RESUMEN

Sertoli cell tumors (SCTs) are rare tumors accounting for <1% of all testicular tumors. Here, we report a rare case of SCT in a 60‑year‑old man presenting as a painless swelling in the right groin since childhood. Clinically, he presented with right‑sided inguinal hernia with absence of the right testis. He had normal left testis and had no gynecomastia or infertility. The specimen of hernial sac showed testis with a 1.6 cm × 1.5 cm nodular mass having gray tan‑cut surface. Histopathologically, the testis showed atrophy and the nodular portion showed tumor cells arranged in tubular and microcystic pattern, with no solid pattern or necrosis. The diagnosis of SCT was confirmed with immunohistochemical staining for inhibin which showed fine granular cytoplasmic positivity. Cryptorchid testis having SCT and presenting as a content of inguinal hernia is a rare occurrence.

RESUMEN

Malignant Sertoli cell tumor was diagnosed in a 5-year-old male Shih Tzu dog. Clinical features of the dog were anorexia, urinary incontinence, constipation, anemia, alopecia, and epistaxis. The dog also had unilateral cryptorchid testis in the abdomen. Several abdominal and thoracic masses were identified on radiography. Grossly, the cryptorchid testicular mass was markedly enlarged to 8 cm in diameter. On cut surface, firm and well demarcated milk-white neoplastic areas were irregularly separated by white fibrous bands. Histologically, the testicular mass was diagnosed as tubular pattern Sertoli cell tumor. In addition, abdominal and mediastinal lymph nodes metastasis were found. Immunohistochemically, the tumor cells were strongly positive for vimentin and neuron specific enolase, but negative for S-100 and cytokeratin.

RESUMEN

Adenomatoid tumor is the common neoplasm of the paratesticular tissue that generally is considered to be of mesothelial derivation. But, there are only few reports on adenomatoid tumor of the epididymis associated with cryptorchid testis in the world. We report an unusual tumor that occurred in epididymis associated with intraperitoneal cryptorchid testis of a 32-year-old man andwas confirmed as an adenomatoid tumor by light microscopy.

RESUMEN

A 32 year-old male was admitted to our hospital complaining of nausea, vomiting, fever and abdominal pain. Physical examination revealed lower abdominal mass associated with severe peritoneal irritation sign. The right testis was not palpated. Though the tumor might be the abdominal cryptorchid testis tumor. the other surgical abdominal conditions of non-urologic origin couldn't be rouled out. So, we performed emergent exploration. The mass had been ruptured and adhered to sigmoid colon. Histological examination revealed mixed germ cell tumor, composed of yolk sac tumor and classic seminoma. We performed systemic chemotherapy with bleomycin. etoposide and cisplatin under diagnosis of mixed germ cell tumor of stage III B-5.

RESUMEN

Seminoma is the most common tumor to occur in abdominal ectopic testes. Intraabdominal seminomas usually presents with secondary signs and symptoms due to the anatomical position of the tumor. We report a case of seminoma developed in abdominal cryptorchid testis associated with torsion. The patient was a 38 year-old male who had complained of left lower abdominal pain radiating to left scrotum. Abdominal ultrasonograph showed a well defined round mass with mixed internal echo in the left lower abdomen. Exploraton revealed that the mass was about 7.5 x 5.5 x 3.5 cm in size and twisted in 180' clockwise and herniated intestine was also seen. We carried out radical orchiectomy. The histopathologic diagnosis was pure seminoma confined to the testis. Postoperative irradiation was performed under the diagnosis of seminoma of stage I.

RESUMEN

Because of its usually abnormal mesorchial attachments the undescended testis is more prone to undergo torsion than normally-sited organ, but the incidence of this complication appears to have lessened in recent years. We present two cases with torsion of the unilateral cryptorchid testis and a review of the literature. Two cryptorchids complained of the painful mass on the inguinal area. Exploration revealed testicular torsion and orchiectomy was done.

RESUMEN

The incidence of impalpable testis is approximately 20 percent of undescended testis population and some may possess vascular pedicle shortening that prohibits scrotal placement in one stage orchiopexy. From July 1987 through April 1988, 12 patients with 17 impalpable testicles were explored by lower midline incision and the following results were obtained : 1. We could identify and prove the location of testis and/or anorchia without postoperative complication : 10 intraabdominal testes (59%), 3 intracanalicular (18%), 4 anorchia (23%). 2. Among 10 intraabdominal testes 5 testes were brought down by transabdominal orchiopexy, 3 testes by Fowler-Stephens method, and 1 testis by staged orchiopexy and remaining testis were removed. 3. Among 3 intracanalicular testes 2 testes were brought down by transabdominal orchiopexy and one testis was removed. 4. Follow up testicular examination of fixed testes revealed no testicular atrophy in all transabdominal orchiopexed testes and no testicular atrophy in 1 of 3 testes which were performed by Fowler-Stephens method.

RESUMEN

From July, 1977 to May, 1988, 148 patients of cryptorchidism had been admitted to our Department of Urology, Chonbuk National University hospital. A clinical evaluation was made on these 148 patients and a histopathological observation was combined to the 47 patients who had an operation for cryptorchidism during the period of May. 1986 to May, 1988. The results were as follows ; 1. cryptorchidism had the highest incidence rate among the anomalies of the genitourinary tracts. (32.8 %). 2. On the age distribution, 6 to 10 years old group was the most frequent (35.1%) and 11 patients (7.4%) were over 20 years old. 3. Inguinal area was the most frequent location (64%) and 28% of the cases were bilateral and the right side was more prevalent (67%). 4. Among the associated anomalies, inguinal hernia was the most common (36%) and 5 patients(3.4%) were associated with hypospadias. S. On histopathological findings of the undescended testis, the diameter of the seminiferous tubule was smaller compared to the normal descended testis and fibrosis of peritubular structure was initiated from earlier age group and the number of the spermatogonia was abruptly diminished after puberty.

RESUMEN

Cryptorchid testis is correlated with high incidence rate of testicular neoplasm. A 44 year-old male was admitted to our hospital due to huge abdominal mass and dyspnea. Physical examination revealed impalpable right testicle in the scrotum. Abdominal CT scan revealed a huge right retroperitoneal mass with central necrosis. AFP and BHCG were increased. 4 courses induction PVB therapy was performed under diagnosis of unresectable bulky stage IIB nonseminomatous germ cell tumor. Thereafter the tumor size and markers were decreased. But the patient was lost for follow-up. One year later, the tumor size and markers were increased Retroperitoneal excision of intraabdominal mass was performed followed by adjuvant PVB therapy due to persistent increased markers.

RESUMEN

A case of torsion of the right cryptorchid testis was herein reported. The patient was 6-year-old boy who was rolled on a staircase and traumatized his right inguinal area. There after painful and tender swelling developed on the area, and it spent 4 days prior to visit our department The involved cryptorchid testis showed dark reddish discoloration and external rotation of 360 degrees and was removed because of no re-establishment of blood supply in spite of detorsion for 30 minutes.

RESUMEN

A case of testicular embryonal carcinoma developed in s cryptorchid testis is presented with a review of the literature. According to Campbell's series, the incidence of malignant cryptorchid testis was 11.6%. and his statistical evaluation was similar to Gilbert and Hamilton's series of 7,000 cases of testicular neoplasms where 840 tumors were developed in undescended testes(12%). Both authors agree that the chance of neoplastic development in a cryptorchid testis is approximately 48 times greater then that developing in a normally descended testis. Several cases of testicular tumor were reported sporadically in Korea, but malignant tumors in cryptorchism were rarely seen in the literatures Only two cases of malignant cryptorchid testis were already reported in Korea.
