RESUMEN

Objective:To analyze the influencing factors of failed induction of labor(IOL)in full-term singleton pregnant women,and to establish a prediction model for failed IOL.Methods:This study retrospectively analyzed the clinical data of 1483 pregnant women with full-term singleton of IOL in the Department of Obstetrics and Gy-necology,Heping Branch of General Hospital of Northern War Zone from January 1,2019 to December 31,2019.According to the outcome of IOL,the pregnant women were divided into the successful group(1108 cases)and the failed group(375 cases)of IOL.The influencing factors of failed IOL were screened to establish the prediction model through multivariate Logistic regression analysis.The receiver operating characteristic(ROC)curves and Hosmer-Lemeshow test were used to assess the predictive performance and fitting degree of the model.Results:Multivariate Logistic regression analysis showed that there were risk factors for failed IOL(OR>1,P<0.05),in-cluding elderly primiparous delivery,with no history of vaginal delivery,education level≤12 years,gestational age<40 weeks,pre-pregnancy overweight or obesity,excessive gestational weight gain,height<160 cm,cervical Bishop score before IOL<4 points,neonatal weight≥3750 g,combined IOL,suspected fetal distress,and the time from IOL to onset of labor≥24 hours,and height≥165 cm,IOL with dinoprostol suppositories were protective fac-tors for failed IOL(OR<1,P<0.05).Antepartum factors and antepartum factors combined with intrapartum fac-tors were separately used to establish model for predicting failed IOL.The area under the ROC curve(AUC)were 0.914 and 0.940,and the Youden index were 0.660 and 0.733,respectively.The prediction accuracy were 87.5%and 88.9%,respectively.Conclusions:This study screened the significant influencing factors of failed IOL,providing a theoretical basis for clinical measures to improve the success rate of IOL and constructing a pre-diction model of failed IOL,which is helpful for obstetricians and pregnant women to decide the mode of delivery together,and ensure the safety of mother and baby;on the other hand,it aims to enhance everyone′s awareness of pregnancy health care and improve the vaginal delivery rate.

@#Awake craniotomy has been gaining popularity for the last decade. It allowed maximum tumor resection while avoiding neurological morbidity. However, this technique presents several challenges to both the neurosurgeon and anesthesiologist. In this case, we present a 33-year-old male who was diagnosed with low-grade glioma in the left parieto-occipital, which required surgical resection. Anatomically, the tumor was located in the language area. Hence, it was decided to perform an awake craniotomy excision of the tumor to allow intraoperative cortical mapping to preserve language functions. Intraoperative, a subdural hematoma was noted, and severe pain occurred. Eventually, this leads to an upward herniation of the brain parenchyma. The crisis was addressed promptly with maneuvers to decrease intracranial pressure. Awake craniotomy was abandoned, and the procedure was converted to general anesthesia without the benefit of intraoperative cortical mapping. It is important to note that complications may arise during the procedure, leading to significant harm and debilitation for the patient. Prompt crisis management is necessary to address these potential issues and ensure the highest level of care is provided.

Background: There are several strategies for the induction of labor (IOL), and pharmacologic agents, including oxytocin and prostaglandins, are most commonly used in clinical practice. Compared with oxytocin, the prostaglandin E2 agent, dinoprostone, has been regarded as moreeffective for the induction of labor of LTPs with a non-dilated cervix. Objective was to compare the PGE2 gel placed intracervically with that of oxytocin given intravenously for induction of labor in both primipara as well as multipara with the comparison of Bishop’s score at the starting of induction and its improvement with both of these inducing agents in those with indications for induction with gestational age greater than 36 weeks.Methods: Hospital based prospective observational study over a period of one year on patients attending the antenatal and high-riskpregnancy clinic and scheduled for induction of labor with Bishop score <4.Results: The number of successful induction was low in primiparae group with low Bishop’s score (1-2). There were greater number of successful inductions in both primi as well as multiparae with higher Bishop’s score.Conclusions: When a primigravida had a low cervical score or an unfavourable cervix, intracervical PGE2 gel was found to be more effective at ripening the cervix and inducing labour. Both medicines were equally effective at inducing labour in cases of multigavidae with any Bishop's score and primi with a higher Bishop's score.

