RESUMEN

ObjectiveTo explore the effect of core muscles training based on spinal fine-tuning manipulation on lumbar facet joint disorders. MethodsFrom February, 2021 to February, 2022, 80 patients with lumbar facet joint disorders in Huadong Hospital Affiliated to Fudan University were randomly divided into control group (n = 40) and observation group (n = 40) randomly. Both groups received routine treatment and spinal fine-tuning manipulation, while the observation group received core muscles training in addition, for six weeks. They were assessed with Japanese Orthopaedic Association (JOA) scores, Short-Form of McGill Pain Questionnaire and World Health Organization Quality of Life-BREF before and after treatment. The recurrence rate was observed after three months follow-up. ResultsThe scores of all the scales improved after treatment (t > 5.751, P < 0.001), and improved more in the observation group than in the control group (t > 2.051, P < 0.05). After three months follow-up, the recurrence rate was 7.89% (3/38) in the observation group, less than 28.13% (9/32) in the control group (χ2 = 5.005, P = 0.025). ConclusionCombination of core muscles training may improve lumbar function, reduce lumbar pain, reduce recurrence and improve quality of life for patients with lumbar facet joint disorders.

OBJECTIVE@#To explore the effect of facet joint degeneration in adjacent segments on the incidence of adjacent segment disease (ASD) after lumbar fusion and fixation.@*METHODS@#A retrospective analysis was performed on 138 patients who underwent L5S1 posterior lumbar interbody fusion (PLIF) from June 2016 to June 2019. Patients were divided into a degeneration group (68 cases) and a non-degenerative group (70 cases) based on the presence or absence of L4,5 facet joint degeneration before surgery (graded using the Weishaupt standard). Age, gender, body mass index (BMI), follow-up time, and preoperative L4,5 intervertebral disc degeneration (graded using the Pfirrmann standard) were collected for both groups. Clinical outcomes were evaluated using the visual analogue scale (VAS) and Oswestry disability index (ODI) at 1 and 3 months after surgery. The incidence and time of ASD after surgery were analyzed.@*RESULTS@#There were no significant differences between the two groups in age, gender, BMI, follow-up time, or preoperative L4,5 intervertebral disc degeneration. Both groups showed significant improvement in VAS and ODI at 1 and 3 months after surgery (P<0.001), with no significant difference between the groups(P>0.05). However, there was a statistically significant difference in the incidence and timing of ASD between the groups (P<0.05). The degeneration group had 2 cases of ASD in gradeⅠdegeneration, 4 cases of ASD in gradeⅡdegeneration, and 7 cases of ASD in grade Ⅲ degeneration. There was a statistically significant difference between the number of patients with grade Ⅲ degeneration and those with gradesⅠandⅡASD (P<0.0167, Bonferroni correction).@*CONCLUSION@#Preoperative degeneration of adjacent articular processes will increase the risk of ASD after lumbar fusion fixation, whereas gradeⅢ degeneration will further increase the risk.

@#Introduction: Degenerative spondylolisthesis burdens the future economy. Elderly individuals with this condition suffer from back pain, limb paralysis, reduced activity, and diminished quality of life. Surgery offers relief but is risky and expensive. As healthcare improves, the aging population grows, and spondylolisthesis incidence rises. Early detection and risk factor management are vital for prevention. Therefore, this study aimed to evaluate the correlation of the risk factors for degenerative lumbar spondylolisthesis. Methods: A retrospective observational study was done in Dr Soetomo General Academic Hospital, Surabaya from January 2018 to December 2019. Forty-five subjects with degenerative spondylolisthesis diagnosed by MR Imaging enrolled in this study. Intervertebral disc, facet joint, multifidus muscle, and ligamentum flavum at level L1 until level L5 were examined using MR imaging. Correlation analysis was measured using the Spearman correlation test and risk factor analysis was measured using the logistic regression test. Results: There are significant correlations between intervertebral disc degeneration, facet joint osteoarthritis, multifidus muscle fatty infiltration, and thickening of ligamentum flavum with degenerative spondylolisthesis. The risk factors at levels L3-4 and L5-S1 are intervertebral disc degeneration and L4-5 is multifidus muscle fatty infiltration and facet joint osteoarthritis. Conclusion: This study confirmed the risk factors that correlated with degenerative lumbar spondylolisthesis.

