups were significant different ( F=68.63, P<0.05). During the memory reproduction period, there were significant differences in active avoidance number and active avoidance time among the four groups ( F=8.17, 8.28, both P<0.05). The active avoidance numbers in 10 min and 15 min exposure groups((2.50±0.26) times, (2.33±0.06) times)were significantly lower than those in the control group ((8.33±3.72) times) (both P<0.05), and the active avoidance time ((6.25±0.40)s, (6.61±1.63)s) were significantly higher than those in the control group((3.69±1.41)s) (both P<0.05). The active avoidance numbers in 10 min and 15 min exposure groups were significantly lower than that in 5 min exposure group (both P<0.05). (3) The results of immunofluorescence staining showed that the numbers of SOX2-positive cells in the four groups were statistically significant ( F=5.33, P<0.05). The SOX2-positive cells in 15 min exposure group (4.33±1.12) was significantly lower than that in control group (7.67±1.52) ( P<0.05). The numbers of NeuN-positive cells in the four groups were significantly different ( F=11.06, P<0.05), and the NeuN-positive cells in the 10 min and 15 min exposure groups((105.67±8.50), (88.33±9.50)) were significantly lower than that in the control group (127.00±6.56) ( P<0.05). The NeuN-positive cells in 15 min exposure group were significantly lower than that in 5 min exposure group (110.67±8.32) ( P<0.05). (4) Western blot results showed that the relative expression of SOX2 and NeuN proteins in the four groups was statistically significant ( F=11.560, 7.035, both P<0.05). The relative expression of SOX2 and NeuN proteins in the 15 min exposure group were significantly lower than those in control group (both P<0.05). The relative expression of SOX2 protein in 15 min exposure group was significantly lower than that in 5 min exposure group ( P<0.05). Conclusion:Acute exposure to warm pressure charge explosion simulated gas can lead to anxiety-like behavior, learning and memory deficits in rats, and significantly reduce the protein expression levels of hippocampal dentate gyrus neural stem cells and mature neuronal marker molecules SOX2 and NeuN.

Epidemic cerebrospinal meningitis shows a high degree of consistency with the law of transmission among wei (卫)-qi-ying (营)-blood, in terms of the onset of the season, contagiousness, symptoms, pathogenesis, as well as characteristics of the transmission. It is proposed to use epidemic cerebrospinal meningitis as an example to explore the underlying disease of wei-qi-ying-blood syndrome differentiation system. Epidemic meningitis invades the brain from the upper respiratory tract along the nervous system, and its overall pathogenesis follows from entering the lung system (prodromal period) to entering the blood (bacteremia period, sepsis period) and then entering the brain (shock period). According to the four-dimensional qualitative principle of epidemic pathogen tropism, it corresponds to disease of both wei and qi syndrome, then blazing of both qi and ying syndrome, and then heat blocking pericardium, exuberant heat stirring wind, and internal block and external collapse syndrome. This article explored the laws of transmission among wei-qi-ying-blood and its underlying diseases described in On Warm Heat (《温热论》), and revealed the original appearance of the disease model under the laws of transmission among wei-qi-ying-blood to guide the clinical practice.

Based on the theory of ascending and descending of center qi, it is believed that yang deficiency and water dampness, as well as abnormal circulation of center qi, are the core pathogenesis of psoriasis. The common pathogenic evolution of psoriasis includes wood constraint and blood stagnation, lung metal fluid exhaustion. In the later stage of psoriasis, a mixture of deficiency and excess patterns, as well as cold and heat in complexity, are often observed. Treatment focuses on warming yang and resolving dampness to restore proper qi ascending and descending, and the therapeutic formulas such as Ganjiang Lingzhu Decoction (甘姜苓术汤), Zexie Decoction (泽泻汤), Zhenwu Decoction (真武汤), and Linggui Zhugan Decoction (苓桂术甘汤) are suggested based on the nature and location of the disease. To unblock the qi movement through discharging the liver and rectifying the lung, Xiaochaihu Decoction (小柴胡汤) and Danzhi Xiaoyao Powder (丹栀逍遥散) are commonly used as the foundation with modifications. To balance yin and yang through clearing the heart and nourishing the kidneys, a self-designed Changzhong Decoction (畅中汤) is utilized. Following the theory of ascending and descending of center qi, it is suggested to combine cold and warm medicinals and mediate the center earth, so as to promote the circulation of the center qi, restore the clear and the turbid, and keep four-dimensional smooth flow of qi, providing a reference for traditional Chinese medicine treatment for psoriasis.

