RESUMEN

Background: It is essential for any health-care facility, regardless of its size, to implement a maintenance programme for medical equipment. The maintenance not only has a positive impact on the safety and effectiveness of healthcare technology, but also increases the lifetime of equipment and thus helps to save scarce investment resources. It also enhances the demand for health services. Demand for services availability is crucial of functioning healthcare technology.Methods: A comprehensive descriptive study of the maintenance of equipment at a tertiary care corporate hospital in India from January 2012 to May 2012. A convenient sampling method was used to capture data from Key Participants which included two categories of staff viz technicians and managers/administrators. There were 40 technicians and 10 managers who responded to the questionnaire. Both the primary and secondary sources of data were used.Results: Breakdown data analysis showed that the major cause of breakdown was human errors. About 40% of breakdown that occurred in the hospital was mainly because of manual errors followed by electrical and mechanical issues.  Average down time for the time period of 5 months came to around 19 hours, which was quite high. Analysis of KAP questionnaire showed that about 55% technicians were not having the educational qualification required for handling the medical equipment, about 15% were not given training at the time of purchase of the equipment. Similarly, few managers were not aware about maintenance practices and importance of maintenance management for smooth and efficient functioning.Conclusions: Good and effective maintenance practices can reduce the cost of maintenance of equipment that arises due to breakdown. The training of equipment users and maintenance managers reduces the equipment downtime. To reduce the possibility of equipment malfunction following service or repair, all personnel involved in maintaining and servicing equipment should be trained.

RESUMEN

Since the outbreak of Novel Coronavirus Pneumonia(NCP), hospitals have taken the fight against the virus as its own responsibility, and keep standing in the front line of epidemic prevention and control. The continuous input of anti-epidemic forces in hospitals also brings challenges to the medical supplies support, including the management of protective supplies and the maintenance of medical equipment. In the face of increasing security pressure, the medical materials support team broke the game on multiple fronts. Firstly, the team implements active material procurement strategy, sets material distribution priority according to risk level, releases materials uniformly based on stock and use, and implements traceability management of donated materials to ensure material supply. Secondly, centralized allocation management of equipment, emergency installation, advanced maintenance and emergency maintenance work is effectively completed. Thirdly, disinfection strategies for items and equipment are developed safely and effectively with the aid of disinfection equipment functions. At last, personnel management and training have been strengthened. These measures have provided strong support for the orderly prevention and control of the epidemic.

RESUMEN

Assurance of the safety,reliability and effectiveness of in-use medical equipments is key to improving medical risk management and applied quality of medical equipments of a hospital.By means of regular field survey of in-use medical equipments of the hospital,the authors identified key risk exposures in the current equipment quality control system,and took targeted corrective measurements.Data of routine equipment maintenance and that of appraisal were compared and analyzed.Based on such,appraisal points are set dynamically,and a standardized and customized appraisal program is developed for the safety and quality evaluation of medical equipments,featuring comprehensive coverage and operability.Such efforts effectively supplement daily maintenance work and provide data basis for reliability and usability analysis of medical equipments.

Objetivo: Identificar as implicações da não manutenção dos equipamentos hospitalares na qualidade do atendimento cirúrgico. Método: Trata-se de uma pesquisa quantitativa, exploratória, descritiva, observacional, realizada em um hospital filantrópico do interior de Minas Gerais. Aplicou-se a técnica de observação direta e a avaliação de registros de manutenção preventiva e corretiva dos equipamentos cirúrgicos. Resultados: Durante o período de observação, verificou-se que os equipamentos que mais apresentaram falhas durante a cirurgia foram: bisturi elétrico, intensificador e foco cirúrgico. Os dados de funcionalidade e manutenção dos equipamentos foram comparados com recomendações do fabricante e com a literatura científica. Conclusão: A não manutenção dos equipamentos cirúrgicos pode prolongar a recuperação pós-operatória, aumentar a morbidade e a mortalidade e levar a um impacto financeiro desnecessário para a instituição. Espera-se que os resultados deste estudo possam motivar a equipe multiprofissional à realização da manutenção preventiva dos equipamentos antes das cirurgias.


Objective: To identify the implications of non-maintenance of hospital equipment for the quality of surgical care. Method: This is a quantitative, exploratory, descriptive, observational study carried out at a philanthropic hospital in the countryside of Minas Gerais. The technique of direct observation was applied, as well as the evaluation of records related to preventive and corrective maintenance of surgical equipment. Results: During the observation period, the equipment presenting most failures during surgical procedures were: electric scalpel, intensifier, and surgical focus. Equipment functionality and maintenance data were compared with manufacturers' recommendations and the scientific literature. Conclusion: Failure in surgical equipment maintenance can prolong patients' postoperative recovery, increase morbidity and mortality, and lead to unnecessary financial impact for the institution. It is hoped that the results of this study motivate the multiprofessional team to perform preventive maintenance of equipment before surgeries.


