La prestación de servicios para la atención integral de la salud requiere la participación de diversos profesionales con competencias específicas para brindar la mejor atención posible a la población. La inclusión del nutricionista en los diferentes niveles del sistema de salud es crucial para garantizar una atención integral en las diversas etapas de la vida. Una distribución inequitativa de nutricionistas en los niveles de atención puede conducir a una fragmentación de la atención y la pérdida de oportunidades para abordar los problemas relacionados con la alimentación y nutrición. Por ello, se desarrolló un estudio con el objetivo de describir la distribución de nutricionistas en los establecimientos de salud según el nivel de atención, y evaluar el cumplimiento de las recomendaciones de recursos humanos establecidas por la norma técnica de las UPSS de Nutrición y Dietética. Se desarrolló un estudio descriptivo y transversal, mediante el análisis de fuentes secundarias. Se utilizaron los datos de recursos humanos por IPRESS de SUSALUD, del año 2022. Se consideró la Norma técnica de UPSS de Nutrición y Dietética para evaluar el cumplimiento de las recomendaciones de recursos humanos. Entre los principales resultados, se identificó que el 7.93% de los establecimientos de salud del primer nivel de atención tienen al menos un nutricionista. En el segundo nivel de atención, el 96.35% de los establecimientos de salud no cumplen con las recomendaciones de recursos humanos de la norma técnica, y ningún establecimiento del tercer nivel de atención cumplió esta recomendación. La distribución de nutricionistas en los diversos niveles de atención del sistema de salud peruano es desigual. Es necesario mejorar la planificación de recursos humanos en el sistema de salud peruano para garantizar una atención integral de la salud a la población.
The provision of services for comprehensive healthcare requires the involvement of various professionals with specific competencies to provide the best possible care to the population. The inclusion of nutritionists at different levels of the healthcare system is crucial to ensure comprehensive care at various stages of life. An unequal distribution of nutritionists across healthcare levels can lead to fragmented care and missed opportunities to address nutrition-related issues. Therefore, a study was conducted with the aim of describing the distribution of nutritionists in healthcare facilities according to the level of care and evaluating compliance with the human resources recommendations established by the technical standard of Nutrition and Dietetics UPSS. A descriptive and cross-sectional study was conducted using the analysis of secondary sources. Human resources data from SUSALUD for the year 2022 were used. The Technical Standard of UPSS of Nutrition and Dietetics was considered to assess compliance with human resources recommendations. Among the main findings, it was identified that 7.93% of first-level healthcare facilities have at least one nutritionist. In the second level of care, 96.35% of healthcare facilities do not comply with the human resources recommendations of the technical standard, and no third-level healthcare facility met this recommendation. The distribution of nutritionists across various levels of care in the Peruvian healthcare system is unequal. It is necessary to improve human resources planning in the Peruvian healthcare system to ensure comprehensive healthcare for the population..

RESUMEN Objetivo: Determinar el perfil de los investigadores en ciencias médicas y de la salud de la categoría Monge Medrano, calificados por el Consejo Nacional de Ciencia, Tecnología e Innovación Tecnológica (CONCYTEC) y registrados en el Registro Nacional Científico, Tecnológico y de Innovación Tecnológica (RENACYT) en el año 2022. Materiales y métodos: Estudio de enfoque cuantitativo, tipo descriptivo y de corte transversal. Se revisaron las Hojas de vida afines a la Ciencia y Tecnología (CTI Vitae) de 706 investigadores registrados en Renacyt durante el mes de setiembre del 2022. Se recopilaron las características demográficas, de formación académica, de actividad profesional y de producción científica mediante una ficha de recolección de datos y, a partir de esto, se elaboró una base de datos. Resultados: El 65,3 % de investigadores fueron de sexo masculino, un 80,0 % procedía de Perú y un 66, 7 % dominaba dos o tres idiomas; los más frecuentes eran español, inglés, portugués, francés, italiano, alemán y quechua. A nivel académico, el máximo grado académico obtenido fue el de doctor (71,2 %), la primera carrera profesional no siempre fue una carrera estrictamente de la salud, el 80,3 % reportó estudios de maestría y el 71,2 %, estudios de doctorado; además, el 75,5 % indicó una universidad como institución de primera filiación. A nivel profesional, el 38,2 % registró más de 20 años de experiencia laboral, el 84 % como docente y el 51,7 % tenían experiencia como evaluador o formulador de proyectos de investigación; el 74,8 % reportó haber recibido un premio o distinción en su carrera, asimismo, 133 investigadores contaban con un índice h entre 4 y 6. Adicionalmente, el 94,5 % (667 investigadores) tenía artículos de producción científica, y de estos, el 41,4 % había redactado entre uno y cinco artículos donde era primer autor; además, el 65,9 % de los investigadores reportó haber realizado asesoría de tesis en pregrado y posgrado. Conclusiones: El investigador en ciencias médicas y de la salud es predominantemente masculino, domina al menos dos idiomas, incluido el inglés, tiene grado de doctor y reporta tener producción científica. La universidad es la institución más frecuente de filiación de los investigadores.
ABSTRACT Objective: To determine the profile of medical and health sciences researchers belonging to the Carlos Monge Medrano group, qualified by Consejo Nacional de Ciencia, Tecnología e Innovación Tecnológica (CONCYTEC National Council of Science, Technology and Technological Innovation) and registered in Registro Nacional Científico, Tecnológico y de Innovación Tecnológica (RENACYT National Scientific, Technological and Technological Innovation Registry) in 2022. Materials and methods: A quantitative, descriptive and cross-sectional study. The CTI Vitae Hojas de vida afines a la Ciencia y Tecnología (CTI Vitae Résumés related to Science and Technology) data sheets of 706 researchers registered in Renacyt were reviewed during September 2022. Demographic characteristics, academic background, career and scientific output were collected in a data collection sheet, which was used to create a database. Results: Out of all researchers, 65.3 % were males, 80.0 % came from Peru, and 66.7 % were fluent in two or three languages, the most frequent being Spanish, English, Portuguese, French, Italian, German and Quechua. As for their academic background, the highest degree was a doctorate (71.2 %), the first reported program was not always strictly a health sciences program, 80.3 % pursued master's studies and 71.2 % pursued doctoral studies. In addition, 75.5 % indicated a university as their primary affiliation. Regarding their career, 38.2 % had more than 20 years of work experience, 84 % served as educator, 51.7 % had experience as research project evaluator or developer and 74.8 % received an award or distinction during their career. Moreover, 133 researchers had an h-index between 4 and 6. Furthermore, 94.5 % (667 researchers) drafted scientific papers, out of whom 41.4 % were the lead author in one to five articles and 65.9 % served as undergraduate and graduate thesis advisor. Conclusions: Medical and health sciences researchers are mostly males, are fluent in at least two languages including English, have a doctorate degree and have scientific output. Universities are the most frequent institution of affiliation reported by researchers.