During the carcinoma transformation of colitis, the imbalance of “metabolic-immune interaction” resulted from abnormal energy and metabolic substrates flow and direction was the key process, which caused by intercellular metabolic competition. Based on traditional Chinese medicine (TCM) theory and clinical research, we found that “fire failing to warm earth” is the key pathogenesis of colon-cancer transformation. “Fire” was a synonym for TCM to understand the energy metabolism, combined modern medical research findings, we thought energy metabolism disorders was a microcosmic manifestation of the “fire decline”, while abnormal immune function was the biological basis of “earth deficiency”. The imbalance between “metabolism-immune interaction” and the “fire failing to warm earth” pathogenesis of colitis-cancer transformation demonstrated the different understanding of the same pathological mechanism between western medicine and TCM. For treatment, it could be effectivce to delay the transformation of colitis-cancer by synergistically regulated the energy metabolism - “replenish fire” and enhanced the immune function - “nourish earth”, which was called the methods of replenishing fire to nourish earth.

Ischemia-reperfusion injury after lung transplantation is the main cause of primary graft dysfunction, which will subsequently reduce the function of lung allograft and lower the overall survival rate of lung transplant recipients. As a physiological regulatory molecule, hydrogen molecule has the functions of anti-inflammation, easing oxidative stress, alleviating direct cell injury and mitigating epithelial edema. Recent studies have demonstrated that hydrogen molecule and its products (hydrogen and hydrogen-rich solution) could significantly mitigate ischemia-reperfusion injury and postoperative complications after lung transplantation. In this article, the protective effect and exact mechanism of hydrogen molecule and its products in lung transplantation were reviewed, aiming to provide theoretical basis for the application of hydrogen molecule and its products as a novel treatment for lung transplantation-related complications, enhance the overall prognosis and improve the quality of life of lung transplant recipients

OBJECTIVE@#To observe the clinical efficacy of transcutaneous electrical acupoint stimulation (TEAS) combined with warm acupuncture in treating breast cancer associated with upper limb lymphedema (BCRL).@*METHODS@#This was a retrospective cohort study using a paired control design. Fifty-two BCRL patients were assigned to the control group (27 cases) and the treatment group (25 cases). The patients in the control group were treated with lymphedema comprehensive detumescence treatment (CDT) for 4 weeks, including systematic therapy composed of manual lymphatic drainage, compression bandage, skincare, and functional exercise. The patients in the treatment group were treated with TEAS combined with warm acupuncture based on the control group methods. Each treatment lasted for 30 min and was applied twice a week for 4 weeks. The arm circumference (AC) of different positions of the affected limb and the degree of swelling of the affected limb were evaluated before the first treatment and after the last treatment. The clinical efficacy was evaluated according to the degree of edema before and after treatment. All adverse events during treatment were recorded.@*RESULTS@#The patients' AC and the swelling feeling of the affected limb in the treatment group and the control group were both reduced compared with those before treatment. Compared with the control group, AC of the wrist joint transverse stria, the midpoint between the wrist joint transverse stria and the elbow joint transverse stria in the treatment group were significantly reduced (P<0.05). The decrease in AC diameter at the midpoint between the elbow joint transverse stria and the axillary transverse stria was the most significant (P<0.01). The swelling degree of the affected limbs in the treatment group was significantly lower than before treatment, and was significantly lower compared with the control group after treatment (P<0.01). The total effective rate was 72% in the treatment group, significantly higher than that in the control group (55.56%, P<0.05). No serious adverse events occured in either group.@*CONCLUSIONS@#TEAS combined with warm acupuncture can effectively reduce AC and swelling feeling of the affected limb in patients with BCRL. The effect is better than that of CDT therapy alone. (Registration No. ChiCTR2200062075).

ABSTRACT Introduction: Competitive gymnastics training is extremely debated in the Chinese scientific community. It is only when coaches choose proper physical training methods, they can fundamentally improve gymnasts' physical fitness. Objective: Analyze the effect of warm-up exercises on gymnasts' physical fitness. At the same time, this paper analyzes the factors that influence the indicators of gymnasts' physical fitness. Methods: This paper selects several gymnasts as research objects. The gymnasts perform physical training with interval training warm-up for two months. Data of gymnasts' physical constitution and the indicators related to their function are tracked and tested. Morphological indicators include height, circumference and body composition; functional indicators include multiple physiological and biochemical indicators (blood and urine analysis). Results: Gymnasts increased urea and creatine kinase concentrations after warm-up physical training. However, there was no statistical difference (P>0.05). The primary functional status of the athletes was stable. The results showed a normal resting heart rate and a reduction of blood pressure in the elite athletes. Conclusion: Gymnasts using interval training can maintain high intensity levels for prolonged periods. The indicators of physical fitness were above average. However, players' fitness to complete the movement set needs to be improved. Level of evidence II; Therapeutic studies - investigation of treatment outcomes.