Purpose: To evaluate the effectiveness of repeat deep anterior lamellar keratoplasty (DALK) in patients of previous failed DALK. Methods: A retrospective analysis of records of seven patients who had undergone repeat DALK following the failure of the primary DALK was done. The indications for repeat surgery, time elapsed since the first surgery, and pre? & postoperative best?corrected visual acuity (BCVA) were noted for all the patients. Results: The follow?up period ranged between one? to four?year post repeat DALK. The indication of primary DALK was keratoconus with vernal keratoconjunctivitis (VKC) (n = 3), corneal amyloidosis (n = 2), Salzman nodular keratopathy (n = 1), and healed keratitis (n = 1). The need for repeat surgery arose when the BSCVA dropped to less than 20/200. The time interval elapsed since the first surgery ranged from two months to four years. Postoperatively, the BSCVA improved from 20/120 to 20/30 at the end of one?year post repeat DALK in all except one patient. All regrafts were clear at the most recent examination, performed after a mean period of 18 months after the secondary graft. No complication was encountered during the resurgery. The dissection of the host bed was easier in the second surgery owing to weaker adhesions. Conclusion: The prognosis for repeat DALK for failed DALK is excellent, and the outcomes of secondary grafts were comparable to those of primary DALK grafts. Re DALK offers the advantage of an easier dissection and lower chances of graft rejection compared to penetrating keratoplasty.

Background: Percutaneous device closure of atrial septal defect (ASD) has become an increasingly popular procedure as it offers several advantages. However, it is associated with infrequent, but life?threatening complications such as device embolization. Objective: To analyze the risk factors, common sites of embolization, associated complications, timing of embolization, and the treatment executed. Settings and Design: A retrospective study was performed at a tertiary referral center for cardiac services. Material and Methods: Pre?procedure, intra?procedure, and post?procedure data of patients whose ASD device embolized was collected retrospectively and analyzed for risk factors, common sites of embolization, associated complications, timing of embolization, and the treatment executed. Results: Thirty devices were embolized, out of which 13 were retrieved percutaneously in the Catheter laboratory, whereas 17 patients underwent surgery. Fourteen patients had an unfavorable septal morphology for device closure. Ten devices were embolized in the catheter laboratory, five in the intensive care unit, and two in the ward. The devices were embolized to almost all chambers of the heart and great vessels. One patient had an inferior vena cava rim tear while attempting percutaneous retrieval. One patient required a short period of total circulatory arrest (TCA) for retrieval of the device from ascending aorta, while another required a lateral position for retrieval from descending aorta. One patient required re?exploration for bleeding, while another had an air embolism and succumbed. Conclusions: Once embolization occurs, the risks associated increase manifold. Most of the surgical extractions are uneventful; however, there could be certain complications that may need repair of valvular apparatus, the institution of TCA, or the need for the lateral position. Air embolization though very rare can occur which could be fatal.

Background: Increasing cesarean section delivery rates in India and world is a serious maternal health concern. It is important to understand the trends, reasons behind this change and to find ways to achieve optimum cesaerean section (CS) rates. As per the latest data national family health survey 2019-21 (NFHS 5), CS rates at population level in India seems to be 22% while WHO recommends 10-15% threshold. So, we aim to analyze trend of CS and evaluate it according to Robson’s 10 group classification at tertiary care hospital in Rewa, Madhya Pradesh, India.Methods: A cross sectional study was conducted at department of obstetrics and gynecology, Shyam Shah medical college Rewa for 3 years from May 2019 to April 2022 on all deliveries occurring on or after 28 weeks of gestation by cesarean section.Results: Total of 26552 deliveries over 3 years period were analyzed, of these 7484 were CSs (28.18%). Overall C section rate increased from 18.97% in 2019 to 39.95% in 2022. Major contributors to this increase were Robson’s group 5-32.58%, Robson’s group 1-29.45% and Robson’s group 2-12.22%.Conclusions: Robson’s group 1, 2 and 5 were major contributors to overall increased cesarean section rates. Fetal compromise, meconium aspiration risk, obstructed labor and cesarean scar tenderness were underlying indications for most of the cesarean sections done. Efforts should be made to implement standard protocol to reduce primary cesarean section rates.

@#Intraosseous hemangioma is a benign, rare neoplasm that accounts to 0.5 - 1% of all benign tumors of bones.1, 2 While most hemangiomas arise from soft tissues, it is uncommon for it to arise from bones.2 The most common sites of growth are in the vertebral body and the calvarium with frontal bone making up approximately 45% of calvarial cases.2,3 However, they are also encountered in the head and neck with sites such as the skull (53%), mandible (10.7%), nasal bones (9%), and cervical spine (6%).4 In the mandible, the body is mostly affected and 65% are found in the molar and premolar region.1 They are more common in adult females with peaks at the second and fifth decades of life.1-3 Hemangioma of the mandible is difficult to diagnose due to its nonspecific clinical presentation and radiographic features. It mimics various mass lesions in the mandible such as giant cell granuloma, fibrous dysplasia, multiple myeloma, osteosarcoma, ameloblastoma and keratocysts. Therefore, a comprehensive history taking and physical examination plus examination of the imaging studies available and tissue biopsy all play important roles in arriving at the final diagnosis.5 We present the case of an aggressive mandibular hemangioma in a young boy and our management involving a failed fibular free flap reconstruction.