OBJECTIVE@#To compare and analyze the feasibility of autologous facet joint bone block as an alternative to polyetheretherketone (PEEK) cage in lumbar intervertebral fusion surgery for patients with osteoporosis.@*METHODS@#From December 2018 to June 2021, the case data of patients with osteoporosis (T value ≤ -2.5 on dual energy X-ray bone density) who underwent posterior lumbar interbody fusion in the Fourth Medical Center, Chinese PLA General Hospital were retrospectively reviewed. All the cases were followed up for no less than 12 months and were divided into two groups according to the differences of interbody fusion materials: the autologous facet joint bone block group (autogenous bone group) and the PEEK cage group (PEEK group). The general data [such as age, gender, body mass index (BMI), primary diagnosis, distribution of fusion segments, bone mineral density of lumbar (BMD), incidence of preoperative complications], the perioperative data (such as duration of operation, intraoperative blood loss, postoperative drainage, perioperative allogeneic blood transfusion rate), and the incidence of postoperative complications were compared between the two groups. Imaging parameters (disc height, lumbar lordosis angle, segment lordosis angle, segmental lordosis angle, disc height improvement rate, and fusion rate) and lumbar functional scores [visual analogue scale (VAS), Oswestry disability index (ODI), Japanese Orthopedics Association (JOA) score for lower back pain] were compared to evaluate the clinical efficacy between the kinds of intervertebral fusion materials 1 week, 3 months and 6 months postoperative and at the last follow-up.@*RESULTS@#A total of 118 patients were enrolled, including 68 cases in the autogenous bone group and 50 cases in the PEEK group, there were no statistical differences in age, gender, BMI, primary diagnosis, distribution of fusion segments, BMD, incidence of preoperative complications, duration of operation, intraoperative blood loss, postoperative drainage, perioperative allogeneic blood transfusion rate, incidence of postoperative complications, all the preoperative imaging parameters and all the lumbar function scores between the two groups (P>0.05). Postoperative superficial surgical site infections occurred in 3 patients in the autogenous bone group and 2 patients in the PEEK group. At the last follow-up, 3 cases of intervertebral graft collapse occurred in the autogenous bone group and 5 cases in the PEEK group, 1 case of graft subsidence in the autogenous bone group and 1 case in the PEEK group. All the imaging parameters showed significant differences between postoperation and preoperation (P < 0.05), and all the imaging parameters showed significant differences between 1 week and 3 months postoperative in both groups (P < 0.05). The height, angle of fusion gap in the autogenous bone group were lower than those in the PEEK group 1 week postoperatively (P < 0.05), and the fusion gap height improvement rate in the autogenous bone group was lower than that in the PEEK group (P < 0.05). The cases in both groups started to show final fusion 3 months after surgery, and the fusion rate in the autogenous bone group was 75% 6 months postoperatively, which was significantly higher than the rate of 56% in the PEEK group (P < 0.05), and there was no statistically significant difference in the final fusion rate between the two groups (P>0.05). The ODI, the postoperative VAS score was significantly lower than that in preoperation, while the postoperative JOA score was significantly higher than that in preoperation (P < 0.05). The ODI was lower while the JOA score was higher of the autogenous bone group than that of the PEEK group 6 months postoperatively (P < 0.05).@*CONCLUSION@#In osteoporosis patients, good interbody fusion rate and improvement of lumbar vertebral function can be obtained by using autologous facet joint bone block or PEEK cage, while the fusion rate and the improvement of lumbar function with autologous facet joint bone block are better than those with PEEK cage 6 months post-operatively. PEEK cage is superior to autologous facet joint bone block in intervertebral distraction and improvement of lumbar lordosis. Significant disc space subsidence occurred in osteoporotic patients within 3 months after lumbar interbody fusion, and the subsidence of PEEK cage was more obvious than that of autologous facet joint bone block.

Introducción: Los científicos han explorado las capacidades intelectuales de los animales, su importancia para la salud física y emocional de los humanos, su uso como sujetos en la investigación biomédica y psicológica, pero se ha prestado poca atención a las valoraciones que hacen las personas del común sobre estas prácticas sociales. El objetivo de esta investigación fue presentar los resultados sobre las valoraciones de prácticas de divertimiento, producción industrial, investigación biológica y psicológica, que involucran animales, a partir de las dimensiones: ética, legal, las capacidades intelectuales, los beneficios para la salud física y emocional de las personas y la preocupación por su bienestar. Método: Se construyó un instrumento online a partir una frase mapa compuesto por 28 ítems, que exploran la evaluación de 255 personas, hombres y mujeres sobre estas prácticas, bajo la condición de tener o no mascota. Las respuestas se analizaron mediante un análisis de escalamiento multidimensional. Resultados: Queda en evidencia la preocupación de las personas por las condiciones en que se usan los animales, en particular, de quienes tienen o han tenido mascota. Conclusiones: Los hallazgos se discuten a partir del soporte de las facetas incluidas en la frase mapa y la propuesta para nuevos estudios sobre prácticas sociales que involucre animales en la población general.


Introduction: Scientifics have explored the intellectual capacities of animals, their importance for the physical and emotional health of humans, its use as subjects for biomedical and psychological research. However, little attention has been involved to studying how lay people think about these types of social practices. The objective of this research was to present the results of the evaluations of social practices involving animals in entertainment, industrial production, biological and psychological research. The practices were evaluated on the following dimensions: ethics, legal, intellectual capacities, benefits for the physical and emotional health of people; and concern for their well-being. Method: An instrument based on a mapping sentence and composed of 28 items was responded online by 255 people, men and women with the condition of having a pet or not. Item responses were analyzed using multidimensional scaling analysis. Results: It is evident people's concern for the conditions in which the animals are used particularly in people that have or has have pets. Conclusions: The findings are discussed on the evidence that support the mapping sentence, and on the proposal for new studies on social practices involving lay people.