Objetivo: Identificar las implicaciones del no mantenimiento de los equipos hospitalarios en la calidad de la atención quirúrgica. Método: Se trata de un estudio observacional, descriptivo, exploratorio y cuantitativo realizado en un hospital filantrópico del interior de Minas Gerais. Se aplicó la técnica de observación directa y la evaluación de registros de mantenimiento preventivo y correctivo de los equipos quirúrgicos. Resultados: Durante el período de observación, se verificó que los equipos que presentaron el mayor número de fallas durante la cirugía fueron: bisturí eléctrico, intensificador y foco quirúrgico. La funcionalidad del equipo y los datos de mantenimiento se compararon con las recomendaciones del fabricante y la literatura científica. Conclusión: La falta de mantenimiento del equipo quirúrgico puede prolongar la recuperación postoperatoria, aumentar la morbilidad y la mortalidad y generar un impacto financiero innecesario para la institución. Se espera que los resultados de este estudio motiven al equipo multiprofesional a realizar el mantenimiento preventivo del equipo antes de las cirugías

Objective To propose a decision tree-based early warning model so as to predict the medical equipment quantity and quality under special conditions to quantize maintenance and detection staffs allocation,components supply,emergency planning and etc.Methods A data set was established based on the verification report,data on performance parameters detection and daily management log,and then underwent pretreatment.A decision tree came into being with the algorithms of ID3,CHAID and etc.The data rule corresponding to the decision tree was transformed into a series of early warning information.The model was verified by applying it to medical equipment usage management.Results The decision tree developed was applied to analyzing a vehicle-mounted water purifying device maintenance and usage records in some logistics support vehicle,and it's found the main factors contributing to the failures included migration distance along non-paved road,ambient temperature and service time.Conclusion The decision tree-based early warning model for medical equipment quantity and quality gains high feasibility and practical values.

Objective To explore the management methods of medical equipment maintenance. Methods Based on the staffing of hospital engineering technician, the difference between levels of hospitals in medical equipment maintenance management was explored from the aspects of maintenance, preventive maintenance, use training of medical equipment, medical equipment maintenance management system and etc. Results The management methods of medical equipment maintenance were different corresponding to the configurations of the departments of clinical engineering. Conclusion The maintenance management method corresponding to the staffing of hospital engineering technician as well as the correct use of modern medical equipment contribute to insuring the safety and reliability and enhancing the availability and utilization rate.

Objective To develop a field X-ray vehicle frame motion control simulation circuit based on Proteus.Methods Corresponding component models of Proteus were selected to construct the circuit according to the circuit block diagram.SCM control program was developed and debugged.The frame motion control simulation was executed through dynamic simulation of motor running,simulation results analysis with different parameters and verification of the circuit efficacy.Resnlts DC motor positive and negative rotation controlled by the button was realized,which solved the problems of X-ray vehicle frame motion.Conclusion Proteus can be used to implement system simulation of analog circuit,digital circuit,SCM and peripheral circuit as well as to construct virtual circuit lab.The equipment maintainer enhances medical equipment maintenance based on circuit simulation,circuit model analysis,failure simulation and failure position determination.

Objective: To observe and summarize the routine maintenance situation, and research relevant countermeasure and provide the optimal solution based on the problems in maintenance of medical equipment. Methods: To adopt SWOT method to analyze the problem of equipment maintenance, and make effective countermeasures by using relevant statistical methods. Results: The SWOT method that was applied in maintenance work has improved accuracy of maintenance work for equipment, and increased operation rate of equipment and serviceability rate, at the same time, the satisfied degree of medical staffs and patients has been increased. Conclusion: Through SWOT method to analyze the situation of maintenance for equipment, the work key points can be effectively improved, and the blindness and dead ends of equipment maintenance management can be found. Therefore, this method can reduce the labour intensity of maintenance staffs, increase work efficiency, and provide essential guarantee for medical work of hospital.

Objective:To analyze the demands of maintenance management system of medical equipment, and realize the scientization and normalization of plan design for maintenance management system of medical equipment.Methods: On the basis of current status of maintenance management system of medical equipment, the demands of various department during management process were analyzed, and then a systematic maintenance management plan was designed to face to all of medical equipment in hospital.Results: The preponderances of this system were discussed from three aspects: equipment maintenance, equipment archive and equipment benefit.Conclusion: The application of this system can accelerate the progress of maintenance for medical equipment, enhance maintenance awareness of clinical medical staffs for medical equipment, improve the transparency of equipment scrapping and disposal, and provide effective references for demonstration and purchase of medical equipment.