OBJECTIVE@#To investigate the short-term effectiveness of transverse antecubital incision in the treatment of failed closed reduction of Gartland type Ⅲ supracondylar humeral fractures (SHFs) in children.@*METHODS@#Between July 2020 and April 2022, 20 children with Gartland type Ⅲ SHFs who failed in closed reduction were treated with internal and external condylar crossing Kirschner wire fixation through transverse antecubital incision. There were 9 boys and 11 girls with an average age of 3.1 years (range, 1.1-6.0 years). The causes of injuries were fall in 12 cases and fall from height in 8 cases. The time from admission to operation ranged from 7 to 18 hours, with an average of 12.4 hours. The healing of the incision and the occurrence of complications such as nerve injury and cubitus varus were observed after operation; the elbow flexion and extension range of motion after removing the gypsum, after removing the Kirschner wire, and at last follow-up were recorded and compared, as well as the elbow flexion and extension and forearm rotation range of motion at last follow-up between healthy and affected sides; the Baumann angle was measured on the X-ray film, and the fracture healing was observed. At last follow-up, the effectiveness was evaluated according to the Flynn elbow function evaluation criteria.@*RESULTS@#All incisions healed by first intention, and there was no skin necrosis, scar contracture, ulnar nerve injury, and cubitus varus. Postoperative pain occurred in the radial-dorsal thumb in 2 cases. The gypsum was removed and elbow flexion and extension exercises were started at 2-4 weeks (mean, 2.7 weeks) after operation, and the Kirschner wire was removed at 4-5 weeks (mean, 4.3 weeks). All the 20 patients were followed up 6-16 months, with an average of 12.4 months. The fracture healing time was 4-5 weeks, with an average of 4.5 weeks, and there was no complication such as delayed healing and myositis ossificans. The flexion and extension range of motion of the elbow joint gradually improved after operation, and there were significant differences between the time after removing the gypsum, after removing the Kirschner wire, and at last follow-up ( P<0.017). There was no significant difference in the flexion and extension of the elbow joint and the forearm rotation range of motion between the healthy and affected sides at last follow-up ( P>0.05). There was no significant difference in Baumann angle between the time of immediate after operation, after removing the Kirschner wire, and at last follow-up ( P>0.05). According to Flynn elbow function evaluation standard, 16 cases were excellent and 4 cases were good, the excellent and good rate was 100%.@*CONCLUSION@#The treatment of Gartland type Ⅲ SHFs in children with failed closed reduction by internal and external condylar crossing Kirschner wire fixation through transverse antecubital incision has the advantages of complete soft tissue hinge behind the fracture for easy reduction and wire fixation, small incision, less complications, fast fracture healing, early functional recovery, reliable reduction and fixation, and can obtain satisfactory results.

Objective@#To describe a novel glaucoma drainage device fashioned from a scleral buckle and nasolacrimal silicone tube. @*Method@#This is a case report and a description of a surgical technique.@*Results@#A 6-year-old boy with congenital glaucoma who underwent 2 failed filter surgeries had medically uncontrolled intraocular pressures (IOP). Due to limited funds for a second glaucoma drainage device, the author utilized a portion of a scleral buckle and nasolacrimal silicone tube – the same raw materials used in the Schocket implant – to fashion a novel, non-valved glaucoma shunt that was implanted in the patient’s eye. Post-operatively, IOP was 8-10mmHg. This was sustained for 12 months without the need for supplemental anti-glaucoma medications. Postoperative complications included tube migration resulting in localized corneal decompensation and cataract formation. @*Conclusion@#The alternative GDD made from a portion of a scleral buckle and silicone tube may be an effective and economical alternative to the GDDs available in the market.