RESUMO Introdução: O treinamento de ginástica competitiva é extremamente debatido na comunidade científica chinesa. Somente quando os treinadores escolhem métodos adequados de treinamento físico, eles podem melhorar fundamentalmente a condição física dos ginastas. Objetivo: Analisar o efeito de exercícios para aquecimento sobre a aptidão física dos ginastas. Ao mesmo tempo, este trabalho analisa os fatores que influenciam os indicadores da aptidão física dos ginastas. Métodos: Este trabalho seleciona vários ginastas como objetos de pesquisa. Os ginastas realizam treinamento físico com aquecimento por treinamento intervalado durante dois meses. Dados de constituição física dos ginastas e os indicadores relacionados à sua função são rastreados e testados. Os indicadores morfológicos incluem altura, circunferência e composição corporal; os indicadores funcionais incluem múltiplos indicadores fisiológicos e bioquímicos (análise de sangue e urina). Resultados: Os ginastas aumentaram a concentração de ureia e creatinaquinase após o treinamento físico de aquecimento. Entretanto, não houve diferença estatística (P>0,05). O estado funcional primário dos atletas demonstrou-se estável. Os resultados mostraram um ritmo cardíaco em repouso normal e uma redução da pressão sanguínea nos atletas de elite. Conclusão: Ginastas que utilizam treinamento intervalado podem manter níveis de alta intensidade por períodos prolongados. Os indicadores de aptidão física foram acima da média. Entretanto, a aptidão física dos jogadores para completar o conjunto de movimentos precisa ser aprimorada. Nível de evidência II; Estudos terapêuticos - investigação dos resultados do tratamento.