@#<strong>BACKGROUND AND OBJECTIVE</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">Staffing shortages and health inequities are persistent barriers in the Philippines toward achieving universal health care. To ensure an adequate and responsive health workforce, there is a need to evaluate the Human Resources for Health (HRH) status across health facilities, particularly those in underserved communities. Hence, this study aims to determine the staffing requirements and workload pressure among primary care facilities in selected geographically isolated and disadvantaged areas (GIDAs) in the Philippines.</p><strong>METHODS</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">The study utilized the workload indicators of staffing need (WISN) methodology from the World Health Organization to determine the staffing and workload situation among three health worker cadres (physicians, nurses, and midwives) in the study sites. Particularly, six primary care facilities (four rural health units and two community hospitals) located in Surigao del Norte were involved in the study. WISN-related data (health service statistics, available working time, and health professionals’ workload components) were collected through records review, focus group discussions, and key informant interviews. The WISN software was used to analyze the staffing levels and workload pressure in the selected facilities.</p><strong>RESULTS</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">A total of 40 health workers, including physicians (n = 5, 13%), nurses (n = 21, 52%), and midwives (n = 14, 35%) participated in the study. The findings noted varying levels of staffing and workload pressures among the three cadres in selected primary care facilities, which were influenced by several factors. Particularly, health facilities with additional human resources obtained from deployment programs indicated adequate staffing and low to normal workload pressures. However, further analysis revealed potential HRH maldistribution and reliance on the temporary nature of the staff augmentation program in delivering primary care services, which need to be addressed to optimize health workforce planning. Service workload may also have been impacted by the temporary closure of health facilities due to disasters. Among the few cadres tha reported staffing shortage and high workload pressure, these were due to higher service demands, increased task delegation, and inadequate service coordination. Hence, context-specific challenges and situational factors in GIDAs need to be considered when determining the staffing and workload requirements.</p><strong>CONCLUSION</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">There is a need to improve the capacities of health facilities and local government units (LGUs) to engage in evidence-based HRH planning through the WISN methodology. Doing so could improve staffing and workload distribution among health care facilities in the country. Moreover, interorganizational collaboration (DOH, LGUs, and health facilities) should be strengthened to improve delineation and prevent duplication/omission of health services, rationalize HRH distribution and augmentation, and streamline the priority health services based on the local contextual factors.</p>

Objective: The Japanese government’s physician workforce reform, which commenced in April 2024, introduced regulations on physicians’ working hours. However, in areas facing physician shortages such as rural regions and surgical medical specialties, healthcare provision relies heavily on the extended working hours of each physician. The anticipated impact of this reform, when implemented, was significant.Materials and Methods: Using publicly available government data, we estimated the current working hours of physicians in various medical specialties in each prefecture across Japan. Subsequently, we calculated the ratio of surplus or deficit physicians when hypothetically assuming that all physicians adhered to the regulatory upper limit of 58.4 working hours per week nationwide.Results: Assuming that all physicians work to the regulated maximum, there would be a shortage of doctors in various medical specialties across Japan, such as surgery, neurosurgery, orthopedic surgery, obstetrics and gynecology, and emergency medicine. Geographically, shortages of doctors are observed in rural prefectures such as those in the Tohoku region, particularly in emergency- and surgery-related specialties, indicating a critical shortage of physicians in rural areas. Additionally, it has become evident that even in medical specialties with a calculated surplus of physicians nationwide, the margin of surplus is generally only a few percentage points.Conclusion: Currently, rural areas and surgical medical specialties in Japan have limited leeway in the physician workforce, and the strict application of workforce reform may lead to a severe shortage of physicians in these areas. It is noteworthy that as similar reforms may subsequently be implemented in other countries, analogous challenges would arise. Thus, the implementation of workforce reform requires a flexible approach to minimize its negative effects, which widen the existing disparity in the workforce.

ABSTRACT Objective. To create and validate criteria for prioritizing problems related to policies and management of the health workforce. Methods. This methodological study was divided into three stages. First, the criteria were elaborated by means of a systematized literature review. Second, the criteria were evaluated online by a committee of judges comprised of eight specialists. In the third stage, an evaluation was carried out by the target audience in a hybrid workshop. The participants evaluated the material using the Suitability Assessment of Materials instrument, adapted for the research. Results. Three prioritization criteria (relevance, window of opportunity and acceptability) and a scoring scale were developed based on the literature review. In the evaluation by the committee of judges, the approval percentage of the criteria and prioritization method was 84%. Modifications were made based on suggestions in relation to the material presented to the specialists. In the pre-test stage, the approval percentage varied by item, with six of them reaching a maximum approval of 100% (corresponding to approximately 46% of the items), four reaching 92% and three achieving 83% each, indicating positive results. Conclusions. The developed criteria were considered valid for use in the context of policies and management in the area of human resources for health.
RESUMEN Objetivo. Crear y validar criterios para priorizar los problemas relacionados con las políticas y la gestión de los recursos humanos para la salud. Métodos. Este estudio metodológico se dividió en tres etapas. En la primera se elaboraron los criterios mediante una revisión sistematizada de la bibliografía. En la segunda un comité de ocho especialistas evaluó en línea los criterios. Y la tercera consistió en una evaluación por parte del público destinatario en un taller híbrido. Los participantes evaluaron el material utilizando el instrumento de evaluación de la idoneidad de los materiales, que fue adaptado para la investigación. Resultados. Sobre la base de la revisión de la bibliografía, se elaboraron tres criterios para la asignación de prioridades (relevancia, ventana de oportunidad y aceptabilidad) y una escala de puntuación. En la evaluación realizada por el comité de especialistas, el porcentaje de aprobación de los criterios y del método de asignación de prioridades fue del 84%. Se realizaron modificaciones basadas en sugerencias planteadas con respecto al material presentado a los especialistas. En la etapa posterior de prueba preliminar, el porcentaje de aprobación varió en los distintos puntos, de tal manera que en seis puntos (es decir, en aproximadamente el 46% de los puntos) se alcanzó una aprobación máxima del 100%, en cuatro una aprobación del 92% y en tres una aprobación del 83% en cada uno, lo que indica unos resultados positivos. Conclusiones. Se consideró que los criterios elaborados son válidos para su uso en el contexto de las políticas y la gestión en el ámbito de los recursos humanos para la salud.

