RESUMEN

World Health Organization (WHO) reported that a total of 559 meningitis cases and 18 deaths were reported in the southeast of Niger from November 1, 2022 to January 27, 2023.Children were the major of patients.Among the laboratory confirmed cases, 93.7%(104/111) were epidemic cerebrospinal meningitis.Since 2021, the epidemic of meningococcal disease has attacked the gay and bisexual men in Florida, USA.This paper briefly reviews the occurrence and development of the two outbreaks of epidemic cerebrospinal meningitis, as well as the causes and countermeasures.The current prevention and control measures of meningitis is discussed based on the " Defeating meningitis by 2030: Global Road Map" issued by WHO in 2021.This paper is expected to attract the attention and to improve awareness and vigilance of domestic clinical and public health workers, which would lead to strengthen the surveillance, prevention and control of meningitis.

RESUMEN

Epidemic cerebrospinal meningitis shows a high degree of consistency with the law of transmission among wei (卫)-qi-ying (营)-blood, in terms of the onset of the season, contagiousness, symptoms, pathogenesis, as well as characteristics of the transmission. It is proposed to use epidemic cerebrospinal meningitis as an example to explore the underlying disease of wei-qi-ying-blood syndrome differentiation system. Epidemic meningitis invades the brain from the upper respiratory tract along the nervous system, and its overall pathogenesis follows from entering the lung system (prodromal period) to entering the blood (bacteremia period, sepsis period) and then entering the brain (shock period). According to the four-dimensional qualitative principle of epidemic pathogen tropism, it corresponds to disease of both wei and qi syndrome, then blazing of both qi and ying syndrome, and then heat blocking pericardium, exuberant heat stirring wind, and internal block and external collapse syndrome. This article explored the laws of transmission among wei-qi-ying-blood and its underlying diseases described in On Warm Heat (《温热论》), and revealed the original appearance of the disease model under the laws of transmission among wei-qi-ying-blood to guide the clinical practice.

RESUMEN

Objective To improve the diagnosis and treatment of epidemic cerebrospinal meningitis in children.Methods A retrospective review was performed and a total of 69 cases of children epidemic cerebrospinal meningitis were collected and classified into ordinary and fulminant epidemic cerebrospinal meningitis from 1999 to 2011.Disease prevalent season,age,clinical characteristics,laboratory examination,treatment,prognosis and risk factors were analyzed.Results The mean age(33 male and 36 female) was (6.85 ±5.12)years old,ranging from 56 days to 15 years old.A total of 60 cases began on January to May; 6 cases began on June to October; 3 cases began on November to December.All of the 42 cases of ordinary epidemic meningitis were recovered; 7 cases of fulminant epidemic meningitis(n =27) were dead.All of the patients had fever; 64 cases(92.75%) had Petechiae petechial skin; 36 cases(52.17%) had headache; 49 cases(71.01%) vomited; 65 cases(94.2%) had positive neck resistance; 15 cases(21.74%) had convulsion; 35 cases(50.72%) had drowsiness or coma; 32 cases (46.38%) had positive pathological signs and 24 cases (34.78%) had low blood pressure or shock.Blood tests revealed that 63.77% of the patients leukocytes raised and 8.70% of the patients reduced; 42.03% of the patients platelet reduced; 55.07% of the patients had electrolyte disturbance and 89.86% of the patients had coagulation disorders,among which 46.38% fibrinogen reduced.The rash printed piece and cerebrospinal fluid smear tests found that Gram-negative bacteria positive rate were 12.5% and 19.35% respectively; the positive rate in bacterial culture of cerebrospinal fluid and blood culture were 12.90% and 7.25% respectively.Drug sensitive test showed that penicillin and ceftriaxone were sensitive to bacteria.The differences of age,leukocyte and platelet counts,fibrinogen and altered mental status between ordinary and fluminant epidemic meningitis were significant (P < 0.05).Conclusion The prevalent season is from January to May and the prevalent age is less than 5 years for children with epidemic cerebrospinal meningitis.Ceftriaxone or penicillin should be the preferred choice;age,lower leukocyte and platelet counts,lower fibrinogen and altered mental status should be used to predict the severity of the disease.

