Fever of unknown origin(FUO)is a difficulty in clinical diagnosis and treatment.Patients with FUO come to seek medical consultation usually with fever as the main complaint,and the accompanying symptoms and signs are generally atypical.The pathogenesis of FUO remains conflicting in the field of modern western medicine,and its treatment is still focused on empirical anti-inflammatory management,which has the deficiency of delayed diagnosis,limited therapeutic options,poor therapeutic effects,and obvious adverse reactions.In the field of traditional Chinese medicine(TCM),FUO generally results from the dysfunction of zang-fu organs and the imbalance of yin and yang,and has the clinical features of long duration of illness,unknown etiology,complexity of illness,recurrent attacks,and difficult to be cured.Based on the six-meridian syndrome differentiation,Chief Physician LI Chuang-Peng pointed out that the pathogenesis of FUO is characterized by the combined disease of shaoyin and yangming,and put forward the therapeutic principle of warming shaoyin and unblocking yangming.He proposed the use of Dahuang Fuzi Decoction(mainly composed of Rhei Radix et Rhizoma and Aconiti Lateralis Radix Praeparata)plus Yiyi Fuzi Baijiang Powder(mainly composed of Coicis Semen,Aconiti Lateralis Radix Praeparata and Patriniae Herba)to subside fever and eliminate pathogen,together with Asari Radix et Rhizoma for guiding the medicine directly to the shaoyin.Moreover,therapies of strengthening and activating spleen and stomach,nourishing yin to produce fluid,and unblocking the blood vessels can be used for eliminating the pathogen and supporting the healthy qi.

Objective To explore the characteristics of the five circuit phases and six climatic factors(wuyun liuqi,also translated as five circuits and six qi)at birth of the patients with hepatic diseases,and to analyze the correlation between the incidence of hepatic diseases and the characteristics of wuyun liuqi of birthdates of the patients.Methods A total of 5 057 inpatients with hepatic diseases admitted to Guangdong Integrated Chinese and Western Medicine Hospital Affiliated to Guangzhou University of Chinese Medicine from May 2021 to May 2023 were collected,and then the characteristics of wuyun liuqi at birth of the patients were statistically analyzed.Results The incidence of hepatic diseases was closely related to the dominant circuit,dominant qi,guest qi,joining of guest qi with dominant qi,and circuit qi.In terms of the correlation of the prevalence of hepatic diseases with the dominant circuit(regular yearly evolutive phase)of the birthdate,the prevalence was the highest in the population born at gold circuit and was the lowest in the population born at fire circuit;in terms of the dominant qi(regular yearly circuit qi),the prevalence was the highest in the population born at taiyin wet-earth phase and was the lowest in the population born at taiyang cold-water phase;in terms of the guest qi(alterable yearly circuit qi),the prevalence was the highest in the population born at yangming dryness-gold phase and was the lowest in the population born at shaoyin monarch-fire phase;in terms of joining of guest qi with dominant qi,the prevalence was the highest in the population born at clockwise phase of guest qi generating or restricting dominant qi and was the lowest in the population born at adaptive phase of dominant qi and guest qi;in terms of the combination of circuit qi,the prevalence was the highest in the population born in the year of shunhua(qi generating circuit)and was the lowest in the population born in the year of suihui(yearly circuit phase being the same as the circuit qi of the year earthly branch).The differences were all statistically significant(P<0.05 or P<0.001).Conclusion The incidence of hepatic diseases is correlated with the dominant circuit,dominant qi,guest qi,joining of guest qi with dominant qi,and combination of circuit and qi at the birthdate of the patients to some extent.

Objective To observe the clinical effect of Yangming meridian acupuncture combined with acupuncture push based on the theory of"treating flaccence and taking Yangming"in the treatment of Guillain-Barre syndrome.Methods 52 cases of patients with conventional rehabilitation combined with Yangming meridian acupuncture combined with acupuncture based on the theory of"treating potence and taking Yangming alone"were taken as the study group and 52 cases of patients with conventional rehabilitation alone as the control group.Limb muscle strength score,clinical efficacy,limb sensory function,limb motor function,upper limb median nerve electrophysiology and daily living ability were compared between the two groups.Results After treatment,muscle strength scores of proximal lower extremity,distal lower extremity,proximal upper extremity,distal upper extremity,median sensory nerve action potential(SNAP),motor conduction velocity(MCV),sensory conduction velocity(SCV)and modified Barthel index(MBI)were increased in 2 groups(P<0.05).The study group was more obvious(P<0.05).The total effective rate of the study group(86.54%)was higher than control group(69.23%)(P<0.05).After treatment,the sensory function and motor function of limbs in 2 groups were better than before treatment(P<0.05),especially in the study group(P<0.05).After treatment,the distal motor latency(DML)of upper limb median nerve was decreased in 2 groups(P<0.05),and more significantly in the study group(P<0.05).Conclusion Yangming meridian acupuncture combined with acupuncture pushing based on the theory of"treating impotence and taking Yangming"is effective in the treatment of Guillain-Barre syndrome,which can improve the muscle strength,sensation and motor function of limbs,and regulate nerve electrophysiology.