O presente estudo metodológico foi dividido em três fases. Primeiro, foram elaborados critérios por meio de revisão sistematizada da literatura. A seguir, os critérios foram avaliados on-line por uma comissão de juízes composta por oito especialistas. Na terceira fase, o público-alvo fez uma avaliação dos critérios em uma oficina de formato híbrido. Os participantes avaliaram o material usando o instrumento Suitability Assessment of Materials, adaptado para esta pesquisa. Resultados. Com base na revisão da literatura, foram elaborados três critérios de priorização (relevância, janela de oportunidade e aceitabilidade) e uma escala de pontuação. Na avaliação da comissão de juízes, a porcentagem de aprovação dos critérios e do método de priorização foi de 84%. Foram feitas alterações com base em sugestões relacionadas ao material apresentado aos especialistas. Na fase de pré-teste, a porcentagem de aprovação variou de acordo com o item. Seis deles (aproximadamente 46% dos itens) atingiram aprovação máxima de 100%, quatro atingiram 92% e três atingiram 83%, indicando resultados positivos. Conclusões. Os critérios desenvolvidos foram considerados válidos para uso no contexto de políticas e gerenciamento na área de recursos humanos em saúde.

Abstract Objective: to analyze the availability (in terms of stock and composition) and accessibility (in terms of geographical distribution) of the nursing workforce in Brazil. Method: this is a descriptive, cross-sectional study with retrospective data collection, identified by combining databases available on institutional websites and structured according to indicators from the World Health Organization's "National Health Workforce Accounts". The study considered nursing professionals at senior level (nurses) and middle level (nursing auxiliaries and technicians). Indicators of stock, composition, distribution (by age group and gender) and the ratio of nurses to doctors were included. Results: there was an increase in the number of personnel between 2005 and 2010, mainly in middle and technical level professionals. There are more personnel aged between 36 and 55, with a predominance of women in all categories, despite the increase in men. There was an uneven distribution of personnel across the country's regions, with the Southeast having the largest number of professionals. The ratio of nurses to doctors is less than one in the South and Southeast. Conclusion: despite the large number of nurses, their distribution is uneven. The growth of nursing technicians has significantly outstripped that of nurses, indicating more intensive technical training policies than those found in higher education.
Resumo Objetivo: analisar a disponibilidade (estoque e composição) e acessibilidade (distribuição geográfica) da força de trabalho de enfermagem no Brasil. Método: estudo descritivo e transversal, com coleta retrospectiva de dados, identificados por meio da combinação de bancos de dados disponíveis em sites institucionais e estruturados de acordo com indicadores das "Contas Nacionais da Força de Trabalho em Saúde" da Organização Mundial da Saúde. Consideraram-se, para o estudo, profissionais de enfermagem de nível superior (enfermeiros) e nível médio (auxiliares e técnicos de enfermagem). Indicadores de estoque, composição, distribuição (por faixa etária e sexo) e razão de enfermeiros para médicos foram incluídos. Resultados: houve aumento no número de profissionais, entre 2005 e 2010, principalmente nos profissionais de nível médio e técnico. Há mais profissionais entre 36 e 55 anos, com predominância do sexo feminino em todas as categorias, apesar do aumento do sexo masculino. Observou-se distribuição desigual de profissionais nas regiões do país, sendo a região Sudeste a com maior número de profissionais. A razão de enfermeiros por médico é inferior a um nas regiões Sul e Sudeste. Conclusão: apesar do elevado contingente de enfermeiros, sua distribuição é desigual. O crescimento de técnicos de enfermagem superou significativamente o de enfermeiros, indicando políticas de formação técnica mais intensivas que as encontradas no ensino superior.
Resumen Objetivo: analizar la disponibilidad (en términos de número y composición) y accesibilidad (en términos de distribución geográfica) de la fuerza de trabajo de enfermería en Brasil. Método: estudio descriptivo y transversal, con recolección retrospectiva de datos, identificados a través de una combinación de bases de datos disponibles en sitios web institucionales y estructurados en función de los indicadores de las "Cuentas Nacionales del Personal de Salud" de la Organización Mundial de la Salud. La población del estudio fueron profesionales de enfermería de nivel superior (enfermeros) y nivel medio (auxiliares y técnicos en enfermería). Se incluyeron indicadores de número, composición, distribución (por franja etaria y sexo) y razón entre enfermeros y médicos. Resultados: aumentó el número de personal entre 2005 y 2010, principalmente la de nivel medio y técnico. La mayoría del personal tiene entre 36 y 55 años y predomina el sexo femenino en todas las categorías, pese a que aumentó la cantidad de trabajadores do sexo masculino. Se observó que la distribución de personal en las diferentes regiones de Brasil era desigual y la región Sudeste es la que tiene la mayor cantidad de profesionales. La proporción de enfermeros por médico es inferior a uno en las regiones Sur y Sudeste. Conclusión: a pesar de que la cantidad de enfermeros es elevada, la distribución es desigual. El aumento de la cantidad de técnicos en enfermería superó significativamente al de enfermeros, lo que indica que las políticas de formación técnica son más intensivas que las observadas en la educación superior.

