RESUMEN

Laser corneal refractive surgery is an effective way for the correction of myopia, and its long-term stability is a common concern of surgeons and patients. However, refractive regression is still one of the most common postoperative long-term complications, which has a great impact on postoperative refractive state and visual quality. Refractive regression may be related to many factors, such as the remodeling of corneal epithelium and stroma, and the imbalance between corneal biomechanics and intraocular pressure. Although enhancement surgery could be a useful way to correct refractive regression, there is a risk of corneal ectasia and other complications. Some intraocular pressure lowering medications can be used to rebuild the balance of intraocular pressure and corneal biomechanics, and to prevent postoperative refractive regression by affecting the thickness of corneal epithelium. To a certain extent, the type, the timing and the way of intraocular pressure lowering medications application may have an impact on the prevention and treatment effect of refractive regression.

RESUMEN

The human eye has various axes and angles, of which the angle Kappa is an important indicator of the centrality of the human eye and is widely used in ophthalmic surgery. Proper preoperative evaluation and application of the angle Kappa facilitated the achievement of optimal postoperative visual quality. Chord mu is a new term that has emerged recently to better express the angle Kappa. The two concepts are not well understood clinically, limiting their usefulness. Therefore, to better understand the angle Kappa and chord mu, the definitions and connections between them are presented separately in this paper. Meanwhile, the application of angle Kappa in strabismus surgery was summarized, the method for compensating large angle Kappa in corneal refractive surgery and the clinical significance of angle Kappa in predicting postoperative centrality of multifocal intraocular lens(MIOL)in phacoemulsification combined with MIOL implantation were discussed, with a view to providing references for clinical work.

AIM: To observe the therapeutic effect of the diquafosol sodium combined with intense pulsed light(IPL)on meibomian gland dysfunction(MGD)dry eye after refractive surgery.METHODS:A total of 64 patients(128 eyes)with MGD dry eye diagnosed within 6mo after laser corneal refractive surgery in our hospital from March 2021 to December 2021 were selected. They were randomly divided into control group and experimental group. A total of 33 patients(66 eyes)in the control group were treated with sodium hyaluronate combined with IPL, and 31 patients(62 eyes)in the experimental group were treated with diquafosol sodium combined with IPL. Ocular symptom scores were performed before each IPL treatment in both groups to examine non-invasive tear breakup time(NIBUT), tear meniscus height, lipid layer grade of tear film, meibomian gland deletion rate and uncorrected visual acuity.RESULTS:After IPL treatment, ocular symptom scores and meibomian gland deletion rate score of two groups were decreased continuously. NIBUT, tear meniscus height and lipid layer grade of tear film were increased continuously, and there was no significant change in uncorrected visual acuity. NIBUT of patients in the experimental group was better than that in the control group before the third IPL treatment(6.24±0.27s vs. 5.51±0.24s, P=0.046).CONCLUSIONS:Both diquafosol sodium and sodium hyaluronate combined with IPL showed good therapeutic effect on MGD dry eye, but there was no significant difference in the short-term efficacy between the two groups.

Objetivo: Determinar los resultados refractivos en pacientes operados de catarata con cirugía refractiva corneal, según el cálculo del poder dióptrico de la lente intraocular con la fórmula Barrett True K. Métodos: Se realizó un estudio pre-experimental, del tipo antes y después, en el cual fueron incluidos 18 pacientes (31 ojos). En ellos se analizaron variables demográficas y clínicas. La principal variable de salida fue la predictibilidad del componente esférico ± 0,50 D, ± 1,0 D según la longitud axial. Resultados: Fueron estudiados pacientes con un promedio de edad de 59,4 años, predominantemente del sexo femenino (66,7 por ciento). El 77,4 por ciento fue operado con queratotomía radial. Con la cirugía de catarata se produjo una mejora ostensible de la agudeza visual no corregida (mediana preoperatoria: 0,12 y mediana posoperatoria: 0,60). Solo el 9,7 por ciento de los ojos analizados presentó una agudeza visual sin corregir de 20/20 y el 90,3 por ciento de 20/40 o más. La cantidad de ojos con un equivalente esférico de ± 0,50 disminuyó en la medida en que aumentó la longitud axial (corta: 100 por ciento; normal: 57,1 por ciento; larga: 22,7 por ciento), no así la predictibilidad del componente esférico de ± 0,50, que aumentó (corta: 50,0 por ciento; normal: 57,1 por ciento; larga: 63,6 por ciento). Conclusiones: La fórmula Barrett True K resulta útil para el cálculo de la lente intraocular en pacientes operados de catarata y cirugía refractiva corneal previa(AU)