[Objective]To explore the influence of WANG Yangming's theory of mind in the Ming Dynasty on ZHANG Jingyue's thought of treating depression.[Methods]With the help of historical literature research methods of distinguishing academic chapters and examining the origin and development of mirror studies,this study aims to sort out ZHANG Jingyue's grand perspective based on the ontology,physical application,and cultivation of Yangming's theory of mind,establishes a new theory of traditional Chinese medicine for the treatment of depression,and elucidates the inheritance and development of ZHANG Jingyue's thought of treating depression on Yangming's theory of mind.[Results]The core issues of Yangming's theory of mind were the relationship between human mind and emotions,as well as the relationship between emotion and mood.ZHANG Jingyue established a theory of depression diagnosis and treatment that combined emotion and mood,illness due to depression,discussed depression from deficiency,and treated depression through knowledge and action,reflecting his medical thinking's inheritance and development of Yangming's theory of mind.[Conclusion]Based on the analysis of the origin and development of Yangming's theory of mind,it can be concluded that ZHANG Jingyue's thought of treating depression inherits and exerts the profound meaning of Yangming's theory of mind,develops and constructs a highly innovative theory for the diagnosis and treatment of depression in traditional Chinese medicine,and analyzes his thought of treating depression under the approach of Yangming's theory of mind,which is beneficial for the diagnosis and treatment of depression in traditional Chinese medicine.

This study explored the ideas and methods of acupuncture for Guillain-Barré Syndrome (GBS) with the core principle of “to treat flaccidity, select the yangming (阳明) channel only”. The main pathological mechanism of GBS is deficiency of qi and blood in the yangming channel, malnutrition of all sinews, diminished spleen and stomach function leading to the production of pathogenic damp-heat qi, which obstructs the meridians, and gradually affects the liver and kidneys, consuming essence and damaging blood. Concurrently, dysfunction of the dumai (督脉) pivotal mechanism and lack of moisture in sinews and vessels result in symptoms such as skin numbness, paralysis, and muscle wastage. In clinical diagnosis and treatment, a combination of syndrome and channel differentiation is taken. Treatment primarily focuses on acupoints of yangming channel, aiming to supplement qi and blood, and acupoints of du mai are combined to open the vessel and fill the marrow. Specific acupoints are selected based on syndrome differentiation, providing comprehensive regulation to promote harmonization of qi and blood, relieve meridians, and the smooth generation and circulation of whole body fluids. This, in turn, enhances the strength of muscles and bones, and fosters a robust and freely moving body.

OBJECTIVE@#To observe the clinical effect of electroacupuncture at acupoints of yangming meridians for sarcopenia.@*METHODS@#A total of 60 patients with sarcopenia were randomized into an observation group and a control group, 30 cases in each group. In the control group, conventional nutrition intervention for sarcopenia was adopted. In the observation group, on the basis of the treatment in the control group, acupuncture was applied at bilateral Binao (LI 14), Quchi (LI 11), Zusanli (ST 36), Yanglingquan (GB 34), etc.,ipsilateral Quchi (LI 11) and Zusanli (ST 36) were connected to electroacupuncture, with discontinuous wave, 2 Hz in frequency, 1-10 mA in intensity, 2 times a week, with a interval of 3 days. A total of 12-week treatment was required in the two groups. Before and after treatment, the appendicular skeletal muscle mass index (ASMI), grip strength, 6 m-walking time, body fat percentage and body moisture percentage were observed in the two groups.@*RESULTS@#Compared with those before treatment, after treatment, ASMI and grip strength were increased while 6 m-walking time was shortened in the two groups (P<0.05); body fat percentage was decreased while body moisture percentage was increased in the observation group (P<0.05). After treatment, in the observation group, ASMI, grip strength and body moisture percentage were increased (P<0.05), 6 m-walking time was shortened and body fat percentage was decreased (P<0.05) compared with those in the control group.@*CONCLUSION@#Electroacupuncture at acupoints of yangming meridians can effectively improve the skeletal muscle mass, muscle function, body fat percentage and body moisture percentage in patients with sarcopenia, and make the distribution of muscle and fat more reasonable.