ABSTRACT The 35th Brazilian Congress of Surgery marked a turning point for surgical education in the country. For the first time, the Brazilian College of Surgeons included Global Surgery on the main congressional agenda, providing a unique opportunity to rethink how surgical skills are taught from a public health perspective. This discussion prompts us to consider why and how Global Surgery education should be expanded in Brazil. Although Brazilian researchers and institutions have contributed to the fields expansion since 2015, Global Surgery education initiatives are still incipient in our country. Relying on successful strategies can be a starting point to promote the area among national surgical practitioners. In this editorial, we discuss potential strategies to expand Global Surgery education opportunities and propose a series of recommendations at the national level.
RESUMO O 35º Congresso Brasileiro de Cirurgia foi marcado por discussões inovadoras para a educação cirúrgica no país. Pela primeira vez, o Colégio Brasileiro de Cirurgiões incluiu a Cirurgia Global na pauta principal do congresso, proporcionando uma oportunidade única de repensar como as habilidades cirúrgicas são ensinadas a partir de uma perspectiva de saúde pública. Essa discussão nos leva a considerar por que e como o ensino da Cirurgia Global deve ser expandido no Brasil. Embora pesquisadores e instituições brasileiras tenham contribuído para a expansão do campo desde 2015, as iniciativas de educação em Cirurgia Global ainda são incipientes em nosso país. Basear-se em estratégias bem-sucedidas pode ser um ponto de partida para promover a área entre os profissionais de cirurgia nacionais. Neste editorial, discutimos potenciais estratégias para expandir as oportunidades de educação em Cirurgia Global e propomos uma série de recomendações a nível nacional.

RESUMO Introdução: Em julho de 2021, o Brasil tinha 373 cursos de Medicina autorizados pelo MEC, dos quais 229 de instituições privadas, com um valor médio de mensalidade de R$ 8.242,70. O grande volume de recursos envolvidos em mensalidades, bolsas e fomentos (Prouni, Fies etc.) justifica o esforço envolvido neste trabalho. Objetivo: A revisão visou identificar as metodologias de análise de custo de formação de médicos, classificando-as segundo categorias de nível de ensino e abrangência do custo. Método: A metodologia escolhida foi a revisão de escopo, dado que a produção na área de foco é irregular e com lacunas metodológicas. Resultado: As buscas em português e inglês resultaram na seleção de 24 textos, que foram agrupados em nove grupos, elaboradas a partir do foco de seus conteúdos combinados com os interesses da pesquisa. Conclusão: A inexistência de consenso metodológico e a busca por otimizar recursos e avaliar a eficácia do ensino reforçam a necessidade do desenvolvimento de metodologias de apuração de custos da graduação em Medicina.
ABSTRACT Introduction: Brazil had 373 medical courses authorized by the Ministry of Education in July 2021, with 229 being in private institutions and an average monthly tuition rate of R$8,242.70. The vast number of resources involved in tuition, scholarships, and grants (PROUNI, FIES, and so forth) justifies the effort required for this activity. Objective: The review sought to find approaches for analyzing the cost of training doctors, categorizing them according to the level of education and cost scope. Method: Given the irregularity and methodological inadequacies around interest, the chosen methodology was a scoping review. Result: The literature search in Portuguese and English yielded 24 articles, which were divided into nine categories depending on their content focus and the study interests. Conclusion: The lack of methodological agreement, as well as the pressure to optimize resources and evaluate educational efficacy, highlight the need for the development of methods to determine the costs of undergraduate medical education.

Problema: Disposição laboral insuficiente dos profissionais alocados na Atenção Primária à Saúde. Relevância: O dimensionamento correto qualifica os processos e a gestão. Objetivo: Avaliar a capacidade laboral da rede da Atenção Primária à Saúde do município de Primavera do Leste/Mato Grosso, Brasil. Métodos: Estudo epidemiológico descritivo exploratório, do tipo transversal, de análise secundária do Sistema de Notificação de Agravos de Notificação, e-SUS AB e o e-Gestor. Foram utilizados indicadores de saúde e parâmetros para a estratificação do grau de vulnerabilidade territorial. A produção e o número de profissionais foram calculados por unidade de saúde. Resultados: A análise mostrou que a produtividade e o quantitativo ideal das categorias profissionais de médicos, enfermeiros, técnicos de enfermagem, agentes comunitários, dentistas, técnicos e auxiliares de saúde bucal estiveram abaixo do recomendado. Considerando que os dados compilados se referem ao ano de 2021, período de excepcionalidade da pandemia, a explicação para essa redução da produção ideal pode estar na diminuição dos procedimentos e consultas programadas em todas as unidades de saúde. Os desafios são complexos e envolvem fatores como crescimento populacional acelerado, profissionais sem vínculo efetivo e com alta rotatividade, bem como participação mais efetiva da gestão para a inclusão desse tema nas pautas estratégicas da saúde.
Issue: Insufficient workforce allocation in Primary Health Care. Relevance: Accurate staffing enhances processes and management. Objective: To evaluate the workforce capacity of the Primary Health Care network in the municipality of Primavera do Leste/Mato Grosso, Brazil. Methods: An exploratory descriptive epidemiological study, of a cross-sectional type, with a secondary analysis of the Notifiable Disease Notification System (SINAN), e-SUS AB, and the e-Manager (e-Gestor). Health indicators and parameters for the stratification of territorial vulnerability levels were used. The output and number of professionals were calculated per health unit. Results: The analysis revealed that the productivity and ideal quantity of professional categories, including doctors, nurses, nursing technicians, community agents, dentists, dental technicians, and dental assistants, were below the recommended levels. Considering that the compiled data refer to the year 2021, a period of exceptional circumstances because of the pandemic, the explanation for this reduction in ideal production may be due to the decrease in procedures and scheduled appointments across all health units. The challenges are complex and involve factors such as rapid population growth, professionals without a formal contract and with high turnover, as well as a more effective management participation to include this issue in the strategic health agendas.

