RESUMEN

BACKGROUND:How to provide sufficient skin resources for scar plastic surgery and repair of extensive deep burn patients while avoiding the re-proliferation of scar tissue in the surgical area has always been an important topic in burn and wound repair research. OBJECTIVE:To observe the clinical application effects of artificial dermis combined with autologous scar epidermis in the repair of scar after extensive burns. METHODS:Retrospective analysis was performed on 73 patients with scar hyperplasia and contracture deformity after extensive burns in Bengbu Third People's Hospital Affiliated to Bengbu Medical College from January 2021 to January 2023.The patients were divided into three groups according to the treatment method:Group A(n=21,artificial dermis combined with autologous scar epidermis transplantation was used for treatment),group B(n=27,scar epidermis was transplanted after scar release in the functional site),and group C(n=25,functional site scar release after transplantation of thick skin treatment).Skin survival and infection at the receiving site, wound healing time at the receiving site and the donor site were recorded in the three groups. The scar status and functional recovery of the recipient area and donor area were evaluated by the Vancouver Scar Scale and activities of daily living. RESULTS AND CONCLUSION:(1)The skin infection rate was lower in group B than that in groups A and C(P<0.05).The survival grade was higher in group B than that in groups A and C(P<0.05).(2)The wound healing time at the receiving site was longer in group A than that in groups B and C(P<0.05).The wound healing time at the receiving site was longer in group C than that in group B(P<0.05).The wound healing time at the donor site was longer in group C than that in groups A and B(P<0.05).(3)Vancouver Scar Scale score was higher in group B than that in groups A and C at 12 months postoperatively(P<0.05).Vancouver Scar Scale score was higher in group C than that in groups A and B at 6 and 12 months postoperatively(P<0.05).The excellent grade of activities of daily living in groups A and C was significantly higher than that of group B at 12 months postoperatively(P<0.05).(4)The results showed that the application of artificial dermis combined with autologous scar epidermis composite transplantation in the treatment of scar contracture after extensive burn could not only achieve the same effect as that of intermediate-thickness skin, but also avoid postoperative scar re-hyperplasia at the donor site and shorten the time of complete wound healing at the donor site.Compared with scar epidermal transplantation, this treatment has obvious advantages.

RESUMEN

According to the infection characteristics of batches of patients with extensive burns in dust explosion, the article focused on the concept and mode of whole-range fine management.Based on the characteristics and rules of infection prevention in bat-ches of patients with extensive burns, the measurement and examination of infection management were refined, the infection monitoring indexes were designed scientifically, the infection prevention scheme and monitoring table were formulated.By early and whole-range intervention of infection prevention, quantitative evaluation, fine management and precise control at different times, all levels of infec-tion management teams could fully serve their purposes in order to realize effective infection prevention.

RESUMEN

Objective To explore effect of CYP3A4 gene polymorphism and genetic testing on the efficacy of fentanyl in patients with extensive burns. Methods A prospective randomized controlled study, 149 patients with extensive burns were picked from Feburary to July in 2015.Patients were randomly divided into matched group ( n=56 ) and experimental group ( n=93 ) . The matched group was treated with fentanyl 5 g/kg to finish implementation of anesthesia induction.Depends on the gene detection results, the patients in experimental group were treated with 6 g/kg, 5 g/kg or 4 g/kg fentanyl. The VAS score at different time after operation in the experimental group and the control group were compared , and the dosage of fentanyl was observed.Patients whose VAS score was greater than 7 should treated with intramuscular injection of pethidine 100 mg, recorded all patients with pethidine,s additional cases and the times. At the same time, the adverse reactions of the two groups were recorded before and after hospital discharge. ResuIts The score of VAS was not significantly different after wake up immediatly after the surgery , and the score of VAS was significantly lower in the experimental group patients in 6h,12h after the surgery (P<0.05).The dosage of fentanyl in the experimental group and the number of the cases and the times were significantly decreased (P<0.05).The adverse reactions after the surgery such as nausea, vomiting, itching, drowsiness, and adverse reactions were significantly lower in the experimental group (P<0.05).ConcIusion CYP3A4 gene detection in patients with large area of burn in the individual drug use is important, because of patients’ genotype adjusting fentanyl dosagecan, it enhances significantly the analgesic effect, reduces the amount of drug use, and effectively reduces the adverse reactions.

RESUMEN

To introduce the treatment methods and weaknesses of the advanced wounds in extensive burns patients, explain the application of ultrasonic debridement in wound repairment. Introduce the treatment equipment of the advanced wounds, and present the idea to combine bath treatment and ultrasonic debridement. To develop a ultrasonic-therapeutic tank. It is important to improve treatment methods of the advanced wounds in extensive burns patients.

RESUMEN

Objective To investigate the clinical effects of ulinastatin in treating extensive burns patients of shock period. Methods 15 patients with extensive burns were assigned to the treatment group(8 eases)and the control group(7 cases)randomly, patients in the control group were given the routine therapy, while those in the treatment group Were also given ulinastatin in the early stage of shock period. The life symptoms, urine amount, shock lasting time and complications were observed in the fourth day after injury. Results Compared to the control group, the life symptoms were steadier, urine amount was more of equivalent, shock lasting time was shorter, and complications were less in the treatment group. Conclusion In the early stage of the shock period for extensive burns patients, ulinastatin can help the patients live through the shock period steady.