Resumen: Introducción: El dolor en columna lumbar afecta a un alto porcentaje de la población y presenta un significativo impacto socioeconómico. El síndrome facetario lumbar tiene una prevalencia entre 15-31% con incidencia a lo largo de la vida de hasta 52% en algunas series. Debido al empleo de distintos tipos de tratamiento y diversos criterios de selección de los pacientes, la tasa de éxito varía en la literatura. Objetivo: Comparar resultados de tratamiento con rizólisis aplicando radiofrecuencia pulsada versus crioablación en pacientes con diagnóstico de síndrome facetario lumbar. Material y métodos: De Enero de 2019 a Noviembre de 2019, ocho pacientes fueron divididos aleatoriamente en dos grupos: el grupo A tratados con radiofrecuencia pulsada y el grupo B tratados con crioablación. Se evaluó el dolor con la escala visual análoga y el índice de discapacidad de dolor lumbar de Oswestry a las cuatro semanas, además de tres y seis meses. Resultados: El seguimiento fue de seis meses. De forma inmediata los ocho pacientes (100%) refirieron mejoría de los síntomas y del dolor. De los cuatro pacientes que se encontraban en limitación funcional intensa, uno pasó a estar sin limitación funcional, dos de ellos pasaron a limitación funcional mínima y uno a limitación funcional moderada al primer mes; se reportaron diferencias estadísticamente significativas. Conclusiones: Ambos tratamientos controlan el dolor en el corto plazo; también hay una mejoría de las capacidades físicas. La morbilidad que acompaña a la neurólisis, ya sea radiofrecuencia o crioablación, es muy baja.


Abstract: Introduction: Lumbar spine pain affects a high percentage of the population and has a significant socioeconomic impact. Lumbar facet syndrome has a prevalence between 15-31% with lifetime incidence of up to 52% in some series. Due to the use of different types of treatment and different patient selection criteria, the success rate varies in the literature. Objective: To compare results of treatment with rhizolysis applying pulsed radiofrequency versus cryoablation in patients diagnosed with lumbar facet syndrome. Material and methods: From January 2019 to November 2019, eight patients were randomly divided into two groups: group A treated with pulsed radiofrequency and group B treated with cryoablation. Pain was assessed with the visual analogue scale and the Oswestry low back pain disability index at four weeks, in addition to three and six months. Results: Follow-up was six months. Immediately the eight patients (100%) reported improvement in symptoms and pain. Of the four patients who were in intense functional limitation, one of them became without functional limitation, and two of them went to minimum functional limitation and one to moderate functional limitation at the first month, statistically significant differences were reported. Conclusions: Both treatments control pain in the short term; there is also an improvement in physical abilities. The morbidity accompanying neurolysis either radiofrequency or cryoablation is very low.

Objective To investigate the effect of facet joint asymmetry on lumbar biomechanics in normal and patients with adolescent lumbar disc hemiation (ALDH). Methods Mimics 21.0, 3-Matic Medical 13.0, Geomagic Wrap 2017, HyperMesh 2019 and finite element software ABAQUS 2021 were combined to establish three-dimensional finite element models of nonnal lower lumbar spine and adolescent lumbar disc hemiation. According to the difference between the left and right facet joints, three cases of nonnal and ALDH patients aged 13-18 years old were selected, with a total of 6 cases. The stress of intervertebral disc under different torque loads (neutral position, lateral flexion and rotation) was analyzed and compared. Results 1. Three dimensional finite element models of L3-L, segments were established in 6 male nonnal and adolescent patients with lumbar disc herniation, and the stress and displacement nephogram of lumbar disc hemiation in nonnal neutral position, flexion, extension, lateral flexion and rotation were obtained; 2. The stress of L4_, annulus fibrosus increased when the facet joint angle of nonnal adolescents was symmetrical in the neutral position; 3. The stress of annulus fibrosus was greater than that of nucleus pulposus under different conditions, and the stress of annulus fibrosus was flexion > neutral position > extension; 4. In patients with ALDH, the left side of the facet joint was larger than the right 10 ° model, and the stress in the posterior side of the annulus fibrosus of L4_, segment increased significantly under extension condition. Under lateral flexion condition, the left stress of the left annulus fibrosus was compared with the right stress of the right annulus fibrosus, t = l. 575, P<0. 05, the difference was statistically significant, the right stress of the right annulus fibrosus was greater than the left stress of the left annulus fibrosus. Under the rotating condition, the stress on the left and right sides of the fiber ring was greater than that on the back side. Conclusion 1. Compared with nonnal and patients with ALDH, the stress of intervertebral disc increases under different postures, which ma)' increase the shear load of intervertebral disc and aggravate the process of intervertebral disc degeneration; 2. When the degree of left-right asymmetry of facet joint is more than 10 degrees, the stress on the side with small degree is greater. Facet joint asymmetry can lead to the overload of vertebral bod)' and intervertebral disc, leading to spinal instability; 3. The stress of intervertebral disc in extension is less than that in neutral position and flexion. Extension can relieve the pressure of intervertebral disc and play an auxiliary role in the recovery of patients with ALDH.

