RESUMEN

EIB (Exercise-Induced Bronchoconstriction) describes the narrowing that accurs in the airway follow a short period of exercise. EIB is found in 8-10% of normal children population as occult bronchospasm during or after physical activities. The mecanisms of EIB are related to rapid ventilation and mouth brathing which cause beat and water loss during breathing leading to bronchoconstriction. Peak Expiratory Flow Rate (PEFR) measured pre and post-exercise in students aged 12-16 years in girl intrmediate school. Any female shows PEFR values reduction 15% after 6 minutes continuous free running considered as asthmatic patient, this give an incidence rate of asthmatic patient of 9% in female students in this age. Treatment of EIB, Zafirlukast treatment gives (85.7%) protection rate. While salbutamol inhalation gives a protection rate 88%. Only 66.6% of girls with EIB give an improvement in PEFR values after sodium cromoglycate treatment. A regular measurement of PEFR in school students appears to be a good indicator of EIB, while inhalation of salbutaol 15 minutes before exercise give a good protection against EIB attacks at least for 4 hours (AU)

RESUMEN

Objective To investigate the influence of Zafirlukast on the inflammatory factors and immune function of patients with seasonal allergic asthma.Methods 182 patients with seasonal allergic asthma from the department of respiratory in our hospital were selected and divided into two groups, 91 cases in each group.The control group treated with budesonide and formoterol fumarate powder for inhalation, the experiment group received Zafirlukast on the basis of the control group,serum levels of inflammatory cytokines,peripheral blood T lymphocyte subsets,serum immunoglobulin levels, lung function,and the clinical effect were compared after the treatment.Results Compared with the control group, serum levels of TNF-,IL-4,IL-6 were lower, level of IL-10 was higher in the experiment group after treatment, CD8 +T lymphocyte levels was lower, CD3 +, CD4 +T lymphocyte and CD4 +/CD8 +ratio levels were higher after treatment, serum levels of IgE, IgG, IgA were lower after treatment, levels of PEF,FEV1 ,FEV1/FVC were higher after treatment, the difference was statistically significant (P<0.05).The effective rate in the experiment group 94.51% was higher than the control group 81.32% (P<0.05).Conclusion Zafirlukast can significantly reduce the levels of serum inflammatory factors in patients with seasonal allergic asthma, improve the cellular immune function, reduce the level of immunoglobulin, effectively improve lung function, shorten the length of hospital stay, has better clinical curative effect.

BACKGROUND: Capsular contracture is the most troublesome complication in breast implant surgery. Although capsule formation can be seen as a normal reaction to a foreign body, it can induce pain, hardness, deformity, and other pathologic problems. Surgical intervention is required in severe cases, but even surgery cannot guarantee a successful outcome without recurrence. This experimental study confirms that single topical administration of leukotriene antagonist zafirlukast (Accolate, Astrazeneca) reduces peri-implant capsule formation and prevents capsular contracture. METHODS: Twelve smooth-surfaced cohesive gel implants were implanted in New Zealand White rabbits. These miniature implants were designed to be identical to currently used products for breast augmentation. The rabbits were divided into 2 groups. In the experimental group (n=6), the implant and normal saline with zafirlukast were inserted in the submuscular pocket. In the control group (n=6), the implant and normal saline alone were used. Two months later, the implants with peri-implant capsule were excised. We evaluated capsule thickness and collagen pattern and performed immunohistochemical staining of myofibroblasts, transforming growth factor (TGF)-beta1, 2. RESULTS: The thickness of the capsules in the experimental group was reduced in both dorsal and ventral directions. The collagen pattern showed parallel alignment with low density, and the number of myofibroblasts as well as the amounts of TGF-beta1 and TGF-beta2 were reduced in the experimental group. CONCLUSIONS: We suggest that single topical administration of leukotriene antagonist zafirlukast can be helpful in reducing capsule formation and preventing capsular contracture via myofibroblast suppression, modulation of fibroblastic cytokines, and anti-inflammatory effect.

RESUMEN

Objective To investigate the effects of Zafirlukast on emotions and Th1/Th2 cytokines in asthmatic children.Methods One hundred and eight asthmatic children were randomly allocated to 3 groups.The children in group A inhaled Pulmicort aerosol 200 ?g twice every day.On the basis of inhaled Pulmicort aerosol,the children in group B took orally Zafirlukast orally 10 mg once every day.The children in group C only took orally Zafirlukast 10 mg once every day.The course of treatment of all children were 4 weeks.Anxiety,depression status and the levels of IFN-? and IL-4 in blood serum were respectively detected before and after treatment.Results Scores of anxiety and depression after treatment in 3 groups were significantly lower than those before treatment(Pa0.05).Conclusions Zafirlukast or Glucocorticoid inhalation has the same effect on improving emotional status and accommodating disequilibrium of Th1/Th2 for asthmatic children,but effect of Zafirlukast combined with Glucocorticoid inhalation are better.

RESUMEN

Churg-Strauss syndrome (CSS) is an eosinophil associated disease, characterized by vaculitis and granulomatosis on small vessel, asthma and eosinophil tissue infiltration. Several cases of CSS have been reported in patients treated with cysteinyl leukotriene receptor antagonist and weaned-off systemic or inhaled corticosteroids. A 39-year-old man with a history of childhood allergic rhinitis was diagnosed with bronchial asthma and his symptoms were well controlled with fluticasone propinate, salmeterol and zafirlukast. However four months later, he was admitted again with prominent skin lesions, tingling sensation on both extremities and fever. We diagnosed CSS with a history of sinusitis and bronchial asthma, marked peripheral eosinophilia, vasculitis and neuropathy. His symptoms and laboratory findings promptly improved after ten days of intravenous corticosteroid and cessation of zafirlukast without recurrence for six months during the follow-up period. We conclude that zafirlukast may cause CSS and attention should be taken when using zafirlukast.