Objective To discuss the practicability of application of fault tree analysis in innovation to electrical system of SL-IE radiotherapy simulator.Methods The structure and principle of the electrical system were analyzed,the fault tree for the electrical system was built,and the qualitative analysis and quantitative analysis to the fault tree were made.The electrical system was innovated with effective redundant method,and the qualitative analysis and quantitative analysis were made after innovation.Theoretical analysis and the actual validation were made for the fault tree of electrical system after innovation.Finally,several notes were provided for the innovation to the similar medical equipments.Results The fault tree analysis method was applied to innovating the electrical system,and theoretical analysis indicated that the failure probability to total system reduced obviously.Actually,the actual effect after innovation also validated the correctness of the theoretical analysis.Conclusion Application of fault tree analysis in innovation to electrical system of SL-IE radiotherapy simulator is practical.But,the selection to the innovation event,the change to maintainence after innovation and the event to innovate carefully should be paid attention to.

RESUMEN

Objective To apply quality control circle (QCC) to reducing non-timely maintenance rate of medical equipment.Methods QCC was established,and some measures were taken to observe its effect in decreasing non-timely maintenance rate of medical equipment.Results The non-timely maintenance rate fell from 12.1% to 4.5%,and 0.3 million Yuan was saved for the manpower cost.Conclusion QCC decreases the non-timely maintenance rate of medical equipment,increases the efficiency of the maintainer,shortens the downtime of the fault medical equipment,enhances hospital economic benefit and patient satisfaction and provides references for other hospitals.

No cotidiano dos Centros de Material e Esterilização (CMEs), observa-se que as lavadoras termodesinfetadoras submetidas a intervenções técnicas para correção de falhas são liberadas para uso sem evidência de operação conforme os parâmetros requeridos para realizar limpeza e termodesinfecção eficientes. Considerando a importância da prevenção das infecções relacionadas à assistência à saúde, este estudo apresenta como inovação a sistematização dos ensaios requeridos para a liberação de termolavadoras após intervenções técnicas, assim como as informações necessárias para a conservação desses equipamentos em condições ótimas de operação. A liberação segura do equipamento deve incluir a avaliação dos parâmetros de temperatura e tempo em comparação aos dados obtidos na qualificação, a conferência do volume de detergente admitido durante a limpeza, a avaliação da eficácia da limpeza com monitores comercialmente disponíveis, o estabelecimento de um controle de mudanças e um protocolo para direcionar a requalificação, atendendo à legislação nacional e Às recomendações internacionais.


It is seen, in the daily routine of Material and Sterilization Centers (CMEs, acronym in Portuguese), that thermal washer disinfectors submitted to technical interventions for correcting failures are released for use without evidence of operation following the required parameters for the effective performance of cleaning and thermal disinfection. Given the importance of preventing healthcare-related infections, this study presents innovated systematization of assays required for the release of thermal washers after technical interventions, as well as the necessary information to preserve such equipment in optimal operation conditions. Safe release of the equipment should include the evaluation of temperature and time parameters compared to data obtained during qualification, the conference of admitted detergent volume during cleaning, the evaluation of cleaning effectiveness with commercially available monitors, the establishment of a change control, and a protocol for directing the requalification, following the Brazilian regulations and international recommendations.


En lo cotidiano de los Centros de Material y Esterilización (CMEs), se observa que las lavadoras termodesinfectantes sometidas a intervenciones técnicas para corrección de fallas son liberadas para uso sin evidencia de operación según los parámetros requeridos para realizar limpieza y termodesinfección eficientes. Considerando la importancia de la prevención de las infecciones relacionadas a la asistencia a la salud, este estudio presenta como innovación la sistematización de los ensayos requeridos para la liberación de termolavadoras tras intervenciones técnicas, así como las informaciones necesarias para la conservación de esos equipos en condiciones excelentes de operación. La liberación segura del equipo debe incluir la evaluación de los parámetros de temperatura y tiempo en comparación a los datos obtenidos en la calificación, la conferencia del volumen de detergente admitido durante la limpieza, la evaluación de la eficacia de la limpieza con monitores comercialmente disponibles, el establecimiento de un control de cambios y un protocolo para direccionar la recalificación, atendiendo la legislación nacional y las recomendaciones internacionales.