Repairing glans dehiscence after failed hypospadias repair is challenging for pediatric surgeons. Here, we introduced and evaluated a newly modified Mathieu technique, Mathieu combined tunnel (MCT), which involves multiple custom-designed flaps for the shortage of flap source material after repeated operations; we also constructed a tunnel to avoid the glans incision that may carry new risks of dehiscence. This retrospective study included 26 patients who were consecutively admitted to the First Affiliated Hospital of Sun Yat-Sen University (Guangzhou, China) for glans dehiscence repair after failed hypospadias repair from October 2014 to October 2020; sixteen patients underwent surgery using the MCT (MCT group) and ten patients underwent surgery using the tubularized incised plate (TIP) technique (TIP group). The operative time, blood loss, postoperative complications, normal urethral meatus rate, success rate, and Hypospadias Objective Penile Evaluation (HOPE) score were compared between the two groups. The MCT group achieved an overall satisfactory penile appearance and voiding function, with a higher rate of normal urethral meatus (15/16, 93.8%) and a lower rate of glans dehiscence (1/16, 6.2%), compared with the TIP group (70.0% and 30.0%, respectively). However, these differences were not statistically significant, possibly because of the limited number of patients (all P > 0.05). Mean postoperative HOPE scores were similar in the MCT group (mean ± standard deviation: 8.83 ± 0. 89) and TIP group (8.94 ± 0.57) (P > 0.05). No significant differences were found between the two groups in terms of blood loss and success rate, nor in the rates of various complications (e.g., fistula, urethral stricture, and glans dehiscence). In conclusion, the MCT technique appears to be feasible and reliable for repairing glans dehiscence after failed hypospadias repair.

Resumen OBJETIVO: Evaluar los desenlaces con la aplicación del balón de Bakri para contener la hemorragia posparto. MATERIALES Y MÉTODOS: Estudio descriptivo, analítico, observacional y retrospectivo efectuado en pacientes atendidas en el Hospital Central, San Luis Potosí, a quienes de enero 2012 a diciembre 2017 se les colocó un balón de Bakri para contener la hemorragia posparto. Se excluyeron los expedientes incompletos y las pacientes a quienes se colocó el balón en otra institución. El análisis estadístico se procesó en una hoja de Excel Office 2013. RESULTADOS: Se realizaron 143 colocaciones del balón de Bakri, 84.06% (n = 121) durante el posparto, 9% (n =13) en la poscesárea y 6.2% (n = 9) durante la cesárea. La colocación del balón se consideró exitosa en 90.2% (n = 129); hubo 14 (9.79%) fallas en el control de la hemorragia que ameritaron otro procedimiento. El 48.9% de las pacientes no tenían factores de riesgo. El promedio de permanencia del balón fue de 20 h. Se registró una muerte obstétrica directa, por choque hipovolémico. CONCLUSIONES: El éxito del balón de Bakri para contener la hemorragia obstétrica fue similar al reportado en la bibliografía. Su aplicación es una estrategia de segunda línea, secundaria a atonía y complicaciones bajas.


Abstract OBJECTIVE: To evaluate the outcomes with the application of the Bakri balloon to contain postpartum hemorrhage. MATERIALS AND METHODS: Descriptive, analytical, observational and retrospective study carried out in patients attended at the Central Hospital in San Luis Potosi who between the months of January 2012 to December 2017 had a Bakri balloon placed to contain postpartum hemorrhage. Incomplete records and patients who had the balloon placed in another institution were excluded. Statistical analysis was processed in an Excel Office 2013 spreadsheet. RESULTS: 143 Bakri balloon placements were performed, 84.06% (n = 121) during postpartum, 9% (n =13) post cesarean section and 6.2% (n = 9) during cesarean section. Balloon placement was considered successful in 90.2% (n = 129); there were 14 (9.79%) failures to control bleeding that warranted another procedure. Of the patients, 48.9% had no risk factors. The average length of stay was 20 hours. One direct obstetric death was recorded, due to hypovolemic shock. CONCLUSIONS: The success of the Bakri Balloon in containing obstetric hemorrhage was similar to that reported in the literature. Its application is a second line strategy, secondary to atony and low complications.

Resumen La fibrosis epidural es un hallazgo clínico y radiológico el cual se encuentra relacionado directamente en su génesis con los procedimientos quirúrgicos realizados en la columna vertebral. Esta patología es la causa más frecuente del síndrome de cirugía fallida de columna, manifestada clínicamente como un dolor equiparable en severidad al presentado previo a la intervención quirúrgica realizada. La fibrosis epidural es parte del proceso de cicatrización usual de los tejidos circundantes de la columna vertebral, en otras palabras, es esperable que esta suceda, sin embargo, solo una mínima cantidad de las personas intervenidas desarrollan clínicamente la sintomatología. La importancia del análisis de esta patología radica en que estas valoraciones son ingresadas al Departamento de Medicina Legal por asunto de reagravación de alguna secuela ocasionada por algún evento traumático. Es imprescindible conocer a profundidad los diferentes aspectos etiológicos, fisiopatológicos, clínicos, radiológicos y terapéuticos que involucran esta patología vertebral, así como, el respectivo análisis médico legal en este tipo de casos, tomando en consideración todos los elementos de juicio que permitan relacionar este hallazgo con algún hecho denunciado. En el presente artículo se realizará un análisis médico legal de un caso de reagravación por fibrosis epidural valorado en el Departamento de Medicina Legal de Costa Rica, donde el peritado interpone una denuncia contra el ente asegurador al ser rechazada la posibilidad de reapertura del riesgo laboral presentado.


