RESUMEN

This article aims to describe a clinical case of the use of the injectable restorative technique with composite resin in a patient with an esthetic complaint in the anterior teeth. Producing direct restorations in anterior teeth when performed with composite resin demands a long clinical time. Results often depend on the operator's ability to reproduce proper form, function, and esthetics. Lately, the versatility of composite resins has increased. As this material evolved, new techniques were introduced to simplify the restorative process and save time. The restorative technique with injectable fluid composite resin, in addition to delivering faster and with a certain ease, is capable of increasing the predictability of treatment results. However, it is known that flowable resins have lower wear resistance and color stability compared to conventional resins. Thus, a version of this material with a higher filler content in its composition was introduced to the market, to improve wear resistance and gloss retention. Long-term studies evaluating the use of low-viscosity resins are needed. The injectable technique can accurately reproduce the morphology obtained in the wax-up of the case, generating a satisfactory esthetic and functional result. Despite simplifying the fabrication of restorations when compared to the conventional technique, it is still necessary for the operator to have some experience and aptitude. The clinical time for the production of restorations has been considerably reduced. The material showed good flow, shine, and polish in the immediate result. However, the finishing and polishing of the restorations required a longer clinical period than usual and in the short- term follow-up, flaws in the gingival margins and a new finishing and polishing procedure were necessary. Therefore, the technique described in this article can be considered a restorative alternative, but it is still not able to replace the tradit ional method of making previous esthetic restorations.


Este artículo tiene como objetivo describir un caso clínico del uso de la técnica restauradora inyectable con resina compuesta en un paciente con una queja estética en los dientes anteriores. Producir restauraciones directas en dientes anteriores cuando se realizan con resina compuesta demanda un largo tiempo clínico. Los resultados a menudo dependen de la capacidad del operador para reproducir la forma, la función y la estética adecuadas. Últimamente, la versatilidad de las resinas compuestas ha aumentado. A medida que este material evolucionó, se introdujeron nuevas técnicas para simplificar el proceso de restauración y ahorrar tiempo. La técnica restauradora con resina compuesta fluida inyectable, además de una entrega más rápida y segura, es capaz de aumentar la previsibilidad de los resultados del tratamiento. Sin embargo, se sabe que las resinas fluidas tienen menor resistencia al desgaste y estabilidad del color en comparación con las resinas convencionales. Así, se introdujo en el mercado una versión de este material con un mayor contenido de relleno en su composición, con el objetivo de mejorar la resistencia al desgaste y la retención del brillo. Se necesitan estudios a largo plazo que evalúen el uso de resinas de baja viscosidad. La técnica inyectable es capaz de reproducir fielmente la morfología obtenida en el encerado del caso, generando un resultado estético y funcional satisfactorio. A pesar de simplificar la fabricación de restauraciones en comparación con la técnica convencional, todavía es necesario que el operador tenga cierta experiencia y aptitud. El tiempo clínico para la producción de restauraciones se ha reducido considerablemente. El material mostró buena fluidez, brillo y pulido en el resultado inmediato. Sin embargo, el acabado y pulido de las restauraciones requirió de un período de tiempo clínico más largo de lo habitual y en el seguimiento a corto plazo, fueron necesarios defectos en los márgenes gingivales y un nuevo procedimiento de acabado y pulido. Por lo tanto, la técnica descrita en este artículo puede considerarse una alternativa restauradora, pero aún no es capaz de reemplazar el método tradicional de realizar restauraciones estéticas previas.

Bone and soft tissue conditions are important for successful implant treatment. But, the placement itself is also very important. Implants which is installed in the wrong position result in the biological, esthetical and mechanical problems. In order to place an implant in the correct position, the final restoration and diagnostic wax-up should be considered prior to the surgery. If the artificial teeth for the interim denture are directly transferred from the diagnostic wax-up, the operator can try the form of diagnostic wax-up in the mouth. If the surgical template is produced by duplicating the interim denture, the implant can be placed in the planned position. In this case, the polymethyl methacrylate (PMMA) artificial tooth was precisely milled by the digital duplication of diagnostic wax-up. And interim denture was fabricated by using these milled teeth. After the patient adapted for a sufficient period, the implant was placed at the planned position with surgical template produced by duplicating the interim denture. After confirming sufficient osseointegration, the final prostheses were made to reflect the shape of diagnostic wax-up. Through this procedure, the satisfactory functional and esthetic outcome could be acquired.