Objective:To evaluate the accuracy of an ultrasound-guided robot-assisted system for percutaneous facet joint puncture in an animal experiment by registration of ultrasound images and two-dimensional X-ray fluorescence images.Methods:Six specimens of fresh adult sheep lumbar spine were prepared and soaked in gelatin solution. The specimens contained a total of 48 facet joints with 24 ones respectively on the left and right sides. Half of the joints were assigned by the random number table method into a robot-assisted group which were subjected to percutaneous facet joint puncture assisted by the ultrasound-guided robot-assisted system while the contralateral ones into a freehand group which were subjected to percutaneous facet joint puncture by freehand. In the robot-assisted group, the left facet joints were 3 L 1, 3 L 2, 3 L 3 and 3 L 4 ones while the right facet joints were 2 L 1, 3 L 2, 5 L 3 and 2 L 4 ones. In this self-control animal experiment, both the robot-assisted punctures and the freehand punctures were carried out in the same specimen. The puncture success rate, puncture needle adjustment, positioning deviation, orientation deviation and operation time were recorded. Results:The positioning deviation and orientation deviation in the robot-assisted group [(2.21 ± 1.12) mm and 1.51° ± 0.47°] were significantly lower than those in the freehand group [(3.26 ± 1.44) mm and 2.24° ± 0.89°] ( P < 0.05). All the articular facet joint punctures were successfully accomplished for the first time in the robot-assisted group while those required multiple adjustments in the freehand group. In the robot-assisted group, the total operation time [(463.84 ± 34.93) s] was significantly longer than that in the freehand group [(298.40 ± 27.48) s], but the puncture time [(37.97 ± 6.87) s] was significantly shorter than that in the freehand group [(261.61 ± 33.15) s] ( P < 0.05). Conclusion:The ultrasound-guided robot-assisted system for percutaneous facet joint puncture can lead to accurate puncture with limited needle adjustments.

Objective:To investigate the efficacy of flexion-lateral curvature-supination reduction combined with primary anterior surgery for the treatment of lower cervical dislocation with unilateral facet inter-locking.Methods:A retrospective analysis was performed in the 32 patients who had been admitted to Department of Spine Surgery, Honghui Hospital for lower cervical dislocation with unilateral facet interlocking from November 2015 to October 2018. According to their treatments, they were divided into 2 groups. In the emergency group treated by flexion-lateral curvature-supination reduction combined with primary anterior surgery, there were 13 males and 3 females, aged from 24 to 63 years. In the traction group treated by cranial traction reduction combined with secondary anterior surgery, there were 12 males and 4 females, aged from 20 to 64 years. The operation time, intraoperative blood loss, hospital stay, bone graft fusion, American Spinal Injury Association (ASIA) grade and Japanese Orthopaedic Association (JOA) score were compared between the 2 groups.Results:There was no significant difference in the preoperative general data between the 2 groups, showing they were comparable ( P<0.05). All patients were followed up from 26 to 40 months. The hospital stay for the emergency group [(7.2±1.2) d] was significantly shorter than that for the traction group[(10.9±1.2) d] ( P<0.05). There was no significant difference in the operation time, blood loss, ASIA grade or JOA score between the 2 groups ( P>0.05). All patients achieved osseous fusion of intervertebral space. Conclusion:Compared with traditional methods, flexion-lateral curvature-supination reduction combined with primay anterior surgery shows no significant difference in the recovery of neurological function but leads to a shorter hospital stay.

Resumen A través de un estudio correlacional, esta investigación explora la influencia de posibles beneficios sociales y sus variables asociadas en la adopción de Comportamientos Urbanos Responsables (CUR). Teniendo en cuenta la complejidad del problema y el número de variables a explorar, se empleó el modelo de Teoría de Facetas. Además, se diseñó un cuestionario derivado de una "frase mapa" que presenta en forma de viñetas situaciones urbanas hipotéticas que exploran las variables en estudio. Participaron 177 individuos que estimaron la probabilidad de que un individuo involucrado en una viñeta adopte o no un CUR, a partir de variables como la distancia temporal y la magnitud del beneficio, la saliencia del anuncio de este beneficio, y la aprobación social y el reproche. Un análisis de distancias mínimas (SSA) mostró que los participantes reconocen estas variables a la hora de optar por un CUR. La distancia temporal para obtener el beneficio y la magnitud grande del beneficio resultaron estar altamente correlacionadas con la mayor estimación sobre la probabilidad de que un individuo elija un CUR. Se sugiere formular estrategias educativas y políticas públicas que conduzcan al desarrollo de la cooperación, la responsabilidad y la autorregulación, con el fin de contribuir a mejorar la calidad de vida urbana y la convivencia en el medio urbano.