Based on the theory of "brain-gut communication" and "heart-stomach disease simultaneously", the thinking and method of treating chronic fatigue syndrome (CFS) from yangming meridian were discussed. CFS is related to brain and heart. Based on the analysis of meridian circulation, zang-fu function and the indication characteristics of yangming meridian, the indications of yangming meridian are closely related to brain and heart, so it is proposed to start from yangming meridian and use Chinese herbs combined with acupuncture to treat CFS, including the four methods of clearing away heat and moisturizing dryness, cooling blood and removing blood stasis, promoting qi to clear the organs, and strengthening and replenishing deficiency. It has certain guiding and reference significance for clinical treatment of CFS.

In this study, the specific manifestation of the "" is analyzed from the aspect of word meaning, and the relationship between the "" and stomach is further discussed through the perspectives of and the exterior-interior relationship. In addition, combined with typical clinical case, it is suggested that attention should be paid on the relationship between exterior and interior syndromes, and the clinical application of and method (activating and promoting circulation of organs) should be strengthened, which could expand the new thinking of clinical treatments for viscera diseases through acupuncture and moxibustion.

BACKGROUND: Traditional Chinese medicine has proposed the theory that “treatment for flaccidity aims at Yangming meridian” in the Internal Canon of Medicine. However, relatively few reports focus on electroacupuncture at the Stomach Meridian of Foot-Yangming for the treatment of spinal cord injury. From the perspectives of Bcl-2 and Bcl-2 expression, the mechanism of electroacupuncture intervention at the Stomach Meridian of Foot-Yangming for the treatment of spinal cord injury was preliminarily explored in this study. OBJECTIVE: To investigate the effect of electroacupuncture on the expression of Bax and Bcl-2 in injured segments of spinal cord injury rats. METHODS: Sixty-four Sprague-Dawley female rats (SPF level) were randomly divided into four groups: control group, pre-labeling group, post-injury labeling group and electroacupuncture group, with 16 rats in each group. The control group and the pre-labeling group were given intraperitoneal injection of Brdu (5 mg/kg) for 10 continuous days prior to the operation to label the cells that had differentiation ability and vitality in the spinal cord. The control group underwent laminectomy at T10 segment on the 11th day of labeling, while a model of spinal cord injury was induced by the clamp method in the pre-labeling group. The post-injury labeling group and the electroacupuncture group were injected intraperitoneally with BrdU (5 mg/kg) for 10 continuous days to mark the activated and proliferated cells. In the electroacupuncture group, electroacupuncture stimulation was done at Zusanli and Futu acupoints of Foot-Yangming Stomach Meridian on the 3rd day after modeling, 5 times a week. Four rats were taken from each group at 3, 10, 17, and 24 days after modeling (0, 7, 14, and 21 days after electroacupuncture stimulation). Basso, Beattie and Bresnahan scores were used to evaluate the motor function, and the expressions of Bax and Bcl-2 protein and gene in the spinal cord were detected using qRT-PCR and western blot. RESULTS AND CONCLUSION: At 3-24 days after surgery, the neurological scores in the pre-labeling group, post-injury labeling group, and electroacupuncture group were significantly lower than those in the control group (P < 0.05). The neurological scores of the electroacpuncture group were significantly higher than those in the pre-labeling group and post-injury labeling group (P < 0.05). With the prolongation of spinal cord injury time, the mRNA and protein expressions of Bax increased first and then decreased, while the mRNA and protein expression of Bcl-2 decreased first and then increased. The ratio of Bax/Bcl-2 decreased gradually with the lapse of electroacupuncture intervention time. To conclude, electroacupuncture stimulation of the Stomach Meridian of Foot-Yangming can reduce apoptosis of nerve cells, and promote recovery of motor function in spinal cord injury rats by regulating the expression of Bcl-2 and Bax.