RESUMEN Introducción: El entrenamiento de la gimnasia de competición es muy debatido en la comunidad científica china. Sólo cuando los entrenadores eligen métodos de entrenamiento físico adecuados, pueden mejorar fundamentalmente la forma física de los gimnastas. Objetivo: Analizar el efecto de los ejercicios de calentamiento en la condición física de los gimnastas. Al mismo tiempo, este documento analiza los factores que influyen en los indicadores de la aptitud física de los gimnastas. Métodos: Este trabajo selecciona a varios gimnastas como objeto de investigación. Los atletas realizan un entrenamiento físico con calentamiento por medio de un entrenamiento por intervalos durante dos meses. Se hace un seguimiento de los datos, de la constitución física de los gimnastas y de los indicadores relacionados con su función. Los indicadores morfológicos incluyen la altura, la circunferencia y la composición corporal; los indicadores funcionales incluyen múltiples indicadores fisiológicos y bioquímicos (análisis de sangre y orina). Resultados: Los gimnastas aumentaron las concentraciones de urea y creatina quinasa después del entrenamiento físico de calentamiento. Sin embargo, no hubo diferencias estadísticas (P>0,05). El estado funcional primario de los atletas era estable. Los resultados mostraron una frecuencia cardíaca en reposo normal y una reducción de la presión arterial en los atletas de élite. Conclusión: Los gimnastas que utilizan el entrenamiento por intervalos pueden mantener niveles de intensidad elevados durante períodos prolongados. Los indicadores de aptitud física estaban por encima de la media. Sin embargo, es necesario mejorar la aptitud de los jugadores para completar el conjunto de movimientos. ABSTRACT Introduction: The development of table tennis and its rule changes increased the importance of physical training with the decline of ball speed and rotation, highlighting the importance of strong quality in the game. Objective: To study the effect of the muscular warm-up phase on improving physical conditioning in table tennis players. Methods: We randomly selected 20 high school table tennis players aged between 14 and 16 years as experimental objects through the analysis of pre-test data, combined with the characteristics of the group, where a physical training plan was generated. Results: After 12 weeks of training, the measured values were analyzed and compared, and it was found that this program can effectively improve the functional movement level of male table tennis players in high school students; the total test score was significantly improved (P<0.01), and the rate of increase was 22%. Conclusion: Muscle warm-up prior to physical training is important in improving athletic ability, enhancing athletic performance, and preventing sports injuries. RESUMO Introdução: O desenvolvimento do tênis de mesa e suas mudanças de regras aumentaram a importância do treinamento físico com o declínio da velocidade e rotação da bola, destacando-se a importância da qualidade da força no jogo. Objetivo: Estudar o efeito da fase de aquecimento muscular na melhoria do condicionamento físico nos tenistas de mesa. Métodos: Selecionou-se aleatoriamente como voluntários 20 mesatenistas do ensino médio com idades entre 14 e 16 anos como objetos experimentais, através da análise dos dados pré-teste, combinados com as características do grupo, onde foi gerado um plano de treinamento físico. Resultados: Após 12 semanas de treinamento, os valores medidos foram analisados e comparados, e verificou-se que este programa pode melhorar efetivamente o nível de movimento funcional dos tenistas de mesa masculinos em estudantes do ensino médio, a pontuação total do teste foi significativamente melhorada (P<0,01), e a taxa de aumento foi de 22%. Conclusão: O aquecimento muscular prévio ao treinamento físico é um fator importante para melhorar a capacidade atlética, melhorar o desempenho atlético e evitar lesões esportivas. RESUMEN Introducción: El desarrollo del tenis de mesa y sus cambios de reglas aumentaron la importancia del entrenamiento físico con la disminución de la velocidad y la rotación de la pelota, destacando la importancia de la calidad de la fuerza en el juego. Objetivo: Estudiar el efecto de la fase de calentamiento muscular en la mejora de la condición física de los jugadores de tenis de mesa. Métodos: Seleccionamos aleatoriamente como voluntarios a 20 jugadores de tenis de mesa de secundaria con edades comprendidas entre los 14 y los 16 años como objetos experimentales, a través del análisis de los datos del pre-test, combinados con las características del grupo, donde se generó un plan de entrenamiento físico. Resultados: Después de 12 semanas de entrenamiento, se analizaron y compararon los valores medidos, y se descubrió que este programa puede mejorar eficazmente el nivel de movimiento funcional de los jugadores de tenis de mesa masculinos en estudiantes de secundaria, la puntuación total de la prueba mejoró significativamente (P<0,01), y la tasa de aumento fue del 22%. Conclusión: El calentamiento muscular previo al entrenamiento físico es un factor importante para mejorar la capacidad atlética, aumentar el rendimiento deportivo y prevenir las lesiones deportivas. Objetivou-se descrever o perfil dos sistemas de criação de caprinos e ovinos da Ilha de São Luís no estado do Maranhão. Para isso, realizou-se um estudo descritivo, exploratório e quali-quantitativo com 80 produtores rurais selecionados pela técnica Snowball, no período de fevereiro a abril de 2021. Em relação ao perfil do produtor rural, verificou-se predominância do gênero masculino (93,8%), apresentando faixa etária entre 25 a 60 anos (86,2%). A maioria dos produtores apresentavam ensino médio completo (23,8%) ou ensino superior completo (23,8%) e 88,8% dos produtores não estavam vinculados a organizações coletivas, associações, cooperativas ou sindicatos. Nas propriedades rurais o sistema de criação predominante era o semi-extensivo (73,8%) com presença de apriscos (76,2%) e as criações eram consorciadas com caprinos e ovinos (58,8%). A principal causa de morte nos animais verificada foi a verminose (58,8%). Conclui-se que se faz necessário o estabelecimento de medidas de estímulo às criações na Ilha de São Luís por meio de políticas públicas voltadas aos produtores locais, requerendo, sobretudo, assistência técnica, orientações zootécnicas, educação sanitária e acesso ao crédito, para, assim, deixar de ser apenas uma atividade de subsistência e passar a gerar maiores rendas para as famílias produtoras.


The objective of this research was to describe the profile of the goat and sheep rearing systems on the Island of São Luís in the state of Maranhão. For this, a descriptive, exploratory and qualitative-quantitative study was carried out with 80 rural producers selected by the Snowball technique, from February to April 2021. Regarding the profile of the rural producer, there was a predominance of males (93.8%), aging between 25 and 60 years (86.2%). Most producers had completed high school (23.8%) or higher education (23.8%) and 88.8% were not members of collective organizations, associations, cooperatives or unions. In rural properties, the predominant rearing system was semi-extensive (73.8%) with the presence of sheepfolds (76.2%) and mutual goats and sheep farming (58.8%). The main cause of death of animals was warm infections (58.8%). In conclusion, it is necessary to establish measures to support sheep and goat farming on the Island of São Luís through public policies focused on local producers, which require, above all, technical assistance, zootechnical guidelines, health education and access to credit in order to the farms stop being just a subsistence activity and start generating higher incomes for producing families.

