Abstract Objective To evaluate the expulsion and continuation rates of the copper intrauterine device (IUD) inserted in the immediate postpartum period in a Brazilian public university hospital. Materials and Methods In the present cohort study, we included women who received immediate postpartum IUD at vaginal delivery or cesarean s March 2018 to December 2019. Clinical data and the findings of transvaginal ultrasound (US) scans performed 6-weeks postpartum were collected. The expulsion and continuation rates were assessed 6-months postpartum using data from the electronic medical records or by telephone contact. The primary outcome was the proportion of IUDs expelled at 6 months. For the statistical analysis, we used the Student t-test, the Poisson distribution, and the Chi-squared test. Results There were 3,728 births in the period, and 352 IUD insertions were performed, totaling a rate of 9.4%. At 6 weeks postpartum, the IUD was properly positioned in 65.1% of the cases, in 10.8% there was partial expulsion, and in 8.5% it had been completely expelled. At 6 months postpartum, information was obtained from 234 women, 74.4% of whom used IUD, with an overall expulsion rate of 25.6%. The expulsion rate was higher after vaginal delivery when compared with cesarean section (68.4% versus 31.6% respectively; p = 0.031). There were no differences in terms of age, parity, gestational age, final body mass index, and newborn weight. Conclusion Despite the low insertion rate of copper IUDs in the postpartum period and a higher expulsion rate, the rate of long-term continuation of intrauterine contraception was high, indicating that it is a useful intervention to prevent unwanted pregnancies and to reduce short-interval birth.
Resumo Objetivo Avaliar as taxas de expulsão e continuação do dispositivo intrauterino (DIU) de cobre inserido no pós-parto imediato em um hospital universitário brasileiro. Materiais e Métodos Neste estudo de corte transversal, foram incluídas parturientes submetidas à inserção de DIU de cobre no pós-parto imediato entre março de 2018 e dezembro de 2019. Foram coletados dados clínicos e da ultrassonografia (US) transvaginal realizada após seis semanas. As taxas de expulsão e de continuação foram avaliadas após seis meses por meio de dados do prontuário ou por contato telefônico. O resultado principal foi a proporção de DIUs expelidos em seis meses. Para análise estatística, utilizaram-se o teste t de Student, a distribuição de Poisson, e o teste do Qui quadrado. Resultados Houve 3,728 nascimentos no período, e foram inseridos 352 DIUs, em uma taxa de 9,4%. Com 6 semanas, o DIU estava bem posicionado em 65,1% dos casos, em 10,8%, houve expulsão parcial, e, em 8,5%, fora totalmente expelido. Aos 6 meses de pós-parto, foram obtidas informações de 234 mulheres, 74,4% das quais usavam DIU, com uma taxa de expulsão geral de 25,6%. A taxa de expulsão foi maior após o parto vaginal do que após cesariana (68,4% versus 31,6%, respectivamente; p = 0,031). Não houve diferenças quanto à idade, paridade, idade gestacional, índice de massa corpórea final, e peso do recém-nascido. Conclusão Apesar da baixa taxa de inserção e alta taxa de expulsão, a taxa de continuação em longo prazo da contracepção intrauterina com DIU de cobre foi elevada, o que indica que se trata de intervenção útil para prevenir gestações indesejadas em curto intervalo de tempo.

El dispositivo intrauterino (DIU), ha sido utilizado en nuestro medio y en el mundo durante muchos años como método anticonceptivo. Una complicación infrecuente posterior a su inserción es la migración fuera del útero. La localización vesical y complicación con la vesicolitiasis, forman parte de las complicaciones asociadas a la migración. Presentamos un caso de migración de un DIU a la vejiga asociado a la formación de un cálculo entorno a ese dispositivo. Se describe el cuadro clínico, los estudios realizados y el tratamient
The intrauterine device (IUD) has been used in our environment and in the world for many years as a contraceptive method. One of the complications is the migration outside the uterus, being the bladder location unusual and much more the secondary bladder stone. We present a case of migration of an IUD to the bladder associated with the formation of a calculus around this foreign body. The clinical record, study and treatment are described.

A partir de que Richter R, en 1909, realizó la primera publicación sobre el uso de dispositivos intrauterinos con fines anticonceptivos, la forma, los materiales, las indicaciones, así como las contraindicaciones han evolucionado hasta convertirse en el método de planificación familiar más usado en el mundo. El riesgo de perforación durante la inserción es bajo; sin embargo, ha sido referida. En la literatura se ha descrito el uso de la laparoscopia para la extracción de dispositivos intrauterinos migrados a cavidad abdominal. En la presente publicación se reporta el caso clínico de una mujer de 29 años de edad, con antecedente de colocación de dispositivo intrauterino, dos años previos, por vía vaginal sin complicación aparente; comenta sólo dolor de cólico posterior a la colocación que va cediendo a analgésicos vía oral no especificados, sin sintomatología hasta la fecha. Acude a revisión y realización de citología cervical donde no se encuentran hilos guía del dispositivo intrauterino. Mediante radiografía simple de abdomen y bajo fluoroscopia se determina la localización intraabdominal del testigo radiopaco del dispositivo, es valorada por el Servicio de Cirugía General y se determina a realizar la extracción del mismo por vía laparoscópica sin complicaciones; se encontró el dispositivo con adherencias a omento mayor, sin afección aparente de órganos intraabdominales. La Organización Mundial de la Salud recomienda la extracción inmediata de los dispositivos intrauterinos que han migrado. La migración del dispositivo intrauterino a la cavidad abdominal es una complicación poco frecuente, y la presentación como hallazgo en pacientes asintomáticas es aún menos frecuente; en una revisión sistemática de la literatura, Richdeep S y cols. valoraron 49 artículos en los que encontraron 179 casos de manejo laparoscópico, reportando al omento como el sitio más frecuente de localización (26.7%). La relevancia del presente caso radica en la presentación asintomática dos años posteriores a su inserción, la ruta diagnóstica utilizando radiografías simples y fluoroscopia, así como la extracción sin complicaciones y con excelente resultado, vía laparoscópica.
Since Richter R. in 1909 performed the first publication on the use of intrauterine devices; materials, forms, indications and contraindications of these have evolved to become the method most widely used family planning in the world. Although the risk of perforation during insertion is low, it exists. The literature has described the use of laparoscopy for removal of intrauterine devices migrated to the abdominal cavity. Case report: 29 years old woman who two years before suffers colic pain posterior to application of an intrauterine device with no complications, and diminishes with the use of unspecified analgesics, and stays asymptomatic to date. She attended a review, and underwent to cervical cytology, during the procedure, was impossible to see the strings of the intrauterine device. Plain abdominal radiography and fluoroscopy were done to determine the location of the radiopaque intraabdominal witness. Assessed by the General Surgery Service we determined extraction of the device by laparoscopy and was done without any complication. Findings: strong omentum adhesions to the device, but no apparent damage to other abdominal organs. The World Health Organization recommends the immediate removal of intrauterine devices that have migrated. The migration of the intrauterine devices into the abdominal cavity is a rare complication, and the presentation as an incidental finding in asymptomatic patients is even less common, a systematic review of literature, Richdeep S. et al, evaluated 49 items found 179 cases of laparoscopic management reporting the omentum as the most common site of localization (26.7%). The relevance of this case lies in the asyntomatic presentation two years after insertion, the diagnostic route using radiographs and fluoroscopy, and extraction without complications and with excellent results laparoscopically.
