RESUMEN

Abstract This case report describes an esthetic treatment to improve the shape and alignment of the anterior teeth, reestablishing smile harmony, using feldspathic porcelain veneers. Results of clinical follow up after 36 months are also presented. The advantages, disadvantages and limitations of the technique are detailed with reference to the relevant literature. This suggests that the success of treatment depends on adequate conditions of bonding between the veneers and the tooth complex, which involves parameters such as the strength and durability of the bond interface. Therefore, the clinical success of feldspathic porcelain veneers depends on the accurate selection of cases and correct execution of clinical and laboratory procedures. The rehabilitation involved from first right premolar to the left with feldspathic porcelain veneers made on refractory dies. After the 3-year follow up, excellent clinical results and patient satisfaction were achieved.


Resumo Este relato de caso descreve um tratamento estético para melhorar a forma e alinhamento dos dentes anteriores, restabelecendo a harmonia do sorriso por meio de laminados de porcelana feldspática. Resultados de acompanhamento clínico longitudinal de 36 meses também são apresentados. As vantagens, desvantagens e limitações da técnica são detalhadas com referência na literatura. Isto sugere que o sucesso do tratamento depende de adequadas condições de união entre o laminado e o complexo dental, o qual envolve parâmetros tais como resistência e durabilidade na interface de união. Assim, o sucesso clínico de laminados de porcelana feldspática depende de uma precisa seleção do caso e correta execução dos procedimentos clínicos e laboratoriais. A reabilitação envolveu de primeiro pré-molar direito a esquerdo com laminado de porcelana feldspática confeccionada em matriz refratária. Após 3 anos de acompanhamento, excelentes resultados clínicos e satisfação do paciente foram verificados.

Introducción: Los procedimientos restauradores indirectos son comunes en la clínica diaria actual. Se debe tener cuidado especial en la cementación, pues la gran mayoría de los fracasos ocurren en la interfase diente/restauración. Por esto es indispensable un adecuado tratamiento de superficie de la restauración para lograr una adhesión óptima a la superficie dentaria. Materiales y Métodos: Treinta cilindros de porcelana VITA VM 13 con dimensiones de 5 mm de altura y 7 mm de diámetro fueron separadas aleatoriamente en 3 grupos (n=10) de acuerdo con los tratamientos recibidos. Grupo I (control) - sin acondicionamiento superficial; Grupo II - grabado con ácido fluorhídrico al 9.6 por ciento por 1 minuto; Grupo III - grabado con ácido fluorhídrico al 9.6 por ciento por 2 minutos. Estas fueron probadas en cuanto a su resistencia de unión a la tracción en una máquina universal para pruebas (INSTRON modelo 4411, 3M). Los valores, expresados en MPa, fueron analizados estadísticamente a un nivel de significación del 5 por ciento, usando el Análisis de la Varianza (ANOVA) de un factor. Resultados: En relación con la prueba de resistencia de unión a la tracción, se encontraron los siguientes valores medios para cada grupo: Grupo I (control) - x=3.35 MPa; Grupo II - x=4.64 MPa; Grupo III - x=3.80 MPa. Conclusiones: Según los resultados obtenidos, se puede concluir que el tratamiento de superficie con ácido fluorhídrico durante 1 minuto, promueve microrretenciones que aumentan significativamente los valores de resistencia a la tracción, que puede traducirse en un beneficio clínico mayor.


Aim: Indirect restorative procedures are common in the current daily practice. Special care must be taken in the cement, as the vast majority of failures occurs at the interface tooth /restoration. Therefore a proper surface treatment of the restoration is essential to achieve optimum adhesion to the tooth surface. Materials and Methods: Thirty porcelain cylinders VITA VM 13 with dimensions of 5 mm in height and 7 mm in diameter were randomly divided into 3 groups (n=10) according to the treatments received. Group I (control) - no surface conditioning; Group II- recorded with 9.6 percent hydrofluoric acid for 1 minute; Group III - recorded with 9.6 percent hydrofluoric acid for 2 minutes. Their tensile bond strength was tested in a universal testing machine (Instron Model 4411, 3M). The values, expressed in MPa, were statistically analyzed at a level of significance of 5 percent, using Analysis of Variance (ANOVA) of a factor. Results: In relation to the bond strength test, tensile strength, revealed the following values for each group: Group I (control) - x=3.35 MPa, Group II - x=4.64 MPa, Group III - x=3.80 MPa. Conclusions: According to the results, it can be concluded that the surface treatment with hydrofluoric acid for 1 minute promotes microretentions, which significantly increase the values of bond strength, tensile strength. This can result in a higher clinical benefit.

RESUMEN

PURPOSE: This in vitro study evaluated shear bond strengths of surface treatment porcelains with four porcelain repair systems simulating intraoral bonding of composite resin to feldspathic porcelain or pressable porcelain. MATERIAL AND METHODS: Eighty Porcelain disks were prepared. Group A: forty disk specimens were fabricated with Feldspathic Porcelain(Omega(R)900, Vident, Menlo Park, CA, USA). Group B: forty disk specimens were fabricated with Pressable Porcelain(IPS Empress 2 ingot, Ivoclar-Vivadent, Schaan, Liechtenstein, Germany). Each groups was divided into 4 subgroups and composite resin cylinders were bonded to specimen with one of the following four systems: Clearfil Porcelain Bond(L. Morita, Tustin, CA, USA), Ulradent Porcelain Etch.(Ultradent, Salt Lake City, UT, USA), Porcelain Liner-M(Sun Medical Co., Kyoto, Japan), Cimara Kit(Voco, Germany). After surface conditioning with one of the four porcelain repair systems, substrate surfaces of the specimen were examined microscopically(SEM). Shear bond strengths of specimens for each subgroup were determined with a universal testing machine (5mm/min crosshead speed) after storing them in distilled water at 37+/-1 degrees C for 24 hours. Stress at failure was measured in MPa, and mode of failure was recorded. Differences among four repair systems were analyzed with two way ANOVA and Duncan test at the 95% significance level. RESULTS: In the scanning electron photomicrograph of the treated porcelain surface, hydrofluoric acid etched group appeared the highest roughness. The shear bond strength of the phosphoric acid etched group was not significantly(p>0.05) different between feldspathic porcelain and pressable porcelain. But in no treatment and roughened with a bur group, the shear bond strength of the feldspathic porcelain was significantly higher than that of the pressable porcelain. In hydrofluoric acid etched group, the shear bond strength of the pressable porcelain was significantly higher(p0.05). However in the other groups, there were significant differences in shear bond strengths between feldspathic porcelain and pressable porcelain(p<0.05).