Abstract Through a correlational study, this research explores the influence of possible social benefits and associated variables on the adoption of Responsible Urban Behaviors (RUB). Considering the complexity of the problem and the number of variables to be explored, the Facet Theory model was used. Also, a questionnaire was created, which presented hypothetical urban situations in the form of vignettes that explored the variables under study. A total of 177 individuals participated and estimated the likelihood of an individual involved in a vignette adopting or not an RUB, based on variables such as the time and magnitude of the benefit, salience of the advertisement of such benefit, and social approval and criticism. The smallest space analysis showed that participants recognize these variables when it comes to opting for an RUB. The time to obtain the benefit and the magnitude of the benefit turned out to be highly correlated with the highest estimate of the likelihood of an individual choosing an RUB. This research suggests formulating educational strategies and public policies that lead to the development of cooperation, responsibility, and self-regulation, to help improve the quality of urban life and coexistence in the urban environment.

ABSTRACT Objective To analyze the results of the use of dexmedetomidine (D) in the treatment of patients with degenerative diseases of the lumbar spine using puncture techniques. Methods The study included 77 patients who underwent surgical puncture for degenerative diseases of the lumbar spine with the use of alpha-2-adrenomimetic D: percutaneous laser denervation of the facet joints (n = 46) and posterolateral transforaminal endoscopic discectomy (n = 31). We assessed: the level of sedation using the Ramsay Sedation Scale (RSS) and the Richmond Agitation Sedation Scale (RASS); intraoperative dynamics of the cardiovascular and respiratory system parameters; the level of pain syndrome according to VAS. Results A high intraoperative level of sedation was determined, with RASS -2, -3 and Ramsay III, IV; when transferring a patient to a department (in 90 minutes) this parameter was RASS 0 and Ramsay II. There were no significant changes in central hemodynamics and respiratory depression. The minimum level of pain was determined immediately after surgery, at 30 and 60 minutes after surgery, and before transfer to the department (90 minutes): 6 (4;9); 10 (8;12); 12 (9;13); 16 (13;19) respectively. The absence of the need for additional analgesia on the first postoperative day was verified. Conclusion The use of D significantly reduces the level of pain, while maintaining the necessary verbal contact with the patient, and provides the necessary neurovegetative protection without respiratory depression or lowered hemodynamic parameters during the perioperative period. RESUMO Objetivo Analisar os resultados do uso de dexmedetomidina (D) no tratamento de pacientes com doenças degenerativas da coluna lombar com técnicas de punção. Métodos O estudo incluiu 77 pacientes submetidos à punção cirúrgica em doenças degenerativas da coluna lombar com o uso de um agonista alfa-2 adrenérgico: denervação percutânea das articulações facetárias com laser (n = 46) e discectomia endoscópica transforaminal posterolateral (n = 31). Foram avaliados o nível de sedação usando a Escala de Sedação de Ramsay (RSS) e a Escala de Sedação e Agitação de Richmond (RASS); a dinâmica intraoperatória dos parâmetros dos sistemas cardiovascular e respiratório; o nível de síndrome de dor de acordo com a EVA. Resultados Determinou-se um alto nível intraoperatório de sedação pela RASS (-2, -3) e pela Ramsay (III, IV). Ao transferir um paciente para outro setor (depois de 90 minutos), esse parâmetro era 0 em RASS e II em Ramsay. Não houve alterações significativas na hemodinâmica central e na depressão respiratória. O nível mínimo de dor foi determinado imediatamente após a cirurgia, 30 e 60 minutos depois da cirurgia e antes da transferência para o outro setor (90 minutos depois): 6 (4; 9); 10 (8; 12); 12 (9; 13); 16 (13; 19), respectivamente. Constatou-se que não era necessária analgesia adicional no primeiro dia pós-operatório. Conclusões O uso de D reduz significativamente o nível de dor mantendo a comunicação verbal necessária com o paciente e fornece a proteção neurovegetativa necessária sem depressão respiratória e os parâmetros hemodinâmicos reduzidos durante o período perioperatório. RESUMEN Objetivo Analizar los resultados del uso de dexmedetomidina (D) en el tratamiento de pacientes con enfermedades degenerativas de la columna lumbar con técnicas de punción. Métodos El estudio incluyó a 77 pacientes con enfermedades degenerativas de la columna lumbar que se sometieron a punción quirúrgica mediante el uso de un agonista adrenérgico alfa-2: denervación percutánea de las articulaciones facetarias con láser (n = 46) y discectomía endoscópica transforaminal posterolateral (n = 31). Fueron evaluados el nivel de sedación mediante la Escala de Sedación de Ramsay (RSS) y la Escala de Sedación y Agitación de Richmond (RASS); la dinámica intraoperatoria de los parámetros de los sistemas cardiovascular y respiratorio; el nivel del síndrome de dolor según la EVA. Resultados Se determinó un alto nivel de sedación intraoperatoria en RASS (-2, -3) y por Ramsay (III, IV)Al transferir un paciente a otro sector (después de 90 minutos), este parámetro fue 0 en RASS y II en Ramsay. No hubo cambios significativos en la hemodinámica central y la depresión respiratoria.El nivel mínimo de dolor se determinó después de la cirugía, 30 y 60 minutos después de la cirugía y antes del traslado al otro sector (90 minutos después): 6 (4; 9); 10 (8; 12); 12 (9; 13); 16 (13; 19), respectivamente. Se verificó que no era necesaria analgesia adicional el primer día postoperatorio. Conclusiones El uso de D reduce significativamente el nivel de dolor al mismo tiempo que se mantiene la necesaria comunicación verbal con el paciente y brinda la protección neurovegetativa necesaria sin depresión respiratoria y parámetros hemodinámico reducidos durante el período perioperatorio. Objective To analyze the biomechanical characteristics of lumbar fusion by 3 internal fixation methods using three-dimensional (3D) finite element (FE) method. Methods The FE fixation models of physiological L4-5, unfixed fusion L4-5, translaminar facet screw (TLFS), lumbar pedical screw+translaminar facet screw (LPS+TLFS), bilateral pedical screw (BPS) with complete osteotomy or partial osteotomy of facet joint were established, respectively. The biomechanical characteristics of L4 centrum and implants under six motion states (spinal flexion, extension, lateral bending and axial rotation) in L4-5 fusion model and three fixation models were compared by FE analysis. Results The average maximal displacements of L4 centrum in L4-5 unfixed fusion model, TLFS model, TLFS+LPS model, BPS model were 1.410 8, 0.629 8, 0.336 9, 0.252 8 mm (complete osteotomy of facet joint) and 1.296 7, 0.844 9, 0.340 9, 0.273 8 mm (partial osteotomy of facet joint); the average maximal displacements of cage were 0.479 9, 0.319 5, 0.167 6, 0.126 4 mm (complete osteotomy of facet joint) and 0.378 7, 0.348 4, 0.183 5, 0.137 2 mm (partial osteotomy of facet joint);the average maximum stresses of screws and rods during 6 motions in TLFS model, TLFS+LPS model, BPS model were 178.34, 79.55, 56.33 MPa (complete osteotomy of facet joint) and 142.29, 103.02, 59.69 MPa (partial osteotomy of facet joint). Conclusions In percutaneous transforaminal lumbar interbody fusion, the fixation effect of BPS model was similar to that of LPS+TLFS model. BPS model could achieve the best spinal stability, and LPS+TLFS model was also a good fixation method. The stability of TLFS model alone was relatively poor, but it was still better than that of cage bone graft without internal fixation. In the absence of internal fixation, preservation of the articular process significantly increased stability of the spine.