Objective To analyze the epidemiological characteristics and disease control of epi-demic cerebrospinal meningitis (meningitis) in Xi′an in order to optimize the measurement for disease con-trol and prevention .Methods The reported cases , pathogen surveillance data and immunization coverage for meningitis were analyzed to evaluate the epidemic status , the number of healthy carriers and the disease control efficacy.Results The incidence of meningitis was 22.51/100 000 from 1951 to 1984 in Xi′an. Since the implementation of Expanded Immunity Program in 1984 , the incidence dropped significantly from the average incidence of 4.87/100 000 in 1980′s to 0.59/100 000 in 1990′s.The average incidence of men-ingitis was 0.095/100 000 from 2001 to 2011 and then showed a dramatic downtrend with no cases reported during 2012 to 2013 .Conclusion The analysis suggested that an expanded coverage of vaccine immuniza -tion and an enhanced disease surveillance system for meningitis should be used as the comprehensive meas -urements for the prevention and control of disease .

Objective To investigate the epidemiology and clinical features of epidemic cerebrospinal meningitis of children in Shanghai area in the last 2.5 years. Methods A retrospective study was conducted in 27 hospitalized patients with epidemic cerebrospinal meningitis by analyzing the clinical manifestation,epidemiology and laboratory test fingdings from Jan.2006 to Jun.2008.Statistical analysis of the clinical manifestations of cases in serogroup A,B and C was performed according to Neisseria meningitidis infection. Results There were sporadic cases of children with epidemic cerebrospinal meningitis in Shanghai area in the last 2.5 years.The seasonal distribution illustrated the peak occurrence from Feb.to Apr.Among all these cases,92.6% were floating children.Common types(74.1%) were the predominant clinical manifestations and the others were fulminate types(25.9%).Four cases in each of serogroup A and B,and 8 cases of serogroup C were identified by serotyping on 14 Neisseria meningitidis strains and genotyping on the other 2 cases′ cerebrospinal fluid specimens.All the patients in serogroup A with meningococcal diseases were not vaccinated before and 25.0% of them belonged to fulminate types.Half of the cases had petechia on the skin and the prevalence of skin necrosis was 25.0% and 50% had complications of nervous system.No fulminate type was present in cases of serogroup B meningococcal disease and only 1 case had slight petechia on the skin.However,intensive convulsions occurred in 75.0% of the patients,and all of them had complication of nervous system,with a few had sequela.Among the patients in serogroup C meningococcal disease,62.5% patients were vaccinated with serogroup A Neisseria meningitidis.In serogroup C,Neisseria meningitidis infection was found in each age group,with petechia on skin,skin necrosis,immunity correlation fever and complications of nervous system in 87.5%,50.0%,75.0% and 12.5% of the patients,respectively. Conclusions 1.Epidemic cerebrospinal meningitis occurred sporadically in children in Shanghai area,the majority of patients were floating populations.2.Clinical manifestation was typical in patients of serogroup A meningococcal disease.Subdural effusion or ventricalitis was more common in patients of serogroup B meningococcal disease.In serogroup C,Neisseria meningitidis was the most prevalent serotype leading to epidemic cerebrospinal meningitis.More cases were found in patients infected with serogroup C,Neisseria meningitidis with severe shock and immunity correlation fever compared with the other groups.Prophylactic vaccination of serogroup C polyvalent meningococcal organism should be recommended.

RESUMEN

Objective To study the effect of early application of heparinate in fulminant epidemic cerebrospinal meningitis in children.Methods Twenty eight patients of fulminant epidemic cerebrospinal meningitis were reviewed and summarized.Results After application heparinate, there were 14 out of 28 platelet increase,12 patients died of diffuse intravascular coagulation.Conclusion Early application heparination is safe and effective in fulminant epidemic cerebrospinal meningitis,and side effect such as hemorrhage is not found.