Abstract The epidural fibrosis is a clinical and radiological finding that is related with the surgical procedures performed on the spine. This pathology is the most frequent cause of failed back syndrome or post laminectomy syndrome, that is clinically manifested by an acute pain comparable in severity with the surgical intervention. Epidural fibrosis is part of the usual healing process of the surrounding tissues of the spine, in other words, it's to be expected as a result of this procedures, nevertheless, only a small number of people who have been intervened, actually develop the symptoms clinically. The importance of this pathology lies in the fact that these assessments are attended in the Legal Medicine Department due to the aggravation issues of any sequel caused by a traumatic event. It is essential to know such things as the etiology, pathophysiology, clinical manifestations, radiological findings, and the therapeutic aspects that involve this vertebral disease, as well as, the respective forensic analysis, in order to use all the elements of judgment that allows to relate what is documented with the denounced findings.

Introducción: El índice de vulnerabilidad CAPE es un índice global de política exterior que identifica a los países a los que se dará prioridad para recibir ayuda exterior. Ofrece un enfoque evidenciado, estructurado y razonado para utilizar la ayuda en acuerdos bilaterales con la salud mental como base. La presente versión está diseñada específicamente para la región de América Latina y el Caribe (ALC), que comprende 33 países. Objetivos: Identificar los países a ser priorizados para la ayuda externa, a través de la versión ALC del Índice de Vulnerabilidad CAPE (CAPE VI-LAC). Materiales y métodos: Al igual que con la versión global del Índice de Vulnerabilidad CAPE, consideramos varios índices o medidas a nivel de país que indican el estado de salud o que puede influir en la salud. Para el análisis, calificamos a los 20 peores países. Utilizamos 26 indicadores validados y disponibles internacionalmente para explorar y realizar el análisis. Resultados: Las cifras y el mapa muestran los 32 países que figuraron entre los 20 peores en al menos un indicador y también los 12 peores dentro de la CAPE VI-LAC en su conjunto. De los 33 países de ALC, sólo San Cristóbal y Nieves no figuraba en ninguno de los 20 países peores en ningún momento. Conclusión: Lo que podemos concluir con un alto grado de certeza es que los 12 países con peores puntuaciones son posiblemente estados frágiles; países donde los gobiernos no tienen el control o la autoridad completos, a menudo son represivos y corruptos, participan en graves abusos de los derechos humanos y se caracterizan por la inestabilidad política de diversas formas, la desventaja por los cambios climáticos extremos, la pobreza extrema, la desigualdad social y étnica divisiones, incapaces de proporcionar servicios básicos y sufren focos de insurgencia en forma de terrorismo, que a menudo son violentos y brutales. Los gobiernos, los donantes de ayuda, las organizaciones regionales y los profesionales y las asociaciones de salud mental deben trabajar juntos para abordar estas situaciones.


Introduction: The CAPE Vulnerability Index is a global foreign policy index that identifies the countries to be prioritise for foreign aid. It offers an evidenced, structured and reasoned approach to using aid in bi-lateral agreements with mental health as a foundation. The present version is specifically design for Latin America and Caribbean (LAC) region, which comprises of 33 countries. Objectives: To identify the countries to be prioritized for foreign aid, through the LAC version of the CAPE Vulnerability Index (CAPE VI-LAC). Materials and methods: Like with the CAPE Vulnerability Index global version we consider various indices or measures at country level that indicate health status or what may influence health. For the analysis we score the worst 20 countries. We used 26 internationally available and validated indicators to explore and perform the analysis. Results: The figures and map show the 32 countries that featured in the worst 20 in at least one indicator and also the worst 12 within the CAPE VI-LAC as a whole. Of the 33 LAC countries only St Kitts and Nevis did not feature in any of the worst 20 countries at any time. Conclusion: What we can conclude with a great degree of certainty that the worst 12 scoring countries are possibly fragile states; countries where the Governments do not have complete control or authority, are often repressive and corrupt, participate in serious human rights abuses and are characterised by political instability of various forms, disadvantage by the extremes of climate changes, extreme poverty, inequality, social and ethnic divisions, unable to provide basic services and suffer from pockets of insurgency in the form of terrorism, which are often violent and brutal. Governments, aid donors, regional organizations, and mental health professionals and associations should work together in order to address these situations.

RESUMEN

Abstract Introduction: In the management of the anticipated difficult airway (DA), awake intubation is the strategy of choice. In this context, flexible fibroscopy is the tool most widely used as the first choice. However, there is always the rare case where it may fail. Important findings: Six successful rescue cases using videolaryngoscopy following failed fibroscopy in patients with anticipated DA, and 1 case of rescued extubation of an airway previously secured with fiberoptic bronchoscopy. Conclusion: Videolaringoscopy may be an adequate tool to use as a backup plan for the management of an anticipated DA.