【Objective】 To compare the efficacy of percutaneous vertebroplasty (PVP) and facet blocking (FB) in the treatment of osteoporotic vertebral compression fractures. 【Methods】 From January 2015 to June 2019, 162 patients with osteoporotic vertebral compression fracture were treated with PVP and FB. Among them, PVP group (86 cases) received PVP treatment and FB group (76 cases) received FB treatment. We compared the perioperative related indexes, the height of the anterior edge of the injured vertebrae, the sagittal kyphosis Cobb angle, visual analogue score, dysfunction index and lumbar dysfunction score of preoperative and postoperative follow-up (1 day, 1 week, 1 month, 3 months, 6 months, and 12 months) in both groups. 【Results】 There was significant difference in operation time between PVP group [(34.25±6.04)min)] and FB group [(21.47±4.58)min] (P0.05), but there was significant difference in the height of anterior margin of injured vertebrae between the two groups at 3 days, 6 months and 12 months after operation (P0.05), but after operation(P0.05). 【Conclusion】 Both PVP group and FB group have effectively relieved pain. The results of this study suggest that FB can become an alternative treatment for PVP in some patients, such as the poor, elderly and pain-tolerant ones.

@#Lumbar decompressive laminectomy is a standard treatment for degenerative lumbar spinal stenosis, but in some cases, can lead to iatrogenic spondylolysis and delayed segmental instability. Iatrogenic spondylolysis occurs in most cases in pars interarticularis, but rare cases have also been reported, pediculolysis in pedicle and laminolysis in lamina. Minimally invasive spine surgery (MIS) is known to have a low risk of developing these iatrogenic spondylolyses, and unilateral biportal endoscopy is the MIS that has been drawing attention. We present a case of a 72-year-old female who was diagnosed with L4-5 unstable non-isthmic spondylolisthesis and severe right central disc extrusion 10 weeks after UBE assisted unilateral laminotomy for bilateral decompression (ULBD) at the consecutive segments of L3-4 and L4-5. Pre-operative imaging studies revealed severe central stenosis without spondylolisthesis at L3-L4 and L4- L5 along with L4-L5 facet tropism. She was managed by anterior lumbar interbody fusion and cement augmented pedicle screw fixation, which resulted in the complete resolution of her clinical and neurologic symptoms.