BACKGROUND: The authors found a striking similarity between the qi and blood theory and nerve repair of spinal cord injury in terms of improving blood-oxygen microenvironment in tissues. The hypothesis is that electroacupuncture can improve the blood-oxygen microenvironment of the spinal cord and promote nerve regeneration by regulating hypoxia-inducible factor 1a and vascular endothelial growth factor signal transduction. OBJECTIVE: To investigate the effect of electroacupuncture intervention on the expression of hypoxia-inducible factor 1a and vascular endothelial growth factor in injured segments of spinal cord injury rats. METHODS: Totally 120 Sprague-Dawley female rats were enrolled to make spinal cord injury models by clamping the spinal cord (20 seconds) using a microvascular clamp. Rat models were then randomly divided into three groups: Ashi point group, Stomach Meridian of Foot-Yangming group and blank control group, 40 rats in each group. Electroacupuncture at two Aishi points or at both sides of Futu and Zusanli points was started on the 3rd day after modeling. Each rat was scored on the Basso, Beattie and Bresnahan locomotor rating scale at 1, 2, 3,4, and 5 weeks after intervention. Injured spinal cord specimens were then taken and observed histomorphologically. Hypoxia-inducible factor-1a and vascular endothelial growth factor protein and mRNA expressions were detected using immunohistochemistry staining, real-time fluorescence quantitative PCR and western blot assay. The study protocol was approved by the Animal Ethic Committee of the First Affiliated Hospital of Guangxi University of Chinese Medicine in China (approval No. 201712001). RESULTS AND CONCLUSION: The lower limb function score, hypoxia-inducible factor-1a and vascular endothelial growth factor gene and protein expression in the two electroacupuncture intervention groups were significantly higher than those of the blank control group. The number of neurons in Ashi point and Stomach Meridian of Foot -Yangming groups was significantly higher than that of the blank control group with the lapse of intervention time. Electroacupuncture intervention can effectively improve the lower limb function score of spinal cord injury rats, increase the number of neurons, and up-regulate the mRNA and protein expression of hypoxia-inducible factor-1 a and vascular endothelial growth factor, thus effectively promoting the neurological recovery of the spinal cord.

<p><b>OBJECTIVE</b>To analyze the distribution characteristics of hirsutism and the law of meridian differentiation in polycystic ovarian syndrome (PCOS).</p><p><b>METHODS</b>For the patients of PCOS, the modified Ferriman-Gallwey score (mF-G score) was adopted to evaluate the hirsutism distribution. The clementine data mining software was used to analyze the distribution region of hirsutism and the correlation with meridian.</p><p><b>RESULTS</b>A total of 141 patients of PCOS participated in the study. The most common regions of hirsutism were in the upper lip (50.35%) and the lower abdomen (34.04%). The higher frequencies of hirsutism at two regions were at the upper lip and lower abdomen (24.82%), the upper lip and chest (12.06%) and the upper lip and thigh (11.35%). In the analysis on the correlation of the upper lip, lower abdomen and chest with other regions, hirsutism was more common in the nearby regions. In reference to the running course of meridian, the frequencies of hirsutism from high to low were the conception vessel (63.12%), the thoroughfare vessel (61.70%), the liver meridian of foot-(58.16%) and the stomach meridian of foot-(57.45%). OBJECTIVE: To examine the distribution of heat-sensitive acupoints below the elbow and knee joints of the Yangming Meridians in patients with perennial allergic rhinitis (PAR).. METHODS: A total of 27 PAR volunteers 16-55 years in age were recruited in the present study. In a quiet room, the patient was ordered to take a recumbent or sitting position, and the operator held an ignited moxa-stick to make a circling (about 2 min), sparrow-pecking (2 min) and forwards-backwards moving (1 min) moxibustion perpendicularly over the skin (about 3 cm away) along the running course of the Large Intestine Meridian of Hand Yangming below the elbow joints and the Stomach Meridian of Foot Yangming below the knee joints on both sides. The procedures were repeated again and again until the moxibustion sensation disappeared. The patient was asked to carefully perceive the heat diffusion, diathermic, extending, athermal (cool and itching) feelings, at the heated loci. If one or more of the feelings appeared, it was considered as the thermosensitive phenomenon and that spot was marked as the "thermosensitive spot" or "thermosensitive acupoint".. RESULTS: In those 27 PAR patients, the thermosensitive response was found at Quchi (LI 11), Shousanli (LI 10), Hegu (LI 4), Sanjian (LI 3), and Zusanli (ST 36), constituting 59.25% (16/27), 85.18% (23/27), 44.44% (12/27), 55.55% (15/27), and 11.11% (3/27), respectively, being 92.59% in total. Of the 27 patients, 2, 2, 9, 8, 5 and 1 cases had 0, 1, 2, 3, 4 and 5 thermosenitive acupoints appeared, accounting for 7.41%, 7.41%, 33.33%, 29.63%, 18.52% and 3.70%, respectively. CONCLUSION: There is a certain occurrence rate of the thermosensitive acupoints below the elbow and knee joints of the Yangming Meridians in PAR patients, which may provide a target-point for treating PAR.