Resumen Infection by SARS-CoV-2 virus is not solely limited to the common clinical presentation of acute respiratory distress syndrome, mainly because a wide spectrum of clinical manifestation has been observed. These presentations include, but are not limited to, neurological, cardiovascular, throm- boembolic, hematologic, and autoimmune presentations. Within this wide spectrum, cases of autoimmune hemolytic anemia due to SARS-CoV-2 infection are rising. This is why primary care physicians should be ready to identify this clinical entity appropriately.

RESUMEN

Objective: To observe the clinical efficacy of warm needling moxibustion plus Tuina (Chinese therapeutic massage) for knee osteoarthritis (KOA).Methods: A total of 60 patients with KOA were randomized into an observation group and a control group by the random number table, with 30 cases in each group. The control group was treated with warm needling moxibustion, and the observation group was treated with additional Tuina treatment. The treatments were performed once every other day, for a total of 10 times. The visual analog scale (VAS) and Western Ontario and McMaster Universities osteoarthritis index (WOMAC) were scored before and after treatment, and the surface electromyography (sEMG) signals of quadriceps femoris were collected. Results: The total effective rate of the control group was 76.7%, and that of the observation group was 90.0%. The difference between the two groups was statistically significant (P<0.05). The WOMAC and VAS scores in both groups after treatment were improved, and the integrated electromyography (iEMG) and median frequency (MF) of quadriceps femoris were increased. The intra-group differences were statistically significant (P<0.05). After treatment, the WOMAC and VAS scores in the observation group were lower than those in the control group, and the differences between the two groups were statistically significant (P<0.05). The improvement of quadriceps femoris function in the observation group was more notable than that in the control group, and the iEMG value and MF were significantly higher than those in the control group (P<0.05). Conclusion: Warming needling moxibustion plus Tuina can effectively relieve the pain in patients with KOA, improve their daily activity function, and increase the sEMG signals of the quadriceps femoris.

Objective:To develop an ex vivo normothermic mechanical perfusion(NMP)and compare the effect of air-oxygenated NMP versus oxygen-oxygenated NMP on reducing renal injury from donor after cardiac death(DCD).Methods:All kidneys from DCD rats were subjected to 30 min in situ warm ischemia after cardiac attest.And harvested kidneys were stored for 8h under static cold preservation after NMP for 2h.In experimental groups, kidneys were subjected to either air-oxygenated NMP(group A, n=6)or oxygen-oxygenated NMP(group O, n=6). Sham operation(group C, n=6)and DCD kidneys under static cold preservation without NMP(group SCS, n=6)were employed as controls.The evaluation parameters included creatinine(Cr), aspartate amino transferase(AST)and lactate dehydrogenase(LDH)in perfusate, pathological changes by hematoxylin-eosin(HE)staining, histological criteria, expressions of myeloperoxidase and intercellular adhesion molecular-1(ICAM-1)by immunohistochemistry and Western blot, tumor necrosis factor-alpha(TNF-α)and interleukin-6(IL-6)by enzyme-linked immunoadsorbent assay and level of malondialdehyde(MDA)by thiobarbital method and activity of superoxide dismutase(SOD)by WST-8 in renal tissues.Differences between two groups were analyzed by two-tailed unpaired Student's test and differences among more than two groups by one-way ANOVA.Results:Renal arterial oxygen tensions in NMP perfusate were(576.3±68.2)mmHg with oxygen-oxygenation and(137.0±39.1)mmHg with air-oxygenation.There was significant difference( P<0.05). The pathological injury scores in groups SCS, O and A by HE staining were(7.0±0.1), (5.0±0.9)and(2.5±0.5); injury scores and the expressions of renal proximal tubular epithelial cell vacuolar degeneration in groups O and A were lower than those in group SCS( P<0.05)and injury score in group A was lower than group O( P<0.05). In perfusate, the levels of △Cr, △AST and △LDH in groups O and A were(43.9±52.8)μmol/L and(12.6±3.5)μmol/L, (532.3±52.8)U/L and(49.1±50.4)U/L and(9998.0±2014.4)U/L and(1477.0±810.4)U/L.There were significant differences( P<0.05). In perfused kidneys, the MDA level and SOD activity in groups O and A were(0.192±0.018)mmol/g, (0.162±0.023)mmol/g, (0.6±0.3)×10 3 U/g, (1.7±0.4)×10 3 U/g; TNF-α and IL-6 levels in groups O and A were(124.376±19.635)and(89.331±13.123)ng/g, and(4.038±1.026)×10 3 and(1.774±0.518)×10 3 ng/g.After air-oxygenated NMP, lower renal damage indices were characterized by a lower MDA level and a higher SOD activity, the lower levels of TNF-α and IL-6 and the lower expressions of MPO and ICAM-1 than those in oxygen-oxygenated NMP( P<0.05). Conclusions:NMP with air-oxygenation mimics renal perfusion under physiological conditions and decreases oxidative stress and inflammation injury.It may confer a better retrieval in DCD kidney against warm ischemia injury.