Resumen A pesar de varias décadas de estudios sobre las actitudes hacia el medio ambiente y las variables personales y sociales asociadas a ellas, la educación ambiental no ha logrado generar cambios significativos en la adopción de comportamientos ambientales. La educación en reglas verbales se propone como una alternativa para lograr los cambios comportamentales requeridos, por considerar que sugiere soluciones pragmáticas para reducir la distancia entre el decir-hacer y orientar la investigación futura sobre el comportamiento proambiental. . Un total de 469 participantes de diferentes grupos de edad tomó parte de este estudio: 376 informaron de las frecuencias del seguimiento de reglas proambientales a través de un cuestionario y 93 clasificaron diferentes tipos de comportamientos proambientales en un procedimiento de selección libre, y también en relación con las consecuencias y esfuerzo asociados a estos comportamientos. Los resultados permiten observar los efectos que ha tenido la educación ambiental en la conceptualización del comportamiento proambiental, pero con carencias en el establecimiento de la conexión de estos comportamientos con consecuencias sociales y naturales. Se discuten las implicaciones para la educación ambiental.


Abstract Despite several decades of studies on attitudes to the environment and the personal and social variables associated to them, environmental education has largely failed to make substantial gains in the adoption of pro-environmental behaviors. Education on rule-governed behavior is proposed as an alternative for reducing the distance between saying and doing behavioral change that suggests pragmatic solutions and promising avenues for future investigation on pro-environmental behavior. A total of 469 participants from different age groups took part of this study. 376 reported the frequencies of following pro-environmental rules through a questionnaire, and 93 categorized different kinds of pro-environmental behaviors in a free sorting procedure and also in relation to consequences and effort associated to these behaviors. Even though results evidence the effects of environmental education in recognizing environmental relevant behaviors they lack in establishing the connection of these behaviors with social and natural consequences. Implications for environmental education are discussed.

Incongruities in the terminology and in the Brazilian legislation about percutaneous facet procedures (PFPs) for the treatment of chronic lower back pain are frequently the subject of litigations between health professionals and supplementary healthcare providers. The Brazilian Hierarchical Classification of Medical Procedures (CBHPM, in the Portuguese acronym) describes four types of PFPs, while the Brazilian Unified Supplementary Health Terminology (TUSS, in the Portuguese acronym) describes five distinct lumbar PFPs, which correlate with the ones described on the List of Procedures and Events in Health, created by the Brazilian National Agency of Supplementary Health (ANS, in the Portuguese acronym). In the present paper, we review the terminology of the procedures, proposing the unification of the terminology and the abolition of redundancies in the tables. Finally, we developed a single terminology proposal for the PFPs based on their complexity and objectives to be used for the treatment of lower back pain.

Coronary arteries supply blood to the heart. They run through the epicardial adipose tissue during their course. Occasionally a band of myocardial tissue overlies a segment of the coronary arteries. This band of myocardium overlying the coronary artery is named as Myocardial Bridge (MB). There have been numerous instances of sudden and unexpected cardiac death where the presentation of myocardial bridging was the main clinically pertinent finding post-mortem. We wanted to study the distinct morphological aspects of MBs along with the structural composition of the coronary vessel present beneath, in front of and past the myocardial bridge. METHODSMicroscopy and morphometry were utilized to detail the distinct morphological characteristics of myocardial bridges as well as to take a closer look at the structural composition of the coronary vessel present beneath, in front of and past the myocardial bridge being evaluated. Three segments of the left anterior descending artery (LAD) were utilized to glean testable samples. These samples are as follows: (A) located proximally to the bridge at a distance of 8 mm, (B) alongside the bridge and (C) located distally from the bridge at a distance of 8 mm. RESULTSPrevalence of MB was found to be 40 %. Male to female ratio was 7:1. 87.5 % of the hearts have single MB, 10 % have double MB, 2.5 % have triple MB. Most common vessel affected is LAD. The MBs ranged in length from 4.4 mm to 45 mm with a length of 12.67 mm being the quantifiable mean. Thickness of MB ranged from 0.4 to 1.9 mm with mean thickness of 1.11 mm. The component of the coronary artery that was bridged often showed a diminished tunica intima compared to similar segments taken from proximal and distal sites. It should be noted that the proximal segment is predisposed to atherosclerotic presentation on account of myocardial bridging. CONCLUSIONSA myocardial bridge leads to detrimental changes in the vessel which in turn leads to early presentation of atherosclerosis, arrhythmias, myocardial infarctions and in severe cases, sudden death. Myocardial bridging is frequently localized in the LAD. It can have significant effects on the overall health.

El dolor facetario lumbar es una de las principales causas de dolor lumbar; representa alrededor del 15-56%. La articulación facetaria estabiliza la columna vertebral, tiene un rol fundamental en el soporte, distribución del peso y regulación de los movimientos rotacionales de la columna. Por ello, el conocimiento de la anatomía y de la biomecánica de esta articulación ayuda a tener una mejor comprensión de su participación en la fisiopatología del dolor lumbar y, por ende, mejora su abordaje diagnóstico y terapéutico. Nosotros revisamos aquí los conceptos actuales de embriología, anatomía, biomecánica y la correlación clínica/imagenológica de los cambios asociados a la enfermedad degenerativa facetaria de la columna lumbar.