Objetivo: Determinar la relación entre los estilos de liderazgo y el desempeño laboral del personal médico del Hospital Vitarte Lima, en 2018. Métodos: Estudio descriptivo, correlacional y transversal. Se hizo un muestreo no probabilístico intencionado y se seleccionó a 64 trabajadores. La recolección de datos se hizo mediante un cuestionario previamente validado y un análisis de consistencia interna, por el alfa de Cronbach. Resultados: La estadística descriptiva determinó un 50% en el estilo de liderazgo democrático de las dimensiones y un nivel superior a 60% para el desempeño laboral. La estadística inferencial, mediante la prueba de hipótesis por el coeficiente de correlación de Spearman (rho = 0,85), evidenció una correlación positiva alta en ambas variables. Conclusión: Existe correlación positiva entre el estilo de liderazgo democrático de las dimensiones y el desempeño laboral, por lo que se recomienda la creación de proyectos de mejora de la calidad y capacitación en los trabajadores.
Objective: To determine the relationship between leadership styles and work performance of the medical staff of Hospital Vitarte Lima in 2018. Methods: Descriptive, correlational and cross-sectional study. A non-probabilistic purposive sampling was done and was selected 64 workers. The data collection was done through a previously validated questionnaire and internal consistency by Cronbach's alpha. Results: The descriptive statistics determined a 50% in the democratic leadership style of the dimensions and a level higher than 60% for job performance, the inferential statistics through the hypothesis test by Spearman's correlation coefficient (rho = 0.85) showed a high positive correlation in both variables. Conclusion: There is a positive correlation between the dimensions Democratic Leadership Style and work performance, so it is recommended the creation of quality improvement projects and training for workers.

O objeto de estudo desta dissertação é a formação do sanitarista no nível de graduação. Delimita-se na história dos cursos de graduação em saúde coletiva e no desenho formativo realizado para a nova profissão. Objetiva-se analisar a formação na graduação em saúde coletiva dos cursos existentes no Brasil à luz dos projetos pedagógicos dos cursos. Trata-se de um estudo qualitativo e exploratório com abordagem documental, utilizando o método da análise de conteúdo. Depois da análise de 22 projetos, os resultados demonstraram uma abertura gradual dos cursos, com concentração entre 2008 e 2009, sendo o último aberto em 2019, e que estão organizados em maior número na região Norte, com 27,3%. A maioria dos cursos segue ou se adequou às Diretrizes Curriculares Nacionais do Curso de Graduação em Saúde Coletiva (DCNsCGSC), com predominância de carga horária igual ou superior a 3.200 horas, duração de oito a nove semestres e média de quatro anos, havendo turno noturno em 45,5% dos cursos. A oferta média é de 1.864 vagas/ano, 68% dos cursos são oferecidos por universidades federais, e 77,3% deles recebem a nomenclatura de bacharelado em saúde coletiva. Dos 18 cursos avaliados pelo Ministério da Educação (MEC), 67% são avaliados com conceito 5. A formação permite uma construção do conhecimento de forma gradativa a partir das bases da saúde coletiva. Os cursos articulam-se com os serviços e os sistemas de saúde do Sistema Único de Saúde (SUS) e com a comunidade através de aulas práticas, estágios, pesquisa e projetos de extensão, fortalecendo o contato prático dos alunos com situações reais de saúde da população. Conclui-se que, embora o país esteja vivenciando uma crise na educação superior, os cursos de graduação em saúde coletiva estão na contramão dessa realidade. Apesar de as DCNsCGSC não estarem em vigor devido à não publicação pelo MEC em Diário Oficial, os cursos estão em conformidade com elas. A formação segue as bases da saúde coletiva, de modo que cumpre o objetivo de fortalecimento do SUS para a formação de recursos humanos em saúde.
The object of study of this dissertation is the training of the public health professional at the undergraduate level. It is delimited in the history of undergraduate courses in public health and in the training design carried out for the new profession. The objective is to analyze the undergraduate training in public health of the existing courses in Brazil in the light of the pedagogical projects of the courses. This is a qualitative and exploratory study with a document approach, using the method of content analysis. After analyzing 22 projects, the results showed a gradual opening of courses, with a concentration between 2008 and 2009, with the last one being in 2019, and which were organized in greater numbers in the Northern region, with 27.3%. Most courses follow or fit the National Curriculum Guidelines of the Undergraduate Course in Public Health (DCNCGSC), with a predominance of hours equal to or greater than 3,200 hours, duration of eight to nine semesters, and an average of four years, with night classes in 45.5% of the courses. The average offer is 1,864 places/year, 68% of the courses are offered by federal universities, and 77.3% of them receive the name of bachelor in public health. Of the 18 courses evaluated by the Ministry of Education (MEC), 67% are evaluated with a grade of 5. Training allows a gradual construction of knowledge based on the foundations of public health. The courses are articulated with health services and systems of the Unified Health System (SUS) and with the community with practical classes, internships, research, and extension projects, strengthening students' practical contact with real health situations in the population. Therefore, although the country is experiencing a crisis in higher education, the undergraduate courses in public health are going against this reality. Although the DCNsCGSC are not in force due to the non-publication by the MEC in the Official Gazette, the courses are in accordance with them. The training follows the basis of collective health so that it fulfills the objective of strengthening the SUS for the training of human resources in health.
El objeto de estudio de esta tesis es la formación de profesionales de la salud pública en el grado. Se hace una delimitación de la trayectoria de los cursos de grado en salud colectiva y el diseño formativo realizado para la nueva profesión. La investigación pretende analizar la formación de grado en salud colectiva en Brasil a partir de los proyectos pedagógicos de los cursos. Se trata de un estudio cualitativo y exploratorio, de enfoque documental, y con el método de análisis de contenido. Del análisis de 22 proyectos, los resultados muestran que hubo una ampliación gradual de los cursos en el período entre 2008 y 2009, la última se llevó a cabo en 2019, y que entre las regiones brasileñas donde se concentran destaca más el Norte, con un 27,3%. La mayoría de los cursos siguen o se ajustan a las Directrices Curriculares Nacionales de la Carrera de Grado en Salud Colectiva (DCNsCGSC), tienen carga horaria igual o superior a 3.200 horas, con una duración de ocho a nueve semestres y un promedio de cuatro años, con jornada nocturna en el 45,5%. La oferta promedio es de 1.864 vacantes/año, el 68% de los cursos son en universidades federales y el 77,3% llevan el nombre Licenciatura en Salud Colectiva. De los 18 cursos evaluados por el Ministerio de Educación (MEC), el 67% recibieron calificación 5. La formación permite construir paulatinamente conocimientos a partir de las bases de la salud colectiva. Los cursos se articulan con los servicios y redes de salud del Sistema Único de Salud (SUS) brasileño y con la comunidad mediante clases prácticas, pasantías, proyectos de investigación y extensión, fortaleciendo el contacto de los alumnos con situaciones reales de salud de la población. Se concluye que, aunque el país viva una crisis en la educación superior, las carreras de grado en salud colectiva van en sentido contrario a esta realidad. Si bien las DCNsCGSC no se encuentran vigentes todavía, porque hace falta que el MEC las publique en el Diario Oficial, los cursos están de acuerdo con ellas. La formación sigue las bases de la salud colectiva, de modo que cumpla el objetivo de fortalecer el SUS para la formación de recursos humanos en salud.