Objective:To analyze the correlation between R. E.N.A.L., PADUA, C-index, DAP scoring system and the efficacy and safety of nephron-sparing surgery (NSS) for T 1b renal tumors, and to construct a nomogram model to predict the efficacy and safety of surgery by combining multiple parameters. Methods:The data of 80 patients with stage T 1b renal tumor who received NSS from March 2020 to July 2021 in Changhai Hospital of Naval Military Medical University were retrospectively analyzed. There were 59 males and 21 females, aged (56.9±10.2) years old. The tumor diameter was (4.7±0.9) cm, with 40 cases on the left and 40 on the right sides. Tumors were located in the upper/lower pole in 46 cases and in the middle in 34 cases. The tumors were located in 59 cases laterally, 21 cases medially, and 74 cases were bulging, 16 cases endogenous. There were 53 round tumors, 18 lobular tumors, and 9 irregular tumors. One case underwent open surgery, 43 cases underwent laparoscopic surgery, and 36 cases underwent robotic surgery.42 cases underwent transperitoneal approach, and 38 cases underwent retroperitoneal approach. The composite outcome (MIC) achieved by all three indicators, including negative surgical margins, warm ischemia time <20 min, and no serious complications, was used as the main indicator to evaluate the efficacy and safety of surgery. Secondary indicators were operation time, intraoperative blood loss, postoperative hospital stay, postoperative creatinine changes and hemoglobin changes. Relevant risk factors were analyzed by logistic regression, and a nomogram model for predicting surgical efficacy and safety was constructed. Receiver operating characteristic(ROC) curves were used to compare the predictive power of the nomogram model with other scoring systems. Results:Univariate logistic regression analysis showed that PADUA and R. E.N.A.L. scores were risk factors for MIC achievement( OR=1.419, P=0.038; OR=1.358, P=0.038). However, C-index and DAP were not risk factors for MIC achievement( P>0.05). The results of correlation analysis showed that R. E.N.A.L. score was significantly correlated with postoperative hemoglobin decrease(R 2=0.197). PADUA score was significantly correlated with postoperative hospital stay(R 2=0.186). C-index was significantly correlated with postoperative creatinine increase(R 2=-0.221). DAP was significantly associated with operation time (R 2=0.192). The results of univariate logistic regression analysis showed that body mass index ( OR=1.257, P=0.025), tumor morphology ( OR=18.741, P=0.005), longitudinal location of tumor ( OR=1.992, P=0.038), the relationship between tumor and collection system ( OR=4.886, P=0.002) were risk factors for MIC attainment. A nomogram prediction model was constructed by combining these indicators with the Mayo adhesive probability (MAP) index. The ROC curve showed that the area under the curve (AUC) of the nomogram model and R. E.N.A.L. score, PADUA score, C-index, and DAP were 0.834, 0.645, 0.643, 0.526, and 0.593, respectively. The nomogram model had the highest predictive power for T 1b renal tumors achieving MIC. Conclusions:In the renal tumor scoring system, PADUA and R. E.N.A.L. scores can predict whether the MIC of T 1b renal tumor NSS is achieved or not. The nomogram model composed of patient body mass index, tumor shape, longitudinal position of tumor, relationship between tumor and collecting system and MAP can better predict whether the MIC of T 1b renal tumor NSS is achieved or not.