Resumen Introducción: En el manejo de vía aérea difícil anticipada la estrategia de elección es la intubación con el paciente despierto. En este contexto, la fibroscopia flexible es la herramienta más utilizada como plan A; sin embargo, en raras ocasiones puede fallar. Hallazgos importantes: Se presentan seis casos de rescates exitosos con videolaringoscopia luego de fibroscopia fallida en pacientes con vía aérea difícil anticipada, y un caso de rescate de extubación de vía aérea asegurada previamente con fibrobroncoscopio. Conclusión: La videolaringoscopia puede ser una adecuada herramienta como plan B para el manejo de la vía aérea difícil anticipada.

Background: In acute coronary syndrome, prompt restoration of myocardial blood flow is essential to optimize myocardial salvage and decrease mortality. Coronary artery reperfusion, if performed in a timely manner improves clinical outcomes compared to no reperfusion. Fibrinolysis and percutaneous interventions can restore blood flow in an acutely occluded coronary artery in most of the patients; but in a few subset of patients coronary artery bypass grafting (CABG) is needed to effectively restore blood flow.Methods: A retrospective study was conducted among all CABGs performed during March 2016 - February 2018. During this period, 366 CABGs were performed and 57 patients underwent emergency CABG as per the inclusion criterias. Pre-operative, intra-operative and post-operative data was analysed of these patients. Patients were divided into four groups based on the time of surgery from the time of onset of myocardial infarction.  Results: 57 patients underwent emergency CABG with males constituting 94.7% of the study population. 78.9% of them had an ejection fraction less than 35%. 52% presented with low cardiac output status, 10% needed intra-aortic balloon pump support and 15% required mechanical ventilation prior to surgery. All patients received an average of 2.8 grafts. Mortality was 5%Conclusions: Immediate surgical revascularisation of patients presenting with acute MI is feasible. Emergency CABG not only treats the culprit lesion but also achieves complete revascularization and offers a clear advantage for patients. The optimal timing of CABG for patients with acute MI remains difficult to establish.

ABSTRACT Objective To identify the incidence and possible risk factors associated with rod breakage in patients who underwent vertebral column resection (VCR) or pedicle subtraction osteotomy (PSO) to treat complex deformities of the spine. Methods Retrospective analysis of a series of 32 patients operated from 2014 to 2018 in a single center. The patients were analyzed for demographic (sex, age), biometric (BMI), radiographical (pre- and postoperative angular variations), and surgical (arthrodesed and osteotomized levels) characteristics. Descriptive analyses were performed for the numerical variables (mean, standard deviation, maximum, median, and minimum) and frequency analysis was performed for the categorical variables. Logistic regression analysis was performed for the dependent variable "rod breakage", using a stepwise technique to select the variables for the best model, assuming statistical significance of 0.05. Results Of the 32 patients selected, rod breakage occurred in 34.4%. Mean age was 36.6 years (± 19.8), ranging from 10 to 74 years, and the mean BMI was 25.1 (±6.0). Most patients were subjected to VCR (75.0%), were males (56.2%) and did not smoke (90.6%). Logistic regression analysis showed that "arthrodesed levels" were was positively associated with rod breakage (OR 1.72; CI95%: 1.13-3.10; p<0.05). The other factors were not associated with breakage. Conclusion Rod breakage is a frequent complication after three-column osteotomy, especially in long constructions. RESUMO Objetivos Identificar a incidência e possíveis fatores de risco relacionados à quebra de hastes em pacientes submetidos à ressecção colunar vertebral (RCV) ou osteotomia de subtração pedicular (OSP) para o tratamento cirúrgico de doenças complexas da coluna vertebral. Métodos Análise retrospectiva de uma série de 32 pacientes operados de 2014 a 2018, em um único centro. Os pacientes foram avaliados quanto às características demográficas (sexo, idade), biométricas (IMC), radiológicas (variação angular antes e após a correção) e cirúrgicas (níveis artrodesados, níveis osteotomizados). As análises descritivas foram realizadas para as variáveis numéricas (média, desvio-padrão, máximo, mediana e mínimo) e, para as variáveis categóricas foi realizada a análise da frequência. Realizou-se análise de regressão logística para a variável dependente "quebra", utilizando a técnica stepwise para seleção das variáveis que compõem o melhor modelo, considerando o nível de significância de 0,05. Resultados Foram selecionados 32 pacientes; a proporção de quebra foi de 34,4%. Observou-se que a média de idade foi de 36,6 anos (± 19,8 anos), distribuindo-se entre 10 e 74 anos, e o IMC médio foi 25,1 (± 6,0). Verificou-se que a maior parte dos indivíduos foi submetida a técnica RCV (75,0%), era do sexo masculino (56,2%) e não fumava (90,6%). Na análise de regressão logística, "níveis artrodesados" associou-se positivamente à quebra (OR 1,72; IC95%; 1,13-3,10; p < 0,05). Os outros fatores não se associaram à quebra. Conclusão A quebra de hastes é uma complicação frequente das osteotomias das três colunas, principalmente, em construções longas. Nível de evidência III; Estudo Retrospectivo.

