RESUMEN

Objective To analyze the outcome of oncology and pregnancy on patients with early-stage cervical cancer after undergoing fertility function retention surgery via laparoscopy.Methods Sixty-seven patients with early cervical cancer treated in Huangshi Central Hospital from January 2011 to January 2015 were selected as the research subject;27 patients in the research group were given fertility-sparing operation,also known as transvaginal cervical cold knife conization(CKC) and laparoscopic pelvic lymphadenectomy (PLD) or total laparoscopic uterine cervix extensive resection(RT)),and the patients in the control group (40 cases)were given laparoscopic uterine extensive resection (RH);the perioperative condition,intraoperative condition and postoperative recurrence of two groups were compared,and the postoperative pregnancy status of the patients in the research group were analyzed.Results Among the 27 patients in the research group,2 patients whose intraoperative pathological examination results did not meet the standards for the fertility-sparing operation were finally treated with RH,and the other 25 patients were successfully given CKC+PLD or RT;In the research group,the intraoperative blood loss,postoperative exhaust time and hospital stay were better than those in the control group,and the differences were statistically significant (intraoperative blood loss:(85.6 + 25.5) ml,(102.5 ± 31.2) ml,t =2.429,P =0.018;postoperative exhaust time:(1.4± 0.3) d,(1.7 ±0.5) d,t =3.065,P =0.003;hospital stay:(10.2±2.4) d and (12.3±3.5),t =2.913,P=0.005),there was no statistically significant difference between the two groups in the operation time,incidence of urinary retention and postoperative infection (P>0.05);the differences between the two groups in the number of removed pelvic lymph nodes,length of cardinal ligament,length of uterosacral ligament,length of removed vaginal were not statistically significant(P>0.05);after 24 months of follow-up,the difference in postoperative recurrence rate of the two groups(4.0% (1/25) vs.5.0%(2/40)) was not statistically different (P>0.05);among 25 patients with fertility-sparing operation,20 patients had pregnancy desire,and 6 patients were successfully pregnant(6/ 20),including 2 cases of full-term labor,2 cases of preterm delivery and 2 cases of abortion.Conclusion Fertility-sparing surgery via laparoscope of early cervical cancer has good curative effect and reliable security,but the outcome of pregnancy is not ideal,and need to be improved.

Breast cancer is one of the most common malignant tumors in women,the trend of young women with breast cancer in China is obvious.Some young patients in the diagnosis of breast cancer are unmarried,so they have a certain degree of expectation of the protection of ovarian function and reali-zation of the right.In the treatment of breast cancer,the doctors should understand the effects to gonadal function.In chemotherapy,for some patients with ovarian function inhibitors have a protective effect to o-varian fuction.At present,there are some methods to protect the reproductive function,such as cryopreser-vation of oocytes,cryopreservation of embryos and preservation of ovarian tissues and so on.But thoses methods for the protection of ovarian function and reproductive function are not yet perfect.

RESUMEN

Objective To investigate the clinical effect of laparoscopy combined with leuprolide on patients with mild endometriosis and infertility and effects on fertility function of patients. Methods A total of 60 patients with mild endometriosis and infertility in our hospital were randomly divided into the control group (n=30) and the observation group (n=30). The control group was treated with laparoscopic surgery, the observation group was treated with leuprorelin treatment, and the levels of FSH, E2, luteinizing hormone and PROG were tested by automatic biochemical analyzer between two groups before and after treatment, the pregnancy rate as assessment of reproductive function in patients. Clinical curative effect and effect on fertility of 2 groups were compared. Results Levels of FSH, E2, LH and PROG in the observation group were lower than those in the control group (P<0.05) in 3 months after treatment, and the pregnancy rates in the observation group after 1 months, 2 months and 3 months were higher than that in the control group (P<0.05). Conclusion The treatment of Laparoscopy combined with Leuprolide for mild endometriosis and infertility in patients has ideal effect, and is worthy of popularization and application.

Objective To explore the clinical effect of the open and laparoscopic surgery in the treatment of ectopic pregnancy to retain the clinical efficacy of reproductive function analysis. Methods Four hundred and fifty-six patients with ectopic pregnancy were selected as our subjects. Two hundred and eleven cases were served as laparotomy group( open),and 245 cases were laparoscopic group( laparoscopic). The data of intraoperative blood loss,operative time,postoperative anal exhaust time,average hospital stay,tubal patency rate,intrauterine pregnancy rate and again repeat ectopic pregnancy rate were recorded. Results Patients in two groups were successfully completed surgery. The operative time,intraoperative blood loss,postoperative anal exhaust time,average hospitalization time were(55. 1 ± 13. 5)min,(63. 5 ± 18. 3)ml,(25. 7 ± 5. 6)h,(6. 1 ± 2. 0)d respectively in laparotomy group,and(41. 3 ± 15. 5)min,(41. 1 ± 13. 3)ml,(13. 5 ± 5. 1)h,(3. 6 ± 1. 4)d respectively in laparoscopic group,and the differences were significant( t =2. 045,2. 263,3. 131, 3. 152,P﹤0. 05). The tubal patency rate,intrauterine pregnancy,ectopic pregnancy rate in laparotomy group were 81. 0%( 171/211 ),59. 7%( 126/211 ) and 26. 5%( 56/211 ) respectively,and 75. 5%( 185/245 ), 53. 5%( 131/245 ) and 22. 9%( 56/245 ) respectively in laparoscopic group,and the differences were not significant(χ2 =2. 254,2. 130,1. 242;P ﹥0. 05 ). Conclusion Laparoscopic surgery in the treatment of ectopic pregnancy is superior to laparotomy in terms of small trauma,less bleeding,faster recovery,shorter hospitalization time,and postoperative tubal patency rate.