Low back pain is a very common reason for emergency room consultation, it is found in approximately 60% of adults, and, within it, facet lumbar pain is one of the main causes, accounting for about 15-56% of low back pain cases. The facet joint stabilizes the spine, helps to distribute loads and has a fundamental role in support, weight distribution, and rotational movements regulation of the spine. Consequently, knowledge of the anatomy and biomechanics of this joint is helpful to have a better understanding of their contribution to the low back pain pathophysiology and, therefore, improving diagnostic and therapeutic approaches. This paper aims to review the current concepts of embryology, anatomy, biomechanics, and clinical/imaging correlation of the changes associated with lumbar degenerative facet disease

RESUMEN

Vernal keratoconjunctivitis(VKC)is a chronic bilateral inflammation of the conjunctiva, commonly associated with a personal or family history of atopy. It is characterized by severe itching, foreign body sensation, thick ropy discharge, photophobia and conjunctival injection. VKC has palpebral, limbal and mixed forms. The classical conjunctival sign in palpebral VKC is the presence of giant papillae, which are predominantly seen on the superior tarsal conjunctiva. The limbal form occurs in dark skinned individuals and the papillae tend to occur at the limbus and have a thick gelatinous appearance. Clinical findings and laboratory investigations support the presence of IgE mediated type1 hypersensitivity reaction. Involvement of CD4 T helper (Th2) driven type IV hypersensitivity has also been confirmed. There has been an increase in the prevalence of allergic disorders in recent years and exaggerated manifestations of these diseases have been recognized in patients living with Human immunodeficiency virus

Trata-se de um estudo de corte transversal e caráter correlacional, no qual, objetivou-se investigar as associações à cerca de características sociodemográficas, estresse percebido, estresse precoce e mindfulness em uma população de funcionários da categoria técnico administrativo da Universidade de São Paulo campus Ribeirão Preto. A amostra foi composta por 929 indivíduos, locados em 14 diferentes unidades do campus. Para coleta de dados que ocorreu no período de junho a dezembro de 2017, foram utilizados o Questionário Sociodemográfico, Perceived Stress Scale (PSS-14), Childhood Trauma Questionnaire e Questionário das Cinco Facetas de Mindfulness (FFMQ-BR). Os dados foram submetidos à estatística descritiva e analítica. A idade média foi de 46,15 (DP=10,57) anos e o valor de correlação para a faceta 1(não julgar a experiência interna) foi (p=,033; p<0.05). Este resultado demonstra que existem possibilidades, de que quanto maior a idade, menor é o julgamento da experiência interna. A variável estresse percebido demonstrou valores de correlação positiva com as facetas 1(p=0,345; p< 0.001); faceta 2 (agir com consciênciapiloto automático) (p=0,518; p< 0.001) e faceta 7 (agir com consciência-distração) (p=0,444 p< 0.001). Assim quanto maior o "não julgamento da "experiência interna" (faceta 1); "agir com consciência-piloto automático" (faceta 2) e "agir com consciência-distração" (faceta 7), há uma tendência à maiores níveis de estresse percebido. Tratando-se de alguns subtipos de estresse precoce, foram observados que o "Abuso Emocional" (r=0,206; p< 0.001); "Abuso físico r=0,108; p< 0.001) e "Negligência Emocional (r=0,119; p< 0.001) atingiram valores estatisticamente significativos com a faceta 1. Desta maneira, a presença do estresse precoce, ao que tudo indica, parece estar associada em julgar com menor frequência a experiência interna. Os resultados sugerem que estas varáveis são de suma importância e associam-se com os desfechos a respeito do nível de mindfulness mensurado pelas facetas da FFMQ


This cross-sectional and correlational study aimed to investigate the associations of sociodemographic characteristics, perceived stress, early stress and mindfulness in a population of administrative technical staff at the University of São Paulo campus. Ribeirão Preto. The sample consisted of 929 individuals, located in 14 different campus units. For data collection that took place from June to December 2017, we used the Sociodemographic Questionnaire, Perceived Stress Scale (PSS-14), Childhood Trauma Questionnaire and the Five Facet Mindfulness Questionnaire (FFMQ-BR). Data were submitted to descriptive and analytical statistics. The mean age was 46.15 (SD = 10.57) years and the correlation value for facet 1 (not judging internal experience) was (p = .033; p <0.05). This result demonstrates that there are possibilities that the older you are, the lower your judgment of inner experience. The perceived stress variable showed positive correlation values with facets 1 (p = 0.345; p <0.001); facet 2 (act with autopilot awareness) (p = 0.518; p <0.001) and facet 7 (act with awareness-distraction) (p = 0.444 p <0.001). Thus the greater the "non-judgment of" internal experience "(facet 1); "Acting with autopilot awareness" (facet 2) and "acting with distraction awareness" (facet 7), there is a tendency to higher levels of perceived stress. With regard to some subtypes of early stress, it was observed that "Emotional Abuse" (r = 0.206; p <0.001); "Physical abuse r = 0.108; p <0.001) and "Emotional Neglect (r = 0.119; p <0.001) reached statistically significant values with facet 1. Thus, the presence of early stress seems to be associated with judging the experience less frequently. internal. The results suggest that these variables are of paramount importance and are associated with outcomes regarding the level of mindfulness measured by the FFMQ facets