[Objective]Research on treatment based on syndrome differentiation of palmar hyperhidrosis(PH), for the purpose of systematically and comprehensively understanding PH, and to improve clinical curative effect. [Methods]The pathological basis, clinical manifestation and therapeutic principle and methods are systematically summarized.[Result]PH is relevant to the cold, heat, deficiency and excess of taste, disorder of Qi and blood of heart, and the block of damp and hot. Exuberant heat in Yangming, deficiency cold of spleen and stomach, and food stagnation can lead to PH. Malnutrition of heart spirit and dysfunction of heart caused by insufficiency of heart yin also contribute to PH. Another factor is the block of damp-heart. The treatment for PH caused by exuberant heat in Yangming is Baihu Decoction or Chengqi Decoctions. PH caused by deficiency cold of spleen and stomach can be cured by Lizhong Decoction. The treatment for PH caused by food stagnation is Baohe Wan. Insufficiency of heart yin and disharmony between Ying and Wei need to nourish blood for tranquillization and harmonize Ying and Wei. The block of damp or damp-heat leading to PH need drug resolving dampness with aromatics or clearing heat and promoting diuresis for cure. [Methods]The pathological basis, clinical manifestation and therapeutic principle and methods are systematically summarized.[Result]PH is relevant to the cold, heat, deficiency and excess of taste, disorder of Qi and blood of heart, and the block of damp and hot. Exuberant heat in Yangming, deficiency cold of spleen and stomach, and food stagnation can lead to PH. Malnutrition of heart spirit and dysfunction of heart caused by insufficiency of heart yin also contribute to PH. Another factor is the block of damp-heart. The treatment for PH caused by exuberant heat in Yangming is Baihu Decoction or Chengqi Decoctions. PH caused by deficiency cold of spleen and stomach can be cured by Lizhong Decoction. The treatment for PH caused by food stagnation is Baohe Wan. Insufficiency of heart yin and disharmony between Ying and Wei need to nourish blood for tranquillization and harmonize Ying and Wei. The block of damp or damp-heat leading to PH need drug resolving dampness with aromatics or clearing heat and promoting diuresis for cure. [Conclusion]PH has a complex pathogenesis, which needs treatment based on syndrome differentiation.

[bjective]This paper is about to expound the true meaning of the 374th clause of Treatise on Febrile Diseases. [Methods] Under the guidance of dynamic thinking of Treatise on Febrile Diseases, with the help of other clauses about Zaoshi syndrome, Small Decoction for Ordering the Qi, Gardenia and Prepared Soybean Decoction, to understand the 374th clause.[Results] The 374th clause is not alone, it should be combined with the 375th clause, they tell us when we treat the combination of Shi and Re syndrome of Yangming disease, we should use Small Decoction for Ordering the Qi first, then use Gardenia and Prepared Soybean Decoction. At the same time if we want to understand the 374th clause, we should combine it with the 368th clause to 375th clause, it tells us the treatment and difference between the diarrhea of cold, wet heat, Shi and Re, deficient heat etc. [Conclusion]This clause institutes the principle of the treatment for Shi and Re syndrome of Yangming disease, completing the system of treating diarrhea. It also tells us how to combine two decoctions.
RESUMEN
Objective To observe the effect of thumb pressing at Yangming Meridian on the function rehabilitation after total knee arthroplasty (TKA) for knee osteoarthritis (KOA).Method Sixty KOA patients after TKA were randomized into a treatment group (thumb pressing at Yangming Meridian plus conventional rehabilitation) and a control group (single conventional rehabilitation),30 cases each.The treatment group was intervened by thumb pressing and kneading manipulation at the acupoints of Foot Yangming Meridian and Foot Taiyin Meridian plus rehabilitation training of passive flexion and extension by CPM machine,while the control group was intervened by rehabilitation training of passive flexion and extension by CPM machine.The American Hospital for Special Surgery (HSS) for knee joint,the American Knee Society score (KSS) and the Western Ontario and the McMaster Universities Osteoarthritis Index (WOMAC) of KOA were used to evaluate the function.Result After the treatment,the HSS,KSS and WOMAC scores were changed significantly in both groups (P<0.05).The improvements of HSS,KSS and WOMAC scores in the treatment group were superior to those in the control group,and the differences were statistically significant (P<0.05).Conclusion The efficacy of thumb pressing at Yangming Meridian plus conventional rehabilitation for post-TKA KOA is superior to single conventional rehabilitation.