It is reported that the causes of unaesthetic proportion of anterior teeth vary widely. Especially, when the unaesthetic tooth proportion of the mandibular incisors arises due to the wear of the anterior teeth accompanied by the compensation of the alveolar bone, it may cause serious functional and aesthetic problems. In such case, it should be considered that the evaluation of vertical dimension and tooth proportion as well as smile line, soft tissue and hard tissue morphology. And, increase of vertical dimension or clinical crown lengthening followed by prosthodontic restorations is needed to improve the interdental mesial/distal, width/length ratio considering the anterior guidance. This case report demonstrates functional and aesthetic improvements through systematic diagnosis and treatment procedures in a 48-year-old male patient with unaesthetic anterior teeth proportion because of tooth wear accompanied by the compensation of alveolar bone and defect of several central incisors due to chronic periodontitis.

The maxillary anterior teeth play an important role in esthetics. The esthetic of maxillary anterior teeth is closely related to tooth morphology and also harmony with gingiva. Precise diagnosis and treatment plan are essential to satisfy patient's demand, and sometimes surrounding soft tissue management is involved to achieve the goal. Gingivectomy can be considered as one method to make esthetic restoration possible. As well as esthetics, function has to be considered in maxillary anterior teeth restoration. Definitive cast of abutment and diagnostic cast waxed up labially were superimposed with model scanner, so can provide former comfortable occlusion. This case report demonstrates functional and esthetic improvements of two patients through gingivectomy and the data of superimposed image of casts.

Para obter um resultado mais satisfatório no enceramento, devemos sempre ter um bom conhecimento do material utilizado, das cores naturais e da anatomia propriamente dita. Da forma aqui abordada é possível se ter uma prévia muito mais rápida de como será a prótese, sem muita dificuldade, podendo até se realizar a prova em boca, pois há o reforço da fibra de algodão, que alimenta a resistência e não é quebradiça.


In order to obtain better results through wax-up it is important to have proper knowledge on the utilized material,natural color and anatomy of the teeth. The presented approach enables a fast and simple preview of the prostheses and since due to the lint reinforcement it also allows testing in the patients' mouth.

The treatment of areas demanding esthetic requirements, such as maxillary anterior teeth, should take into account the achievement of a healthy, harmonious to the surrounding tissue, and an attractive smile line. In this case report, smile line, soft tissue and hard tissue morphology, and the anatomy and proportion of the tooth, must be considered. In patients with unesthetic maxillary anterior ratio due to inadequate gingival contour and diastema caused by peg lateralis, it would be challenging to achieve an esthetic restoration by orthodontic treatment alone. In such case, after orthodontic treatment, dento-gingivo-facial compositional diagnosis and analysis, followed by gingivectomy and prosthodontic restoration is needed to improve the interdental mesial/distal, width/length ratio to achieve a satisfactory esthetic result. In addition, when improving the tooth proportion of peg lateralis by prosthodontic treatment, Porcelain laminate veneer (PLV), which results in less tooth structure loss, reproduction of similar shade to that of the proximal tooth and high transparency, is recommended. This case report demonstrates esthetic improvements by prosthodontic restoration through systematic diagnosis and treatment procedure in patients with unesthetic maxillary anterior proportion after orthodontic treatment due to peg lateralis by means of two female patients aged twenty years old.

Partial edentulism has multiple implications in relation to function, esthetics and future rehabilitative treatment. This case report illustrates the management of a patient with extreme consequences of partial edentulism. The main clinical findings were unopposed remaining teeth, overeruption of the remaining teeth, loss of vertical dimension of occlusion, and significant disfigurement of the occlusal plane. Following the diagnostic procedure, a well-coordinated prosthodontic treatment involving liaison with other dental disciplines was indicated. The management involved an innovative combination of fixed and removable prostheses in conjunction with crown lengthening surgery and strategic implant placement. Series of provisional prostheses were applied to facilitate the transition to the final treatment.

BACKGROUND: In order to restore severely attrited teeth properly, surgical intervention in the form of a crown-lengthening procedure may be required. And also, proper diagnosis and treatment sequencing is critical to obtain a successful results. Adequate diagnostic wax-up ensures good esthetics and healthy periodontal tissue. CASE DESCRIPTION: This clinical case report describes a diagnostically based protocol for restoration on mandibular anterior teeth with crown lengthening procedure and the treatment of partially edentulous mandible combined with an edentulous maxilla. In addition, the effort to prevent the combination syndrome was described. CLINICAL IMPLICATION: An interdisciplinary diagnosis and examination through visualization of the desired results ensure conservative and more predictable outcome.