Objective:To assess the association between warm ischemia time (WIT) and renal function in patients undergoing laparoscopic partial nephrectomy.Methods:A total of 344 patients treated with laparoscopic partial nephrectomy in Peking University People’s Hospital were included. There were 240 males (69.8%) and 104 females (30.2%) with a median age of 57 (23-89) years.The median BMI was 25.6 (16.7-36.0) kg/m 2.213 cases (61.9%) were associated with hypertension.There were 66 (19.2%) patients with diabetes mellitus. There were 92 cases (26.7%) with smoking history. The median preoperative creatinine was 73 (32-170) μmol/L. The median preoperative estimated glomerular filtration rate (eGFR) was 95 (33-142) ml/(min·1.73m 2). The maximum diameter of the tumor was 2.5 (7-9) cm.314 (91.3%) patients with renal cancer stage T 1. All patients underwent warm ischemia during the operation. The patients were divided into three groups for analysis. Restricted cubic spline regression analysis was used to assess the association between WIT as a continuous variable and percentage change of eGFR. Analysis of covariance was used to compare postoperative eGFR among the three groups, and to adjust for preoperative eGFR and tumor diameter. Results:There were statistically significant differences in the percentage change of postoperative eGFR ( P=0.009) and tumor diameter ( P<0.001) among the three groups. Restricted cubic spline regression analysis showed that with the prolongation of WIT, the percentage change of postoperative eGFR gradually decreased, and the curve began to stabilize after 30 minutes (R 2=0.044, P=0.015). The results of covariance analysis showed that after adjusting for baseline preoperative eGFR and tumor size, the effect of WIT on postoperative eGFR was significantly different among the three groups ( F=3.864, P=0.022). The postoperative eGFR in the WIT<20 min group was significantly higher than that in 20 min≤WIT<30 min group( P=0.009) and WIT≥30 min group( P=0.017). There was no significant difference in postoperative eGFR between the two groups with longer WIT( P=0.806). Conclusions:In partial nephrectomy, patients with WIT less than 20 minutes had higher postoperative eGFR levels than those with WIT greater than 20 minutes. However, when WIT exceeded 20 minutes, prolonged ischemia time did not lead to further decline in renal function.

Objective: To observe the clinical efficacy of warm needling moxibustion plus lumbar traction for lumbar disc herniation (LDH) and its effect on Oswestry disability index (ODI) score, present pain intensity (PPI) score, and the degree of straight leg raising.Methods: A total of 146 patients with LDH were randomized into a combination group and a control group by the random number table method, with 73 cases in each group. The control group was treated with lumbar traction, and the combination group was treated with lumbar traction plus warm needling moxibustion. The curative efficacy of the two groups was compared. Also, the lumbar function, pain, straight leg raising degree, and inflammatory factor levels before and after treatment were compared. Results: The total effective rate of the combination group was 93.2%, which was higher than 78.1% of the control group (P<0.05). After treatment, the lumbar ODI score, the PPI scores of lumbar pain and lower-limb pain, tumor necrosis factor (TNF)-α and interleukin (IL)-6 levels, and straight leg raising test score in both groups were significantly decreased (P<0.05). The Japanese Orthopaedic Association (JOA) score significantly increased (P<0.05). The improvements of the above items in the combination group were all superior to those in the control group (P<0.05). Conclusion: Warm needling moxibustion plus lumbar traction in the treatment of LDH can improve lumbar function, relieve pain, and reduce inflammatory response of the body.

Objective: To observe the clinical efficacy of warm needling moxibustion plus isokinetic muscle strength training for knee osteoarthritis (KOA).Methods: A total of 135 patients with KOA due to Yang deficiency and coagulated cold were randomized into a warm needling moxibustion group, an isokinetic muscle strength training group, and a combined group by the random number table method, with 45 cases in each group. The warm needling moxibustion group was treated with warm needling moxibustion. The isokinetic muscle strength training group was treated with isokinetic muscle strength training. The combined group was treated with warm needling moxibustion plus isokinetic muscle strength training. The Western Ontario and McMaster Universities osteoarthritis index (WOMAC) and visual analog scale (VAS) were scored before and after treatment, and isokinetic indicators of peak torque (PT), total work (TW) and average power (AP) were evaluated. Results: The total effective rate of the combined group was 92.5%, which was significantly higher than 83.3% in the warm needling moxibustion group (P<0.05) and 72.5% in the isokinetic muscle strength training group (P<0.05). After treatment, the scores of WOMAC (total, pain, stiffness, and function) and VAS, and isokinetic indicators (PT, TW, and AP) were all improved compared with those before treatment (P<0.05) in all three groups. The differences among the three groups were statistically significant (P<0.05). The WOMAC total score and score of stiffness in the combined group were lower than those in the warm needling moxibustion group and the isokinetic muscle strength training group (P<0.05), and the scores in the warm needling moxibustion group were lower than those in the isokinetic muscle strength training group (P<0.05). The WOMAC score of pain and VAS score in the warm needling moxibustion group and the combined group were lower than those in the isokinetic muscle strength training group (P<0.05). The differences between the warm needling moxibustion group and the combined group were not statistically significant (P>0.05). The WOMAC function score in the combined group was lower than that in the warm needling moxibustion group and the isokinetic muscle strength training group (P<0.05), while there was no statistical difference between the warm needling moxibustion group and the isokinetic muscle strength training group (P>0.05). PT, TW, and AP in the combined group were higher than those in the warm needling moxibustion group and the isokinetic muscle strength training group (P<0.05), and they were higher in the isokinetic muscle strength training group than in the warm needling moxibustion group (P<0.05). Conclusion: Warm needling moxibustion plus isokinetic muscle strength training has a better effect in the treatment of KOA due to Yang deficiency and coagulated cold than either warm needling moxibustion or isokinetic muscle strength training alone.