Resumo No contexto do trabalho hospitalar durante a COVID-19, é preciso reconhecer que as atividades de apoio exercidas por maqueiros, agentes de limpeza e auxiliares administrativos são imprescindíveis para o processo de trabalho. Este artigo analisou resultados de uma etapa exploratória de uma pesquisa ampla com esses trabalhadores dentro de uma unidade hospitalar referência para COVID-19 no estado da Bahia. Foram selecionadas três entrevistas semiestruturadas produzidas a partir da perspectiva do "fazer falar" sobre o trabalho, utilizando pressupostos da etnometodologia e da ergonomia. A análise incidiu sobre as atividades de trabalho de um maqueiro, um agente de limpeza e uma auxiliar administrativa, problematizadas a partir da díade essencialidade-invisibilidade. O estudo demonstrou que esses trabalhadores são invisibilizados pela desvalorização social de suas atividades e nível de escolaridade e resistem apesar das circunstâncias e do sobretrabalho; e evidenciou a essencialidade desses serviços pela interdependência entre o trabalho de apoio e o trabalho assistencial e suas contribuições para a segurança do paciente e da equipe. Conclui-se que é necessário criar estratégias para que esses trabalhadores sejam valorizados social, financeira e institucionalmente.
Abstract In the context of hospital work during the COVID-19 pandemic, it should be acknowledged that the nonclinical support activities carried out by stretcher bearers, cleaning agents and administrative assistants were essential for the work process. This article analyzed the results of an exploratory stage of broader research with these workers in a COVID-19 hospital reference unit in the state of Bahia. Three semi-structured interviews were selected, using assumptions of ethnomethodology and ergonomics, to encourage these workers to talk about their work, The analysis concentrated on the work activities of stretcher-bearers, cleaning agents and administrative assistants from a visibility perspective. The study showed that these workers were rendered invisible by the scant social respect for their activities and level of schooling that prevailed despite the circumstances and excess workload; and revealed the essential nature of these services due to the interdependence between support work and care work and their contributions to patient and team safety. The conclusion drawn is that it is necessary to establish strategies such that these workers are valued socially, financially, and institutionally.

Objetivos: Aplicar la dinámica de sistemas para estimar la evolución de la incidencia y la prevalencia de hipoacusia en personas mayores en países de bajos, medios y altos ingresos, así como el acceso al tratamiento, y evaluar la influencia de la implementación de estrategias sanitarias sobre estos indicadores. Metodología: Los análisis se realizaron mediante simulación con dinámica de sistemas según parámetros globales. Para ello, se desarrolló un diagrama de bucles causal, integrando la incidencia, la prevalencia y el tratamiento de hipoacusia con el nivel de desigualdad, factores de riesgo, uso de dispositivos de ayuda auditiva, fuerza laboral de audiólogos y otorrinolaringólogos según el nivel de ingresos del país. Luego, se construyó un diagrama de flujo para ejecutar las simulaciones durante un período de 100 años. Además, se ejecutaron cuatro simulaciones con estrategias sanitarias (reducción de factores de riesgo, mejora en el uso dispositivos de ayuda auditiva, aumento del número de audiólogos y otorrinolaringólogos) y se estimó el porcentaje de cambio respecto al modelo basal. Resultados: Los países de bajos ingresos mostraron una mayor incidencia y prevalencia de hipoacusia, menor acceso a tratamiento adecuado y una mayor prevalencia de hipoacusia sin tratar o con tratamiento inadecuado. La reducción de factores de riesgo creció en un 15 y 33 % la población con audición normal en los próximos 50 y 100 años, respectivamente. Además, la mejora en el uso de dispositivos de ayuda auditiva logró una reducción del 60 % de la población con tratamientos inadecuados o sin tratamiento, y el aumento de audiólogos y otorrinolaringólogos incrementó un 250 % el acceso a un tratamiento adecuado. Conclusiones: La evolución de la salud auditiva está condicionada por factores económicos, donde los entornos más desfavorecidos muestran peores indicadores. Además, la implementación de estrategias combinadas favorecería la salud auditiva en el futuro.
Objectives: To estimate the evolution of the incidence and prevalence of hearing loss in the elderly in low-, middle- and high-income countries by means of system dynamics simulation according to global parameters and to analyze the influence of the implementation of health strategies. Methodology: A causal loop diagram was developed to relate the incidence, prevalence and treatment of hearing loss to the level of inequality, risk factors (RF), use of hearing aids (HA), audiologist and otolaryngologist (ENT) workforce by country income level. A flow chart was then constructed to run the simulations over a 100-year period. In addition, four simulations were run with health strategies (reduction of RF, improvement in HA use, increase in the number of audiologists and ENT specialists) and the percentage change from the baseline model was estimated. Results: Low-income countries showed a higher incidence and prevalence of hearing loss, less access to adequate treatment, and a higher prevalence of untreated or inadequately treated hearing loss. The reduction of RF increased the population with normal hearing by 15% and 33% over the next 50 and 100 years, respectively. In addition, the improvement in the use of ha achieved a 60% reduction in the population with inadequate or untreated treatment, and the increase in audiologists and ENT specialists improved the access to adequate treatment by 250%. Conclusions: The evolution of hearing health is conditioned by economic factors, where the most disadvantaged environments show worse indicators. In addition, the implementation of combined strategies would favor hearing health in the future. System dynamics is a very useful methodology for health managers because it enables to understand how a disease evolves and define what are the best health interventions considering different scenarios.
Objetivos: Aplicar a dinâmica do sistema para estimar a evolução da incidência e prevalência da perda auditiva em pessoas idosas em países de baixo, médio e alto rendimento, bem como o acesso ao tratamento, e avaliar a influência da implementação de estratégias de saúde sobre estes indicadores. Metodologia: As análises foram conduzidas utilizando simulação da dinâmica do sistema com base em parâmetros globais. Para tal, foi desenvolvido um diagrama do laço causal, integrando a incidência, prevalência e tratamento da perda auditiva com o nível de desigualdade, fatores de risco, utilização de aparelhos auditivos, mão-de-obra de audiologistas e otorrinolaringologistas por nível de rendimento nacional. Foi então construído um fluxograma para executar as simulações ao longo de um período de 100 anos. Além disso, foram realizadas quatro simulações com estratégias de saúde (reduzindo os fatores de risco, melhorando a utilização de aparelhos auditivos, aumentando o número de audiologistas e otorrinolaringologistas) e foi estimada a mudança percentual em relação ao modelo de base. Resultados: Os países de baixos rendimentos mostraram maior incidência e prevalência de perda auditiva, menor acesso a tratamento apropriado e maior prevalência de perda auditiva não tratada ou tratada de forma inadequada. A redução dos fatores de risco aumentou a população com audição normal em 15 e 33% durante os próximos 50 e 100 anos, respectivamente. Além disso, uma melhor utilização de aparelhos auditivos permitiu uma redução de 60% na população mal tratada ou não tratada, e o aumento do número de audiologistas e especialistas em ORL aumentou em 250% o acesso ao tratamento adequado. Conclusões: A evolução da saúde auditiva é condicionada por fatores económicos, com os ambientes mais desfavorecidos a apresentarem indicadores piores. Além disso, a implementação de estratégias combinadas favoreceria a saúde auditiva no futuro.