Objective:To explore the function of virtual reality in clinical laboratory equipment maintenance teaching depends on present condition of Luohe medical college. Methods: 90 college students from laboratory medicine department were selected as experiment target. They were divided into control group and experiment group and each group were 45 students. All the chosen students had no significant difference in subjects, ages and hobbies. Traditional teaching was used in the control group and virtual reality was introduced in the teaching of the experiment group. Results: Scores of written exam and training operation were analyzed with SPSS19.0. It showed that the scores of the experiment group were better than those of the control group. Conclusion: Application of virtual reality in clinical laboratory equipment maintenance teaching can improve students’ scores and effects.

Background: Medical equipments are necessary to effectively monitor, treat and support the care of patients by doctors in the management of their medical conditions. Proper maintenance affects the performance and safety of equipment. As equipments are part of health care provision in hospital, it is necessary to have well planned and managed equipment maintenance system in the organization. Aim: The study revealed the medical equipment maintenance system in Physiology Department of tertiary care government teaching institute. Material and methods: A retrospective observational study, during February to June 2014 of fifty two equipments costing more than 10,000 rupees was done for their maintenance system in the department. Information about equipment was collected by onsite inspection and departmental equipment records in structured proforma. Results: In the study, 45 mechanical and 7 electrical equipments costing less than 100,000 rupees were evaluated. It was found that 94.2% equipments did not have any service contract. From time of installation in the department it was found that most equipment i.e. 53.8% had crossed the life period of 15 years. Maximum i.e 85.7% electrical equipments had crossed their average life period. There was no proper maintenance of logbook and non availability of the user manual by the side of 84.6% of the equipments in department. Service history in worksheet of all 52 instruments was not recorded, post warranty maintenance in all equipments was by local personals only which was needed in 90.4% equipments. Out of equipments needing the maintenance and repair, it was corrective type in 98.2% times and reactive type in 1.8% times. There was no special training of the technicians about the maintenance of equipments. 73.1% equipments were working while 21.2% were awaiting repair. The procedure of ‘maintenance request" was noted in log book for all equipments. 3 of the equipment were having the long down time period above 5 years. Non functional equipment with long down time period was due to non availability of the spare parts in 21.2% equipment and vendor was not responding for maintenance in 5.7% equipment Conclusion: There was need of proper equipment maintenance system in department. The long down time period of equipment was due to old technology, non-availability of parts and non-responding vendors.

The problems of some hospital in medical equipment maintenance management were introduced from the aspects of maintenance time, maintenance record, maintenance service and self maintenance. Some measures were proposed including simplifying maintenance process through PDCA, informatizing maintenance record, cooperating with third-party maintainer and establishing self maintenance team, so that precision and informatized medical equipment management can be realized to maximize the benefit of medical equipment management.

O objetivo deste estudo foi definir uma metodologia para a definição de intervalos de contratos de manutenção e recomendações gerais em um grande hospital da cidade do Rio de Janeiro, Brasil. A confiabilidade de 161 dispositivos de monitoramento e tratamento cardíaco foi estudada por meio de quatro indicadores de suas condições de uso. Esses indicadores, juntamente com o ano de fabricação do equipamento, foram usados como preditores em um modelo de regressão logística que teve como desfecho a ocorrência de falha no equipamento. A seguir, o Mean Time Between Failures (MTBF) foi estimado para os indicadores apontados como relevantes pelo modelo. Apenas uma variável (intensidade de uso) foi considerada estatisticamente significante no modelo preditivo final. O MTBF, de acordo com as categorias dessa variável (baixo-médio-alto), tiveram valores compreendidos entre 121,3 (baixo) e 5,8 (alto) dias. Os resultados indicam grandes inadequações nos períodos de contratos de manutenção utilizados pelo hospital e sugerem que estratégias de manutenção mais eficientes seriam períodos de inspeção compatíveis com os intervalos de MTBF identificados.


The objective of this study was to define a methodology to assist the definition of maintenance contract intervals and overall recommendations for a large hospital in the city of Rio de Janeiro, Brazil. The reliability of 161 cardiac monitoring and treatment devices was studied. Four indicators of equipment use conditions were developed, and, along with the year of manufacture of the equipment, were used as predictors in a logistic regression that had the occurrence of failure as outcome. The mean time between failures (MTBF) was then estimated for the categories of the indicators deemed relevant by the regression. Only one variable (intensity of use) was statistically significant in the final predictive model, and the MTBF, according to each category of this variable (low-average-high), ranged from 121.3 (low) to 5.8 days (high). The results indicated large inadequacies in the maintenance contract periods used by the hospital and suggested that a much better maintenance strategy would be the inspection periods compatible with the identified MTBF intervals.

XML dynamic swapper of network data applied, a network management platform for medial equipment maintenance is developed, which is based on hospital LAN communication. J2EE-based multiplayer B/S structure is adopted. With the platform, routine management, malfunction declaration, maintenance service and data totaling can be carried out effectively.