Objective: Determine refractive outcomes in patients undergoing cataract corneal refractive surgery based on intraocular lens dioptric power calculation with the Barrett True-K formula. Methods: A pre-experimental before/after study was conducted of 18 patients (31 eyes). Demographic and clinical variables were analyzed. The main output variable was spherical component predictability ± 0.50 D, ± 1.0 D according to axial length. Results: Mean age was 59.4 years; female sex prevailed (66.7 percent). Of the patients studied, 77.4 percent underwent radial keratotomy. Cataract surgery led to notable uncorrected visual acuity improvement (preoperative mean: 0.12; postoperative mean: 0.60). Only 9.7 percent of the eyes examined had an uncorrected visual acuity of 20/20, whereas 90.3 percent had 20/40 or more. The number of eyes with a spherical equivalent of ± 0.50 fell as axial length rose (near: 100 percent; normal: 57.1 percent; far: 22.7 percent), unlike ± 0.50 spherical component predictability, which rose from near: 50.0 percent; normal: 57.1 percent; far: 63.6 percent. Conclusions: The Barrett True-K formula is useful for intraocular lens calculation in patients undergoing previous cataract and corneal refractive surgery(AU)

@#AIM: To investigate the vision-related quality of life after corneal refraction surgery of the new recruits in a navy unit. <p>METHODS:New recruits in a navy unit during 2014-2016 were interviewed druing recheck ophthalmic examination through a questionnaire, including basic data and the Visual Function Questionnaire-25(VFQ), and also checking the visual acuity, contrast sensitivity(CS), optometry.<p>RESULTS: Among 2 760 new recruits, 340 persons(12.32%)had undergone corneal refractive surgeries, which included corneal stroma surgery(89.71%)and corneal surface surgery(10.29%). According to the preoperative myopia spherical equivalent, the subjects were divided into three groups: mild myopia group(≤-3.00D, 134 cases), moderate myopia group(>-3.00 D to < -6.00D, 159 cases), and high myopia group(≥-6.00D, 47 cases). The postoperation spherical equivalent range ±1.0D is 128 cases(95.52%), 153 cases(96.23%), 38 cases(80.85%)in mild myopia group, moderate myopia group and high myopia group respectively. The contrast sensitivity(CS)were statistically significant among three different diopter groups under natural light and glare conditions(<i>P</i><0.05), the CS of the low myopia group and moderate myopia group were higher than in high myopia group(<i>P</i><0.05). The visual-related quality of life scores were statistically significant among three group(<i>P</i><0.05). The scores of low(6.25±8.61)points and moderate myopia groups(6.87±8.51)points were significantly better than that of high myopia group(9.84±9.76)points(<i>P</i><0.05).<p>CONCLUSION:The vision-related quality of life in new recruits after corneal refractive surgery in the unit is related with preoperative diopter of refractive error. The vision-related quality of life in recruit with lower diopter are better than in recruit with higher dioper.

@#AIM: To investigate the vision-related quality of life after corneal refraction surgery of the new recruits in a navy unit. <p>METHODS:New recruits in a navy unit during 2014-2016 were interviewed druing recheck ophthalmic examination through a questionnaire, including basic data and the Visual Function Questionnaire-25(VFQ), and also checking the visual acuity, contrast sensitivity(CS), optometry.<p>RESULTS: Among 2 760 new recruits, 340 persons(12.32%)had undergone corneal refractive surgeries, which included corneal stroma surgery(89.71%)and corneal surface surgery(10.29%). According to the preoperative myopia spherical equivalent, the subjects were divided into three groups: mild myopia group(≤-3.00D, 134 cases), moderate myopia group(>-3.00 D to < -6.00D, 159 cases), and high myopia group(≥-6.00D, 47 cases). The postoperation spherical equivalent range ±1.0D is 128 cases(95.52%), 153 cases(96.23%), 38 cases(80.85%)in mild myopia group, moderate myopia group and high myopia group respectively. The contrast sensitivity(CS)were statistically significant among three different diopter groups under natural light and glare conditions(<i>P</i><0.05), the CS of the low myopia group and moderate myopia group were higher than in high myopia group(<i>P</i><0.05). The visual-related quality of life scores were statistically significant among three group(<i>P</i><0.05). The scores of low(6.25±8.61)points and moderate myopia groups(6.87±8.51)points were significantly better than that of high myopia group(9.84±9.76)points(<i>P</i><0.05).<p>CONCLUSION:The vision-related quality of life in new recruits after corneal refractive surgery in the unit is related with preoperative diopter of refractive error. The vision-related quality of life in recruit with lower diopter are better than in recruit with higher dioper.