Objetivo : Describir el diagnóstico situacional de un grupo de Unidades Productoras de Servicios de Medicina de Rehabilitación (UPS-MR) del Perú evaluadas en el 2019 y 2020. Material y métodos : Estudio observacional, transversal y descriptivo del análisis secundario de datos de la evaluación que realizó la Dirección Ejecutiva de Investigación y Docencia en Prevención de Riesgos y Promoción de la Salud del Instituto Nacional de Rehabilitación "Dra. Adriana Rebaza Flores" AMISTAD PERÚ - JAPÓN a UPS-MR durante los años 2019 y 2020. Las variables fueron: logro en el equipamiento, infraestructura, documentos/herramientas de gestión y manuales y guías de práctica clínica, funciones, recursos humanos, procedimientos y logro total. Cada logro tenía como puntaje mínimo 0% y como puntaje máximo 100%. Además, se realizó un análisis secundario para comparar el logro de las UPS-MR según nivel de categorización y región. Resultados: Catorce UPS-MR fueron incluidas en la investigación. Seis (42,9%) fueron de categoría III-1 y 6 (42,9%) eran de la región Lima. El mayor logro fue en las funciones (74,1%) y el menor logro en los recursos humanos y procedimientos (41,9% y 40,8%). Finalmente, el logro total tuvo una media de 58,3%±14,6%. Conclusiones : Las UPS-MR tienen el mayor logro en las funciones y los menores logros en recursos humanos y procedimientos. El logro total fue alrededor del 50%.
SUMMARY Objective : To describe the situational diagnostic of a group of units producing medical rehabilitation services (UPMRS) in Peru from 2019 to 2020. Methods: This a cross-sectional study that included a secondary analysis of an evaluation performed by the Dirección Ejecutiva de Investigación y Docencia en Prevención de Riesgos y Promoción de la Salud of the Instituto Nacional de Rehabilitación "Dra. Adriana Rebaza Flores" AMISTAD PERÚ - JAPÓN a UPMRS from 2019 to 2020. Variables included in the analyses were achievement of infrastructure, equipment, documents of administration, guidelines of clinical practice, human resources and total achievement. The minimal score per achievement was 0% and the maximum was 100%. Achievements stratified by region and categorization were made. Results: Fourteen PMRS were included; six (42.9%) were category III-1 and six (42.9%) were from Lima Region. Best achievement was in functions (74.1%) and worst was in human resources and procedures (41.9% and 40.8%, respectively). Finally, total achievement had a mean score of 58.3%±14.6%. Conclusions: UPMRS had better achievements in functions and worse in human resources and procedures, total achievement was 50%.

Resumo Trata-se de uma revisão narrativa cujo objetivo é compreender o estado da arte da literatura sobre programas de Agentes Comunitárias de Saúde (ACS) no mundo, identificando suas nomenclaturas, práticas, formação e condições trabalhistas. A grande concentração de programas de ACS ainda ocorre em países de baixa e média renda da África (18), Ásia (12) e América Latina (05), com algumas poucas experiências em países de alta renda na América do Norte (02) e Oceania (01). No total foram catalogadas 38 experiências, tendo sido descritas as práticas de cuidado, vigilância, educação, comunicação em saúde, práticas administrativas, de articulação intersetorial e mobilização social. Caracterizou-se os níveis e duração das formações das ACS, assim como as diversas condições de trabalho em cada país. Em grande parte, o trabalho é precarizado, muitas vezes voluntário e realizado por mulheres. A revisão proporcionou um panorama comparativo que pode contribuir para enriquecer o olhar de gestores e tomadores de decisão em contextos de implantação, ampliação e reconfiguração de tais programas.
Abstract This is a narrative review whose objective is to understand the state of the art of the literature on Community Health Worker (CHW) programs worldwide, identifying their nomenclatures, practices, training, and working conditions. The major concentration of CHW programs can still be found in low- and middle-income countries in Africa (18), Asia (12), and Latin America (05), with a few experiences in high-income countries in North America (02) and Oceania (01). In total, 38 experiences were cataloged, and the practices of care, surveillance, education, health communication, administrative practices, intersectoral articulation, and social mobilization were described. The levels and duration of CHW training were characterized, as were the different working conditions in each country. Much of the work is precarious, often voluntary and carried out by women. This review provided a comparative overview that can contribute to enrich the view of managers and decision-makers in contexts of the implementation, expansion, and reconfiguration of such programs.