Métodos Análisis retrospectivo de una serie de 32 pacientes operados desde 2014 hasta 2018, en un único centro. Los pacientes fueron evaluados cuanto a las características demográficas (sexo, edad), biométricas (IMC), radiológicas (variación angular antes y después de la corrección) y quirúrgicas (niveles artrodesados, niveles osteotomizados). Los análisis descriptivos fueron realizados para las variables numéricas (promedio, desviación estándar, máximo, mediana y mínimo) y, para las variables categóricas fue realizado el análisis de frecuencia. Se realizó análisis de regresión logística para la variable dependiente «rotura¼, utilizando la técnica stepwise para selección de las variables que componen el mejor modelo, considerando el nivel de significancia de 0.05. Resultados Fueron seleccionados 32 pacientes; la proporción de rotura fue de 34,4%. Se observó que el promedio de edad fue de 36,6 años (± 19,8 años), distribuyéndose entre 10 a 74 años, y el IMC promedio fue de 25,1 (± 6,0). Se verificó que la mayor parte de los individuos fue sometida a la técnica RCV (75,0%), era del sexo masculino (56,2%) y no fumaba (90,6%). En el análisis de regresión logística, los "niveles artrodesados" se asociaron positivamente a la rotura (OR 1,72; IC 95%; 1,13-3,10; p <0,05). Los otros factores no se asociaron a la rotura. Conclusión La rotura de varillas es una complicación frecuente de las osteotomías de tres columnas, principalmente en construcciones largas. Nivel de evidencia III; Estudio Retrospectivo.

OBJECTIVE@#To observe the effect of warming acupuncture on uterine blood perfusion in the patients with failed high-quality freeze-thawed embryo transfer (FET) and explore its effect mechanism on the improvement of clinical pregnancy rate after re-tranfer.@*METHODS@#A total of 72 patients of failed high-quality FET were randomized into an observation group and a control group, 36 cases in each one. In the observation group, after the menstrual period ended, warming acupuncture started at the acupoints located on the abdomen, e.g. Qihai (CV 6), Guanyuan (CV 4), Zhongji (CV 3) and Qugu (CV 2) and those on the lumbar sacral region, e.g. Shenshu (BL 23), Mingmen (GV 4) and Yaoyangguan (GV 3), 50 min in each treatment, once daily, at the interval of 1 day after 4-day treatment. The treatment was discontinued till the patients were at the ovulatory stage. In the control group, capsules were taken orally and continuously after the end of menstrual period, 3 capsules each time, three times a day and stopped at the ovulatory stage. The treatment of one menstrual cycle was taken as one course and the treatment for 3 menstrual cycles was required. Before and after treatment, the uterine artery pulsation index (PI), endometrial thickness, endometrial type, uterine blood perfusion, the recovery time of sufficient uterine blood flow, the endomentrial receptivity (ER) during the implantation window period and the clinical pregnancy rate were observed in the two groups.@*RESULTS@#After treatment, the endometrial thickness was increased and PI decreased obviously in the two groups (<0.05) and PI in the observation group was lower than that in the control group (<0.05). After treatment, the proportion of type a and type A of endometrium was increased markedly in the two groups (<0.05) and the proportion in the observation group was higher than the control group (<0.05). After treatment, the case proportion of sufficient uterine blood flow was increased obviously in the two groups (<0.05) and the value in the observation group was higher than the control group [83.3% (30/36) vs 69.4% (25/36), <0.05]. After treatment, the proportion of ER during the implantation window period was increased remarkably in the two groups (<0.05) and the value in the observation group was higher than the control group [72.2% (26/36) vs 50.0% (18/36), <0.05]. The recovery time of sufficient uterine blood flow in the observation group was shorter than the control group (<0.05) and the clinical pregnancy rate was higher than the control group [47.2% (17/36) vs 33.3% (12/36), <0.05].@*CONCLUSION@#Warming acupuncture enhances uterine blood perfusion and improves uterine endometrial receptivity so that the clinical pregnancy rate is increased after re-transfer in the patients with failed high-quality freeze-thawed embryo transfer.