Objective: To observe the efficacy of warm needling moxibustion plus intra-articular injection of sodium hyaluronate for hip involvement in ankylosing spondylitis (AS). Methods: A total of 60 patients with hip involvement in AS were randomly divided into a control group and an observation group, with 30 cases in each group. The patients in the control group were given an intra-articular injection of sodium hyaluronate, once a week. The patients in the observation group were given additional warm needling moxibustion, once a day, with a 2-day interval after five consecutive days of treatment. After 5 weeks, changes in such indicators as visual analog scale (VAS) score, Harris score, Bath ankylosing spondylitis disease activity index (BASDAI), Bath ankylosing spondylitis functional index (BASFI), serum cartilage oligomeric matrix protein (COMP), interleukin (IL)-17 were observed, and the efficacy was evaluated. Six months after treatment, Bath ankylosing spondylitis radiology index-hip (BASRI-hip) was evaluated. Results: After treatment, the total effective rate in the observation group was higher than that in the control group (P<0.05). After treatment, the VAS scores in both groups were lower than those before treatment (P<0.05), and the score of the observation group was lower than that of the control group (P<0.05). After treatment, the Harris scores of both groups were higher than those before treatment (P<0.05), and the score of the observation group was higher than that of the control group (P<0.05). Six months after treatment, the BASRI-hip score of the control group was higher than that before treatment (P<0.05), while the score of the observation group was not significantly different from that before treatment (P>0.05) and was lower than that of the control group (P<0.05). After treatment, the scores of BASDAI and BASFI of both groups were lower than those before treatment (P<0.05), and the scores of the observation group were lower than those of the control group (P<0.05). After treatment, the levels of serum COMP and IL-17 of both groups were lower than those before treatment (P<0.05), and the levels of the observation group were lower than those of the control group (P<0.05). Conclusion: The efficacy of warm needling moxibustion plus intra-articular injection of sodium hyaluronate for hip involvement in AS is better than the intra-articular injection of sodium hyaluronate alone. This combined approach can alleviate hip pain, improve hip functions, delay the destruction of the hip, prevent AS development, and reduce the levels of serum COMP and IL-17.

Objective: To investigate the efficacy and possible mechanism of action of warm needling moxibustion combined with entecavir in the treatment of compensated cirrhosis due to chronic hepatitis B (CHB).Methods: Ninety patients were randomly divided into a control group and an observation group, with 45 patients in each group. The control group was given oral entecavir, and the observation group was given additional warm needling moxibustion. The serum alanine transaminase (ALT), aspartate transaminase (AST), albumin (ALB) levels, portal vein internal diameter, splenic vein internal diameter, spleen thickness, and liver hardness were compared before and after treatment. The serum hyaluronic acid (HA), laminin (LN), procollagen type Ⅲ (PCⅢ), type Ⅳ collagen (Ⅳ-C), interleukin (IL)-21, and platelet-derived growth factor (PDGF) levels were also measured. Results: After treatment, the serum ALT and AST levels decreased (P<0.05), and the serum ALB levels increased (P<0.05) in both groups. The serum ALT and AST levels were lower in the observation group than in the control group (P<0.05), and the ALB level was higher in the observation group (P<0.05). The portal vein internal diameter, splenic vein internal diameter, spleen thickness, and liver hardness values were reduced in both groups after treatment (P<0.05), and were lower in the observation group than in the control group (P<0.05). The serum HA, LN, PCⅢ, and Ⅳ-C levels were reduced in both groups (P<0.05), and were lower in the observation group than in the control group (P<0.05). In the control group, the serum IL-21 level decreased (P<0.05), but the serum PDGF level did not change significantly (P>0.05); in the observation group, the serum IL-21 and PDGF levels decreased significantly (P<0.05) and were lower than those in the control group (P<0.05). Conclusion: Warm needling moxibustion combined with entecavir treatment can improve liver function, reduce the inner diameters of the portal vein and splenic vein, spleen thickness, and liver hardness, and improve liver fibrosis indicators in patients with CHB cirrhosis, which may be related to the reduction of serum IL-21 and PDGF levels.