@#AIM: To evaluate the changes of early visual quality of patients with high myopia after small incision lenticule extraction(SMILE)and femtosecond assisted laser in situ keratomileusis(FS-LASIK)by the double-pass optical quality analysis system Ⅱ(OQAS Ⅱ)and Pentacam corneal topography.<p>METHODS: A prospective non-randomized controlled study was conducted among 148 eyes of 74 patients with high myopia. These patients were treated by the same surgeon in our hospital from March 2020 to September 2020. According to their wishes, 86 eyes with 43 patients were treated with SMILE and 62 eyes with 31 patients were treated with FS-LASIK. The uncorrected visual acuity(UCVA), modulation transfer function cut off frequency(MTF cut off), Strehl ratio(SR), objective scattering index(OSI), predicted visual acuity values(VA 100%, VA 20%, VA 9%), high-order aberration(HOA), horizontal aberration(<i>Z 13</i>), vertical coma(<i>Z -13</i>)and spherical aberration(<i>Z 04</i>)preoperatively, 1 and 7d postoperatively were collected and analyzed.<p>RESULTS:There was no statistical significance in preoperative age, spherical equivalent(SE), UCVA, MTF cut off, SR, OSI, VA 100%, VA 20%, VA 9%, HOA, <i>Z 13</i>, <i>Z -13</i> and <i>Z 04</i>(<i>P</i>>0.05). The OSI of the SMILE group was higher than the FS-LASIK group at postoperative 1d(2.3±2.1 <i>vs</i> 1.8±1.1). The difference in OSI between the two groups was not statistically significant at postoperative 7d(1.2±0.7 <i>vs</i> 1.3±0.7). The HOA and <i>Z04</i> in the FS-LASIK group were higher than the SMILE group at postoperative 1d and 7d(<i>P</i><0.001). The UCVA, <i>Z 13</i>, <i>Z -13</i>, MTF cut off, SR, VA 100%, VA 20%, and VA 9% between the two groups were no statistical significance at postoperative 1d and 7d(<i>P</i>>0.05).<p>CONCLUSION:FS-LASIK is easier to introduce corneal high-order aberration and spherical aberration in the early postoperative period, while SMILE with 2mm incision only introduces higher scatter at postoperative 1d. Therefore, SMILE can obtain better visual quality than FS-LASIK in the early postoperative period among patients with high myopia after corneal refractive surgery.

The main purpose of corneal refractive surgery is not only to improve vision, but also to improve visual quality, which is a higher level of visual ability except vision.At present, most corneal refractive surgeries take the pupil center as the center of examination and treatment, since changes in body position may result in pupil centroid shifts and eye cyclotorsion and postoperative visual quality can be affected by undercorrected astigmatism, induced astigmatism and increased high order aberration that results from eccentric cutting occurring during corneal refractive surgery.The measurement of centroid shifts and eyeball rotation that can be attributed to the change of body position guiding the design of surgery is of great significance to enhance the accuracy of pupil center positioning, improve the safety of surgery and promote the postoperative visual quality.In this article, the relevant literature on the measurement methods of the pupil centroid shifts and ocular cyclotorsion, pupil centroid shifts and ocular cyclotorsion measured by iris recognition technology in response to changes in body position, and the effects of posture-related pupil centroid shifts and ocular cyclotorsion during refractive surgery on postoperative visual quality (postoperative astigmatism, aberration, and modulation transfer function) were reviewed.

@#Given the popularity of corneal refractive surgery and an aging population of patients who have undergone this procedure, those patients will likely require the diagnosis, follow-up, and management of glaucoma in the future. Since glaucoma examination metrics such as intraocular pressure depend largely on corneal properties, patients who plan to undergo corneal refractive surgery must also undergo a thorough glaucoma examination in order for the ophthalmologist to monitor the development and progress of glaucoma. This article presents an overview of related research on the diagnosis of glaucoma before and after corneal refractive surgery in order to provide reference for clinical work.

@#Amblyopia is defined as a decrease in visual acuity that results from abnormal binocular interaction or visual deprivation during critical period of the visual cortex, which cause a variety of visual functions damage. With the development of science and technology, amblyopia treatment has new concept. The new method of amblyopia treatment is occlusion therapy based on corrected refraction, followed by visual function training and pharamacological therapy, instead of traditional treatment which contians only refraction correction and patching therapy. Correction of refractive error is no longer limited to use spectacles, also provided with contact lenses, corneal refractive surgery and intraocular refractive surgery. In addition to using patching, occlusion therapy can use Bangerter filters, contact lenses and LED liquid crystal glasses. The training of visual function aimed to improve three-level visual function and restore visual cortical plasticity, which is especially important in the adult amblyopia. Levodopa and citicoline drugs can be added to occlusion therapy in order to reach a better outcome. We should design the fittest treatment plan according to the individual situation of the patients within varied of methods, adopting diversified combination, personalized therapy.