Objective:To evaluate the incidence of failed induction of labour (FIOL), and determined whether this resulted from non-adherence to the induction of labour protocol.Design and Methods:This study was a retrospective cohort analysis of all term patients who underwent induction of labour from 1st January to 30th June 2018. Materials:GPHC IOL protocol, inpatient charts, nurses and theatre log books, electronic handing over records, Microsoft Excel, Word and www.medcalc.org/calc/odds_ratio.php.Results:They were 388 patients induced that met the eligibility criteria outlined, 298 were included in the study while 90 were excluded because of failure to locate the patient records. 77,5% ofthe term patients induced achieved spontaneous vaginal delivery and 21,8% delivered by cesarean section, the nulliparous patients had a higher incidence of cesarean delivery at 33,1% (P=0,0514), compared with the multiparous patients 10,6% (P=0,0158). Therate of FIOL was 10,4% (31 patients). 48,4% of these diagnosis were not in accordance with the labour induction protocol (P=<0,0001).Conclusions:The incidence of FIOL was 10,4%, of which 48,4% was not in accordance to the protocol. Standardization of management and adherence to the labour induction protocol decreases the time to delivery and cesarean section rate. Recommendations:The diagnosis of FIOL should be made in accordance with the international guidelines adapted by GPHC’s induction protocol.

Purpose: To evaluate the clinical factors associated with repeat Descemet stripping automated endothelial keratoplasty (DSAEK) or penetrating keratoplasty (PKP) in cases of failed DSAEK. Methods: Retrospective observational study of cases with failed DSAEK admitted to our center for a repeat keratoplasty over 5 years (January 2013–Decemeber 2017) was undertaken. Demographic and perioperative details of all cases and type of repeat keratoplasty were recorded. Logistic regression analysis was performed to analyze the factors affecting the type of repeat keratoplasty. Results: total of 94 eyes with failed DSAEK were evaluated. Repeat DSAEK was performed in 66% and PKP in 34% of cases. Significantly increased odds for requiring PKP were observed in association with stromal scarring [odds ratio (OR) = 2.9, P = 0.018)], trainee surgeons (OR = 4.05, P = 0.008), intraoperative complications (OR = 4.58, P = 0.003), scleral fixated intraocular lens or anterior chamber intraocular lens in situ (OR = 33.8, P < 0.001), secondary glaucoma (OR = 3.02, P = 0.015), peripheral anterior synechiae (OR = 8.6, P < 0.001), preoperative corneal thickness (OR = 1.01, P < 0001), time to primary surgery (OR = 1.03, P = 0.03), post-DSAEK host thickness (OR = 1.01, P < 0.001), and time interval from graft failure to regraft (OR = 1.18, P < 0.001). All eyes with congenital hereditary endothelial dystrophy, bee-sting-induced corneal decompensation, Axenfeld-Rieger syndrome, and multiple failed grafts underwent secondary PKP. All cases (nine eyes) that required surgical intervention for secondary glaucoma underwent secondary PKP (P < 0.001). Conclusion: Repeat DSAEK is feasible in up to two-third of cases of failed DSAEK. A PKP is required in one-third of cases, and various preoperative, intraoperative and postoperative factors are associated with unsuitability for repeat DSAEK.

Abdominal pregnancy is a rare form of ectopic pregnancy usually associated with fetal death among other complications, although very rare cases of live births have been reported. There is also a high risk of maternal mortality. A high index of suspicion is required to make a preoperative diagnosis as diagnosis from history, examination and ultrasound is often missed. Misdiagnoses as an intrauterine pregnancy usually occur. This misdiagnosis makes management of patients with an abdominal pregnancy a challenge and may affect treatment outcome. We managed a 35 year old pregnant multipara who was referred to us on account of repeated failed attempts at induction of labour for intrauterine fetal death. Three obstetric ultrasound scans done during the course of patient’s management reported an intrauterine dead fetus. We also failed to achieve uterine evacuation. We resorted to carry out a hysterotomy and following laparotomy, we found an abdominal pregnancy. This finding was unexpected by us, however, we delivered the dead fetus and was able to successfully manage the placenta. Discovering an abdominal pregnancy at surgery carried out for a supposed intrauterine pregnancy is usual for many cases of abdominal pregnancy. Clinicians should be aware of the clinical signs and symptoms that raise a suspicion of abdominal pregnancy as prompt preoperative diagnosis of abdominal pregnancy helps to plan and offer early and appropriate intervention. This reduces the incidence of maternal mortality usually due to massive intra-abdominal haemorrhage arising from delayed diagnosis and poor placenta management.