Background: Globally, there has been a trend of devolving services from the National governments to local governments in order to enhance efficiency, effectiveness, equity, responsiveness, and community participation at the grass root levels. This study evaluated the effects of devolution on healthcare system performance in Machakos Level 5 Hospital in Machakos County. Specifically, it evaluated the effects of devolution on healthcare systems performance on healthcare infrastructure, workforce adequacy and workforce satisfaction. Methods: Descriptive cross-sectional study design was used, with mixed methods of data collection (quantitative and qualitative). Results: The analyzed results revealed that healthcare infrastructure, workforce adequacy and workforce satisfaction improved post devolution period to a great extent. From the inferential statistics analysis, there was a significant improvement in all the three objectives post devolution i.e., p<0.05. Conclusions: Devolution of healthcare system improved the performance of Machakos County Level 5 Hospital. Recommendations were made to roll out the UHC program countrywide, equip all departments, employ more staff, enforce the referral system, similar study be conducted in other county referral hospitals, and similar study be conducted with a focus on other healthcare building blocks.

INTRODUCTION@#Due to COVID-19 pandemic, many have shifted into working at home which led to physical inactivity. This may cause musculoskeletal discomfort, chronic disease, muscle atrophy and spinal imbalance due to improper and prolonged sitting posture. Since mobile devices are relatively available for most of the office workers, there were still a lack of evidence-based mobile applications that can counteract the inactivity through exercises, which led to the researchers to create an application called SitMate that consists of evidence-based exercises which aimed to prevent musculoskeletal discomfort among a business process outsourcing company Workforce Management Personnel (BPO-WMP).@*METHODS@#Eleven participants (18-40 years old) full-time, work-from-home BPO-WMP were randomized into Treatment Group(TG)(n=6) and Control Group (CG)(n=5). The TG received one month intervention with the use of SitMate Application containing relaxation exercises, range of motion exercises and stretching exercises, and notifications for postural correction while the CG continued their usual working schedule.@*RESULTS@#There were no significant differences between two groups on all body parts that were measured using the Cornell Musculoskeletal Discomfort Questionnaire, and no significant differences in the intragroup pre-test and post-test scores on all body parts between TG and CG. For the intra-group post-test of the TG, there were noted improvements on the hip/buttock, right shoulder, upper back (median = 0) and right wrist (median = 1.5). There was also a noted increase in discomfort on the neck (median = 1.5) and lower back (median = 3). For the post-test of the CG, there were noted improvements on the right shoulder, right wrist (median = 0) and lower back (median = 1.5).@*CONCLUSION@#This study has shown that the SitMate application does not effectively reduce the prolonged sitting-related discomfort among the personnel after 1 month of intervention.

@#Introduction: Health is a dynamic, emerging, and interdisciplinary field. To address current health problem challenges, we need a public health workforce constantly adaptable and equipped with the newest skills. This study was to explore the skills needed by public health bachelor’s to work in the health office. Methods: We used qualitative methods. A total of six participants took part in this study. Interviews were held virtually via Zoom Platform in September 2022 using a semi-structured interview guide. Interviews were audio-visual recorded, and transcribed verbatim. The transcripts were coded by members of the research team and a thematic analysis was conducted. Results: The majority of participants get jobs in the office through different work entrances. Our study identifies that participants need technical and interpersonal skills to support their job in the health office. Outside learning contributes to developing skills needed by participants. Conclusion: This study represents an initial endeavor to identify the core work-related skills for public health graduates who work in the health office. Future research is needed for longitudinal larger scale quantitative studies to confirm our qualitative findings. In addition, our findings promote transformations on learning for Bachelor of Public Health Program more comprehensively.

RESUMEN La pandemia de la enfermedad por el coronavirus 2019 (COVID-19, por su sigla en inglés) demostró la necesidad de reforzar el enfoque de género desde una perspectiva integradora y multisectorial para el abordaje de las problemáticas en salud. Este artículo busca destacar la importancia de incorporar y fortalecer el enfoque de género en las políticas de construcción de sistemas de salud resilientes, equitativos y universales. Con este objetivo, se aborda el papel de las mujeres en el sector de salud desde dos ámbitos. El primero da cuenta de las condiciones de acceso de las mujeres a los sistemas de salud de la Región de las Américas, donde resalta la necesidad de superar los obstáculos que impiden la realización plena de su derecho a la salud. El segundo plantea el papel preponderante de las mujeres en el mercado laboral del sector de salud, y la necesidad de aumentar su liderazgo en la toma de decisiones del sector. Por último, se hace un llamado para avanzar en la implementación de recomendaciones para fortalecer el enfoque de género y el papel de las mujeres en las políticas y los sistemas de salud.
ABSTRACT The coronavirus disease 2019 (COVID-19) pandemic demonstrated the need to strengthen the focus on gender from an integrative and multisectoral perspective to address health care problems. This article seeks to highlight the importance of incorporating and strengthening the gender focus in policies for building resilient, equitable, and universal health care systems. With this objective in mind, the role of women in the health sector is addressed from two directions. The first examines women's conditions of access to health systems in the Region of the Americas and highlights the need to overcome the obstacles that prevent the full realization of their right to health care. The second discusses the preponderant role of women in the health labor market, and the need to expand their leadership in decision-making in the sector. Finally, an appeal is made for progress in the implementation of recommendations to strengthen the focus on gender and the role of women in health policies and systems.
RESUMO A pandemia da doença do coronavírus de 2019 (COVID-19) demonstrou a necessidade de fortalecer a abordagem de gênero a partir de uma perspectiva integradora e multissetorial ao tratar das questões de saúde. Este artigo procura ressaltar a importância de incorporar e fortalecer a abordagem de gênero nas políticas para o desenvolvimento de sistemas de saúde resilientes, equitativos e universais. Com esse objetivo, este trabalho aborda o papel das mulheres no setor da saúde a partir de duas perspectivas. A primeira está ligada às condições de acesso das mulheres aos sistemas de saúde na Região das Américas, destacando a necessidade de superar os obstáculos que impedem o pleno exercício de seu direito à saúde. A segunda destaca o papel proeminente das mulheres no mercado de trabalho do setor da saúde e a necessidade de aumentar o seu papel de liderança na tomada de decisões no setor.
